Abstract Background Cardiovascular disease is the leading cause of death in Brazil, and hypertension is its major risk factor. The benefit of its drug treatment to prevent major cardiovascular events was consistently demonstrated. Angiotensin-receptor blockers (ARB) have been the preferential drugs in the management of hypertension worldwide, despite the absence of any consistent evidence of advantage over older agents, and the concern that they may be associated with lower renal protection and risk for cancer. Diuretics are as efficacious as other agents, are well tolerated, have longer duration of action and low cost, but have been scarcely compared with ARBs. A study comparing diuretic and ARB is therefore warranted. Methods/design This is a randomized, double-blind, clinical trial, comparing the association of chlorthalidone and amiloride with losartan as first drug option in patients aged 30 to 70 years, with stage I hypertension. The primary outcomes will be variation of blood pressure by time, adverse events and development or worsening of microalbuminuria and of left ventricular hypertrophy in the EKG. The secondary outcomes will be fatal or non-fatal cardiovascular events: myocardial infarction, stroke, heart failure, evidence of new subclinical atherosclerosis and sudden death. The study will last 18 months. The sample size will be of 1200 participants for group in order to confer enough power to test for all primary outcomes. The project was approved by the Ethics committee of each participating institution. Discussion The putative pleiotropic effects of ARB agents, particularly renal protection, have been disputed, and they have been scarcely compared with diuretics in large clinical trials, despite that they have been at least as efficacious as newer agents in managing hypertension. Even if the null hypothesis is not rejected, the information will be useful for health care policy to treat hypertension in Brazil. OBJETIVO: Investigar o desempenho de crianças ao final do Ciclo I do Ensino Fundamental em memória operacional fonológica e consciência fonológica, bem como a possível relação entre essas habilidades nesta faixa de escolaridade. MÉTODOS: O grupo de pesquisa foi composto por 29 sujeitos de ambos os gêneros, com média de idade de 10 anos, todos regularmente matriculados no 5º ano do Ensino Fundamental com ausência de alterações de linguagem oral e/ou escrita. Foi realizada a avaliação da memória operacional fonológica com a utilização do Teste de Repetição de Pseudopalavras e, posteriormente, utilizou-se o Instrumento de Avaliação Sequencial - CONFIAS para avaliar a consciência fonológica. RESULTADOS: Os escolares apresentaram desempenho adequado na memória operacional fonológica independente da similaridade da pseudopalavra. Para a consciência fonológica, observou-se desempenho melhor no nível silábico e inferior ao esperado para o nível fonêmico. Apesar de muitos estudos afirmarem a correlação entre a memória operacional fonológica e a consciência fonológica, esta não foi observada nesta amostra. CONCLUSÃO: A ausência de correlação encontrada entre essas habilidades traz reflexões quanto a possíveis fatores extrínsecos que podem influenciar o desempenho em consciência fonológica.

In the central nervous system, zinc is released along with glutamate during neurotransmission and, in excess, can promote neuronal death. Experimental studies have shown that metallothioneins I/II (MT-I/II), which chelate free zinc, can affect seizures and reduce neuronal death after status epilepticus. Our aim was to evaluate the expression of MT-I/II in the hippocampus of patients with temporal lobe epilepsy (TLE). Hippocampi from patients with pharmacoresistant mesial temporal lobe epilepsy (MTLE) were evaluated for expression of MT-I/II and for neuronal, astroglial, and microglial populations. Compared to control cases, MTLE group displayed widespread increase in MT-I/II expression, astrogliosis and reduced neuronal population. MT-I/II levels did not correlate with any clinical variables, but patients with secondary generalized seizures (SGS) had less MT-I/II than patients without SGS. In conclusion, MT-I/II expression was increased in hippocampi from MTLE patients and our data suggest that it may be associated with different seizure spread patterns.

We conducted a phase I, double-blind, placebo-controlled trial to evaluate a new 5-valent oral rotavirus vaccine’s safety and immunogenicity profiles. Subjects were randomly assigned to receive 3 orally administered doses of a live-attenuated human-bovine (UK) reassortant rotavirus vaccine, containing five viral antigens (G1, G2, G3, G4 and G9), or a placebo. The frequency and severity of adverse events were assessed. Immunogenicity was evaluated by the titers of anti-rotavirus IgA and the presence of neutralizing antibodies anti-rotavirus. No severe adverse events were observed. There was no difference in the frequency of mild adverse events between experimental and control groups. The proportion of seroconversion was consistently higher in the vaccine group, for all serotypes, after each one of the doses. The 5-valent vaccine has shown a good profile of safety and immunogenicity in this small sample of adult volunteers.

