OBJECTIVES To evaluate the level of satisfaction of individuals with cleft lip and/or palate (CLP) and their parents concerning the esthetic and functional treatment outcomes, the impact of the cleft on everyday life, and potential associations with treatment outcome satisfaction. SUBJECTS AND METHODS The sample consisted of 33 patients (7 CP, 20 unilateral CLP, and 6 bilateral CLP; median age: 17.1, range: 9.0-33.1 years) and 30 parents, who responded to a questionnaire in an interview-guided session. All participants received their orthodontic treatment at the Department of Orthodontics in the University of Athens. RESULTS Patients and their parents were quite satisfied with esthetics and function. Patients with UCLP primarily were concerned about nose esthetics (BCLP about lip esthetics and CP about speech). Increased satisfaction was associated with decreased influence of the cleft in everyday life (0.35 < rho < 0.64, P < 0.05). Parents reported significant influence of the cleft on family life, while patients did not. CONCLUSIONS Despite the limited sample size of subgroups, the main concerns of patients with different cleft types and the importance of satisfying lip, nose, and speech outcomes for an undisturbed everyday life were quite evident. Thus, the need for targeted treatment strategies is highlighted for individuals with cleft lip and/or palate.

This article proposes a combined technique including bone grafting, connective tissue graft, and coronally advanced flap to create some space for simultaneous bone regrowth and root coverage. A 23 year-old female was referred to our private clinic with a severe class II Miller recession and lack of attached gingiva. The suggested treatment plan comprised of root coverage combined with xenograft bone particles. The grafted area healed well and full coverage was achieved at 12-month follow-up visit. Bone-added periodontal plastic surgery can be considered as a practical procedure for management of deep gingival recession without buccal bone plate.

Technical complications in implant prosthetic cases represent a major challenge in dentistry. This case report describes minimally invasive management to recover an implant with a fractured remnant of a zirconia abutment, including provisional rehabilitation during a sequential treatment protocol in the esthetic zone. A patient was treated with a screw-retained one-piece implant-supported reconstruction made of a customized zirconia abutment with direct ceramic veneering in the maxillary right central incisor position. During the prosthetic try-in, a fracture in the apical portion of the abutment was evident. The first rescue attempt led to fracture of the retrieval instrument. Immediately, an individualized wired construction was applied to bond the existing fractured reconstruction to the neighboring teeth to maintain the peri-implant mucosal architecture. Because the implant screw canal was blocked, a customized round bur had to be manufactured and was placed in the implant axis with a specific bracket tool from the service set to protect the interior implant threads. Then, the drills of the service set were guided by the newly created access to remove the fractured remnants. The implant screw was retapped and the area rinsed with chlorhexidine solution. All remnants were removed without the need for surgical intervention. Neither the implant connection nor the bone-to-implant interface was damaged. The stepwise treatment approach with the customized round bur combined with the system-specific drills of the service set saved the blocked implant so that the patient could be successfully rehabilitated with a new implant reconstruction.

AIM To associate the dimension of the facial bone wall with clinical, radiological, and patient-centered outcomes at least 10 years after immediate implant placement with simultaneous guided bone regeneration in a retrospective study. MATERIAL AND METHODS Primary endpoint was the distance from the implant shoulder (IS) to the first bone-to-implant contact (IS-BIC10y ). Secondary endpoints included the facial bone thickness (BT10y ) 2, 4, and 6 mm apical to the IS, and the implant position. At baseline, the horizontal defect width (HDWBL ) from the implant surface to the alveolar wall was recorded. At recall, distance from the IS to the mucosal margin (IS-MM10y ), degree of soft tissue coverage of the mesial and distal aspects of the implants (PISm10y , PISd10y ; Papilla Index), pocket probing depth (PPD10y ), and patient-centered outcomes were determined. Width of the keratinized mucosa (KM), Full-Mouth Plaque and Bleeding Score (FMPS, FMBS) were available for both time points. RESULTS Of the 20 patients who underwent immediate implant placement with simultaneous guided bone regeneration and transmucosal healing, nine males and eight females with a median age of 62 years (42 min, 84 max) were followed up for a median period of 10.5 y (min 10.1 max 11.5). The 10-year implant survival rate was 100%. Multivariate regression analysis revealed a correlation of the IS-BIC10y , controlled for age and gender, with four parameters: HDWBL (P = 0.03), KMBL -10 (P = 0.02), BT10 4 mm (P = 0.01), and BT10 6 mm (P = 0.01). CONCLUSION Within the conditions of the present study, the horizontal defect width was the main indicator for the vertical dimension of the facial bone. The facial bone dimension was further associated with a reduction in the width of the keratinized mucosa and the dimension of the buccal bone.

