Lucia Rippel, expelled from her place of birth, walked 220 miles across the fractured landscape of Europe, with her two children and all her belongings dragged in a cart. In 2015, Ildikó Rippel and Rosie Garton retraced her footsteps, crossing borders, climbing fences, bleeding, crying, and blistering, and carrying their flat-pack children. Enquadramento: Apesar de o leite materno ser globalmente aceite como o alimento mais completo e efetivo para assegurar a saúde do bebé, além dos claros benefícios para a mãe, continua-se a verificar que a taxa de amamentação está ainda longe da pretendida. Objetivos: Identificar a evidência científica dos determinantes da interrupção do aleitamento materno aos 6 meses de vida do bebé. Métodos: Foi realizada uma revisão sistemática da literatura sobre as dificuldades que levam à interrupção do aleitamento materno aos 6 meses de vida do bebé. Efetuou-se uma pesquisa na PUBMED, The Cochrane Library, Scielo e Google Académic de estudos publicados entre janeiro de 2010 a outubro de 2015, partindo dos critérios de inclusão previamente definidos, os estudos selecionados foram posteriormente avaliados. Dois revisores avaliaram a qualidade dos estudos a incluir, utilizando a grelha para a avaliação crítica de um estudo. Após a avaliação crítica da qualidade, foram incluídos no corpus de estudo 4 artigos com scores entre 87.5% e 95.0%. Resultados: Como metasíntese e seleção da análise dos artigos, inferiu-se que os fatores que levam à interrupção do aleitamento materno aos 6 meses de vida do bebé são: as preocupações com a lactação, perda de peso do bebé, doença da mãe ou a necessidade de tomar medicação, bloqueio dos ductos mamários, problemas relacionados com a gestão psicossocial, conflitos no seu estilo de vida, o posicionamento e pega inadequados, queixa de leite insuficiente ou fraco, dor à amamentação, fissuras, ingurgitamento mamário, a ansiedade materna e o choro da criança. Conclusões: Perante a evidência científica, as causas de abandono do aleitamento materno são multifatoriais e estão associadas à mãe, ao bebé e à saúde. A promoção do aleitamento materno exige programas de educação baseados na evidência, contribuindo para melhoria dos índices nacionais de prevalência da amamentação até, pelo menos aos 6 meses de vida do bebé, que se deseja cada vez mais consentânea com as melhores práticas internacionais. The ability to sensitively care for others’ wellbeing develops early in ontogeny and is an important developmental milestone for healthy social, emotional, and moral development. One facet of care for others, prosocial comforting, has been linked with important social outcomes such as peer acceptance and friendship quality, underscoring the importance of determining factors involved in the ability to comfort. Although social support has been linked with a number of important social outcomes, no study has directly examined whether felt social support can foster children’s positive behavior toward others. The purpose of the current investigation was to use an experimental priming paradigm to demonstrate that felt social support a) enhances children’s ability to respond prosocially to the distress of others and b) decreases children’s expressions of personal distress when faced with the distress of another person. Participants were 94 4-year-old children (M = 53.56 months, SD = 3.38 months; 52 girls). Children were randomly assigned to either view pictures of mothers and children in close, personal interactions (supportive social interaction condition), happy women and children in separate pictures, presented side-by-side (happy control condition), or pictures of colorful overlapping shapes (neutral control condition). Each set of 20 pictures was presented in the context of a categorization computer game that participants played 4 times throughout the course of the study. Immediately following the first three computer games, children were given the opportunity to comfort someone who was distressed; twice it was the adult experimenter working with the child, and once it was an unseen infant crying over a monitor that participants had been trained to use. Comforting behaviors and distress/arousal were coded in 10-second time segments and yielded a global comforting score and a distress proportion score for each task. Results indicated that priming condition had no effect on either prosocial comforting behavior or expressions of personal distress. I discuss these null findings in light of the available literatures on priming mental representations in children and on prosocial comforting, and suggest some future directions for continued investigation in both fields.

Antecedentes: en Cuenca el uso de plantas medicinales en patologías tradicionales según la cosmovisión andina se mantiene vigente, en los mercados es habitual ver personas que buscan esta medicina, haciendo importante un estudio de la problemática. Objetivo: estudiar las patologías andinas relacionadas al uso de plantas medicinales en los mercados 10 de Agosto, el Arenal y 9 de Octubre de la ciudad de Cuenca. Método: investigación cualitativa, enfoque narrativo e interpretativo, con participación de seis mujeres de sabiduría de los mercados de Cuenca. Las técnicas empleadas: observación directa y entrevistas semi-estructuradas, diario de campo, grabaciones y fotografías. Los datos se procesaron en categorías y diagramas en Atlas.ti, se presentan en narraciones y esquemas. Resultados: las patologías andinas tratadas comúnmente; en el recién nacido, el mal de ojo; lactantes, el espanto; escolares, el shungo; adolescentes y adultos los nervios. Etiologías: estación del año, ambientes desagradables, castigo divino y causas humanas. Sintomatología compromete el estado general: irritabilidad, llanto y decaimiento. Tratamiento: fe cristiana, ritual de limpia y herramientas complementarias. Para la limpia se usa un atado de plantas medicinales, las más utilizadas: chilchil, ruda, eucalipto y flor de guano. Conclusiones: La patología andina tiene leyes y principios propios, las enfermedades mayormente atendidas: el mal de ojo, espanto, mal aire y shungo. Sintomatología es de tipo general. El tratamiento se basa en el ritual con el atado de plantas medicinales y el huevo de gallina. Cada enfermedad se explica en base a una medicina de tipo mágico-religiosa.

