Enrofloxacin (ENR) is an antimicrobial used both in humans and in food producing species. Its control is required in farmed species and their surroundings in order to reduce the prevalence of antibiotic resistant bacteria. Thus, a new biomimetic sensor enrofloxacin is presented. An artificial host was imprinted in specific polymers. These were dispersed in 2-nitrophenyloctyl ether and entrapped in a poly(vinyl chloride) matrix. The potentiometric sensors exhibited a near-Nernstian response. Slopes expressing mV/Δlog([ENR]/M) varied within 48–63. The detection limits ranged from 0.28 to 1.01 µg mL−1. Sensors were independent from the pH of test solutions within 4–7. Good selectivity was observed toward potassium, calcium, barium, magnesium, glycine, ascorbic acid, creatinine, norfloxacin, ciprofloxacin, and tetracycline. In flowing media, the biomimetic sensors presented good reproducibility (RSD of ± 0.7%), fast response, good sensitivity (47 mV/Δlog([ENR]/M), wide linear range (1.0 × 10−5–1.0 × 10−3 M), low detection limit (0.9 µg mL−1), and a stable baseline for a 5 × 10−2 M acetate buffer (pH 4.7) carrier. The sensors were used to analyze fish samples. The method offered the advantages of simplicity, accuracy, and automation feasibility. The sensing membrane may contribute to the development of small devices allowing in vivo measurements of enrofloxacin or parent-drugs.

Fungi have been considered a potential source of natural anticancer drugs. However, studies on these organisms have mainly focused on compounds present in the sporocarp and mycelium. The aim of this study was to assess the anticancer potential of fungal spores using a bioassay-guided fractionation with cancer and normal cell lines. Crude extracts from spores of the basidiomycetous fungus Pisolithus tinctorius were prepared using five solvents/solvent mixtures in order to select the most effective crude extraction procedure. A dichloromethane/methanol (DCM/MeOH) mixture was found to produce the highest extraction yield, and this extract was fractionated into 11 fractions. Crude extracts and fractions were assayed for cytotoxicity in the human osteocarcinoma cell line MG63, the human breast carcinoma cell line T47D, the human colon adenocarcinoma cell line RKO, and the normal human brain capillary endothelial cell line hCMEC/D3. Cytotoxicity was assessed by the 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2H-tetrazolium bromide (MTT) reduction assay. The results showed a reduction in cancer cell viability of approximately 95% with 4 of 11 fractions without a significant reduction in viability of hCMEC/D3 cells. Data demonstrated that spores of P. tinctorius might serve as an interesting source of compounds with potential anticancer properties.

INTRODUCTION: Adults with repaired tetralogy of Fallot (TOF) may be at risk for progressive right ventricular (RV) dilatation and dysfunction, which is commonly associated with arrhythmic events. In frequently volume-overloaded patients with congenital heart disease, tissue Doppler imaging (TDI) is particularly useful for assessing RV function. However, it is not known whether RV TDI can predict outcome in this population. OBJECTIVE: To evaluate whether RV TDI parameters are associated with supraventricular arrhythmic events in adults with repaired TOF. METHODS: We studied 40 consecutive patients with repaired TOF (mean age 35 +/- 11 years, 62% male) referred for routine echocardiographic exam between 2007 and 2008. The following echocardiographic measurements were obtained: left ventricular (LV) ejection fraction, LV end-systolic volume, LV end-diastolic volume, RV fractional area change, RV end-systolic area, RV end-diastolic area, left and right atrial volumes, mitral E and A velocities, RV myocardial performance index (Tei index), tricuspid annular plane systolic excursion (TAPSE), myocardial isovolumic acceleration (IVA), pulmonary regurgitation color flow area, TDI basal lateral, septal and RV lateral peak diastolic and systolic annular velocities (E' 1, A' 1, S' 1, E' s, A' s, S' s, E' rv, A' rv, S' rv), strain, strain rate and tissue tracking of the same segments. QRS duration on resting ECG, total duration of Bruce treadmill exercise stress test and presence of exercise-induced arrhythmias were also analyzed. The patients were subsequently divided into two groups: Group 1--12 patients with previous documented supraventricular arrhythmias (atrial tachycardia, fibrillation or flutter) and Group 2 (control group)--28 patients with no previous arrhythmic events. Univariate and multivariate analysis was used to assess the statistical association between the studied parameters and arrhythmic events. RESULTS: Patients with previous events were older (41 +/- 14 vs. 31 +/- 6 years, p = 0.005), had wider QRS (173 +/- 20 vs. 140 +/- 32 ms, p = 0.01) and lower maximum heart rate on treadmill stress testing (69 +/- 35 vs. 92 +/- 9%, p = 0.03). All patients were in NYHA class I or II. Clinical characteristics including age at corrective surgery, previous palliative surgery and residual defects did not differ significantly between the two groups. Left and right cardiac chamber dimensions and ventricular and valvular function as evaluated by conventional Doppler parameters were also not significantly different. Right ventricular strain and strain rate were similar between the groups. However, right ventricular myocardial TDI systolic (Sa: 5.4+2 vs. 8.5 +/- 3, p = 0.004) and diastolic indices and velocities (Ea, Aa, septal E/Ea, and RV free wall tissue tracking) were significantly reduced in patients with arrhythmias compared to the control group. Multivariate linear regression analysis identified RV early diastolic velocity as the sole variable independently associated with arrhythmic history (RV Ea: 4.5 +/- 1 vs. 6.7 +/- 2 cm/s, p = 0.01). A cut-off for RV Ea of < 6.1 cm/s identified patients in the arrhythmic group with 86% sensitivity and 59% specificity (AUC = 0.8). CONCLUSIONS: Our results suggest that TDI may detect RV dysfunction in patients with apparently normal function as assessed by conventional echocardiographic parameters. Reduction in RV early diastolic velocity appears to be an early abnormality and is associated with occurrence of arrhythmic events. TDI may be useful in risk stratification of patients with repaired tetralogy of Fallot.

