Muscle fatigue is described as a cause of injuries among the many related to the running practice. Therefore, the purpose of this study was to analyze the behavior of the amplitude (RMS) and median frequency (MF) of EMG signal of the iliocostalis (CI), rectus femoris (RF), vastus lateralis (VL), vastus medialis (VM) biceps femoris (long head) (BFCL), tibialis anterior (TA) and gastrocnemius (lateral) (LNG) from the right lower limb, and the behavior of the parameters of amplitude (AP) and frequency (PF) spent in different percentages of the maximum speed during incremental protocol of treadmill running. 10 volunteers participated in this study, athletes, male, aged between 18 and 30 years with no history of injury in lower limbs and similar anthropometry. The protocol consisted of a treadmill test with initial velocity of 10 km.h-¹ and increments of 1 km.h-¹ each three minutes until volitional exhaustion, without rest interval. Synchronized collections were made of electromyographic and kinematic data. The signals were obtained through an acquisition module of biological signals (Telemyo 900 - Noraxon - USA) and software (Myoresearch - Noraxon - USA) calibrated with a sampling frequency of 1000 Hz, gain 2000 times. The raw data were filtered with a 60Hz notch filter, high pass and low pass 20Hz to 500Hz. To capture the image was used a digital video recorder (model NV-GS320, PANASONIC brand), and for image scanning and kinematic data collection was used the software Peak Motus 9.0 (ViconPeak). To obtain the values of RMS and FM analyzed the last ten passes of each speed through a specific routine (Matlab). To obtain these variables AP (m) and FP (stride I min) were analyzed for the last ten past each speed, using specific software (Peak Motus 9.0). O estudo neurofisiológico, na modalidade da eletroneuromiografia (ENMG), determina e quantifica a integridade de componentes da unidade motora. Os principais dados fornecidos pelo exame eletroneuromiográfico são os estudos de condução nervosa motora, sensitiva e eletromiografia. No entanto, vários fatores podem interferir sobre a resposta nervosa à eletroestimulação, tais como: idade, sexo, temperatura, umidade e outros. O objetivo deste trabalho foi verificar o efeito da exposição cirúrgica dos nervos ciático, fibular comum, tibial e do músculo tibial cranial no teste neurofisiológico em ratos. Foram utilizados 20 ratos, Wistar, machos com aproximadamente 80 dias, divididos em dois grupos. No grupo normal o exame foi realizado sem a exposição cirúrgica do nervo fibular. No grupo cirúrgico houve a exposição do nervo fibular comum. Com o modelo experimental utilizado, concluiu-se que o teste neurofisiológico realizado em animais com nervos e músculos expostos cirurgicamente é viável, uma vez que a alteração da temperatura do animal não interferiu significativamente nos valores dos parâmetros eletrofisiológicos observados. Além disso, a exposição de nervos e músculos permite estimular um ponto exato no nervo alvo.

INTRODUÇÃO: A entorse de tornozelo é uma das lesões mais comuns em atletas. Uma forma de avaliar a frouxidão ligamentar pode ser através da medida da amplitude passiva dos movimentos de inversão e eversão do pé para estimar a resistência passiva das estruturas capsuloligamentares do tornozelo, o qual pode ser chamado de torque de resistência passiva. Existem poucos estudos que utilizam a avaliação do torque passivo do tornozelo para avaliar a resistência da cápsula e dos ligamentos. OBJETIVO: O objetivo deste estudo foi comparar o torque passivo dos movimentos de inversão e eversão do pé em atletas com e sem história de entorse de tornozelo. MÉTODO: Participaram do estudo 32 atletas de basquetebol e voleibol feminino (16,06 ± 0,8 anos, 67,63 ± 8,17kg, 177,8 ± 6,47cm). Seus tornozelos foram divididos em dois grupos: grupo controle (29), composto por tornozelos sem sintomas, e grupo entorse de tornozelo, composto por tornozelos que sofreram lesão (29). O torque dos movimentos passivos do tornozelo foi registrado por um dinamômetro isocinético, e a atividade dos músculos fibular longo e tibial anterior foi medida por um eletromiógrafo. As atletas realizaram duas repetições do movimento de inversão e eversão, nas velocidades de 5, 10 e 20°/s e, em seguida, o mesmo protocolo foi repetido apenas para o movimento de inversão máxima do pé. RESULTADOS: O torque de resistência passiva durante os movimentos de inversão e eversão do pé foi menor no grupo com entorse do tornozelo. Este grupo também mostrou menor torque durante o movimento de inversão máxima do pé. Não foram observadas diferenças entre o movimento de inversão e eversão. CONCLUSÕES: A entorse de tornozelo leva a um menor torque de resistência passiva, indicando redução da resistência dos ligamentos colaterais do tornozelo e uma frouxidão articular mecânica.