Introduction: Cayler cardio-facial syndrome is a rare syndrome associated with asymmetric crying faces with congenital heart disease. We report a newborn that was diagnosed as case of Cayler Cardio-facial syndrome based on clinical features and was confirmed with FISH analysis. Case Presentation: A term male baby, born to non-consanguineous couple through normal vaginal delivery was diagnosed to have asymmetric crying faces with deviation of angle of mouth to left side at the time of birth. The baby had normal faces while sleeping or silent. Mother was known case of hypothyroidism and was on treatment. Baby was diagnosed as case of Cayler Cardio-facial Syndrome and was investigated with echocardiogram, brain ultrasound, total body X-ray examination, X-ray of cervico-thoracic vertebral column and fundus examination. Echocardiogram showed muscular VSD, brain ultrasound was normal and fundus examination showed tortuous retinal vessels. Whole body X-ray and lateral X-ray of cervico-thoracic vertebral column were not suggestive of any skeletal abnormalities. The other associated malformation was right ear microtia. Baby FISH karyotype analysis showed deletion of 22q11.2 deletion. Baby was discharged and now on follow-up. Conclusions: Cayler syndrome is a rare syndrome which must be suspected if a baby has asymmetrical cry pattern and normal facies when baby sleeps. Patient must be evaluated with echocardiography to find out associated cardiac malformations. These infants should undergo FISH analysis for 22q11.2 deletion syndrome.

Background: Among different categories of sedative agents, benzodiazepines have been prescribed for more than three decades to patients of all ages. The effective and predictable sedative and amnestic effects of benzodiazepines support their use in pediatric patients. Midazolam is one of the most extensively used benzodiazepines in this age group. Oral form of drug is the best accepted route of administration in children. Objectives: The purpose of this study was to compare the efficacy and safety of a commercially midazolam syrup versus orally administered IV midazolam in uncooperative dental patients. Second objective was to determine whether differences concerning sedation success can be explained by child‘s behavioral problems and dental fear. Patients and Methods: Eighty eight uncooperative dental patients (Frankl Scales 1,2) aged 3 to 6 years, and ASA I participated in this double blind, parallel randomized, controlled clinical trial. Midazolam was administered in a dose of 0.5 mg/kg for children under the age 5 and 0.2 mg/kg in patients over 5 years of age. Physiologic parameters including heart rate, respiratory rate, oxygen saturation and blood pressure were recorded. Behavior assessment was conducted throughout the course of treatment using Houpt Sedation Rating Scale and at critical moments of treatment (injection and cavity preparation) by North Carolina Scale. Dental fear and behavioral problems were evaluated using Child Fear Schedule Survey-Dental Subscale (CFSS-DS), and Strength and Difficulties Questionnaire (SDQ). Independent t-test, Chi-Square, and Pearson correlation were used for statistical analysis. Results: Acceptable overall sedation ratings were observed in 90% and 86% of syrup and IV/Oral group respectively; Chi-Square P = 0.5. Other domains of Houpt Scale including: sleep, crying and movement were also not significantly different between groups. Physiological parameters remained in normal limits during study without significant difference between groups. Conclusions: “Orally administered IV midazolam” preparation can be used as an alternative for commercially midazolam syrup.

Antecedentes: Los trastornos gastrointestinales funcionales de la infancia (TGFI) son manifestaciones gastrointestinales crónicas en cualquier parte del tubo digestivo sin daño estructural o bioquímico los cuales se pueden clasificar según los criterios de ROMA III. Se desconoce su prevalencia en niños latinoamericanos menores de 4 años. Objetivos: Estimar la prevalencia de los TGFI y cada una de sus entidades en menores de 2 años y explorar sus factores asociados. Metodología: Estudio corte trasversal con muestra aleatoria (n=323) tomada de la población de una institución de salud en la ciudad de Bogotá, realizando mediante encuesta diligenciada por los padres. El análisis se realizó por medio del software SPSS© utilizando estadística descriptiva y análisis bivariado, como medida de asociación se calculó las Razones de Disparidad (RD) con IC95%. Resultados: Se encontró una prevalencia de TGFI de 22.1%, diarrea funcional 14.6%, disquecia 12%, regurgitación 9.2%, estreñimiento 3.3%, vómito cíclico 2%, cólico infantil 1.6% y rumiación 0%. La administración de tetero durante la estancia hospitalaria neonatal se asocia con vómito cíclico RD= 6 IC 95% (1.076 – 33.447) p=0.021. La administración de formula infantil durante los primeros 6 meses de vida se asocia con diarrea funcional RD= 0.348 IC 95% (0.149 – 0.813) p=0.012 Conclusiónes: Los TGFI son una causa frecuente de molestias en los menores de 2 años de edad. Sugerimos realizar la validación del cuestionario “questionnaire on infant/toddler gastrointestinal symptoms rome version III” con el fin de mejorar la validez y precisión de los hallazgos en estudios futuros.