PURPOSE: To evaluate whether prostatic arterial embolization (PAE) might be a feasible procedure to treat lower urinary tract symptoms associated with benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: Fifteen patients (age range, 62-82 years; mean age, 74.1 y) with symptomatic BPH after failure of medical treatment were selected for PAE with nonspherical 200-μm polyvinyl alcohol particles. The procedure was performed by a single femoral approach. Technical success was considered when selective prostatic arterial catheterization and embolization was achieved on at least one pelvic side. RESULTS: PAE was technically successful in 14 of the 15 patients (93.3%). There was a mean follow-up of 7.9 months (range, 3-12 months). International Prostate Symptom Score decreased a mean of 6.5 points (P = .005), quality of life improved 1.14 points (P = .065), International Index of Erectile Function increased 1.7 points (P = .063), and peak urinary flow increased 3.85 mL/sec (P = .015). There was a mean prostate-specific antigen reduction of 2.27 ng/mL (P = .072) and a mean prostate volume decrease of 26.5 mL (P = .0001) by ultrasound and 28.9 mL (P = .008) by magnetic resonance imaging. There was one major complication (a 1.5-cm(2) ischemic area of the bladder wall) and four clinical failures (28.6%). CONCLUSIONS: In this small group of patients, PAE was a feasible procedure, with preliminary results and short-term follow-up suggesting good symptom control without sexual dysfunction in suitable candidates, associated with a reduction in prostate volume.

Squamous anal cell carcinoma is a rare malignancy that represents the 1.5% to 2% of all the lower digestive tract cancers. However, an increased incidence of invasive anal carcinoma is observed in HIV-seropositive population since the widespread of highly active antiretroviral therapy. Human papillomavirus is strongly associated with the pathogenesis of anal cancer. Anal intercourse and a high number of sexual partners appear to be risk factors to develop anal cancer in both sexes. Anal pain, bleeding and a palpable lesion in the anal canal are the most common clinical features. Endo-anal ultrasound is the best diagnosis method to evaluate the tumor size, the tumor extension and the infiltration of the sphincter muscle complex. Chemoradiotherapy plus antiretroviral therapy are the recommended treatments for all stages of localized squamous cell carcinoma of the anal canal in HIV-seropositive patients because of its high rate of cure. Here we present an HIV patient who developed a carcinoma of the anal canal after a long time of HIV infection under highly active antiretroviral therapy with a good virological and immunological response.

