Background Timely assessment of the burden of HIV/AIDS is essential for policy setting and programme evaluation. In this report from the Global Burden of Disease Study 2015 (GBD 2015), we provide national estimates of levels and trends of HIV/AIDS incidence, prevalence, coverage of antiretroviral therapy (ART), and mortality for 195 countries and territories from 1980 to 2015. Methods For countries without high-quality vital registration data, we estimated prevalence and incidence with data from antenatal care clinics and population-based seroprevalence surveys, and with assumptions by age and sex on initial CD4 distribution at infection, CD4 progression rates (probability of progression from higher to lower CD4 cell-count category), on and off antiretroviral therapy (ART) mortality, and mortality from all other causes. Our estimation strategy links the GBD 2015 assessment of all-cause mortality and estimation of incidence and prevalence so that for each draw from the uncertainty distribution all assumptions used in each step are internally consistent. We estimated incidence, prevalence, and death with GBD versions of the Estimation and Projection Package (EPP) and Spectrum software originally developed by the Joint United Nations Programme on HIV/AIDS (UNAIDS). We used an open-source version of EPP and recoded Spectrum for speed, and used updated assumptions from systematic reviews of the literature and GBD demographic data. For countries with high-quality vital registration data, we developed the cohort incidence bias adjustment model to estimate HIV incidence and prevalence largely from the number of deaths caused by HIV recorded in cause-of-death statistics. We corrected these statistics for garbage coding and HIV misclassifi cation. Findings Global HIV incidence reached its peak in 1997, at 3·3 million new infections (95% uncertainty interval [UI] 3·1–3·4 million). Annual incidence has stayed relatively constant at about 2·6 million per year (range 2·5–2·8 million) since 2005, after a period of fast decline between 1997 and 2005. The number of people living with HIV/AIDS has been steadily increasing and reached 38·8 million (95% UI 37·6–40·4 million) in 2015. At the same time, HIV/AIDS mortality has been declining at a steady pace, from a peak of 1·8 million deaths (95% UI 1·7–1·9 million) in 2005, to 1·2 million deaths (1·1–1·3 million) in 2015. We recorded substantial heterogeneity in the levels and trends of HIV/AIDS across countries. Although many countries have experienced decreases in HIV/AIDS mortality and in annual new infections, other countries have had slowdowns or increases in rates of change in annual new infections. Interpretation Scale-up of ART and prevention of mother-to-child transmission has been one of the great successes of global health in the past two decades. However, in the past decade, progress in reducing new infections has been slow, development assistance for health devoted to HIV has stagnated, and resources for health in low-income countries have grown slowly. Achievement of the new ambitious goals for HIV enshrined in Sustainable Development Goal 3 and the 90-90-90 UNAIDS targets will be challenging, and will need continued eff orts from governments and international agencies in the next 15 years to end AIDS by 2030.

[EN]Previous studies have reported an association between a more pro-inflammatory diet profile and various chronic metabolic diseases. The Dietary Inflammatory Index (DII) was used to assess the inflammatory potential of nutrients and foods in the context of a dietary pattern. We prospectively examined the association between the DII and the incidence of cardiovascular disease (CVD: myocardial infarction, stroke or cardiovascular death) in the PREDIMED (Prevención con Dieta Mediterránea) study including 7216 high-risk participants.

La sténose aortique est la cardiopathie valvulaire la plus fréquente retrouvée chez les patients agés. Suite à l’apparition des symptômes, la survie des patients diminue de façon drastique en l’absence d’un remplacement valvulaire aortique. Cependant, une proportion considérable de ces patients n’est pas opérée en raison d’un risque chirurgical élevé, l’âge étant l’une des principales raisons de refus d’un remplacement valvulaire aortique chirurgical. Ce défaut dans la prise en charge des ces patients a favorisé le développement du remplacement valvulaire aortique par cathéter où implantation valvulaire aortique par cathèter (TAVR ou TAVI), qui a représenté une révolution dans le traitement de la sténose aortique. Cette intervention est actuellement un traitement de routine chez les patients à haut risque chirurgical atteints d’une sténose aortique, même si la chirurgie cardiaque n’est pas contre-indiquée. Ces dernières années ont vu un changement de profil des candidats potentiels vers une population à plus faible risque. Cependant, plusieurs préoccupations demeurent. L’une des plus importantes est la survenue des arythmies et de troubles de conduction, notamment le bloc de branche gauche et le bloc auriculo-ventriculaire, qui sont des complications fréquemment associées au TAVR. Malgré l’évolution de la technologie et le développement de nouveaux dispositifs réduisant le taux global de complications, aucune amélioration n’a pas été intégrée pour prévenir l’apparition de telles complications. De plus, l’utilisation de certains dispositifs de nouvelle génération semble être associée à un risque accru de troubles de conduction, et par conséquent, l’incidence de ces complications pourrait augmenter dans le futur. Cependant, L’impact et l’évolution de ces complications sont inconnus. Ce travail de recherche évalue l’incidence et l’évolution des troubles de conduction suite au TAVR et l’impact des blocs de branche gauche de novo et de l’implantation d’un pacemaker sur les résultats cliniques et échocardiographiques.