O objetivo do presente estudo foi comparar o sinal eletromiográfi co, a frequência e a amplitude de passada entre diferentes intensidades de corrida: 60%, 80% e 100% da velocidade máxima em dois protocolos incrementais. Participaram deste estudo 11 corredores do sexo masculino. Os protocolos de corrida foram realizados com velocidades iniciais de 10 km.hr-1, com incrementos de 1 km.hr-1 a cada três minutos até a exaustão, que diferiram em relação ao intervalo entre cada estágio incremental: 30 e 120 segundos. Foram analisados valores RMS dos músculos iliocostal lombar, reto femoral, vasto lateral, vasto medial, bíceps femoral, tibial anterior, e gastrocnêmio, e a amplitude e frequência de passada. Os valores RMS mostraram aumento entre as intensidades para quase todos os músculos, e não foram influenciados pelo tipo de protocolo utilizado para maioria dos músculos. A frequência e amplitude de passada apresentaram contribuições percentuais diferenciadas para o aumento da velocidade de corrida.

INTRODUCTION Persistent traumatic peroneal nerve palsy, following nerve surgery failure, is usually treated by tendon transfer or more recently by tibial nerve transfer. However, when there is destruction of the tibial anterior muscle, an isolated nerve transfer is not possible. In this article, we present the key steps and surgical tips for the Ninkovic procedure including transposition of the neurotized lateral gastrocnemius muscle with the aim of restoring active voluntary dorsiflexion. SURGICAL TECHNIQUE The transposition of the lateral head of the gastrocnemius muscle to the tendons of the anterior tibial muscle group, with simultaneous transposition of the intact proximal end of the deep peroneal nerve to the tibial nerve of the gastrocnemius muscle by microsurgical neurorrhaphy is performed in one stage. It includes 10 key steps which are described in this article. Since 1994, three clinical series have highlighted the advantages of this technique. Functional and subjective results are discussed. We review the indications and limitations of the technique. CONCLUSION Early clinical results after neurotized lateral gastrocnemius muscle transfer appear excellent; however, they still need to be compared with conventional tendon transfer procedures. Clinical studies are likely to be conducted in this area largely due to the frequency of persistant peroneal nerve palsy and the limitations of functional options in cases of longstanding peripheral nerve palsy, anterior tibial muscle atrophy or destruction.

Low level laser therapy (LLLT) is known for its positive results but studies on the biological and biomodulator characteristics of the effects produced in the skeletal muscle are Still lacking. In this Study the effects of two laser dosages, 5 or 10 J/cm(2), on the lesioned tibial muscle were compared. Gerbils previously lesioned by 100 g load impact were divided into three groups: GI (n = 5) controls, lesion non-irradiated; GII (n = 5), lesion irradiated with 5 J/Cm(2) and GIII (n = 5), lesion irradiated with 10 J/cm(2), and treated for 7 consecutive days with a laser He-Ne (lambda = 633 rim). After intracardiac perfusion, the muscles were dissected and reduced to small fragments, post-fixed in 1% osmium tetroxide, dehydrated in increasing alcohol concentrations, treated with propylene oxide and embedded in Spurr resin at 60 degrees C. Ultrafine Cuts examined on a transmission electron microscope (Jeol 1010) revealed in the control GI group a large number of altered Muscle fibers with degenerating mitochondria, intercellular substance containing degenerating cell fragments and budding blood capillaries with Underdeveloped endothelial cells. However, groups GII and GIII showed muscle fibers with few altered myofibrils, regularly contoured mitochondria, ample intermembrane spaces and dilated mitochondrial crests. The clean intercellular Substance showed numerous collagen fibers and capillaries with multiple abluminal processes, intraluminal protrusions and several pinocytic vesicles in endothelial cells. it was concluded that laser dosages of 5 or 10 J/cm(2) delivered by laser He-Ne (lambda = 633 rim) during 7 consecutive days increase mitochondrial activity in muscular fibers, activate fibroblasts and macrophages and stimulate angiogenesis, thus suggesting effectivity of laser therapy tinder these experimental conditions. The relative advantages of cruciate retaining or cruciate resecting total knee replacement are still controversial. If the posterior cruciate ligament (PCL) is preserved, it should be properly balanced. In a previous study, it was demonstrated that increasing the flexion gap leads to an anterior translation of the tibia relative to the femur. Based on these results, we hypothesized that cutting the PCL increases the flexion gap and lessens anterior tibial translation.

