Experimental murine L. major infection is characterized by the expansion of distinct CD4+ T cell subsets. The Th1 response is related to production of IFN-g and resolution of infection, whereas Th-2 response with production of IL-4 and IL-10 and dissemination of infection. The objective of this study was to measure the circulating levels of IFN-g, IL-10 and TNF-a in patients with visceral leishmaniasis (VL) before, during and at the end of therapy and to examine the association between cytokine levels and activity of VL. Fifteen patients with VL were evaluated. The cytokine determinations were done by using the enzyme-linked immunoassay (ELISA) before, during and at the end of therapy. At baseline, we detected circulating levels of IFN-g in 13 of 15 patients (median = 60 pg/ml); IL-10 in 14 of 15 patients (median = 141.4 pg/ml); and TNF-a in 13 of 14 patients (median = 38.9 pg/ml). As patients improved, following antimonial therapy, circulating levels of IL-10 showed an exponential decay (y = 82.34 e–0,10367x, r = –0.659; p < 0.001). IFN-g was no longer detected after 7/14 days of therapy. On the other hand, circulating levels of TNF-a had a less pronounced decay with time on therapy, remaining detectable in most patients during the first seven days of therapy (y = 36.99-0.933x, r = –0.31; p = 0.05). Part of the expression of a successful response to therapy may, therefore, include reduction in secretion of inflammatory as well as suppressive cytokines. Since IL-10 and IFN-g are both detected prior to therapy, the recognized cellular immune depression seen in these patients may be due to biological predominance of IL-10 (type 2 cytokine), rather than lack of IFN-g (type 1 cytokine) production.

Thirty one infective endocarditis (IE) fatal cases whose diagnosis was first obtained at autopsy were studied. The clinical data of these patients (Group 1) showed significant differences compared to other 141 IE cases (Group 2). The average age of 53 years in Group 1 patients was 18 years higher than that of Group 2. The Group 1 patients had a low frequency of IE predisposing heart disease. Both patient groups presented fever (about 87%), but a significant low frequency of cardiac murmur (25.8%) was observed in Group 1 patients and echocardiography tests were performed in only 16.1%, suggesting that IE diagnosis was not suspected. Likewise, although most Group 1 patients appeared with severe acute illness, they did not present the classic IE clinical presentation. Blood cultures were performed in only 64.5% of the Group 1 patients. However, bacteria were isolated in 70% of these blood cultures and Staphylococcus aureus was isolated in 71.4%. The bacteria attacked mitral and aortic valves. Complications such as embolizations and cardiac failure occurred in almost half of the cases and they also presented with infections of the lungs, urinary tract, and central nervous system. Medical procedures were performed in practically all fatal cases whose diagnosis was first obtained at autopsy. Sepsis occurred in about half of the patients and it was followed by shock in more than 25%. This form of IE must be suspected in mature and in old febrile hospitalized patients having infection predisposing diseases, embolization, and suffering medical procedures.

Acute generalized exanthematous pustulosis (AGEP) usually presents with the acute appearance of oedematous and erythematous lesions, on which multiple sterile pustules appear, associated with fever. Almost 90% of cases are associated with drugs, with antibiotics (penicillins and macrolides) being the most frequent causative agents. We describe a 36-year-old female patient, which started diltiazem 120mg/day for hypertension. After 6 days of therapy, multiple erythematous and oedematous lesions appeared, with associated multiple small non-follicular pustules. Oral corticosteroids were started, with progressive and complete improvement. Patch-tests were performed, which revealed positivity for diltiazem. Although a rare entity, AGEP must be considered in cases of acute eruptions with disseminated pustules and fever. The use of patch tests in this disease may be useful as positive reactions are frequent.