The use of minimally invasive surgery for the treatment of lung cancer has been growing worldwide. Between May 2008 and November 2012, we performed 24 videothoracoscopic anatomical lung resections in our department. This includes 22 lobectomies and 2 anatomic segmentectomies, which is known to be a more complex surgery, since it demands a finer dissection of sub-lobar structures. We report the clinical cases of two patients who underwent anatomic segmentectomies. The first one was a 63 year old woman, smoker and with a history of breast cancer 20 years earlier. An incidental 9 mm node was found in the lingula. The patient underwent an anatomic lingulectomy and the frozen section was suggestive of a primary lung cancer. Therefore, we proceeded to a full lymphadenectomy. The final pathology evaluation showed a typical carcinoid tumour (pT1aN0). The second patient was a 50 year old woman, a smoker and with a heavy family history of lung cancer. In a screening CT scan a 8 mm ground glass opacity was identified in the left lower lobe (segment VI). After a VATS wedge resection of the node the frozen section evaluation was compatible with adenocarcinoma. We then proceeded to an anatomic segmentectomy with lymphadenectomy. The definitive pathology evaluation confirmed that it was a pT1a N0 bronchioloalveolar adenocarcinoma. The patients now have 5 and 2 months of follow up respectivelly and neither of them has signs of recurrence and the surgical incision showed a good aesthetic result. Anatomic segmentectomy is the indicated surgery especially in patients with low grade tumours, in early stage lung cancers or in patients without pulmonary function for a lobar resection, and it can be done safely using VATS.

SUMMARY Pityriasis versicolor is the most common of the diseases caused by Malasseziayeasts. The aim of this study is to determine the prevalence of pityriasis versicolor and its etiological aspects in the metropolitan area of Porto Alegre, Brazil. A retrospective crosssectional study with data from patients of a reference hospital from 1996 to 2011 was performed. Collected data included: date, age, gender, ethnicity, anatomical region of lesion and the direct mycological examination results. Among the positive results in the direct mycological examination, 5.8% (2,239) were positive for pityriasis versicolor. The angular coefficient (B) was -0.3%/year, showing a decrease over the years. The disease was more prevalent in men (7.1% of men versus 5.1% of women that underwent the direct mycological examination); younger age (median 31 years old); "pardo" and black people (3.7% more than expected in the sample); trunk (73.44% of the affected anatomic sites). Lesions in rare sites (groin, genitals, legs, feet and hands) were also observed in this study. In conclusion, due to the decrease in the prevalence of pityriasis versicolor, long-term epidemiological studies in the metropolitan area of Porto Alegre, Brazil, are needed to continue the monitoring of this disease.

No presente trabalho são comparados dois estudos, ambos com a duração de nove anos, versando os casos de cardite e cardiopatia reumática observados no Serviço de Cardiologia Pediátrica do Hospital de Santa Marta, tendo o primeiro estudo decorrido de Outubro de 1969 a Setembro de 1978, e o segundo de Outubro de 1978 a Setembro de 1987. Dos 38 casos do segundo estudo, 26 tiveram o primeiro surto de febre reumática em Portugal, tendo a cardite surgido como manifestação isolada em 18 casos (69%), associada a poliartrite em 5 casos (20%), e associada a coreia em 3 casos (11%). A insuficiência mitral isolada foi a lesão valvular mais frequente (80%) e 84% das crianças tiveram apresentação clínica inicial na classe funcional I e II da classificação da NYHA. A adesão à profilaxia secundária da febre reumática foi de 78% num grupo de 18 crianças, com um seguimento em média de 2,7 anos, tendo-se modificado neste grupo os sinais de lesão da válvula mitral no sentido da melhoria. Os 12 casos referenciados dos países africanos de língua oficial portuguesa são analisados em separado. Comparativamente ao primeiro estudo, verificou-se um decréscimo de 12,5 para 2,3 casos/ano de cardite reumática, uma redução de mortalidade, assim como uma diferente apresentação clínica, no sentido de um diagnóstico mais precoce, dum predomínio actual de formas menos graves, e de uma maior adesão à profilaxia secundária.

