La resistencia antimicrobiana es un problema frecuente en la práctica médica y de considerable impacto negativo en la mortalidad del paciente. Para los pacientes con microorganismos multiresistentes involucrados en infecciones generalmente severas, las opciones terapéuticas efectivas para su manejo se hacen escasas y en muchos de ellos cuando son resistentes a carbapenemicos solamente el empleo de combinaciones antibióticos se presentan como única alternativa. Se realizo un análisis retrospectivo de una serie de casos de infecciones por Acinetobacter Baumannii resistentes a carbapenemicos durante un periodo de 5 años en una Institución de 4to nivel de Atención,observando el éxito terapéutico obtenido con los diferentes tratamientos antibióticos instaurados y las características de los pacientes comprometidos en estos procesos infecciosos. El éxito terapéutico definido como Cura, se observo en el 53 % de los casos. Se considero tratamiento con resultado Indeterminado en el 20% de los casos y Falla en el 26% de los casos. Presento mejores resultados la combinación de Tigeciclina con Sulbactam y Amikacina que en monoterapia o frente a otras combinaciones que no incluían esta. El 83% de los pacientes tienen el antecedente de haber estado en UCI. El órgano mas frecuentemente involucrado es el Pulmonar (24%)y en el 30 % de los pacientes son bacteremicos, pero en quienes no se pudo determinar el origen de esta. La Tigeciclina en combinación con Sulbactam y Amikacina, parecen ser la mejor terapia antibiótica en el tratamiento de A. baumannii resistente a carbapenemicos.

El aumento en la resistencia bacteriana es un problema global que afecta también a nuestro país y que puede llevar a fracasos terapéuticos, peores desenlaces clínicos, uso de terapia combinada, mayor riesgo de interacciones medicamentosas y de reacciones adversas asi como a aumento de los costos del tratamiento. Tigeciclina, un nuevo antibiótico de la clase de las glicilciclinas ha mostrado una promisoria actividad antibacteriana in vitro contra bacterias comunes incluyendo multiresistentes que cada vez son más prevalentes en nuestro país especialmente a nivel hospitalario. Este estudio fue desarrollado para determinar la actividad de tigeciclina en nuestro medio y conocer su potencial utilidad.

Introducción: La neutropenia febril es una condición frecuente en los pacien¬tes pediátricos con cáncer y una de las mayores causas de morbimortalidad. Es necesario conocer la frecuencia su comportamiento. Objetivo: Caracterizar los eventos de neutropenia febril en niños con cáncer en la clínica Infantil Colsubsidio, Bogotá desde Enero de 2014 hasta Diciembre de 2014. Materiales y Métodos: Estudio transversal en niños de 1 mes a 18 años con cáncer y neutropenia febril atendidos de Enero 2014 a Diciembre 2014. Resultados: Se presentaron 74 eventos de un total de 41 pacientes con un máximo de 3 eventos por paciente, 20 mujeres y 21 hombres con una edad promedio de 8 años. 61% de los pacientes tenían diagnostico de Leucemia Linfoide Aguda .Se clasificaron como Fiebre sin causa clara con 43 eventos (58.11%), colitis neutropenica con 10 eventos (13.51%) y neumonía que registro 7 eventos (9%). El 70% de los pacientes presentaron al ingreso menos de 100 neutrofilos y una PCR con un promedio de 97.4. En un 6,76% de los pacientes se diagnostico bacteriemia. Los Hemocultivos fueron positivos en 5 pacientes (7 %), siendo el S epidermidis y el S mitis los gérmenes más frecuentes La mortalidad fue nula. Conclusiones: Esta estudio permitió conocer el comportamiento de la neutropenia febril en nuestra institución conociendo los organismos principalmente aislados, el perfil de resistencia y la respuesta al manejo antibiótico. Es importante determinar estudios prospectivos y analíticos con el fin de establecer factores de riesgo asociados a complicaciones y mortalidad.