O desenvolvimento teórico dos Mecanismos de Coping (MC) tem como base uma dialéctica relacionada com os seus principais factores determinantes: individuais e situacionais (na base das duas abordagens do coping: disposicional e constitucional). Actualmente a classificação dos MC mais utilizada é baseada em duas dimensões: coping focado na emoção, e coping focado na resolução de problemas. Considera-se essencial que os métodos de classificação dos MC tenham em conta a coexistência de elementos disposicionais estáveis com uma variabilidade situacional dos MC. São abordados alguns instrumentos de medição de coping, baseados em diferentes pressupostos teóricos. O coping pode influenciar a saúde através de vários mecanismos (sistema neuroendócrino, comportamentos relacionados com os riscos para a saúde e adesão terapêutica) e é incluído em dois dos principais modelos teóricos de saúde (Moos & Schaefer e modelo de Leventhal). Com base numa revisão da literatura, concluiu-se que os estilos de coping mais prevalentes no pré transplante foram: aceitação, coping activo, e procura de suporte, sendo os menos utilizados: auto culpabilização e evitação. No pós transplante o coping activo e procura de suporte continuam a ser os estilos de coping preferenciais, a par da confrontação, autoconfiança, recurso à religião e coping focado no problema. Os estilos de coping (Evasivo, Emotivo, Fatalistico) estão associados a uma menor capacidade de controlo pessoal sobre a doença, a confrontação a uma maior qualidade de vida, o evitamento à redução da qualidade de vida e ao aumento dos níveis de depressão e a negação ao aumento da não adesão. A compreensibilidade, a sensação de controlo sobre a doença, os estilos de coping «relacionados com a expressão dos afectos» e a negação variam ao longo da evolução do doente transplantado.

Baseados numa análise dos problemas metodológicos dos ensaios clínicos controlados aleatórios e da meta-análise, tecem algumas considerações sobre a utilização destes métodos na prática clínica.

Introdução - Síndroma maligna dos neurolépticos (SMN) é um efeito secundário raro potencialmente fatal destes fármacos. Relato de caso - Sexo masculino, 18 anos, encefalopatia não progressiva, atraso global do desenvolvimento, epilepsia, perturbação do comportamento (medicada com haloperidol). Iniciou febre elevada, desidratação, hematemeses e hematúria. Apresentava hipertermia, taquicardia, polipneia, tensão arterial instável, hipertonia generalizada e deterioração da consciência. Laboratorialmente destacava-se neutrofilia, trom bocitopénia, proteína C reactiva ligeiramente aumentada, elevação da creatina-cinase, alterações hepáticas e renais. Sépsis e SMN foram hipóteses de diagnóstico. Apesar da suspensão do haloperidol e início da terapêutica de suporte, ocorreu agravamento progressivo e óbito. Conclusão – Os antipsicóticos são frequentemente utilizados no atraso do desenvolvimento com alterações do comportamento. Os efeitos adversos graves exigem elevado grau de suspeição e início rápido de terapêutica.

We conducted a serological survey to determine the presence of hantavirus infection in rodents in Uberlândia, Minas Gerais as well as to identify and characterize associated factors. Rodents were captured using Sherman live-capture traps set in rural and peri-urban environments. A total of 611 rodents were captured. There was a higher trap success in peri-urban areas (26.3%) and a higher prevalence of antibodies among rodents captured in rural areas (2.9%). Necromys lasiurus was the most common species (42.2%) and the more frequently infected (4.6%). One Calomys tener (1/141; 0.7%) and one Calomys sp. (1/14; 7.1%) were also positive for the hantavirus infection. In N. lasiurus, antibody prevalence correlated with population density (p < 0.01), age class (p = 0.003) and presence of scars (p = 0.02). The data confirm that horizontal transmission is the main mechanism that maintains the virus in nature. The higher seropositivity in N. lasiurus is consistent with genetic studies that associate this species with an Araraquara virus reservoir; the seropositivity of C. tener and Calomys sp. may indicate the occurrence of spillover infection or the presence of other circulating hantaviruses.

Ao longo das últimas décadas tem-se assistido a um crescente número de crianças que necessita de avaliação e acompanhamento em Psiquiatria da Infância e Adolescência. Actualmente estima-se que entre 10 a 20% das crianças tenham um ou mais problemas de Saúde Mental, sendo que apenas um quinto destas recebem o tratamento apropriado. Assim, no sentido de se poder proceder a uma melhor planificação e gestão de recursos realizou-se uma análise do movimento assistencial de uma Equipa de Pedopsiquiatria entre 2004 e 2007 bem como uma análise descritiva detalhada da população de utentes do ano 2007. No período de quatro anos estudado foram observadas 1923 crianças num total de 9609 consultas. Verificou-se um predomínio claro de crianças e adolescentes do sexo masculino e a média etária observada foi de 9,89 anos. A caracterização da população consultada no ano de 2007 (480 utentes), revelou também uma preponderância das estruturas familiares nucleare e, ao nível da área de residência destaca-se um predomínio de crianças do concelho de Sintra (71%). Quanto à origem do pedido, em 29% os casos foram referenciados pelo Médico Assistente e em 26% pela Escola; os motivos de consulta mais frequentes foram os problemas de comportamento (30%) e as dificuldades de aprendizagem escolar (15%). Foi possível ainda caracterizar os pedidos de consulta por fonte de referenciação. No que concerne ao diagnóstico, a nível psicodinâmico, a Organização Depressiva predomina (56%) enquanto, quando analisados os diagnósticos do Eixo I de acordo com a DSM-IV-TR, se verifica um predomínio das Perturbações do Humor (30%) seguindo-se as Perturbações do Comportamento e Défice de Atenção (24,8%). Os tempos de espera médios entre o pedido e a primeira consulta variaram entre 55 dias (casos provenientes do Serviço de Urgência) e os 141 dias (casos sinalizados pela escola). Apenas com uma análise aprofundada da realidade assistencial nesta área permitirá a planificação e implementação de medidas que visem optimizar os Serviços e a resposta que estes dão às crianças, adolescentes e famílias.