A patologia hipertensiva ocorre em 6 a 8% das gravidezes e é a segunda causa de morte materna, contribuindo com 15% para essa taxa de mortalidade. É também causa significativa de morte fetal e morbi-mortalidade neonatal. O objectivo deste trabalho foi avaliar, através de uma análise retrospectiva, as complicações hipertensivas na população de grávidas que tiveram o parto na Maternidade Dr. Alfredo da Costa (MAC) em Novembro de 2005. Durante este período 41 mulheres (10,1%) tiveram complicações hipertensivas. Treze grávidas (31,7%) tinham hipertensão crónica, dezanove (46,3%) HIG, 5 (12,2%) PE moderada e 7 (17%) PE grave, com 3 casos de síndrome HELLP. Registou-se um caso de síndrome HELLP sem hipertensão arterial. Registaram-se 9 casos de parto prematuro (22,5%) e uma morte fetal. A idade gestacional média foi 37 semanas. Assinalaram-se 9,7% de RCIU e nenhum caso de asfixia neonatal.

Introdução: A fasceíte plantar (FP) constitui a causa mais frequente de dor no retropé. Está recomendada uma prova terapêutica inicial conservadora. Objetivos: Caraterizar fatores demográficos e sintomáticos, terapêutica prévia e resultados de monoterapia com laser. Material e Métodos: Estudo observacional retrospetivo (12 meses). O tratamento aplicado foi laser infravermelho (AsGa 904nm; densidade de energia 20J/cm2; potência de pico 200mW). Fez-se análise estatística descritiva e comparativa (nível de significância de 0,05). Resultados: Seguiram-se 32 doentes com FP, com idade média de 54,28 anos (24–86; DP 13,95), 66% mulheres, Índice de Massa Corporal (IMC) médio de 30,18Kg/m2 (17,91-41,13; DP 5,19), 75% unilaterais, tempo médio de evolução de 6,28 meses (0,2-24; DP 6,82), 21 tinham feito previamente farmacoterapia e 12 outros tratamentos de MFR. O nível de dor inicial (END) médio foi 7,31 (5-10; DP 1,38). Dos 26 doentes tratados (6 perdas), 24 (92,3%) reportaram melhoria. O número médio de sessões foi 28,5 (11– 60; DP 14,01). O nível médio de dor final foi 0,75 (0-5; DP 2,37), com melhoria média de 89,7% (28,6%-100%; DP 0,16) do nível de dor. A intensidade de dor inicial foi maior em doentes com IMC superior (p=0,002). Não se encontraram outras relações estatisticamente significativas. Conclusões: A FP caraterizou-se por um nível de dor significativo (moderado a grave). A obesidade pareceu ser um fator de risco. A taxa de melhoria com o tratamento com laser foi muito satisfatória. A terapia com laser de baixa intensidade IV constitui uma boa opção terapêutica, a justificar avaliação suplementar por ensaios prospetivos controlados e randomizados.

Apresentam-se os resultados do estudo prospectivo dos primeiros 145 cateterismos da veia subclávia efectuados na Unidade de Cuidados Intensivos Pediátricos (U.C.I.P.) do Hospital de Dona Estefânia. Foram submetidos a cateterismo 131 doentes com idades compreendidas entre 1 mês e 15 anos. A técnica utilizada foi a de Seldinger (abordagem infraclavicular). A taxa de sucesso foi de 90.0% e o número médio de tentativas de 2.7 ± 2.4. A duração média da cateterização foi de 7.5 ± 7.2 dias. Houve 22 complicações «major». A incidência de sépsis de cateter foi de 6.9%. A remoção dos cateteres foi electiva em 91 (62.8%) casos. Não houve mortalidade atribuível ao cateterismo. Os resultados obtidos estão em conformidade com os divulgados na literatura; a aprendizagem e execução da técnica são relativamente fáceis e a sua aplicação tem alta taxa de sucesso e baixa incidência de complicações.
Avaliação terapêutica do hycanthone em pacientes com período de infecção esquistossomótica conhecido

Neste trabalho são estudados 28 pacientes com período de infecção esquistossomótica conhecido, e que foram tratados com hycanthone, (2,5 mg/kg, dose única, i.m.) em diferentes épocas após o contágio. Através do exame coprológico quantitativo (método de Kato), demonstra-se que a cura parasitológica é menor nos pacientes com infecção recente. De fato, de 15 pacientes tratados, 2 a 8 meses após o contágio, apenas 6 (40,0%) curaram-se. Enquanto que nos pacientes com infecção em tôrno de 1 ano, 84,7% deixaram de eliminar ovos pelas fezes. Mesmo assim, nos pacientes com infecção recente, o tratamento produziu uma acentuada redução no número de ovos de S. mansoni.