The dual path of insertion concept for removable partial denture (RPD) design may be used in esthetically demanding situations. When compared to conventional RPDs, the main advantage of this design is the minimal use of clasps. This clinical report describes the treatment of a patient with an anterior maxillary edentulous area using a dual path RPD. The diagnostic cast was surveyed to ensure the adequacy of the undercuts on the mesial surfaces of the anterior abutments, where rigid minor connectors were placed. Inverted V-shaped canine cingulum rest seats were prepared to provide resistance to tooth movement during function. The dual path RPD concept allows excellent esthetic results, minimizes tooth preparation, and reduces the tendency toward plaque accumulation in a Kennedy class IV partially edentulous arch.

This work examines the urban modernization of San José, Costa Rica, between 1880 and 1930, using a cultural approach to trace the emergence of the bourgeois city in a small Central American capital, within the context of order and progress. As proposed by Henri Lefebvre, Manuel Castells and Edward Soja, space is given its rightful place as protagonist. The city, subject of this study, is explored as a seat of social power and as the embodiment of a cultural transformation that took shape in that space, a transformation spearheaded by the dominant social group, the Liberal elite. An analysis of the product built environment allows us to understand why the city grew in a determined manner: how the urban space became organized and how its infrastructure and services distributed. Although the emphasis is on the Liberal heyday from 1880-1930, this study also examines the history of the city since its origins in the late colonial period through its consolidation as a capital during the independent era, in order to characterize the nineteenth century colonial city that prevailed up to 1890 s. A diverse array of primary sources including official acts, memoirs, newspaper sources, maps and plans, photographs, and travelogues are used to study the initial phase of San Jose s urban growth. The investigation places the first period of modern urban growth at the turn of the nineteenth century within the prevailing ideological and political context of Positivism and Liberalism. The ideas of the city s elite regarding progress were translated into and reflected in the physical transformation of the city and in the social construction of space. Not only the transformations but also the limits and contradictions of the process of urban change are examined. At the same time, the reorganization of the city s physical space and the beginnings of the ensanche are studied. Hygiene as an engine of urban renovation is explored by studying the period s new public infrastructure (including pipelines, sewer systems, and the use of asphalt pavement) as part of the Saneamiento of San José. The modernization of public space is analyzed through a study of the first parks, boulevards and monuments and the emergence of a new urban culture prominently displayed in these green spaces. Parks and boulevards were new public and secular places of power within the modern city, used by the elite to display and educate the urban population into the new civic and secular traditions. The study goes on to explore the idealized image of the modern city through an analysis of European and North American travelogues and photography. The new esthetic of theatrical-spectacular representation of the modern city constructed a visual guide of how to understand and come to know the city. A partial and selective image of generalized urban change presented only the bourgeois facade and excluded everything that challenged the idea of progress. The enduring patterns of spatial and symbolic exclusion built into Costa Rica s capital city at the dawn of the twentieth century shed important light on the long-term political social and cultural processes that have created the troubled urban landscapes of contemporary Latin America.

