A significant number of Brazilian gestational-age women are still not tested for HIV, representing a high risk of transmission to their newborns. The current study sought to identify the number of pregnant women with no previous testing or undocumented for HIV referred to the Gynecology and Obstetrics Department of a Regional Teaching Hospital and included diagnosis of HIV infection determined by a rapid test and perinatal transmission in pregnancy. Medical records of all pregnant women admitted to hospital from January 2001 to December 2005 were reviewed. Pregnant women without HIV results were submitted to a rapid HIV test. Those who tested positive were further tested by ELISA and confirmed by indirect immunofluorescence assay (IIA) or Western blot (WB). The viral load from babies born to HIV-infected mothers was assessed by bDNA. Of the 16,424 pregnant women analyzed (6.6%), 1,089 were undocumented for HIV. Eleven women were positive in rapid testing and 10 were confirmed by ELISA, IIA or WB, with 0.9% seropositivity. Mother/infant pairs received zidovudine monotherapy prophylaxis and infant viral load was lower than 50 copies/mL. A higher number of pregnant women previously tested for HIV during antenatal care was verified, compared to that obtained nationwide.

Toxoplasma gondii causes severe fetal disease during acute infection in pregnant women, thus demanding early diagnosis for effective treatment and fetus preservation. Fetal tests are inefficient and risky, and diagnosis is based on maternal IgM serology, which had weak screening ability due to increased sensitivity, with alternative IgG avidity tests. Here, we performed ELISA and avidity assays using a recombinant T. gondii antigen, rROP2, in samples from 160 pregnant women screened from a large public hospital who were referred due to positive IgM assays. IgG serology and avidity assays were compared using whole T. gondii extract or rROP2. ELISA IgG detection with rROP2 showed good agreement with assays performed with T. gondii extract, but rROP2 IgG avidity assays were unrelated to whole extract antigen IgG avidity, regardless of the chaotrope used. These data show that avidity maturation is specific to individual antigen prevalence and immune response during infection. ELISA rROP2 IgG assays may be an alternative serological test for the diagnosis of toxoplasmosis during pregnancy, although our data do not support their use in avidity assays.

The main serological marker for the diagnosis of recent toxoplasmosis is the specific IgM antibody, along with IgG antibodies of low avidity. However, in some patients these antibodies may persist long after the acute/recent phase, contributing to misdiagnosis in suspected cases of toxoplasmosis. In the present study, the diagnostic efficiency of ELISA was evaluated, with the use of peptides derived from T. gondii ESA antigens, named SAG-1, GRA-1 and GRA-7. In the assay referred to, we studied each of these peptides individually, as well as in four different combinations, as Multiple Antigen Peptides (MAP), aiming to establish a reliable profile for the acute/recent toxoplasmosis with only one patient serum sample. The diagnostic performance of the assay using MAP1, with the combination of SAG-1, GRA-1 and GRA-7 peptides, demonstrated better discrimination of the acute/recent phase from non acute/recent phase of toxoplasmosis. Our results show that IgM antibodies to MAP1 may be useful as a serological marker, enhancing the diagnostic efficiency of the assay for acute/recent phase of toxoplasmosis.

Nunca a internacionalização foi tão importante para as empresas nacionais e tão utilizada pelos empresários. O franchising é, por isso, um modo de expansão internacional que os empreendedores podem utilizar quando procuram novos mercados. O formato do negócio a ser franchisado deve ser previamente testado no mercado nacional e depois adaptado às particularidades dos países em que vai ser comercializado. Esta investigação tem como objetivo compreender as principais motivações e barreiras, que o empresário encontra quando usa o franchising como forma de internacionalização, bem como as vantagens e desvantagens percebidas quando recorre a esta forma de comercialização. Para responder a estas questões de investigação foi aplicado o método qualitativo, através de um estudo de caso à empresa Onebiz, nomeadamente a marca NBB, concretizado através de uma entrevista semi-estruturada e da recolha de dados secundários, designadamente pela consulta do site da empresa. O estudo permitiu concluir que, no caso da marca NBB, que se trata de um formato de negócio que, em determinadas circunstâncias, pode ser usado pelas empresas para entrarem nos mercados externos, embora apresente virtualidades e constrangimentos que devem ser tomados em conta quando se opte por este modo de internacionalização.

