In the microvillar microdomain of the kidney brush border, sodium hydrogen exchanger type 3 (NHE3) exists in physical complexes with the serine protease dipeptidyl peptidase IV (DPPIV). The purpose of this study was to explore the functional relationship between NHE3 and DPPIV in the intact proximal tubule in vivo. To this end, male Wistar rats were treated with an injection of the reversible DPPIV inhibitor Lys [Z(NO(2))]-pyrrolidide (I40; 60 mg center dot kg(-1)center dot day(-1) ip) for 7 days. Rats injected with equal amounts of the noninhibitory compound Lys[ Z(NO(2))]-OH served as controls. Na(+) -H(+) exchange activity in isolated microvillar membrane vesicles was 45 +/- 5% decreased in rats treated with I40. Membrane fractionation studies using isopycnic centrifugation revealed that I40 provoked redistribution of NHE3 along with a small fraction of DPPIV from the apical enriched microvillar membranes to the intermicrovillar microdomain of the brush border. I40 significantly increased urine output ( 67 +/- 9%; P < 0.01), fractional sodium excretion ( 63 +/- 7%; P < 0.01), as well as lithium clearance ( 81 +/- 9%; P < 0.01), an index of end-proximal tubule delivery. Although not significant, a tendency toward decreased blood pressure and plasma pH/HCO(3)(-) was noted in I40-treated rats. These findings indicate that inhibition of DPPIV catalytic activity is associated with inhibition of NHE3-mediated NaHCO(3) reabsorption in rat renal proximal tubule. Inhibition of apical Na(+) -H(+) exchange is due to reduced abundance of NHE3 protein in the microvillar microdomain of the kidney brush border. Moreover, this study demonstrates a physiologically significant interaction between NHE3 and DPPIV in the intact proximal tubule in vivo.

Scavenger or Fc gamma receptors are important for capture and clearance of modified LDL particles by monocytes/macrophages. Uptake via scavenger receptors is not regulated by intracellular levels of cholesterol and in consequence, macrophages develop into foam cells in the arterial intima. The levels of scavenger receptor CD36 are increased in atherosclerotic lesions and there is evidence that some components of oxLDL auto-regulate the expression of this receptor. Fc gamma receptor expression is increased in cardiovascular diseases but it is not known weather their expression is regulated by oxLDL. The biological properties of oxLDLs vary depending on the degree of oxidation. In the present study we investigated the effect of LDL particles showing extensive or low oxidation (HoxLDL and LoxLDL) on the expression of CD36 and Fc gamma RII in a human monocytic cell line (THP-1), differentiated or not to macrophage, and the involvement of PPAR gamma. It was found that both forms of oxLDL are able to increase the expression of CD36 and Fc gamma RII and that this effect is dependent on the degree of oxidation and of the stage of cell differentiation ( monocyte or macrophage). We also showed that the increased expression of Fc gamma RII is dependent on PPAR. whereas that of the CD36 is independent of PPAR gamma. Copyright (c) 2008 S. Karger AG, Basel

Candida albicans is the most common opportunistic fungal pathogen and causes local and systemic disease in immunocompromised patients. Alveolar macrophages (AMs) are pivotal for the clearance of C. albicans from the lung. Activated AMs secrete 5-lipoxygenase-derived leukotrienes (LTs), which in turn enhance phagocytosis and microbicidal activity against a diverse array of pathogens. Our aim was to investigate the role of LTB(4) and LTD(4) in AM antimicrobial functions against C. albicans and the signaling pathways involved. Pharmacologic and genetic inhibition of LT biosynthesis as well as receptor antagonism reduced phagocytosis of C. albicans when compared with untreated or WT controls. Conversely, exogenous LTs of both classes augmented base-line C. albicans phagocytosis by AMs. Although LTB(4) enhanced mainly mannose receptor-dependent fungal ingestion, LTD(4) enhanced mainly dectin-1 receptor-mediated phagocytosis. LT enhancement of yeast ingestion was dependent on protein kinase C-delta (PKC delta) and PI3K but not PKC alpha and MAPK activation. Both LTs reduced activation of cofilin-1, whereas they enhanced total cellular F-actin; however, LTB(4) accomplished this through the activation of LIM kinases (LIMKs) 1 and 2, whereas LTD(4) did so exclusively via LIMK-2. Finally, both exogenous LTB(4) and LTD(4) enhanced AM fungicidal activity in an NADPH oxidase-dependent manner. Our data identify LTB(4) and LTD(4) as key mediators of innate immunity against C. albicans, which act by both distinct and conserved signaling mechanisms to enhance multiple antimicrobial functions of AMs.

