In an overview of some of the central issues concerning the impact and effects of new technology in adolescence, this article questions the reality of the net generation before considering the interplay of new and old technologies, the internet as both communication and lifestyle resource, and newer technologies like text messaging and webcams.

Examples of recent research into adolescent risk behaviors from a variety of disciplines and methodologies, denoting the range of researchers interested in this area and whose interest in communication and language articulates and exemplifies the extent of the field, are surveyed in this article.

Many older adults with hearing impairment continue to have substantial communication difficulties after being fitted with hearing aids, and many do not choose to wear hearing aids. Two group communication education programs aimed at such older people are described. The 'Keep on Talking' program has a health promotion focus, and is aimed at maintaining communication for older adults living in the community. An experimental group (n=120) attended the program, and a control group (n=130) received a communication assessment but no intervention. Significant improvements were found in the experimental participants in terms of knowledge about communication changes with age and about strategies to maintain communication skills. At the follow-up evaluation at 1 year, 45% of the experimental group, compared to 10% of the control group, had acted to improve their communication skills. The 'Active Communication Education' program focuses on the development of problem-solving strategies to improve communication in everyday life situations. Preliminary outcomes have been assessed on a small scale (n=14) to date. It is concluded that communication programs represent an important adjunct to, or supplement for, the traditional approach that focuses on hearing aid fitting.

Patterns of vocal rehabilitation for 37 pharyngolaryngectomy patients and 55 total laryngectomy patients over a 5-year period were compared. An electrolarynx (EL) was introduced as the initial communication mode immediately after surgery for 98% of patients, with 30% of pharyngolaryngectomy and 74% of laryngectomy patients subsequently developing tracheoesophageal speech (TES) as their primary mode of communication. Follow-up with 14 of 37 pharyngolaryngectomy patients and 36 of 55 laryngectomy patients was conducted 1-6 years following surgery and revealed that 90% of the pharyngolaryngectomy patients maintained the use of TES in the long term compared to 69% of the laryngectomy group. Long-term outcomes relating to communication disability and handicap did not differ significantly between the two surgical groups, however the laryngectomy patients had significantly higher levels of wellbeing. Across the whole group of patients, statistical comparison revealed that patients using TES had significantly lower levels of disability, handicap and distress than EL users. Considering that lower levels of disability, handicap and distress are associated with TES, and the data supports that suitably selected patients can maintain functional TES in the long term, increased application of this form of communication rehabilitation should be encouraged where viable for the pharyngolaryngectomy population. Background: Increasingly there is a call from clinicians and researchers for measures that document the impact of aphasia on a person's everyday communication. Do existing assessments of communication disability adequately sample communication activities relevant to our clients? Communication skills and networks change with age. A need exists to determine the everyday communication activities of older people and in particular those with aphasia. Aims: The primary aim of this study was to describe and compare the everyday communication activities of older people with aphasia and healthy older people who are living in the community. A secondary aim was to investigate the content validity of the American Speech-Language Hearing Association Functional Assessment of Communication Skills for Adults (ASHA FACS, 1997) for older Australians. Methods & Procedures: Naturalistic observation was the method of choice for detailing the everyday communication of 15 older people with chronic aphasia following stroke and a matched group of 15 healthy older people who were living in the community. Researchers, in the role of participant observer, took field notes for 8 hours, over three occasions within a week. A total of 240 hours of observation have been coded in terms of communication activity, topic, communication partners, and place of communication. A brief 5-day diary served to check the representativeness of the observational data. After each hour of observation, the researcher checked which ASHA FACS items had been observed. Outcomes & Results: Naturalistic observation provided a rich, rigorous, and systematic methodology for detailing the dynamics and complexities of authentic communication. The most common communication activities for both groups were conversations at home and in social groups. Real-life communication was revealed to serve the dual purposes of transaction and interaction. Results indicate that older people with aphasia engage in similar communication activities to healthy older people although differences were evident in the frequency of communication and in specific activities such as story telling, writing, commenting, and acknowledging. ASHA FACS items were generally relevant to older Australians living in the community. Conclusions: This study demonstrated that communication activity is multifaceted in terms of the type of communication and contextual factors. The observational data describe the effects of aphasia on a person's everyday communication activity and reveal the impact of aphasia on the social functions of communication including sharing information, maintaining and establishing relationships, and telling one's story. Functional communication assessment requires a greater focus on the interactional and uniquely interpersonal aspects of social communication.