A competitividade contemporânea e a globalização impulsionaram as empresas a explorar novos mercados, e a munir-se de recursos distintivos e de valor acrescentado. Para além da expansão dos mercados, Le Boterf (2006) afirma que a solicitação social de confiança tem aumentado, e as empresas têm de ser capazes de proporcionar e garanti-la. Neste sentido a adequação dos Recursos Humanos é imperativo na estratégia das organizações. Este estudo tem como objetivos compreender qual a importância das competências transversais na missão internacional, compreender quais as competências transversais que são mais valorizadas pelos recrutadores e como é feita a sua avaliação. Para conseguir responder a estas questões, emerge a investigação que se segue, com um cariz qualitativo. O método de recolha de dados utilizado foi a entrevista semiestruturada e foram entrevistados dez recrutadores de expatriados. Os resultados sugerem que as competências transversais são características importantes na missão internacional, uma vez que atuam em fatores que influenciam o sucesso da missão. Das competências mais importantes é enumerado o relacionamento interpessoal, trabalho em equipa, comunicação, capacidade de adaptação e a tolerância à ambiguidade. No final do trabalho são apresentados os resultados da investigação, bem como a sua discussão.

After the report of a second case of canine visceral leishmaniasis (CVL) in São Bento da Lagoa, Itaipuaçu, in the municipality of Maricá, Rio de Janeiro State, an epidemiological survey was carried out, through active search, totaling 145 dogs. Indirect immunofluorescence assay (IFA), enzyme-linked immunosorbent assay (ELISA), and rapid chromatographic immunoassay based on dual-path platform (DPP(r)) were used to perform the serological examinations. The parasitological diagnosis of cutaneous fragments was performed by parasitological culture, histopathology, and immunohistochemistry. In the serological assessment, 21 dogs were seropositive by IFA, 17 by ELISA, and 11 by DPP(r), with sensitivity of 66.7%, 66.7% and 50%, and specificity of 87.2%, 90.2% and 94%, respectively for each technique. The immunohistochemistry of bone marrow using the cell-block technique presented the best results, with six positive dogs found, three of which tested negative by the other parasitological techniques. Leishmania sp. was isolated by parasitological culture in three dogs. The detection of autochthonous Leishmania infantum in Itaipuaçu, and the high prevalence of seropositive dogs confirm the circulation of this parasite in the study area and alert for the risk of expansion in the State of Rio de Janeiro.