P>Renal transplant patients with stable graft function and proximal tubular dysfunction (PTD) have an increased risk for chronic allograft nephropathy (CAN). In this study, we investigated the histologic pattern associated with PTD and its correlation with graft outcome. Forty-nine transplant patients with stable graft function were submitted to a biopsy. Simultaneously, urinary retinol-binding protein (uRBP) was measured and creatinine clearance was also determined. Banff`s score and semi-quantitative histologic analyses were performed to assess tubulointerstitial alterations. Patients were followed for 24.0 +/- 7.8 months. At biopsy time, mean serum creatinine was 1.43 +/- 0.33 mg/dl. Twelve patients (24.5%) had uRBP >= 1 mg/l, indicating PTD and 67% of biopsies had some degree of tubulointerstitial injury. At the end of the study period, 18 (36.7%) patients had lost renal function. uRBP levels were not associated with morphologic findings of interstitial fibrosis and tubular atrophy (IF/TA), interstitial fibrosis measured by Sirius red or tubulointerstitial damage. However, in multivariate analysis, the only variable associated with the loss of renal function was uRBP level >= 1 mg/l, determining a risk of 5.290 of loss of renal function (P = 0.003). Renal transplant patients who present PTD have functional alteration, which is not associated with morphologic alteration. This functional alteration is associated to progressive decrease in renal function.

P>Dendritic cells (DCs) play an important role in the clearance of apoptotic cells. The removal of apoptotic cells leads to peripheral tolerance, although their role is still not clear. We show that the uptake of apoptotic thymocytes by DCs converts these cells into tolerogenic DCs resistant to maturation by lipopolysaccharide, modulating the production of interleukin-12 and up-regulating the expression of transforming growth factor-beta(1) latency associated peptide. We also observed that DCs pulsed with apoptotic cells in the allogeneic context were more efficient in the expansion of regulatory T cells (Tregs), and that this expansion requires contact between DCs and the T cell. The Tregs sorted from in vitro culture suppressed the proliferation of splenocytes in vitro in a specific and non-specific manner. In the in vivo model, the transfer of CD4+ CD25- cells to Nude mice induced autoimmunity, with cell infiltrate found in the stomach, colon, liver and kidneys. The co-transfer of CD4+ CD25- and CD4+ CD25+ prevented the presence of cell infiltrates in several organs and increased the total cell count in lymph nodes. Our data indicate that apoptotic cells have an important role in peripheral tolerance via induction of tolerogenic DCs and CD4+ CD25+ Foxp3+ cells that present regulatory functions.