Space in musical semiosis is a study of musical meaning, spatiality and composition. Earlier studies on musical composition have not adequately treated the problems of musical signification. Here, composition is considered an epitomic process of musical signification. Hence the core problems of composition theory are core problems of musical semiotics. The study employs a framework of naturalist pragmatism, based on C. S. Peirce’s philosophy. It operates on concepts such as subject, experience, mind and inquiry, and incorporates relevant ideas of Aristotle, Peirce and John Dewey into a synthetic view of esthetic, practic, and semiotic for the benefit of grasping musical signification process as a case of semiosis in general. Based on expert accounts, music is depicted as real, communicative, representational, useful, embodied and non-arbitrary. These describe how music and the musical composition process are mental processes. Peirce’s theories are combined with current morphological theories of cognition into a view of mind, in which space is central. This requires an analysis of space, and the acceptance of a relativist understanding of spatiality. This approach to signification suggests that mental processes are spatially embodied, by virtue of hard facts of the world, literal representations of objects, as well as primary and complex metaphors each sharing identities of spatial structures. Consequently, music and the musical composition process are spatially embodied. Composing music appears as a process of constructing metaphors—as a praxis of shaping and reshaping features of sound, representable from simple quality dimensions to complex domains. In principle, any conceptual space, metaphorical or literal, may set off and steer elaboration, depending on the practical bearings on the habits of feeling, thinking and action, induced in musical communication. In this sense, it is evident that music helps us to reorganize our habits of feeling, thinking, and action. These habits, in turn, constitute our existence. The combination of Peirce and morphological approaches to cognition serves well for understanding musical and general signification. It appears both possible and worthwhile to address a variety of issues central to musicological inquiry in the framework of naturalist pragmatism. The study may also contribute to the development of Peircean semiotics.

Peruvian orchestral music 1945–2005. Identities in diversity Peruvian music for orchestra has not been studied as a whole before, and is hardly known by Peruvian musicians and public. The aim of the thesis is to give a panoramic view of Peruvian orchestral music after 1945, study the particular historical context in which these works were created and how they reflect the search for a musical identity of its own, be it individual, local, national or Latin American. Identity is a construction that changes permanently, and individuals can share many identities at the same time. This is a central issue in multicultural societies as the Peruvian, and music is an important mean for constructing cultural identity. The hypothesis of this research is that orchestral work is a medium for Peruvian composers to express their relationship with traditional and popular musics of the country in different ways, from quotation of melodies to a more abstract appropiation of concepts or suggestive title references. Representative works by selected composers, of different techniques, styles or special reception are chosen and analyzed. Research methodology includes analysis of works with various methods according to their stylistic and technical features, in order to find the particular ways in which composers have approached or expressed diverse identities. The investigation shows that Peruvian orchestral music includes works in the main stylistic trends and using the main compositional techniques of the modernist and postmodern periods. It also shows that the construction and expression of particular identities through the study and use of other Peruvian musical traditions is a constant interest shared by composers of different age and esthetic. In a multicultural society as the Peruvian, characterized by its diversity, different forms of transcultural composition are an important mean of dealing with identity issues in music. This thesis also includes for the first time a list of all orchestral works composed in the country or by Peruvian composers in the period, their composers and genres. In this research pedagogical live action role playing is examined as a method of teaching history. The objective of this research is to present and define the concept of pedagogical live action role playing as a method of teaching history and examine its capability in teaching historical empathy. Pedagogical live action role playing is a teaching method under development which has elements from drama pedagogics and different learning theories. The theoretical part of this research consists of play theories and drama pedagogical theories which are relevant to live action role playing, and also the concepts of historical empathy and historical thinking. In drama pedagogics the main sources are the researches of Laakso (2004) and Heikkinen (2002, 2004). The historical part uses the researches of VanSledright (2002), Cooper (2000) and Ashby and Lee (1987) as its main sources. The research was carried out at the Viikki Teacher Training School in the class 6b during autumn 2007. The research material consists of three-phased interviews. During all phases three pupils were interviewed, and during two of the phases, also the teacher of the class was interviewed. The methods used in the interviews were thematic interview and stimulated recall -interview. The data gathered from the interviews was analyzed using content analysis. Pedagogical live action role playing has three phases when teaching history: pre-game (preparations), game (live action role play) and post-game (discussion after the game and in the classroom). The teacher's attention and carefulness is important during all phases. The mistakes made in the pre-game face e.g. in writing the characters seem to cause major problems in the pupils' game experiences. Pedagogical live action role playing is capable of teaching historical empathy, but according to my research this has some conditions, from which the most important is the pupil's historical knowledge before the game. Teaching historical empathy was based on esthetic dualition: the pupil made decions about the actions of his character as him/herself. The more background information the pupil had about the era they played, the more the pupil seemed to make decisions based on his historical empathy, aka his understanding of the era of the larp and of the people of the same era.