RESUMO: Atualmente a prática de regular de atividade física é mencionada na literatura como uma estratégia fundamental no tratamento da diabetes tipo 2, com influencia positiva na redução das comorbilidades associadas a esta doença. (Sigal et al., 2006; Praet & van Loon, 2009). No entanto, e apesar deste reconhecimento, a maioria da população com diabetes tipo 2, apresenta baixos níveis de atividade física que na literatura têm sido relacionados com conhecimento deficitário ou inadequado acerca dos benefícios e das recomendações para a mesma (Madden, et. al., 2009). Este estudo foi realizado com o objetivo de determinar o nível de atividade física e de avaliar a sua associação com o conhecimento acerca dos benefícios da atividade física e recomendações específicas para a sua prática. Trata-se de um estudo observacional, de natureza descritiva e do tipo Survey (estudo de levantamento) realizado com uma amostra de 50 indivíduos recrutados a partir da consulta de diabetes de uma Unidade de Saúde Familiar da região de Setúbal. A recolha de dados foi feita através da aplicação conjunta, e de uma só vez, de três questionários (Questionário de caracterização sócio-demográfica; versão portuguesa do International Physical Activity Questionnaire - IPAQ; Questionário de Avaliação de Conhecimentos acerca dos benefícios e recomendações específicas da prática regular de atividade física), adaptados ao método de entrevista telefónica. Os resultados revelaram que a maioria dos participantes tinham baixos níveis de atividade física (60%), apesar de demonstrarem bons conhecimentos acerca dos benefícios da sua prática regular (67%). Nas analises exploratórias verificou-se uma associação estatisticamente significativa entre as variáveis, “género” (p= 0,045) e “existência de recomendação para a prática do exercício por parte de um profissional de saúde” (p=0,017), com os conhecimentos acerca dos benefícios da prática regular de atividade física. São os indivíduos do género feminino e com a existência de recomendação para o exercício por parte dos profissionais de saúde, que tendem a demonstrar um nível mais elevado de conhecimento acerca dos benefícios da atividade física. Os resultados mostram igualmente que apesar de não existir uma associação estatisticamente significativa entre o conhecimento acerca das recomendações específicas para a prática da atividade física (recomendações para o modo, frequência duração e intensidade da atividade física), e o nível de atividade física autorreportada, a maioria dos participantes desconhece estas recomendações (70,3%). Estes resultados sugerem a necessidade de realizar programas educativos dirigidos a este aspeto ou de incluir este tipo de informação nas recomendações dos profissionais de saúde para a prática regular de atividade física em indivíduos com diabetes do tipo 2. -----------ABSTRACT:The practice of physical activity has been referred in the research literature as a key strategy in the management of type 2 diabetes mellitus (T2DM), with positive influence in reducing its associated complications (Sigal et al., 2006; Praet & van Loon, 2009). However, the majority of people with T2DM, presents low levels of physical activity, which has been associated, with poor knowledge about its benefits and/or about the current guidelines’ recommendations for that practice (Madden, et. al., 2009). The purpose of this study was to determine the level of physical activity, in a sample of T2DM patients, and to assess its relationship with knowledge of physical activity benefits and knowledge about specific recommendations for the practice of physical activity. An observational descriptive study was carried out with a sample of 50 T2DM participants, recruited from the medical consultation of one of the Familiar Health Units in the Setúbal Region. Three aggregated questionnaires (sociodemographic questionnaire, Portuguese version of the International Physical Questionnaire- IPAQ; Knowledge evaluation about physical activity benefits and specific recommendations for regular physical activity practice Questionnaire) were administrated by telephone interview, all at the same time. The study’s findings showed that the majority of the participants had low levels of physical activity (60%), regardless their appropriate knowledge concerning the benefits of regular physical activity (67%). The results of this study have also shown that participants have a poor and/ or inappropriate knowledge concerning the specific physical activity recommendations that have a positive impact in this specific condition. The exploratory analyses revealed a statistically significant association between an appropriate knowledge about the benefits of physical activity and both “gender” (p=0,045) and “recommendation for physical activity practice by an health professional” (p=0,017). Female participants, who received recommendations for regular physical activity, showed higher levels of knowledge concerning the benefits of being physically ative. The study’s findings suggest that T2DM patients need appropriate information and knowledge about how they should practice physical activity. Practising physical activity following current specific recommendations about the mode, frequency, intensity and duration has a positive effect on the management of T2DM.

Mastocytosis refers to a group of disorders characterized by the infiltration of clonally derived mast cells to the skin or extracutaneous tissues resulting in a heterogeneous clinical picture. It is a rare hematologic disorder in all its forms. The exact incidence is unknown; it affects patients of any age and males and females equally. Its molecular pathogenesis is incompletely understood. The clinical features of mastocytosis result from both chronic and episodic mast cell mediator release, signs and symptoms arising from diffuse or focal tissue infiltration, and, occasionally, the presence of an associated non-mast cell clonal hematologic disease. The histopathologic analysis is essential for definitive diagnosis but there is no curative treatment. The authors report a clinical case of a 72-year-old woman with no history of allergies, with bicytopenia, weight loss, and diffuse axial osteolytic lesions. This is a rare clinical case of aggressive systemic mastocytosis for which palliative treatment can improve survival and quality of life. A brief review of the literature about this pathology is also included.

The exponential increase in clinical research has profoundly changed medical sciences. Evidence that has accumulated in the past three decades from clinical trials has led to the proposal that clinical care should not be based solely on clinical expertise and patient values, and should integrate robust data from systematic research. As a consequence, clinical research has become more complex and methods have become more rigorous, and evidence is usually not easily translated into clinical practice. Therefore, the instruction of clinical research methods for scientists and clinicians must adapt to this new reality. To address this challenge, a global distance-learning clinical research-training program was developed, based on collaborative learning, the pedagogical goal of which was to develop critical thinking skills in clinical research. We describe and analyze the challenges and possible solutions of this course after 5 years of experience (2008-2012) with this program. Through evaluation by students and faculty, we identified and reviewed the following challenges of our program: 1) student engagement and motivation, 2) impact of heterogeneous audience on learning, 3) learning in large groups, 4) enhancing group learning, 5) enhancing social presence, 6) dropouts, 7) quality control, and 8) course management. We discuss these issues and potential alternatives with regard to our research and background.
