The South American species of the genera Askola and Hagenulopsis are revised. Three new species of Askola from Brazil are described based on male imagos. Askola emmerichi sp. nov. and A. paprockii sp. nov. present spotted wings, but differ in general coloration and details of genitalia; Askola cipoensis sp. nov. is easily distinguished because the male eyes being widely separated on meson of head. Three new species of Hagenulopsis are also described: H. lipeo (from Argentina and Bolivia) and H. zunigae (from Colombia), both described from imagos and nymphs, can be recognized by details of coloration and male genitalia. H. esmeralda sp. nov. from Ecuador, described from imagos, shows a distinct male genitalia and translucent male abdomen. A key to species for the the male and female imagos of Askola and Hagenulopsis species is provided.

Background: Addressing human rights issues brings forth ethical and political responsibilities for occupational therapists and requires new epistemological and educational approaches. The way occupational therapists have faced these challenges has depended upon historical, cultural and social contexts. Aim and method: By means of literature review and historical analysis, this paper reflects on how occupational therapists have dealt with human matters issues and on the contemporary changes within the profession. Results and discussion: The paper portrays how Latin American occupational therapists have engaged in social transformation by choosing not to transform ethical and political problems into technical matters. Taking into account experiences and views from South Africa, Brazil and Chile, the paper outlines the importance of developing political literacy and interdisciplinary professional/postprofessional education to prepare the new generation of occupational therapists to engage in social transformation. Addressing issues of invisibility and lack of access to human rights, the paper reflects on the need of developing conceptual tools and strategies for change, and discusses the transformations being produced in contemporary occupational therapy. Conclusion: Occupational therapists and scientists need to be attentive to human rights issues. They also need to answer the call for interconnectedness in the present-day complex societies, and engage in networking and a cross-bordering dialogue. Nevertheless, although necessary and welcome, international cooperation requires a permanent exercise of cultural sensitivity, political awareness and self-awareness.

Cancers of the upper aerodigestive tract (UADT; including oral cavity, pharynx, larynx and oesophagus) have high incidence rates all over the world, and they are especially frequent in some parts of Latin America. However, the data on the role of the major risk factors in these areas are still limited. We have evaluated the role of alcohol and tobacco consumption, based on 2,252 upper aerodigestive squamous-cell carcinoma cases and 1,707 controls from seven centres in Brazil, Argentina, and Cuba. We show that alcohol drinkers have a risk of UADT cancers that is up to five times higher than that of never-drinkers. A very strong effect of aperitifs and spirits as compared to other alcohol types was observed, with the ORs reaching 12.76 (CI 5.37-30.32) for oesophagus. Tobacco smokers were up to six times more likely to develop aerodigestive cancers than never-smokers, with the ORs reaching 11.14 (7.72-16.08) among current smokers for hypopharynx and larynx cancer. There was a trend for a decrease in risk after quitting alcohol drinking or tobacco smoking for all sites. The interactive effect of alcohol and tobacco was more than multiplicative. In this study, 65% of all UADT cases were attributable to a combined effect of alcohol and tobacco use. In this largest study on UADT cancer in Latin America, we have shown for the first time that a prevailing majority of UADT cancer cases is due to a combined effect of alcohol and tobacco use and could be prevented by quitting the use of either of these two agents.

Purpose LATIN is a multinational case-control study designed to identify risk factors for agranulocytosis and to estimate the incidence rate of the disease in some Latin American countries. Methods Each study site in Brazil, Argentina and Mexico conducted an active search of agranulocytosis patients in hematology clinics and looked for possible associations with drug use. Results The overall incidence rate was 0.38 cases per 1 million inhabitant-years. Agranulocytosis patients more often took medications already associated with agranulocytosis than controls (p=0.01), mainly methimazole (OR 44.2, 95% CI 6.8 to infinity). The population attributable risk percentage (etiologic fraction) was 56%. The use of nutrient supplements was more frequent among patients than controls (p=0.03). Conclusions Agranulocytosis seems to be very rare in Latin America. The lower than expected number of cases identified during the study period precluded estimation of the risk associated to individual drugs, with the exception of methimazol. However, this is the longest series of agranulocytosis cases ever gathered in Latin America, and information on drug exposures was collected prospectively. The conclusion is that drug-induced agranulocytosis does not seem to be a major public health problem in the study regions.