Introducción y objetivos: La endocarditis infecciosa (EI) es una enfermedad grave producida por diversos gérmenes que afectan las válvulas cardiacas y el tejido endomiocárdico. El objetivo fue describir las características epidemiológicas, clínicas, ecocardiográficas y microbiológicas de la endocarditis infecciosa por Staphylococcus aureus (S. aureus) meticilino sensible y resistente de la Fundación Cardioinfantil – Instituto de Cardiología (FCI-IC) en el periodo de tiempo 2010- 2015. Métodos: Cohorte retrospectiva de casos de EI por S. aureus en la FCIIC para el período 2010-2015. Se realizó descripción de las variables generales de la población a estudio utilizando medidas de tendencia central y dispersión. Análisis de desenlaces teniendo cuenta la concentración inhibitoria mínima de vancomicina. Resultados: En el estudio se presentaron 27 casos de EI, con una mayor proporción de pacientes de sexo masculino, con hipertensión, diabetes y hemodiálisis. La fiebre fue la manifestación más frecuente seguida de fenómenos vasculares. La válvula más comprometida fue la mitral, principalmente nativa. Discusión: La presentación clínica de los pacientes con EI por S. aureus es aguda por lo que la fiebre es la principal manifestación clínica presentada, lo anterior favorece un rápido diagnóstico clínico. De las cepas de S. aureus causante de EI no se encontró gérmenes con sensibilidad intermedia ni resistente a la vancomicina según criterios establecidos por CLSI. Se encontró mayor proporción de pacientes con un valor de CMI para vancomicina mayor a 0,5μg/ml lo cual es importante dado que podemos estar enfrentándonos a cepas hetero VISA (hVISA).

As infecções do trato urinário (ITU’s) são doenças infecciosas frequentes na prática clínica veterinária, sendo fundamental uma correta antibioterapia, principalmente pelo crescente desenvolvimento de resistências bacterianas aos antibióticos. Realizou-se um estudo retrospectivo, englobando 86 animais admitidos no Hospital Veterinário do Restelo, submetidos a urocultura, com o objetivo de caracterizar as ITU’s microbianas neste hospital, avaliando a sua epidemiologia e susceptibilidade antibiótica das bactérias isoladas. Da totalidade das uroculturas realizadas (n=86), 28 foram positivas, 18 em canídeos e 10 em felídeos, sem predisposição racial, mais em fêmeas nos canídeos e em machos nos felídeos. A idade média dos animais com ITU foi 8 anos nos canídeos e 10 anos nos felídeos. Todas as ITU´s foram monobacterianas, sendo o microorganismo mais frequentemente isolado a Escherichia coli. Verificou-se multirresistência em 10 das 28 bactérias isoladas. A gentamicina foi o antibiótico com melhor perfil de sensibilidade global e o que apresentou mais resistências foi a tetraciclina. O antibiótico mais prescrito de forma empírica foi a enrofloxacina. Este estudo, especialmente se realizado de forma periódica, poderá ser um contributo para a elaboração de guias institucionais de antibioterapia adequada e minimização do aparecimento de resistências bacterianas.

Aim: To assess the effect of the growth promoter avilamycin on emergence and persistence of resistance in enteric bacteria in the pig. Methods and Results: Pigs ( treated with avilamycin for 3 months and controls) were challenged with multiresistant Salmonella Typhimurium DT104 and faecal counts were performed for enterococci, Escherichia coli, S. Typhimurium and Campylobacter ( before, during and 5 weeks post-treatment). Representative isolates were tested for antibiotic resistance and for the presence of resistance genes. Avilamycin-resistant Enterococci faecalis (speciated by PCR) were isolated from the treated pigs and continued to be detected for the first week after treatment had ceased. The avilamycin- resistance gene was characterized by PCR as the emtA gene and speciation by PCR. MIC profiling confirmed that more than one strain of Ent. faecalis carried this gene. There was no evidence of increased antimicrobial resistance in the E. coli, Salmonella and Campylobacter populations, although there was a higher incidence of tetB positive E. coli in the treated pigs than the controls. Conclusion: Although avilamycin selects for resistance in the native enterococci population of the pig, no resistant isolates were detected beyond 1 week post-treatment. This suggests that resistant isolates were unable to persist once selective pressure was removed and were out-competed by the sensitive microflora. Significance and Impact of the Study: Our data suggest the risk of resistant isolates becoming carcass contaminants and infecting humans could be minimized by introducing a withdrawal period after using avilamycin and prior to slaughter.