Cette étude propose l’analyse de représentations queer dans le cinéma des années 2000. Plus précisément, elle porte sur la façon dont l’abus sexuel au masculin est représenté dans deux films produits en 2004, soit La mauvaise éducation (La mala educación), du réalisateur espagnol Pedro Almodovar, et Mysterious Skin, du réalisateur étatsunien Gregg Araki. À l’aide de la réflexion contenue dans Ça arrive aussi aux garçons : l’abus sexuel au masculin du sociologue Michel Dorais, l’objectif vise à démontrer comment cet événement traumatique influence de manière significative la construction identitaire et sexuelle des personnages principaux. De manière plus générale, ce mémoire positionne ces deux réalisateurs dans la grande et riche lignée du cinéma queer, qui met en scène des désirs hors norme et des identités sexuelles alternatives. Le premier chapitre porte sur les théories queer et ses diverses manifestations au grand écran. Il permet par la suite de réunir Almódovar et Araki dans une même étude et de souligner la pertinence de cette réunion. Le deuxième chapitre s’intéresse, à l’aide d’analyses d’extraits significatifs des films, à la façon dont chacun met en scène l’abus sexuel au masculin et comment cet événement se présente dans la vie des protagonistes. Le dernier chapitre se penche sur la construction identitaire et sexuelle des personnages principaux, afin de mieux comprendre l’incidence de l’abus sexuel. Jumelée aux travaux de Judith Butler, l’approche queer sera donc mise de l’avant dans cette étude qui se montre d’emblée attentive, d’un point de vue cinématographique, aux notions de sexe, de genre et de désir, et ce, à travers l’analyse de plusieurs extraits filmiques et d’éléments à la fois narratifs et structurels particulièrement significatifs quant à la représentation de l’abus sexuel au masculin.

Background and Objectives: Both psychiatric acute units and psychiatric intensive care units (PICUs) focus on acute treatment of behavioral disturbances such as violence and aggressive threats and acts. The aim of the present study is to describe the frequency of violent behavior; such as verbal or physical threats and physical attacks, among patients admitted to psychiatric intensive care unit (PICU). In addition the relationship between the episodes of threats and/or attacks in relation to time of the day, days of the week, and their seasonal variations was explored. Methods: All violent behavior was continuously assessed at the psychiatric emergency department. Data were collected during the period from May 2010 to May 2012. Results: Patients with only one hospitalization were less violent than those who have had two hospitalizations. There was a statistically significant difference in violence among patients without formal secondary education and those who have not formal education. Violent behavior showed two peaks during the day; the first occurring at 1 pm and the second at 8 pm. In regard to seasonality, summer had a higher incidence of violence. The most peaceful seasons of the year were spring and autumn. Conclusions: Violent behavior shows variation in daytime, days of the week and season in acute psychiatric intensive care. Daytime variation shows two peaks of violence at 1 pm and 8 pm, Sundays and Wednesdays being the quietest days regarding violence both in winter and summer. Patient's level of education and hospitalization status partially explain the variation.