Allergies in Latin America is the first cross-national survey that describes the symptoms, impact, and treatment of nasal allergies (NAs) in individuals >= 4 years old in Latin America (LA). In total, 22,012 households across the Latin American countries of Argentina, Brazil, Chile, Colombia, Ecuador, Mexico, Peru, and Venezuela were screened for children, adolescents, and adults with a diagnosis of NA and either symptoms or treatment in the past 12 months. A total of 1088 adults and 457 children and adolescents were included and the sample was probability based to ensure valid statistical inference to the population. Approximately 7% of the LA population was diagnosed with NAs with two of three respondents stating that their allergies were seasonal or intermittent in nature. A general practice physician or otolaryngologist diagnosed the majority of individuals surveyed. Nasal congestion was the most common and bothersome symptom of NAs. Sufferers indicated that their symptoms affected productivity and sleep and had a negative impact on quality of life. Two-thirds of patients reported taking some type of medication for their NAs, with a roughly equal percentage of patients reporting taking over-the-counter versus prescription medications. Changing medications was most commonly done in those reporting inadequate efficacy. The most common reasons cited for dissatisfaction with current medications were related to inadequate effectiveness, effectiveness wearing off with chronic use, failure to provide 24-hour relief, and bothersome side effects (e.g., unpleasant taste and retrograde drainage into the esophagus). Findings from this cross-national survey on NAs have confirmed a high prevalence of physician-diagnosed NAs and a considerable negative impact on daily quality of life and work productivity as well as substantial disease management challenges in LA. Through identification of disease impact on the LA population and further defining treatment gaps, clinicians in LA may better understand and treat NAs, thus leading to improvements in overall patient satisfaction and quality of life. Carios mimon is an argasid tick common on Chiroptera, originally described from larvae collected on bats Mimon crenulatum from Bolivia and Eptesicus brasiliensis from Uruguay. Later it was also registered from Argentina and recently included among the Brazilian tick fauna. In Brazil, this species is very aggressive to man, resulting in intense inflammatory response and pain. It is known only by the larval description and its morphology resembles that from other species currently included into the genus Carios, formerly classified into the subgenus Alectorobius, genus Ornithodoros. Here we describe adults and redescribe the larva of C. mimon, based on light and scanning electron microscopy. Remarks about its morphological similarity with other species of this genus are also discussed. Molecular analysis inferred from a portion of the 16S rRNA mitochondrial gene placed C. mimon in a cluster supported by maximal bootstrap value (100%) with other argasid species (mostly bat parasites in the New World), which have been classified into either the genus Ornithodoros or Carios, depending on the Argasidae classification adopted by different authors.

Para tal, ?? utilizado o marco te??rico que se baseia na teoria de institution building e, especialmente, o modelo conceituai de Esman, acrescido de fatores peculiares aos pa??ses estudados. A Ecole Nationale d'Administration (ENA), da Fran??a, modelo de cria????o dessas escolas, ?? analisada como paradigma. Com base no marco te??rico e no modelo franc??s, a an??lise comparativa propicia inferir que tanto o INAP como a ENAP, nos contextos em que foram criados, n??o se institucionalizaram. Software and information services (SIS) have become a field of increasing opportunities for international trade due to the worldwide diffusion of a combination of technological and organizational innovations. In several regions, the software industry is organized in clusters, usually referred to as "knowledge cities" because of the growing importance of knowledge-intensive services in their economy. This paper has two primary objectives. First, it raises three major questions related to the attractiveness of different cities in Argentina and Brazil for hosting software companies and to their impact on local development. Second, a new taxonomy is proposed for grouping clusters according to their dominant business segment, ownership pattern and scope of operations. The purpose of this taxonomy is to encourage further studies and provide an exploratory analytical tool for analyzing software clusters.