The clearance of apoptotic cells by phagocytes is a fundamental process during tissue remodeling and resolution of inflammation. In turn, the phagocytosis of apoptotic cells generates signals that suppress pro-inflammatory activation of macrophages. These events occur during the resolution phase of inflammation and therefore the malfunctioning of this process may lead to inflammation-related tissue damage. Here, we demonstrate that the calcium-binding protein S100A9, normally abundant in the cytoplasm of neutrophils and also released by apoptotic neutrophils, is involved in the suppression of macrophages after the uptake of apoptotic neutrophils. Both, spontaneous and induced production of inflammatory species (nitric oxide, hydrogen peroxide and TNF-alpha) as well as the phagocytic activity were inhibited when macrophages were in presence of apoptotic neutrophils, conditioned medium from neutrophil cultures or a peptide corresponding to the C-terminal region of S100A9 protein. On the other hand, macrophages kept in the conditioned medium of neutrophils that was previously depleted of S100A9 were shown to resume the activated status. Finally, we demonstrate that the calcium-binding property of S100A9 might play a role in the suppression process, since the stimulation of intracellular calcium release with ionomycin significantly reversed the effects of the uptake of apoptotic neutrophils in macrophages. In conclusion, we propose that S100A9 is a novel component of the regulatory mechanisms of inflammation, acting side-by-side with other suppressor factors generated upon ingestion of apoptotic cells. It currently is unknown whether creatine supplementation is safe for people with or at risk of kidney disease. We report on the short-term effects of creatine supplementation on kidney function in a young man with a single kidney and mildly decreased glomerular filtration rate (GFR). A 20-year-old man who had undergone unilateral nephrectomy and presented with mildly decreased GFR without kidney damage underwent a trial with 35 days of creatine supplementation (20 g/d for 5 days followed by 5 g/d for the next 30 days) and had his kidney function monitored. After the intervention, (51)Cr-EDTA clearance (pre, 81.6 mL/min/1.73 m(2); post, 82.0 mL/min/1.73 m(2)), proteinuria (protein excretion: pre, 130 mg/d; post, 120 mg/d), and electrolyte levels were unchanged. Albuminuria, serum urea level, and estimated creatinine clearance were decreased (pre, 4.6 mg/d; post, 2.9 mg/d; pre, 37 mg/d; post, 28 mg/dL; and pre, 88 mL/min/1.73 m(2); post, 71 mL/min/1.73 m(2), respectively), whereas serum creatinine level was slightly increased (pre, 1.03 mg/dL; post, 1.27 mg/dL), falsely suggesting kidney function impairment. This prospective report suggests that short-term creatine supplementation may not affect kidney function in an individual with a single kidney, mild decreased GFR, and ingesting a high-protein diet (ie, 2.8 g/kg/d). This finding has great relevance considering that creatine-induced kidney disease has been a growing concern, even for healthy people. Am J Kidney Dis 55: e7-e9. The progression to end-stage renal failure is independent of the initial pathogenic mechanism. Metabolic acidosis is a common consequence of chronic renal failure that results from inadequate ammonium excretion and decreased tubular bicarbonate reabsorption. Protoporphyrin IX (PpIX) is the immediate metabolic precursor of the heme molecule. The purpose of this study was to evaluate the levels of erythrocytes protoporphyrin IX at an animal model during progressive renal disease. A total of 36 eight-week-old male Wistar rats were divided into six groups: Normal, 4 and 8 weeks after 5/6 nephrectomy (NX). Renal function was evaluated by creatinine clearance and plasma creatinine levels. The autofluorescence of erythrocytes porphyrin of healthy and NX rats was analyzed using fluorescence spectroscopy. Emission spectra were obtained by exciting the samples at 405 nm. Significant differences between normal and NX rats autofluorescence shape occurred in the 600-700 nm spectral region. A correlation was observed between emission band intensity at 635 nm and progression of renal disease.

Background: The relationship between CETP and postprandial hyperlipemia is still unclear. We verified the effects of varying activities of plasma CETP on postprandial lipemia and precocious atherosclerosis in asymptomatic adult women. Methods: Twenty-eight women, selected from a healthy population sample (n = 148) were classified according to three CETP levels, all statistically different: CETP deficiency (CETPd <= 4.5%, n = 8), high activity (CETPi >= 23.8, n = 6) and controls (CTL, CETP >= 4.6% and <= 23.7%, n = 14). After a 12 h fast they underwent an oral fat tolerance test (40 g of fat/m(2) of body surface area) for 8 hours. TG, TG-rich-lipoproteins (TRL), cholesterol and TRL-TG measurements (AUC, AUIC, AR, RR and late peaks) and comparisons were performed on all time points. Lipases and phospholipids transfer protein (PLTP) were determined. Correlation between carotid atherosclerosis (c-IMT) and postprandial parameters was determined. CETP TaqIB and I405V and ApoE-epsilon 3/epsilon 2/epsilon 4 polymorphisms were examined. To elucidate the regulation of increased lipemia in CETPd a multiple linear regression analysis was performed. Results: In the CETPi and CTL groups, CETP activity was respectively 9 and 5.3 higher compared to the CETPd group. Concentrations of all HDL fractions and ApoA-I were higher in the CETPd group and clearance was delayed, as demonstrated by modified lipemia parameters (AUC, AUIC, RR, AR and late peaks and meal response patterns). LPL or HL deficiencies were not observed. No genetic determinants of CETP deficiency or of postprandial lipemia were found. Correlations with c-IMT in the CETPd group indicated postprandial pro-atherogenic associations. In CETPd the regression multivariate analysis (model A) showed that CETP was largely and negatively predicted by VLDL-C lipemia (R(2) = 92%) and much less by TG, LDL-C, ApoAI, phospholipids and non-HDL-C. CETP (model B) influenced mainly the increment in ApoB-100 containing lipoproteins (R(2) = 85% negatively) and phospholipids (R(2) = 13%), at the 6(th)h point. Conclusion: The moderate CETP deficiency phenotype included a paradoxically high HDL-C and its sub fractions (as earlier described), positive associations with c-IMT, a postprandial VLDL-C increment predicting negatively CETP activity and CETP activity regulating inversely the increment in ApoB100-containing lipoproteins. We hypothesize that the enrichment of TG content in triglyceride-rich ApoB-containing lipoproteins and in TG rich remnants increases lipoproteins` competition to active lipolysis sites, reducing their catabolism and resulting on postprandial lipemia with atherogenic consequences.