This study examines the organisation and transformation of altar space in the modern Evangelical Lutheran Church of Finland in liturgical and architectural perspective. The research data consists of 65 altar spaces in The Finnish Evangelical Lutheran church buildings. All of these were characterised in Church Government records as churches , built 1962 1999 and had been consecrated. The main data was collected by means of observation, photographing, and drawing sketches of altar spaces. The focus of this study concerns the organisation of modern Finnish Evangelical Lutheran altar spaces and, in particular, their changes also in relation to the liturgical movement. The challenge of this approach was especially in discovering the spatial identity of an altar space in terms of unequivocal boundaries. The analysis was realised in three stages. Interiors, the organisation of altar space, as well as architectonic qualities of altar spaces in terms of floor elevations, shapes of ceilings, lighting, and openings in the altar space were analysed. Moreover, attention was focused on furnishing and fixed versus movable pieces of furniture (such as the altar, altar rail, the pulpit, the baptismal font, and lectern). Finally, the potential qualitative and quantitative changes in altar space were examined. All in all, the majority of churches in the data featured elongated church halls with an altar at the end of the nave. To look at the data in chronological perspective, increasingly wide church halls had been built since the 1980s (yet there was only one central hall in which the altar was placed at the middle point of the church). Every third church altar was movable. As for the focal point of this study and the altar in particular, it was my aim to pay attention to the versus populum altar and its development in relation to the (Lutheran) liturgy. Hence, it was meaningful to determine, in terms of interior design, whether liturgists were able to celebrate facing the people attending the service. In the 1960s and 70s, a versus orientem altar featured in more than half of all new Finnish Lutheran churches, yet in 2000 two out of three churches featured a versus populum altar. For architectural and esthetic reasons (and not primarily due to liturgical ideas), also altars standing freely off the walls had been constructed. In terms of the liturgy, versus populum altars had been realised in expectation of increased communication between liturgist and worshippers. However, the analysis indicated that the altar could also become a divider of space. This aspect is a novel finding in relation to earlier and concurrent discussions concerning the liturgical movement. This study concluded, all in all, that altars had been increasingly constructed closer and closer to the worshiping parish and, accordingly, used increasingly often in the versus populum manner. Lecterns were often movable until the millennium this was the case in most altar spaces. Baptismal fonts did not have a permanent place in this data, and the data even included altar spaces with no baptismal fonts in the choir, nor the church hall. The position and status of fonts was generally weakened even if baptism in the Lutheran Church was regarded as one of the two sacraments together with the eucharist. The study concluded that even if baptism is regarded as a sacrament in the church, the position and status of baptismal fonts had weakened overall in newer church architecture. In other words, the tendency of the liturgical movement to emphasise the service and its celebration had obviously had its effect on the placement of baptismal fonts in the church hall. This research indicated that the pieces of furniture that mostly involved (many kinds of) visual and spatial changes included the altar and the lectern. In certain instances, fixed furnishings had been substituted by movable pieces or, moreover, new pieces of furniture and paraphernalia such as music instruments, pieces of art, tables, chairs and plants were brought in. In the Evangelical Lutheran Church of Finland, liturgical changes were principally inspired by the Catholic Church, in which liturgical changes are essentially based on Canon Law. Unlike Finnish Lutheranism, Catholicism provides detailed rules and principles even regarding the design of an altar space. According to this study, in the Finnish Lutheran Church, the primarily functional nature of given guidelines and instructions characterises several practical solutions in furnishing.

