O número de bebés que nascem prematuros tem vindo a aumentar. É indiscutível a importância dos avanços tecnológicos e da pesquisa nas Unidades de Cuidados Intensivos de Neonatologia (UCIN), sendo essencial o papel desempenhado pelos fisioterapeutas nos cuidados prestados a estes bebés. As suas potencialidades só se desenvolvem na medida em que o bebé encontra estimulação e posicionamento adequado. Realizou‑se um estudo piloto na UCIN do Centro Hospitalar Lisboa Norte (CHLN), no período de julho de 2011 a março de 2012, em que se pretendeu avaliar o efeito de um programa de fisioterapia na estabilização do prematuro. Este programa consistiu em posicionar o prematuro em decúbito dorsal, lateral direito e lateral esquerdo e decúbito ventral, permanecendo em cada posição 5 minutos. O objetivo foi avaliar, durante a permanência do bebé na UCIN, a alteração nos seguintes parâmetros: frequência cardíaca (FC) e saturação de oxigénio (SatO2), antes e depois do tratamento. A amostra por conveniência incluiu no estudo 30 bebés prematuros, sendo 13 do género feminino e 17 do género masculino, que obedeceram a critérios específicos de inclusão. Para verificar as variáveis fisiológicas FC e SatO2 foi utilizado um Oxímetro de Pulso BCI 3401. Verificou‑se que a FC diminui e a SatO2 aumentou entre a primeira avaliação e a segunda avaliação, para a amostra total, assim como para os sujeitos do género feminino e os sujeitos do género masculino, para α = 0,05 e valor p≤0,001. Contudo, a diferença entre as médias dos dois momentos de avaliação da FC e da SatO2, em função do género, não revelaram diferenças estatisticamente significativas. Em conclusão, o estudo sugere que o programa de fisioterapia promove a melhoria da SatO2 e diminuição da FC, promovendo uma melhoria da estabilização do prematuro.

The oceans remain a major source of natural compounds with potential in pharmacology. In particular, during the last few decades, marine cyanobacteria have been in focus as producers of interesting bioactive compounds, especially for the treatment of cancer. In this study, the anticancer potential of extracts from twenty eight marine cyanobacteria strains, belonging to the underexplored picoplanktonic genera, Cyanobium, Synechocystis and Synechococcus, and the filamentous genera, Nodosilinea, Leptolyngbya, Pseudanabaena and Romeria, were assessed in eight human tumor cell lines. First, a crude extract was obtained by dichloromethane:methanol extraction, and from it, three fractions were separated in a Si column chromatography. The crude extract and fractions were tested in eight human cancer cell lines for cell viability/toxicity, accessed with the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) and lactic dehydrogenase release (LDH) assays. Eight point nine percent of the strains revealed strong cytotoxicity; 17.8% showed moderate cytotoxicity, and 14.3% assays showed low toxicity. The results obtained revealed that the studied genera of marine cyanobacteria are a promising source of novel compounds with potential anticancer activity and highlight the interest in also exploring the smaller filamentous and picoplanktonic genera of cyanobacteria.

This paper presents a modified Particle Swarm Optimization (PSO) methodology to solve the problem of energy resources management with high penetration of distributed generation and Electric Vehicles (EVs) with gridable capability (V2G). The objective of the day-ahead scheduling problem in this work is to minimize operation costs, namely energy costs, regarding he management of these resources in the smart grid context. The modifications applied to the PSO aimed to improve its adequacy to solve the mentioned problem. The proposed Application Specific Modified Particle Swarm Optimization (ASMPSO) includes an intelligent mechanism to adjust velocity limits during the search process, as well as self-parameterization of PSO parameters making it more user-independent. It presents better robustness and convergence characteristics compared with the tested PSO variants as well as better constraint handling. This enables its use for addressing real world large-scale problems in much shorter times than the deterministic methods, providing system operators with adequate decision support and achieving efficient resource scheduling, even when a significant number of alternative scenarios should be considered. The paper includes two realistic case studies with different penetration of gridable vehicles (1000 and 2000). The proposed methodology is about 2600 times faster than Mixed-Integer Non-Linear Programming (MINLP) reference technique, reducing the time required from 25 h to 36 s for the scenario with 2000 vehicles, with about one percent of difference in the objective function cost value.