There are many viruses that are able to infect the alimentary tract of man. Little is known, however, about the mechanism of infection itself or the pathophysiology of the gut during infection. 'The research reported here is concerned with the differences in susceptibility among suckling mice of various ages inoculated by the intraperitoneal and intragastric routes. Since the normal mode of entry of many viruses to the gut is via the oral route, Coxsackievirus B5, a human enterovirus which does attack this way, was utilized. It is a non-tumor producing RNA virus that has been shown to act similarly in the mouse and human. The virus was pooled in HeLa cell cultures and titered by a plaquing assay in the same cell cultures. CD-l mice, 10, 14, 18, and 22 days old , were infected either orally or intraperitoneally with 5.0 x 10^10 (10 day old animals) and 1.0 x10^9 plaque forming units per animal. Dissections were done at 1 and 3 days post infection with samples of the blood, heart, liver, and gut being taken from each animal. Each sample was titered individually and the data presented as an average of six samples. As a result of previous work, it is known that the gut of a newborn mouse isn't able to decrease the concentration of the infecting dose and therefore provides no defense against an enteric infection with Coxsackievirus B5. In contrat, mature mice are able to reduce the amount of viral dissemination across the gut as well as inhibit replication after absorption has occurred. The results of this study indicate that there is a double barrier system developing in suckling mice that is involved with and directly related to the gastrointestinal tract The first part of this defense is the inhibition of penetration of virus across the gut when the primary site of' infection is the intestinal mucosa. This mechanism develops sometime around 20 to 22 days after birth. At about 16-18 days of age, suckling mice that were challenged intragastrically are able to stop active replication and initiate clearance of virus from the systemic circulation. There are many factors that might contribute to the marked decrease in susceptibility with age of suckling mice. Some of these or possibly a combination of these factors might explain the defense mechanisms described above, but to date, the chemistry or mechanical functioning of the gastrointestinal barrier to enteric viral infection is unknown.

Esta monografia tem como tema central os obstáculos jurídicos enfrentados por instituições culturais na abertura de acervos pessoais. Analisa o desenvolvimento da noção de memória e patrimônio através da história, discorre sobre os mesmos conceitos no panorama jurídico e político brasileiro, estuda a legislação arquivística e constata sua adequação para permitir o exercício dos direitos culturais previstos na Constituição Federal de 1988, desde que através das parcerias entre Governo e iniciativa privada permitidas através da legislação de incentivos à cultura. Oferece como anexo o Manual do Clearance, propondo o desembaraço dos direitos autorais e afins, através de licenças e outros instrumentos, acompanhados da indicação da legislação correlata.

Justificativa: Doses inadequadas de diálise estão associadas a maior morbidade e mortalidade em pacientes com uremia terminal em tratamento hemodialítico. Atualmente os métodos mais utilizados para medir a adequação da hemodiálise baseiam-se no cálculo da depuração fracional de uréia (Kt/V) e da taxa de redução da uréia (URR). Nesse estudo avaliou-se a reprodutibilidade do Kt/V e da URR e analisou-se o número aceitável de medidas mensais destes parâmetros para determinar de forma fidedigna a adequação da hemodiálise. Métodos: Avaliamos 43 pacientes clinicamente estáveis, em cinco sessões de hemodiálise (sessões 1 a 5), durante três semanas. Três diálises consecutivas na primeira semana (sessões 1, 2, 3) e duas sessões de meio de semana, nas duas semanas subseqüentes (sessões 4 e 5). Resultados: Não houve diferença entre a média dos Kt/Vs obtidos em diálises seqüenciais (1, 2 e 3) comparada a média dos Kt/Vs obtidos em diálises no meio da semana (2, 4 e 5). O mesmo aconteceu com a URR e com os coeficientes de variação do Kt/V e da URR. Também não encontramos diferença entre uma medida mensal (Kt/V 2) e três medidas mensais (média dos Kt/Vs 2, 4 e 5). O mesmo ocorreu com a URR. A diferença entre o Kt/V 2 e a média dos Kt/Vs 2, 4 e 5 foi de – 0,02 unidades e entre a URR 2 em relação a média das URRs 2, 4 e 5 foi de – 0,5%. Além disso, se fizermos apenas uma medida mensal (Kt/V 2) 80% dos pacientes estarão a uma distância de no máximo 0,16 unidades abaixo da média dos Kt/Vs 2, 4 e 5 e no máximo 0,12 unidades acima. Em relação a URR 80% dos pacientes estarão a uma distância de no máximo 4,37% abaixo da média das URRs 2, 4 e 5 e no máximo 3,37% acima. O grau de concordância entre o Kt/V 2 e a média dos Kt/Vs 2, 4 e 5 é bom para pontos de corte do Kt/V igual ou superior a 1,3. No caso da URR a concordância é boa para pontos de corte acima de 65%. Conclusão: Concluímos que nesta população de pacientes clinicamente estáveis os parâmetros de adequação da hemodiálise mostraram-se reprodutíveis não havendo necessidade de aumentar o número de medidas mensais da quantidade de diálise ofertada.