BACKGROUND: The pathology of restless legs syndrome (RLS) is still not understood. To investigate the pathomechanism of the disorder further we recorded a surface electromyogram (EMG) of the anterior tibial muscle during functional magnetic resonance imaging (fMRI) in patients with idiopathic RLS. METHODS: Seven subjects with moderate to severe RLS were investigated in the present pilot study. Patients were lying supine in the scanner for over 50min and were instructed not to move voluntarily. Sensory leg discomfort (SLD) was evaluated on a 10-point Likert scale. For brain image analysis, an algorithm for the calculation of tonic EMG values was developed. RESULTS: We found a negative correlation of tonic EMG and SLD (p <0.01). This finding provides evidence for the clinical experience that RLS-related subjective leg discomfort increases during muscle relaxation at rest. In the fMRI analysis, the tonic EMG was associated with activation in motor and somatosensory pathways and also in some regions that are not primarily related to motor or somatosensory functions. CONCLUSIONS: By using a newly developed algorithm for the investigation of muscle tone-related changes in cerebral activity, we identified structures that are potentially involved in RLS pathology. Our method, with some modification, may also be suitable for the investigation of phasic muscle activity that occurs during periodic leg movements.

In two young athletic men we made the diagnosis of chronic tibialis anterior syndrome. Pain during strenuous exercise was localized in the anterolateral aspect of both calves. After stopping exercise the complaints disappeared within 15-20 minutes and not in 2-3 minutes as it would be typical for patients with atherosclerotic peripheral arterial occlusive disease. Diagnosis is based on patient history, normal clinical examination (systolic ankle pressure determined by Doppler-Sonography, electronic segmental oscillography) and increased intramuscular pressure at rest (> 10 mmHg) and after exercise (42 and 35 mmHg). Bilateral fasciotomy was performed in both patients. They are free of pain after 3 respectively 6 months postoperatively.

A unique case of a collegiate athlete who suffered an anterior cruciate ligament injury leading to the formation of a synovial cyst is described. The cyst, localized over the tibial tunnel, resulted from irritation caused by the removal of interference screws.

Anterior cruciate ligament (ACL) tear is a common sports injury of the knee. Arthroscopic reconstruction using autogenous graft material is widely used for patients with ACL instability. The grafts most commonly used are the patellar and the hamstring tendons, by various fixation techniques. Although clinical evaluation and conventional radiography are routinely used in follow-up after ACL surgery, magnetic resonance imaging (MRI) plays an important role in the diagnosis of complications after ACL surgery. The aim of this thesis was to study the clinical outcome of patellar and hamstring tendon ACL reconstruction techniques. In addition, the postoperative appearance of the ACL graft was evaluated using several MRI sequences. Of the 175 patients who underwent an arthroscopically assisted ACL reconstruction, 99 patients were randomized into patellar tendon (n=51) or hamstring tendon (n=48) groups. In addition, 62 patients with hamstring graft ACL reconstruction were randomized into either cross-pin (n=31) or interference screw (n=31) fixation groups. Follow-up evaluation determined knee laxity, isokinetic muscle performance and several knee scores. Lateral and anteroposterior view radiographs were obtained. Several MRI sequences were obtained with a 1.5-T imager. The appearance and enhancement pattern of the graft and periligamentous tissue, and the location of bone tunnels were evaluated. After MRI, arthroscopy was performed on 14 symptomatic knees. The results revealed no significant differences in the 2-year outcome between the groups. In the hamstring tendon group, the average femoral and tibial bone tunnel diameter increased during 2 years follow-up by 33% and 23%, respectively. In the asymptomatic knees, the graft showed homogeneous and low signal intensity with periligamentous streaks of intermediate signal intensity on T2-weighted MR images. In the symptomatic knees, arthroscopy revealed 12 abnormal grafts and two meniscal tears, each with an intact graft. Among 3 lax grafts visible on arthroscopy, MRI showed an intact graft and improper bone tunnel placement. For diagnosing graft failure, all MRI findings combined gave a specificity of 90% and a sensitivity of 81%. In conclusion, all techniques appeared to improve patients' performance, and were therefore considered as good choices for ACL reconstruction. In follow-up, MRI permits direct evaluation of the ACL graft, the bone tunnels, and additional disorders of the knee. Bone tunnel enlargement and periligamentous tissue showing contrast enhancement were non-specific MRI findings that did not signify ACL deficiency. With an intact graft and optimal femoral bone tunnel placement, graft deficiency is unlikely, and the MRI examination should be carefully scrutinized for possible other causes for the patients symptoms.

En el presente texto se documenta la elaboración de una propuesta de intervención fisioterapéutica realizada en una empresa productora de alimentos ubicada en la ciudad de Bogotá. Dicha propuesta está dirigida a analizar los diferentes aspectos de la organización para así determinar la problemática a nivel osteomuscular, y el riesgo que corren los trabajadores de padecer alteraciones a causa de las condiciones de la organización del trabajo, la producción, el individuo y puesto de trabajo. A partir de lo anterior se relacionan los requerimientos físicos, tales como postura, fuerza y movimiento, con otras condiciones para comprender la incidencia o aparición de desórdenes músculo-esqueléticos.