Resumen: La pulcra edición de la Segunda parte de comedias de Pedro Calderón de la Barca por Santiago Fernández Mosquera (Biblioteca Castro, 2007) y la reciente tesis de Fernando Rodríguez-Gallego (2009) sobre las dos versiones de El astrólogo fingido (Zaragoza, 1632 y Madrid, 1637) obligan hoy a una revisión de uno de los casos, tan frecuentes en la producción del dramaturgo madrileño, de reescritura —con miras esta vez a la publicación impresa— de una de sus obras. El examen se centrará, más allá de consideraciones formales y estéticas, en la valoración del alcance ético de las profundas transformaciones sufridas por el personaje del primer protagonista, el falso astrólogo, don Diego de Luna, así como en el análisis de las dos versiones del desenlace.

Os implantes osteointegráveis assumiram condição prioritária na reabilitação da perda dentária unitária ou múltipla em função das elevadas taxas de sucesso e previsibilidade no tratamento e vêm sendo cada vez mais utilizados por especialistas e clínicos. Atualmente existe a preocupação com a manutenção dos tecidos moles periimplantares, principalmente em áreas estéticas. De modo geral, um ano após a instalação dos implantes osteointegráveis ocorre uma perda óssea proximal de 1,5 mm e em média 0,1 mm durante os anos subsequentes. Nos últimos anos, achados clínicos evidenciaram menor perda óssea inicial associada a intermediários de diâmetro reduzido em relação à plataforma dos implantes. Com o objetivo de comparar, por meio de imagens radiográficas o comportamento ósseo proximal ao redor de implantes osteointegráveis com plataformas convencionais e plataformas de diâmetro intermediário reduzido, foi estabelecido o seguinte desenho de estudo clínico prospectivo: em 08 pacientes totalmente edentados, foram instalados 40 implantes, 5 implantes mandibulares por paciente. Cada paciente recebeu 3 implantes com plataforma convencional e 2 com plataforma associada aos intermediários de diâmetro reduzido (cone morse). Foram confeccionadas próteses em resina acrílica e fixadas precocemente aos implantes por intermédio de parafusos, seguindo o modelo protocolo Bränemark. Foram feitas radiografias periapicais padronizadas em intervalos de 21 dias, 3, 6 e 12 meses, após a instalação dos implantes. As imagens radiográficas foram digitalizadas e realizada a subtração radiográfica digital pelo programa emago, sendo comparadas com a radiografia inicial. Os resultados obtidos neste estudo mostraram uma regularidade no remodelamento ósseo ao longo do tempo para todos os implantes, não tendo sido encontradas diferenças significativas entre os diferentes implantes analisados.

O corpus desta dissertação tem por objetivo a análise das obras A Maior flor do Mundo de José Saramago, O Gato e o Escuro de Mia Couto e O Mistério do Coelho Pensante de Clarice Lispector para o público infantil. A análise empreendida tem como referencial teórico a Estética da Recepção de Hans Robert Jauss e a Teoria do Efeito Estético de Wolfgang Iser, correntes que figuram o leitor como elemento atuante no ato da leitura, convidando-o a mergulhar nas entrelinhas dos textos e preencher lacunas ou hiatos, deixados pelos autores que mobilizam o leitor a formular o que não foi dito através da imaginação. A linguagem das obras mencionadas age como instrumento de percepção do leitor e símbolo da infância, pois, tornar-se-ão prazerosa, fantástica e maravilhosa sem a função disciplinadora e moralista que, anteriormente, inibia a criança de se expressar com liberdade. Ao trabalhar com conceitos como recepção, efeito, horizonte de expectativas e leitor implícito, busca-se explicar como se dá a leitura e a sua inserção no contexto das práticas culturais de produção de sentido. As obras estudadas contribuem para a formação de leitores críticos e reflexivos ao questionarem a linguagem literária diante das experiências da vida. As ilustrações das obras não se apresentam somente como simples complementação dos textos verbais, mas também, adquirem significados e expressões estéticas