Introdução: A anfotericina B é a droga de escolha para o tratamento de doenças fúngicas severas, estando associada, no entanto, a alta incidência de nefrotoxicidade. O uso de anfotericinas modificadas está associado a elevado custo. Em grupos de baixo risco o uso de sobrecarga hidrossalina pode ser suficiente para evitar perda severa de função renal. Métodos: Foram estudados prospectivamente pacientes internados em hospital universitário, com idade superior a 12 anos, e que estavam dentro das primeiras 24 horas de uso de anfotericina B. Foram excluídos pacientes em centros de terapia intensiva e que estivessem em uso de drogas vasoativas. Solução salina 0,9% (500 ml) foi infundida antes e após a anfotericina B. Foram coletados exames na inclusão e no término do tratamento. A dosagem de creatinina sérica foi repetida após 30 dias do término do tratamento. Resultados: Foram estudados 48 pacientes. A média de elevação da creatinina sérica foi de 0,3 (0,18-0,41) mg/dl., representando um decréscimo médio de 25 (12,8-36,9) ml/min na depuração de creatinina endógena (DCE). Insuficiência renal aguda (IRA), definida pela elevação maior do que 50% da creatinina basal, ocorreu em 15 pacientes (31,3%). Pacientes que utilizaram antibióticos e aqueles em status pós-quimioterapia ou submetidos a transplante de medula óssea foram os que apresentaram maior risco de desenvolverem IRA. A creatinina e a DCE após 30 dias do término do tratamento não diferiram de seus valores basais. Conclusão: Em pacientes de baixo risco, o uso de anfotericina B com adminstração profilática de solução fisiológica foi associado à alteração pequena e reversível da função renal. Devido ao alto custo, o uso de métodos mais dispendiosos nestes pacientes não parece justificado no momento. Ensaios clínicos randomizados são necessários nesta população.

A implantação do Plano Real em 1994 provocou mudanças na estrutura de receitas dos bancos brasileiros. Em épocas de altas taxas de inflação, o resultado dos bancos era composto substancialmente por rendas oriundas do financiamento da dívida interna do país e em menor parcela de outras receitas como dos empréstimos ao setor privado. Com a estabilização da economia e a globalização dos mercados financeiros mundiais, refletida na entrada de bancos estrangeiros no mercado brasileiro, as taxas de juros tenderam a diminuir, ocasionando uma mudança no foco de atuação dos Bancos que estão se concentrando na intermediação financeira. Neste projeto é apresentada a formação básica do resultado de um banco obtido com a intermediação financeira e explana-se sobre os riscos da atividade bancária. É focado o risco de crédito, abrangendo a descrição das principais metodologias de análise. Estuda-se a Resolução CMN/BACEN nO 2682 que mudou a contabilização das rendas de renegociação de dívidas e estabeleceu parâmetros mínimos para a classificação das operações de crédito alterando os critérios de constituição da provisão para créditos de liquidação duvidosa. É explicado como pode ser utilizado um modelo RARO C - Risk Adjusted Return on Capital - desenvolvido originalmente pelo Bankers Trust - para gerenciamento da Carteira de crédito de um Banco de Varejo típico. Para ilustração e considerando que no mercado brasileiro os dados estatísticos sobre operações de crédito são escassos, além de existirem dificuldades na obtenção de dados de uma Carteira de crédito real relacionadas ao sigilo bancário e estratégias de investimento, o modelo RAROC será aplicado em uma Carteira de crédito fictícia de um Banco de Varejo, especialmente criada para esse fim. O estudo não abrange os recursos necessários para a implementação do modelo, nem customização para outros tipos de Bancos, restringindo-se à análise da utilização da metodologia. Por fim, apresentamos nossas conclusões a respeito da gestão do risco do crédito baseada na utilização de um modelo RAROC.