OBJETIVO: Estudiar la fluctuación estacional de Aedes aegypti y correlacionar su abundancia con factores ambientales. MÉTODOS: Las colectas fueron realizadas entre octubre de 2002 y noviembre de 2003, en la ciudad de Resistencia, província del Chaco, Argentina. Fueron hechos muestreos semanales empleando ovitrampas. El número de huevos colectados fue correlacionado con la temperatura, humedad relativa ambiente, evaporación y precipitaciones registradas en dicha localidad. Se utilizó el test de correlación de Pearson con los respectivos datos climáticos semanales, realizándose correlaciones simples y múltiples. RESULTADOS: La ocurrencia de huevos fue registrada de manera discontinua, desde la última semana de octubre de 2002, hasta la última de junio de 2003, a partir de la cual no fueron encontrados hasta noviembre de 2003. Se observó un pico de abundancia (70%) en noviembre y diciembre, que coincidió con el período de temperaturas altas y mayores precipitaciones. Otro pico, aunque de menor importancia, fue observado en abril y coincidió con las lluvias de otoño. Las correlaciones fueron significativas solamente para las precipitaciones acumuladas mensuales (r=0,57; P<0,05). No se registraron oviposturas en invierno cuando la temperatura media semanal fue inferior a 16,5ºC. CONCLUSIONES: Los resultados muestran correlación entre la oviposición y las precipitaciones, pues los períodos de mayor actividad de Aedes aegypti ocurrieron en el final de la primavera, comienzos del verano y en el inicio del otoño. Estos serían los períodos de mayor riesgo epidemiológico especialmente ante la aparición de personas infectadas.

OBJETIVO: Examinar los temas y cuestiones existenciales del cotidiano de personas con esquizofrenia y de sus cuidadores. MÉTODOS: Estudio cualitativo con grupos focales. Fueron incluidos 146 sujetos con esquizofrenia (55% hombres) y 80 cuidadores (75% mujeres) que provenían de servicios primarios y secundarios de salud de Argentina, Brasil, Chile, España, Inglaterra y Venezuela. Cada grupo tenía de 6 a 10 participantes. Los datos fueron explorados por el proceso de análisis de contenido. RESULTADOS: Fueron identificadas cuatro cuestiones existenciales omnipresentes en los discursos: la necesidad de realización como persona y de encontrar un sentido en la vida; la necesidad de ser respetado y no sufrir discriminación; el conflicto decurrente de la pérdida de autonomía; la importancia de comprender la enfermedad y de reconocerse como enfermo. Las cuestiones existenciales aparecieron fuertemente conectadas a necesidades objetivas, como la falta de ocupación y trabajo, que generalmente resultan en una vida sin propósito y sentido. CONCLUSIONES: Hace falta desarrollar un nuevo tipo de cuidado en salud, en que la consideración por la persona con esquizofrenia y la valoración de su subjetividad sean tan importantes cuanto el tratamiento biológico, así como crear estrategias de promoción de la salud que comprendan mecanismos de inclusión laboral y combato al estigma.

Coccidioidomycosis is an endemic infection with a relatively limited geographic distribution: Mexico, Guatemala, Honduras, Colombia, Venezuela, Bolivia, Paraguai, Argentina and the southwest of the United States. In these countries, the endemic area is restricted to the semiarid desert like regions which are similar to the northeast of Brazil. Case report: The patient is a 32 year-old male, born in the state of Bahia (Northeast of Brazil) and has been living in São Paulo (Southeast) for 6 years. He was admitted at Hospital das Clínicas, at the Department of Pneumology in October 1996, with a 6 month history of progressive and productive cough, fever, malaise, chills, loss of weight, weakness and arthralgia in the small joints. Chest x-rays and computerized tomography disclosed an interstitial reticulonodular infiltrate with a cavity in the right upper lobe. The standard potassium hydroxide preparation of sputum and broncoalveolar lavage demonstrated the characteristic thickened wall spherules in various stages of development. Sabouraud dextrose agar, at 25° C and 30° C showed growth of white and cottony aerial micelium. The microscopic morphology disclosed branched hyphae characterized by thick walled, barrel shaped arthroconidia alternated with empty cells. The sorological studies with positive double immunodiffusion test, and also positive complement fixation test in 1/128 dilution confirmed the diagnosis. The patient has been treated with ketoconazole and presents a favorable clinical and radiological evolution