The effect of a pre-shipment hypochlorite treatment on botrytis incidence was evaluated in a large number of rose cultivars and under different long-term storage conditions. Application parameters, stability and sources of hypochlorite were investigated. Irrespective of the type of packaging and shipment conditions, roses that received a pre-shipment treatment with 100 to 150 mg/L hypochlorite showed a significantly decreased botrytis incidence compared to non-hypochlorite treated roses. The hypochlorite treatment generally was more effective than a comparable treatment with commercial fungicides. Dipping the flower heads for approximately one second in a hypochlorite solution was more effective than spraying the heads. In few cases minor hypochlorite-induced damage on the petal tips was observed at higher concentrations (>200 mg/L). Apart from the effect on botrytis incidence, the treatment resulted in reduced water loss that may have an additional beneficial effect on the eventual flower quality. It is concluded that, apart from other obvious measures to reduce botrytis incidence (prevention of high humidity at the flower heads) a pre-shipment floral dip in 100 to 150 mg/L hypochlorite from commercial household bleach is an easy and cost effective way to reduce botrytis incidence following long term storage/transportation of roses. © 2015, International Society for Horticultural Science. Green bean production accounts for 2.4% of the total value of Australian vegetable production and was Australia's tenth largest vegetable crop in 2008-2009 by value. Australian green bean production is concentrated in Queensland (51%) and Tasmania (34%) where lost productivity as a direct result of insect damage is recognised as a key threat to the industry (AUSVEG, 2011). Green beans attract a wide range of insect pests, with thrips causing the most damage to the harvestable product, the pod. Thrips populations were monitored in green bean crops in the Gatton Research Facility, Lockyer Valley, South-east Queensland, Australia from 2002-2011. Field trials were conducted to identify the thrips species present, to record fluctuation in abundance during the season and assess pod damage as a direct result of thrips. Thirteen species of thrips were recorded during this time on bean plantings, with six dominant species being collected during most of the growing season: Frankliniella occidentalis, F. schultzei, Megalurothrips usitatus, Pseudanaphothrips achaetus, Thrips imaginis and T. tabaci. Thrips numbers ranged from less than one thrips per flower to as high as 5.39 thrips per flower. The highest incidence of thrips presence found in October/November 2008, resulted in 10.74% unmarketable pods due to thrips damage, while the lowest number of thrips recorded in April 2008 caused a productivity loss of 36.65% of pods as a result of thrips damage.

Management of coconut ( Cocos nucifera ) lethal yellowing disease (CLYD), which has killed about eight million coconut trees in Mozambique, has proved challenging. The objective of this study was to investigate the impact of farming practices and related history, on the CLYD incidence in Mozambique. The methodology included a socioeconomic questionnaire to the households and direct observations on the palm farms. The collected data were analysed using logistic regression. Five out of 11 explanatory variables tested, namely farm age, availability of other palm species on the coconut farm, type of coconut varieties grown, root cut practices, and intercropping had a significant (P< 0.05) effect on CLYD incidence. Coconut farms <10 years had higher odds of higher disease incidence compared to the farms between 10 to 40 years old. The presence of other palm species in the coconut farms had two times higher odds of having higher disease incidence levels compared to farms without other palm species. Tall coconut varieties were likely to be more tolerant to CLYD compared to dwarf varieties. Coconut farms with some kind of intercropping had two times higher odds of having higher disease incidence levels compared to pure stands. The practice of cutting coconut roots had three times higher odds of having high disease incidence levels compared to non-practicing farms. Farm age, availability of other palm species on the coconut farm, type of coconut varieties grown, root cut practices and intercropping need to be considered for integrated CLYD management.

This study aimed at determining the incidence and pattern of pneumonia, in slaughtered goats in Kumasi abattoir, Ghana. One thousand three hundred and fifty lungs of goats; (1,012 Sahelian and 338 West Africa Dwarf goats (WAD) lungs) of different ages (less than a year to above 4 years) were used in this study. The frequency of occurrence of pneumonia, the degree of consolidation as a percentage of the total lung volume and histological assessment were determined by standard techniques. Fifty five (55) lungs (39 Sahelian, 16 WAD goats) were pneumonic (4.07% prevalence). The right lungs had a significant higher average lung consolidation percentage (19.11) while the right cranial lobes were more affected (9.37). WAD goats of 1-2 years are mostly affected with an average percentage consolidation of 11.73% while Sahelian goats above 4 years of age were the most affected with 32.59% consolidation. Does of both breeds were more while Sahelian goats had higher consolidation than other breeds. Histological examination revealed the presence of giant cell, fibrinous and suppurative bronchointerstitial pneumonia suggesting complicated viral pneumonia which was observed to be the most important caprine pneumonia in slaughtered goats in Ghana. Transportation and pregnancy stress were the major contributory factor to the pneumonia observed hence effective ante-mortem examinations will help to minimize the slaughter of pregnant does and transportation stress in Ghana.