SUMMARY To evaluate soil contamination by parasites in different developmental stages in public squares used as recreation and leisure areas for children in Belo Horizonte (MG, Brazil), 210 soil samples and 141 canine fecal samples were collected from 42 squares in the city. These samples were analyzed by the Caldwell and Caldwell technique and the Hoffman, Pons, and Janer technique. Of the samples analyzed, 89 (42.4%) soil samples and 104 (73.5%) fecal samples were contaminated with Ancylostoma sp., Toxocara sp., Trichuris sp., or Dipylidium sp. eggs; Giardia sp. cysts; or Isospora sp. oocysts. The commonest parasite was Ancylostoma sp., found in 85% soil and 99% fecal samples, followed by Toxocara sp., found in 43.7% soil and 30.7% fecal samples.

SUMMARY High-risk human papillomavirus (hr-HPV) infection is necessary but not sufficient for cervical cancer development. Recently, P16INK4A gene silencing through hypermethylation has been proposed as an important cofactor in cervical carcinogenesis due to its tumor suppressor function. We aimed to investigate P16INK4A methylation status in normal and neoplastic epithelia and evaluate an association with HPV infection and genotype. This cross-sectional study was performed with 141 cervical samples from patients attending Hospital Moncorvo Filho, Rio de Janeiro. HPV detection and genotyping were performed through PCR and P16INK4A methylation by nested-methylation specific PCR (MSP). HPV frequency was 62.4% (88/141). The most common HPV were HPV16 (37%), HPV18 (16.3%) and HPV33/45(15.2%). An upward trend was observed concerning P16INK4A methylation and lesion degree: normal epithelia (10.7%), low grade lesions (22.9%), high grade (57.1%) and carcinoma (93.1%) (p < 0.0001). A multivariate analysis was performed to evaluate an association between methylation, age, tobacco exposure, HPV infection and genotyping. A correlation was found concerning methylation with HPV infection (p < 0.0001), hr-HPV (p = 0.01), HSIL (p < 0.0007) and malignant lesions (p < 0.0001). Since viral infection and epigenetic alterations are related to cervical carcinoma, we suggest that P16INK4A methylation profile maybe thoroughly investigated as a biomarker to identify patients at risk of cancer.

Objectives: To retrospectively review the hybrid treatment of the aortic arch with supra-aortic debranching and endo- vascular stent-graft repair in a single institution. Methods: From 2007 to 2010, all patients submitted to aortic debranching procedures were entered into a prospective database analysis. For the present study, only patients with sealing zones 0 and 1, according to the Ishimaru classification, were included. Procedure-related morbimortality was analysed for the open and endovascular procedures. Results: During the study period, we electively performed 6 total aortic debranching and 4 partial aortic debranching procedures in 10 patients. According to the etiology the indications were: 6 aortic arch aneurysms, 2 post-dissection aneu- rysms, 1 false aneurysm and 1 type I endoleak following TEVAR. The proximal sealing zone was Ishimaru zone 0 in six patients and zone 1 in four patients. The TEVAR procedure was delayed in all patients with a completion success of 80% (1 patient died from ruptured aortic aneurysm; 1 patient denied the second procedure and was lost to follow-up). The 30d mortality rate was 10% (patient mentioned above). The main morbidity was: 1 axillar venous thrombosis, 1 case of subclinical myocardial infarction, 1 case of terminal renal insufficiency and 1 case of prolonged ventilation. No permanent cerebral or peripheral neurologic deficit was noted. Conclusions: The hybrid repair of the aortic arch is a feasible and reproducible procedure, and our results are similar to the previously published series. Medium and long-term results are necessary to confirm whether the technique can be regarded as a safe alternative to open surgery in high-risk patients.