OBJECTIVE To analyze cervical and breast cancer mortality in Brazil according to socioeconomic and welfare indicators. METHODS Data on breast and cervical cancer mortality covering a 30-year period (1980-2010) were analyzed. The data were obtained from the National Mortality Database, population data from the Brazilian Institute of Geography and Statistics database, and socioeconomic and welfare information from the Institute of Applied Economic Research. Moving averages were calculated, disaggregated by capital city and municipality. The annual percent change in mortality rates was estimated by segmented linear regression using the joinpoint method. Pearson’s correlation coefficients were conducted between average mortality rate at the end of the three-year period and selected indicators in the state capital and each Brazilian state. RESULTS There was a decline in cervical cancer mortality rates throughout the period studied, except in municipalities outside of the capitals in the North and Northeast. There was a decrease in breast cancer mortality in the capitals from the end of the 1990s onwards. Favorable socioeconomic indicators were inversely correlated with cervical cancer mortality. A strong direct correlation was found with favorable indicators and an inverse correlation with fertility rate and breast cancer mortality in inner cities. CONCLUSIONS There is an ongoing dynamic process of increased risk of cervical and breast cancer and attenuation of mortality because of increased, albeit unequal, access to and provision of screening, diagnosis and treatment.

The IEEE 802.15.4 has been adopted as a communication protocol standard for Low-Rate Wireless Private Area Networks (LRWPANs). While it appears as a promising candidate solution for Wireless Sensor Networks (WSNs), its adequacy must be carefully evaluated. In this paper, we analyze the performance limits of the slotted CSMA/CA medium access control (MAC) mechanism in the beacon-enabled mode for broadcast transmissions in WSNs. The motivation for evaluating the beacon-enabled mode is due to its flexibility and potential for WSN applications as compared to the non-beacon enabled mode. Our analysis is based on an accurate simulation model of the slotted CSMA/CA mechanism on top of a realistic physical layer, with respect to the IEEE 802.15.4 standard specification. The performance of the slotted CSMA/CA is evaluated and analyzed for different network settings to understand the impact of the protocol attributes (superframe order, beacon order and backoff exponent), the number of nodes and the data frame size on the network performance, namely in terms of throughput (S), average delay (D) and probability of success (Ps). We also analytically evaluate the impact of the slotted CSMA/CA overheads on the saturation throughput. We introduce the concept of utility (U) as a combination of two or more metrics, to determine the best offered load range for an optimal behavior of the network. We show that the optimal network performance using slotted CSMA/CA occurs in the range of 35% to 60% with respect to an utility function proportional to the network throughput (S) divided by the average delay (D).

OBJECTIVE To analyze whether sociodemographic, occupational, and health-related data are associated with the use of hearing protection devices at work, according to gender. METHODS A cross-sectional study was conducted in 2006, using a random sample of 2,429 workers, aged between 18 and 65 years old, from residential sub-areas in Salvador, BA, Northeastern Brazil. Questionnaires were used to obtain sociodemographic, occupational, and health-related data. Workers who reported that they worked in places where they needed to shout in order to be heard were considered to be exposed to noise. Exposed workers were asked whether they used hearing protection devices, and if so, how frequently. Analyses were conducted according to gender, with estimates made about prevalence of the use of hearing protection devices, prevalence ratios, and their respective 95% confidence intervals. RESULTS Twelve percent (12.3%) of study subjects reported that they were exposed to noise while working. Prevalence of the use of hearing protection devices was 59.3% for men and 21.4% for women. Men from higher socioeconomic levels (PR = 1.47; 95%CI 1.14;1.90) and who had previous audiometric tests (PR = 1.47; 95%CI 1.15;1.88) were more likely to use hearing protection devices. For women, greater perceived safety was associated with the use of protection devices (PR = 2.92; 95%CI 1.34;6.34). This perception was specifically related to the presence of supervisors committed to safety (PR = 2.09; 95%CI 1.04;4.21), the existence of clear rules to prevent workplace injuries (PR = 2.81; 95%CI 1.41;5.59), and whether they were informed about workplace safety (PR = 2.42; 95%CI 1.23;4.76). CONCLUSIONS There is a gender bias regarding the use of hearing protection devices that is less favorable to women. The use of such devices among women is positively influenced by their perception of a safe workplace, suggesting that gender should be considered as a factor in hearing conservation programs.