Background: Little is known about the risk of progression to hazardous alcohol use in people currently drinking at safe limits. We aimed to develop a prediction model (predictAL) for the development of hazardous drinking in safe drinkers. Methods: A prospective cohort study of adult general practice attendees in six European countries and Chile followed up over 6 months. We recruited 10,045 attendees between April 2003 to February 2005. 6193 European and 2462 Chilean attendees recorded AUDIT scores below 8 in men and 5 in women at recruitment and were used in modelling risk. 38 risk factors were measured to construct a risk model for the development of hazardous drinking using stepwise logistic regression. The model was corrected for over fitting and tested in an external population. The main outcome was hazardous drinking defined by an AUDIT score >= 8 in men and >= 5 in women. Results: 69.0% of attendees were recruited, of whom 89.5% participated again after six months. The risk factors in the final predictAL model were sex, age, country, baseline AUDIT score, panic syndrome and lifetime alcohol problem. The predictAL model's average c-index across all six European countries was 0.839 (95% CI 0.805, 0.873). The Hedge's g effect size for the difference in log odds of predicted probability between safe drinkers in Europe who subsequently developed hazardous alcohol use and those who did not was 1.38 (95% CI 1.25, 1.51). External validation of the algorithm in Chilean safe drinkers resulted in a c-index of 0.781 (95% CI 0.717, 0.846) and Hedge's g of 0.68 (95% CI 0.57, 0.78). Conclusions: The predictAL risk model for development of hazardous consumption in safe drinkers compares favourably with risk algorithms for disorders in other medical settings and can be a useful first step in prevention of alcohol misuse.

A vaga de democratizações que teve lugar na América Latina na década de 1980 e 1990 alterou profundamente a paisagem política do Cone Sul, pondo fim a diversos regimes autoritários naquela região. O Chile, dirigido de forma autoritária pelo general Augusto Pinochet desde 1973, foi um dos últimos países onde a transição democrática aconteceu. O processo que teve início com o plebiscito de 1988 culminou com a eleição de Patricio Aylwin, a 11 de Março de 1990 e não foi resultado apenas de esforços internos. Este estudo centra-se na dimensão internacional desse processo. Uma vez que as escassas investigações sobre esta vertente se focam no papel de grandes potências, como os Estados Unidos, e considerando a relação histórica entre Argentina e Chile, por questões também de proximidade fronteiriça, optámos por analisar, nesta dissertação, o papel da Argentina na mudança de regime no Chile. Não se trata apenas de compreender a influência argentina no processo, mas também analisar o impacto da própria democratização no país num processo semelhante no Chile. É, também, por este motivo que este estudo abarca um período temporal que se inicia em 1983 (democratização da Argentina) e termina em 1990 (ano em que o Chile muda de regime, deixando para trás o autoritarismo). Tendo por base o modelo de Levitsky & Way e os conceitos desenvolvidos por Whitehead & Schmitter, esta dissertação baseia-se num quadro analítico que parte do trabalho destes quatro autores. De Levitsky & Way, bebe duas noções-chave: o conceito de leverage (grau de vulnerabilidade de um país à influência externa) e linkage (densidade de laços transfronteiriços). De Whitehead & Schmitter recorre a três conceitos (contágio, consentimento e condicionalidade) que permitem caracterizar, de certo modo, o tipo de influência externa. Além da operacionalização do modelo, este trabalho inclui, também, um capítulo mais descritivo sobre a democratização chilena e os seus antecedentes e uma parte, também ela de cariz mais descritivo, dedicada à dimensão internacional do processo. Este estudo procura, assim, mostrar a importância do contexto internacional na democratização do Chile, alertando para a necessidade de não centrar o estudo das democratizações apenas em factores de ordem interna.

INTRODUCTION: Salmonella sp infections have been reported over recent years in hospitals in Argentina and other countries due to multiresistant strains. The aim of this study was to characterize the extended-spectrum β-lactamases in third-generation cephalosporin-resistant strains of Salmonella enterica serovar Oranienburg. METHODS: We studied 60 strains isolated from children with gastroenteritis and/or extraintestinal complications. The antibiotic susceptibility patterns of the isolates were analyzed and the β-lactamases were characterized using phenotyping and genotyping methods. RESULTS: All the strains were resistant to ampicillin, cefotaxime, cefepime and aztreonam and partially susceptible to ceftazidime, thus corresponding well with the resistance phenotype conferred by CTX-M-type β-lactamases. An isoelectric point enzyme (pI = 7.9) was detected in all of the strains, and this was confirmed by PCR as a member of the CTX-M-2 group. CONCLUSIONS: This is the first report of Salmonella enterica serovar Oranienburg producing β-lactamases of the CTX-M-2 group in a pediatric hospital in Tucumán, Argentina.