In this paper we describe a new genus of Bromehaceae, Lapanthus, restricted to the southern portion of the Espinhaco Range, Minas Germs State, in southeastern Brazil Two new combinations to accommodate species previously described in the genera Orthophytum and cryptanthus and one new synonym are proposed Lapanthus has morphological affinities with both Cryptanthus and Orthophytum but nevertheless differs by the combination of margins of the petals ciliate, presence of lanceolate petal appendages and free stamens, and also by molecular data Cryptanthus and Orthophytum have petals entire along the margins, and the filaments of the most internal whorl are adnate to the petals Lapanthus stands out by having a pair of lanceolate petal appendages, which are almost completely adnate to the petals In Orthophytum, however, appendages are cupuhform or sacciform and they are totally absent in the genus Cryptanthus Lapanthus and Orthophytum present meiotic and mitotic chromosome numbers equal to n=25 and 2n=50, 100 and 150 respectively, while Cryptanthus presents meiotic and mitotic chromosome numbers n=17 and 2n=34, 36, 54 respectively, and this difference is considered to be an autapomorphic feature of Cryptanthus Descriptions of the genus and species, identification keys, illustrations, photographs of living specimens, and taxonomic comments are provided

Morphogenesis and cytodifferentiation are distinct processes in tooth development. Cell proliferation predominates in morphogenesis; differentiation involves changes in form and gene expression. The cytoskeleton is essential for both processes, being regulated by Rho GTPases. The aim of this study was to verify the expression, distribution, and role of Rho GTPases in ameloblasts and odontoblasts during tooth development in correlation with actin and tubulin arrangements and amelogenin and dentin sialophosphoprotein (DSPP) expression. RhoA, Rac1, and Cdc42 were strongly expressed during morphogenesis; during cytodifferentiation, RhoA was present in ameloblasts and odontoblasts, Rac1 and its effector Pak3 were observed in ameloblasts; and Cdc42 was present in all cells of the tooth germ and mesenchyme. The expression of RhoA mRNA and its effectors RockI and RockII, Rac1 and Pak3, as analyzed by real-time polymerase chain reaction, increased after ameloblast and odontoblast differentiation, according to the mRNA expression of amelogenin and DSPP. The inhibition of all Rho GTPases by Clostridium difficile toxin A completely abolished amelogenin and DSPP expression in tooth germs cultured in anterior eye chamber, whereas the specific inhibition of the Rocks showed only a partial effect. Thus, both GTPases are important during tooth morphogenesis. During cytodifferentiation, Rho proteins are essential for the complete differentiation of ameloblasts and odontoblasts by regulating the expression of amelogenin and DSPP. RhoA and its effector RockI contribute to this role. A specific function for Rac1 in ameloblasts remains to be elucidated; its punctate distribution indicates its possible role in exocytosis/endocytosis.

In this paper we investigate the classification of mappings up to K-equivalence. We give several results of this type. We study semialgebraic deformations up to semialgebraic C(0) K-equivalence and bi-Lipschitz K-equivalence. We give an algebraic criterion for bi-Lipschitz K-triviality in terms of semi-integral closure (Theorem 3.5). We also give a new proof of a result of Nishimura: we show that two germs of smooth mappings f, g : R(n) -> R(n), finitely determined with respect to K-equivalence are C(0)-K-equivalent if and only if they have the same degree in absolute value.