Este estudo parte de preocupações filosóficas, teóricas e analíticas para promover exercícios de leitura e de interpretação do poema A Máquina do Mundo (1951), de Carlos Drummond de Andrade. Estabelecendo pela via ensaística a noção de ontologia da propagação, busca localizar a necessidade da ontologia no fundamento pré-paradigmático da ciência. Incorporando referências da Fenomenologia e da Hermenêutica, tenta assumir seus pressupostos e implicações para a constituição do método em Teoria da Literatura e para a definição dos campos básicos para a analítica da existência lógica, empírica e pragmática: os campos que representam as instâncias ontológicas do real, do simbólico e do imaginário. Da assunção fenomenológica e hermenêutica, passa-se a considerações sobre categorias pertencentes ao jargão literário, escolhidas por sua relação com o artefato literário em questão e correspondendo ao âmbito das três instâncias ontológicas: discute-se a Poesia como sendo um fenômeno constituinte, a Literatura como uma manifestação instituinte e o Poema como uma manifestação restituinte do signo literário A Máquina do Mundo. Em seguida, considera afetações e interferências de algumas correntes sociológicas, formalistas e antropológicas, buscando participar do diálogo sobre a possibilidade de aceitação da prática literária como uma prática de valor cognitivo, e não apenas ideológico e estético. Em seu terceiro momento, o estudo busca aplicar os pressupostos ontológicos e epistêmicos para estabelecer o limite dos espaços comparativos tornados possíveis ao poema A Máquina do Mundo e buscando o pano de fundo histórico e historiográfico do século XX a partir de suas possibilidades de relação com a obra de Drummond e segundo a orientação dos vetores dados no poema: a consolidação do veio crepuscular na tópica canônica do Novecentos brasileiro; a implicação da postura ideológica de Drummond na recepção crítica à sua obra; e, por fim, a localização posterior dos topoi cosmológicos e existenciais. Em seu último ensaio, refere-se à questão cosmológica e sua afetação sobre a postura existencial do Caminhante. Recuperam-se pontos de semelhança e de diferença entre as poéticas encontradas em Os Lusíadas, de Luís de Camões e em A Divina Comédia, de Dante Alighieri, para por fim remeter à identificação de uma continuidade temática entre o poema de Drummond e aquele que se costuma designar como pensamento originário na tradição da Filosofia ocidental. A seguir, buscam-se referências para alguns dos elementos formais do poema A Máquina do Mundo segundo sua origem no contexto da Literatura Ocidental. Consideram-se alguns traços característicos dos três seres representados (A Máquina, o Mundo, o Caminhante) e, encerrando-se a tese, procede-se a um exercício de leitura centrado especificamente no poema

O presente trabalho teve como objetivo central compreender como as expressões de gênero de mulheres com experiências afetivo-sexuais com mulheres estão associadas aos cuidados dedicados ao corpo, realizados em prol da saúde. As informações foram coletadas a partir de pesquisa de campo realizada durante quatro meses em espaços de sociabilidade das integrantes do Tambores de Safo, grupo de percussão composto por mulheres feministas, lésbicas e bissexuais que se autodefinem negras, residentes no município de Fortaleza, estado do Ceará. O foco de observação da pesquisa recaiu na performatividade e dispositivos performáticos de gênero apresentados pelas participantes nos ensaios e exibições do grupo. Também foram realizadas entrevistas informais visando explorar bem como articular as noções de saúde e expressões de gênero das mulheres. As análises dos dados etnográficos revelou a importância da ambiência dos espaços de circulação na organização dos arranjos estéticos do grupo. Refletiu-se acerca das coisas que compunham tais arranjos e que funcionaram em campo como dispositivos performáticos de gênero: os cabelos (dreadlock/black) e os objetos (alfaia e xequerê). Os usos do corpo na circulação pelos espaços, na interação com os sujeitos, na aprendizagem das performances do batucar possibilitaram à pesquisadora o conhecimento das corporalidades das mulheres do grupo, bem como das maneiras pelas quais as participantes davam a ver seus corpos e a compreensão das errâncias de gênero resultantes do estabelecimento de certos tipos de performances como estratégia protetiva para manutenção de uma vida menos afetada pela discriminação e mais saudável.