Introdução: O CPAP nasal é o tratamento de eleição para os pacientes com Síndrome da Apneia Obstrutiva do Sono (SAOS). Com a máscara nasal podem ocorrer fugas de ar pela boca, que podem por em causa a aderência do paciente ao tratamento devido muitas vezes ao desconforto que provocam, ao aumento do trabalho respiratório e por afectarem a qualidade do sono. Objectivos: Este estudo tem como principal objectivo verificar a eficácia da banda submentoniana e da máscara facial na correcção das fugas pela boca em pacientes com SAOS. Métodos e Participantes: Uma amostra de conveniência de 15 pacientes (8 homens) com SAOS e a fazerem CPAP com máscara nasal, foi recrutada. Foram divididos em dois grupos A e B, onde no grupo A se colocou a banda submentoniana e no grupo B se alterou a interface para máscara facial. Medidas e Resultados: As variáveis avaliadas neste estudo foram as fugas, o IAH, o percentil 95 da pressão de tratamento, a Sa,O2 e os efeitos adversos das duas intervenções. O nível de fugas reduziu no grupo A de 38±11,27 para 24,55±14,30L/min (p=0,002) e no grupo B de 34,34±16,50 para 18,51±16,22L/min (p=0,008). O IAH aumentou no grupo B de 2,60±2,13 para 4,41±3,88 (p=0,016). Relativamente ao percentil 95 da pressão de tratamento aumentou nos dois grupos (Grupo A de 10,15±2,63 para 12,08±2,45cmH2O (p=0,008) e no Grupo B 10,51±1,88 para 12,64±1,29cmH2O (p=0,008)). A Sa,O2 mínima aumentou e o tempo<90% diminui no grupo A com p=0,008, p=0,031, respectivamente. Quanto ao uso auto-reportado verificaram-se poucos efeitos adversos, salientando-se apenas a facilidade de colocação da banda submentoniana, a secura da boca nos dois grupos, a pressão no queixo provocada pela banda e a dor no dorso do nariz provocada pela máscara facial. Conclusão: Ambas as estratégias provaram ser mais eficazes a reduzir a fuga que a máscara nasal. Foram bem toleradas e com poucos efeitos adversos.

Density-dependent effects, both positive or negative, can have an important impact on the population dynamics of species by modifying their population per-capita growth rates. An important type of such density-dependent factors is given by the so-called Allee effects, widely studied in theoretical and field population biology. In this study, we analyze two discrete single population models with overcompensating density-dependence and Allee effects due to predator saturation and mating limitation using symbolic dynamics theory. We focus on the scenarios of persistence and bistability, in which the species dynamics can be chaotic. For the chaotic regimes, we compute the topological entropy as well as the Lyapunov exponent under ecological key parameters and different initial conditions. We also provide co-dimension two bifurcation diagrams for both systems computing the periods of the orbits, also characterizing the period-ordering routes toward the boundary crisis responsible for species extinction via transient chaos. Our results show that the topological entropy increases as we approach to the parametric regions involving transient chaos, being maximum when the full shift R(L)(infinity) occurs, and the system enters into the essential extinction regime. Finally, we characterize analytically, using a complex variable approach, and numerically the inverse square-root scaling law arising in the vicinity of a saddle-node bifurcation responsible for the extinction scenario in the two studied models. Este projeto foi realizado na Empresa Fernando Jesus Mourão & Cª Lda, durante o ano de 2012, e teve como objetivo a melhoria do processo produtivo. Esta melhoria assentou na identificação de possíveis pontos de desperdício, no estudo e avaliação de soluções e por fim na definição de propostas de melhoria globais que visem ajustar o fluxo do processo produtivo à procura e simultaneamente minimizar custos operacionais. De forma a atingir estas metas, foi feito um levantamento exaustivo do processo produtivo e das respetivas operações. Neste estudo, apesar do elevado numero de oportunidades de melhoria, foi possível identificar duas áreas que se revelaram criticas e fundamentais para a introdução da filosofia de melhoria continua na empresa. Estas áreas que são o objeto de estudo desta dissertação são: o corte laser e a organização do pavilhão 3. Uma vez identificados as áreas críticas, procedeu-se a uma análise e definição das ações de melhoria a empreender de modo a melhorar o fluxo produtivo e a sua organização, e simultaneamente reduzir custos. Na área de corte laser procurou-se implementar a filosofia de produção “Lean” nomeadamente através da ferramenta de análise PDCA (Plan-Do-Check-Act), como ferramenta auxiliar do estudo para elaborar um plano de ação, implementar as ações, analisar os resultados e procurar a manutenção dos mesmos. Na intervenção do pavilhão 3 foi usada a técnica dos 5S para organizar e agilizar o funcionamento do mesmo. Todo o trabalho assenta na filosofia Lean e nos seus princípios, tendo-se por isso utilizado as ferramentas especificas Lean na concretização das tarefas executadas no seu âmbito. Para medir os resultados, antes e depois das ações implementadas, foi utilizada o indicador de eficiência (Overall Equipment Effectiveness) para o caso do processo de corte laser. Após a implementação das medidas definidas no processo de corte laser foi possível observar uma melhoria do indicador OEE de cerca de 20 por cento. Por outro lado as melhorias implementadas através da ferramenta dos 5S no pavilhão 3, trouxe ganhos visíveis de aumento de produtividade que beneficiaram o tempo de resposta da empresa.