Let f : U subset of R(2) -> R(3) be a representative of a finitely determined map germ f : (R(2), 0) -> (R(3), 0). Consider the curve obtained as the intersection of the image of the mapping f with a sufficiently small sphere s(epsilon)(2) centered at the origin in R(3), call this curve the associated doodle of the map germ f. For a large class of map germs the associated doodle has many transversal self-intersections. Protein and caloric malnutrition has been considered one of the most concerned endemic diseases in Brazil and in the world. It has been known that depletion or reduction of proteins as far as meals are concerned can steer irreversible damages upon several organic systems. This study had as aim evaluate the effects the low-protein diet had over the formation and composition of the teeth components. 18 females and 6 males were used for the experiment. 12 from the 18 females had undertaken the low-protein diet (DH) for 03 weeks and the other 6, which remained, and those males had undertaken a controlled diet (DC) for the same period. All animals had the diets during their mating, pregnancy and lactation cycle. As soon as the offsprings had been born, 10 young males and females of each group faced a disease hood analysis to check the teeth germs of their lower fore teeth. The rest of the group had their lactation cycle normally 60 days. Then they were put to death and had their lower fore teeth removed both to be analyzed through a scanning electronic microscopy (SEM) of the structure alterations and to have their calcium checked by an atomic absorption of the phosphorus vanadate-molibdate method and by other minerals EDX method. The animals livers were removed to have their hepatic proteins analyzed as well. The histopatologic study showed that at first day of birth, all animals had their lower fore teeth come out. It was verified that 90% of the animals teeth were in an apposition and calcification period and it was possible to observe the dentin formation from 60% of the 90% already mentioned. Through the SEM method it could be realized that 90% of the animals of the DH group had their lower fore teeth easily broken and no definite shape. In this same group itself, it was also observed long micro fissures 369,66 nm ± 3,45 while the DC group had fissures of 174 nm ± 5,72. Now regarding the calcium and phosphorus concentration, it could be noticed that there was a great reduction of these components and other minerals in the DH group. Almost all minerals, except for the Cl and K, presented higher levels in the DC group enamel.The reduction of the protein input greatly influenced the offsprings´ weight and height. However the hepatic proteins had no important difference between the groups what can make one believe that those animals suffered from protein malnutrition of marasmic kind

Foram coletadas 143 amostras de mãos de humanos e camas hospitalares, através de swabs no caldo BHI, em um hospital escola da cidade de Ribeirão Preto/SP. As amostras coletadas foram incubadas a 37ºC por 24 horas e após este período as culturas foram semeadas em placas de Petri contendo agar Staphylococcus Médium 110. As colônias típicas do gênero Staphylococcus foram colhidas e estocados a 4ºC até o momento de elaboração das provas de catalase, manitol, hemólise, DNAse e coagulase. As cepas isoladas foram analisadas através da técnica de RAPD-PCR para verificar o grau de similaridade. A sensibilidade das cepas isoladas foi testada frente a 10 diferentes antibióticos. Das 92 cepas de Staphylococcus sp isoladas, 67 (72,8%) foram identificados como Staphylococcus coagulase-negativas e 25 (27,2%) como Staphylococcus coagulase-positivas. A análise de similaridade mostrou uma grande heterogeneidade entre as cepas, entretanto foram isoladas algumas cepas com 100% de similaridade. Resistência a oxacilina foi encontrada em 39 (42%) cepas. Duas cepas de estafilococos coagulase-negativos mostraram-se resistentes a vancomicina. Onze cepas (12%) de estafilococos foram consideradas multirresistentes. Medidas de desinfecção das mãos de pessoal e dos leitos hospitalares e a racionalização do uso indiscriminado de antibióticos podem contribuir para a queda da transmissão de patógenos e diminuição da pressão de seleção, e conseqüentemente diminuindo a freqüência e letalidade das infecções nosocomiais.

We investigated the immunohistochemistry expression of claudins -1 and -7 in ameloblastoma and in human dental germs on the pattern of distribution (focal, regional or diffuse), the cells that expressed (if central or peripheral) and the location of that expression in the cell components recital membrane, cytoplasm and nucleus. Among the 29 cases of ameloblastoma, 24 were type solid and 6 unicystic. In 7 mandibular specimens of human fetuses found dental germs from the stage of bud to the crown. We note that the pattern of expression in the dental germs was variable for claudinas studied according to the cell type and stage of differentiation and was invariate only in the cells of stellate reticulum. In epithelium internal of enamel organ, claudin-1 has been decreasing with the progression of differentiation as to claudina-7 that was found in the cells of the peripheral papilla. For ameloblastoma the expression was more significant than that observed in dental germs. Fisher s exact test no found association between the expression of claudinas cells in central and peripheral and the type of ameloblastoma (solid or unicystic). Thus, in general the claudin-1 was positive in the central cell of 93,1% of the cases and in peripheral cells of 51,7%. The claudin-7 was expressed in the cells of all cases central and peripheral cells from 89,7%. For both claudins the distribution was predominantly diffuse cells both in central and peripheral cells. Given our findings it is suggested that the expression of claudins may be indicative of the involvement of these molecules in morphogenetics events culminating with the dental development and that possibly influence the development of neoplastic ameloblastoma