Methods We conducted a phase I, multicenter, randomized, double-blind, placebo-controlled, multi-arm (10) parallel study involving healthy adults to evaluate the safety and immunogenicity of influenza A (H1N1) 2009 non-adjuvanted and adjuvanted candidate vaccines. Subjects received two intramuscular injections of one of the candidate vaccines administered 21 days apart. Antibody responses were measured by means of hemagglutination-inhibition assay before and 21 days after each vaccination. The three co-primary immunogenicity end points were the proportion of seroprotection >70%, seroconversion >40%, and the factor increase in the geometric mean titer >2.5. Results A total of 266 participants were enrolled into the study. No deaths or serious adverse events were reported. The most commonly solicited local and systemic adverse events were injection-site pain and headache, respectively. Only three subjects (1.1%) reported severe injection-site pain. Four 2009 influenza A (H1N1) inactivated monovalent candidate vaccines that met the three requirements to evaluate influenza protection, after a single dose, were identified: 15 μg of hemagglutinin antigen without adjuvant; 7.5 μg of hemagglutinin antigen with aluminum hydroxide, MPL and squalene; 3.75 μg of hemagglutinin antigen with aluminum hydroxide and MPL; and 3.75 μg of hemagglutinin antigen with aluminum hydroxide and squalene. Conclusions Adjuvant systems can be safely used in influenza vaccines, including the adjuvant monophosphoryl lipid A (MPL) derived from Bordetella pertussis with squalene and aluminum hydroxide, MPL with aluminum hydroxide, and squalene and aluminum hydroxide.

[ES] En este proyecto se ha desarrollado un completo sistema de gestión de restaurantes y fidelización de clientes. Para ello se ha investigado cómo los restaurantes realizan su gestión interna e interactúan con sus clientes para mostrar su oferta gastronómica. Como resultado de esa investigación se propone un sistema de gestión automatizado de las cartas, así como de los pedidos que se realizan tanto en el restaurante como fuera de él. Este sistema facilita al gestor del restaurante la posibilidad de tener de forma organizada sus cartas y menús, así como de conocer de forma directa la opinión de sus clientes. La aplicación ha sido diseñada de forma que se facilite el acceso a todos los trabajadores de los restaurantes y se pueda restringir su acceso mediante asignación de roles. Además, se facilita a los restaurantes un sistema completo de estadísticas para ver, de forma gráfica, la evolución tanto en ventas como en opiniones y valoraciones de sus clientes. Los clientes tienen la posibilidad de realizar pedidos desde la plataforma y desde el propio restaurante en una carta digitalizada, así como de conocer y descubrir todos los restaurantes registrados con las valoraciones y comentarios de otros usuarios. Todas estas funcionalidades son posibles gracias a la integración de un portal web y de dos aplicaciones diseñadas para dispositivos móviles (adaptadas a los tamaños de pantalla y resolución más usados actualmente) que se comunican e interactúan de forma directa.

[ES] El presente proyecto final de carrera surge con la idea de cubrir una necesidad social a un servicio no existente en muchos contextos. "I-Found" es un concepto de oficina virtual de objetos perdidos, con el añadido de que no solo gestiona objetos, sino también personas y animales (lo que en este proyecto se denomina un "OPA"). Además ofrece la posibilidad de poner en contacto directo y sin intermediarios a personas que han perdido o les ha sido robado un "OPA", con personas que los haya encontrado y viceversa, dentro de cualquier ámbito posible. Los objetivos principales del proyecto son : el aprendizaje de la plataforma Android para dispositivos móviles y el desarrollo de una aplicación, basada en el concepto "I-Found", totalmente funcional, experimental, libre y gratuita, restringida al ámbito geográfico e institucional de la Universidad de Las Palmas de Gran Canaria. Por este motivo, la app desarrollada se denomina "I-Found@ULPGC", leído en inglés como "I-Found at ULPGC". Esta app demuestra la capacidad de personalización del concepto "I-Found" a múltiples contextos institucionales o geográficos.