An 8-year-old female neutered Siamese cat was presented with a recent history of incomplete excision of an apocrine gland adenocarcinoma from the palmar aspect of the right antebrachium, just proximal to the carpal joint. There was no evidence of metastasis. Wide surgical excision of the previous surgery site was performed resulting in a soft tissue defect. Partial reconstruction was achieved using digital pad transposition of the first digit (dewclaw), forming a local axial pattern flap that was transposed into the adjacent defect. The remaining defect was closed by primary apposition. The skin flap healed successfully. Some breakdown of the skin closed by primary apposition necessitated open wound management. The cosmetic and functional result of the first digital pad transposition was considered excellent, rendering it a useful means to reconstruct soft tissue defects in the carpal region.

Four spirochete strains were isolated from papillomatous digital dermatitis (PDD) lesions in Iowa dairy cattle and compared with two previously described spirochete strains isolated from dairy cattle in California. These six strains shared an identical 16S ribosomal DNA sequence that was 98% similar to Treponema phagedenis and 99% similar to the uncultivated PDD spirochete sequence DDLK-4. The whole-cell protein profiles resolved by sodium dodecyl sulfate-polyacrylamide gel electrophoresis of these six strains were similar. However, these strains showed differences in the antigenic diversity of lipopolysaccharide (LPS). Genetic diversity was also detected by pulsed-field gel electrophoresis of genomic DNA digests, revealing differences among five of the six strains. Serum immunoglobulin G antibodies from dairy cattle with active PDD lesions reacted with the LPS of all but one PDD spirochete strain. Likewise, peripheral blood mononuclear cells from cattle with active PDD lesions produced blastogenic responses to one of the two California isolates. Both antibody and lymphocyte blastogenic responses were reduced in convalescent dairy cattle, suggesting the immune response to these spirochetes has short duration. These results demonstrate genetic and antigenic diversity among T. phagedenis-like treponemes and provide further evidence for the involvement of these spirochetes in the pathogenesis of PDD.

As novas Tecnologias da Informa????o e Comunica????o (TIC), entre as quais se destacam a internet, as redes de computadores, a transmiss??o via sat??lite e a telefonia m??vel, criaram condi????es para o surgimento de Sociedades do Conhecimento. Nesse contexto, o Estado, por interm??dio do governo eletr??nico, ?? o principal instrumento de que os cidad??os disp??em atualmente para enfrentar os desafios impostos pela globaliza????o, por meio de intera????es in??ditas da sociedade, empresas e governos. O objetivo deste trabalho foi estabelecer rela????es entre a governan??a do setor p??blico e o governo eletr??nico, partindo-se de um marco referencial te??rico das duas ??reas de estudo. Para tanto, foram delineados seus escopos e limites de atua????o, instituindo-se v??nculos, de acordo com a????es do Governo Eletr??nico brasileiro: certifica????o digital; Portal da Transpar??ncia; vota????o eletr??nica; preg??es eletr??nicos e Portal da Previd??ncia Social. Conclui-se que, apesar dos avan??os recentes observados pela an??lise de v??rias dimens??es da governan??a, baseados em iniciativas de governo eletr??nico aqui inventariadas, ainda h?? muito que fazer em dire????o ?? inclus??o social no Brasil, assim como em outros pa??ses. As TIC podem vir a ser um poderoso agente de inclus??o digital, apoiando a governan??a com a cria????o de espa??os virtuais para participa????o democr??tica e di??logo c??vico e expans??o da participa????o em tomada de decis??o coletiva, promovendo a igualdade e a cidadania.

OBJETIVO: objetivo desta pesquisa foi investigar o grau de satisfação entre os usuários de Aparelho de Amplificação Sonora (AAS) de tecnologia analógica e digital. FORMA DE ESTUDO: clínico com coorte transversal. MATERIAL E MÉTODO: A amostra foi constituída de 40 usuários de AAS unilateral, sendo 20 de tecnologia analógica (Grupo I) e 20 de tecnologia digital (Grupo II). Os indivíduos são portadores de perda auditiva neurossensorial, de configuração plana, de grau leve a moderado, com idade entre 40 e 95 anos. O instrumento de avaliação utilizado foi o International Outcome Inventory for Hearing Aids (IOI-HA - Versão em Português) proposto por Cox et al. (2002). RESULTADOS: Os resultados mostraram que os usuários do Grupo II utilizam o AAS por mais tempo diariamente e também apresentam um grau de dificuldade menor em determinadas situações com o uso da amplificação, comparado aos usuários do Grupo I. Os usuários do Grupo II relatam que sua perda auditiva não afeta em nada sua relação com terceiros. CONCLUSÃO: Apesar de os resultados revelarem que os usuários do Grupo I apresentam um handicap auditivo mais acentuado comparado aos usuários do Grupo II, o grau de satisfação com relação à amplificação não apresenta diferença significativa entre os dois grupos.
