A study of health knowledge and practices of prostitutes is presented here. The study took place at the V. D. Center in Saigon, Vietnam. It was designed with the objective of obtaining information to be used in preparing an educational program to be offered to the prostitutes at the Center, and for using, in preparing educational materials with focus on V. D. prevention. The outline of a course is also presented.

A review is presented of the interrelationships between arthropod vectors, the diseases they transmit and agricultural development. Particular attention is given to the effects of deforestation, livestock development and irrigation on the abundance of vectors and changing patterns of diseases such as malaria, trypanosomiases, leishmaniasis, Chagas' and some arboviral infections. The question as whether keeping livestock diverts biting away from people and reduces diseases such as malaria - that is zooprophylaxis, or whether the presence of cattle actually increases biting populations is discussed.

A programme for the control of respiratory diseases in children was conceived for the State of S. Paulo, Brazil, in 1986. Its progress thereafter and the epidemiology of the diseases concerned are examined. Apart from an inquiry into the 64 existing State local health authorities, a sample of 18,255 cases of children assisted by the programme at different levels, including both in-patient and outpatient care, is analysed. Each case record included information about identification (child, doctor and health facility), reasons for calling, diagnoses made and outcome of treatment. Further data were also sought from hospitals and from State mortality records. The programme was found to be poorly implemented in the State but, where implemented, it showed itself capable of resolving problems (only 0.5% of the cases could not be handled) as also of changing ongoing trends (more than 50% reduction in hospital admission rates). Individual assessment of each item of the programme indicated its bottlenecks. Regarding the epidemiology of respiratory diseases, it is observed that the major burden to health services comes from children aged less than five, and that the most important diseases are wheezing illnesses and pneumonia. Morevoer, they were found to be significantly associated (p = 0.000) so that a child in the community presenting wheezing diseases is 5 times more likely to develop pneumonia than a child with any other respiratory diagnosis. Similarly, among the under five deaths it was found that the risk for pneumonia is 3 times greater for children who died presenting wheezing diseases than it is for children with any other sort of diagnosis. In conclusion, the programme is deemed to be efficient and effective but its efficacy is marred by administrative flaws. The successful control of respiratory problems in childhood is related to a proper appreciation of the importance of wheezing diseases.

The results from the need to develop methodologies for performing cost analysis in developing countries, principally in the region of Latin America, were studied. It, furthermore, serves to generate knowledge from an economic evaluation in order to support decision-making related to the organization of health systems, particularly in the efficient use of resources which are allocated for the provision of medical services. Two chronic diseases (breast cancer and cardiac valve disease) and two infections (enteritis and bronchopneumonia) were selected for the study. The results recommend the use of a valid methodology for economic cost analysis of any disease to be studied and the use of this information in the decision-making process.

In this work, 14 primary schools of Lisbon city, Portugal, followed a questionnaire of the ISAAC - International Study of Asthma and Allergies in Childhood Program, in 2009/2010. The questionnaire contained questions to identify children with respiratory diseases (wheeze, asthma and rhinitis). Total particulate matter (TPM) was passively collected inside two classrooms of each of 14 primary schools. Two types of filter matrices were used to collect TPM: Millipore (IsoporeTM) polycarbonate and quartz. Three campaigns were selected for the measurement of TPM: Spring, Autumn and Winter. The highest difference between the two types of filters is that the mass of collected particles was higher in quartz filters than in polycarbonate filters, even if their correlation is excellent. The highest TPM depositions occurred between October 2009 and March 2010, when related with rhinitis proportion. Rhinitis was found to be related to TPM when the data were grouped seasonally and averaged for all the schools. For the data of 2006/2007, the seasonal variation was found to be related to outdoor particle deposition (below 10 μm).

Environmental tobacco smoke (ETS) is recognized as an occupational hazard in the hospitality industry. Although Portuguese legislation banned smoking in most indoor public spaces, it is still allowed in some restaurants/bars, representing a potential risk to the workers’ health, particularly for chronic respiratory diseases. The aims of this work were to characterize biomarkers of early genetic effects and to disclose proteomic signatures associated to occupational exposure to ETS and with potential to predict respiratory diseases development. A detailed lifestyle survey and clinical evaluation (including spirometry) were performed in 81 workers from Lisbon restaurants. ETS exposure was assessed through the level of PM 2.5 in indoor air and the urinary level of cotinine. The plasma samples were immunodepleted and analysed by 2D-SDSPAGE followed by in-gel digestion and LC-MS/MS. DNA lesions and chromosome damage were analysed innlymphocytes and in exfoliated buccal cells from 19 cigarette smokers, 29 involuntary smokers, and 33 non-smokers not exposed to tobacco smoke. Also, the DNA repair capacity was evaluated using an ex vivo challenge comet assay with an alkylating agent (EMS). All workers were considered healthy and recorded normal lung function. Interestingly, following 2D-DIGE-MS (MALDI-TOF/TOF), 61 plasma proteins were found differentially expressed in ETS-exposed subjects, including 38 involved in metabolism, acute-phase respiratory inflammation, and immune or vascular functions. On the other hand, the involuntary smokers showed neither an increased level of DNA/chromosome damage on lymphocytes nor an increased number of micronuclei in buccal cells, when compared to non-exposed non-smokers. Noteworthy, lymphocytes challenge with EMS resulted in a significantly lower level of DNA breaks in ETS-exposed as compared to non-exposed workers (P<0.0001) suggestive of an adaptive response elicited by the previous exposure to low levels of ETS. Overall, changes in proteome may be promising early biomarkers of exposure to ETS. Likewise, alterations of the DNA repair competence observed upon ETS exposure deserves to be further understood. As citopatias mitocondriais constituem um importante grupo de doenças metabólicas de expressão clínica heterogénea, para as quais não existe uma terapia eficaz. A maioria destas doenças é causada por uma disfunção ao nível da fosforilação oxidativa (OXPHOS), originando consequentemente uma deficiente produção de energia. O correto funcionamento da OXPHOS resulta de uma interação coordenada entre o genoma nuclear e mitocondrial. Assim, as doenças mitocondriais podem ser causadas por defeitos moleculares no genoma mitocondrial, no nuclear, ou em ambos, originando as doenças da comunicação intergenómica, que resultam na perda ou na instabilidade do DNA mitocondrial (mtDNA), e podem causar quer deleções múltiplas, quer depleção do genoma mitocondrial. A síndrome da depleção do mtDNA constitui um grupo de doenças raras, autossómicas recessivas, que se manifestam maioritariamente após o nascimento, causando a morte de muitos doentes durante a infância ou início da adolescência devido a uma redução acentuada do número de cópias do mtDNA. Trata-se de uma síndrome fenotipicamente heterogénea, apresentando-se sob três apresentações clínicas: hepatocerebral, miopática e encefalomiopática. A caracterização molecular destes doentes é importante não só para permitir a realização de aconselhamento genético e diagnóstico pré-natal adequados, mas também para melhorar a compreensão da fisiopatologia da doença e as opções terapêuticas.

As porfirias são um grupo de oito doenças metabólicas raras, em resultado de uma deficiência enzimática em cada uma das oito enzimas envolvidas na biossíntese do grupo heme. São doenças maioritariamente hereditárias, mas podem também ser adquiridas aquando da exposição a certos fatores ambientais e/ou patológicos. Estes fatores externos, denominados de porfirinogénicos também têm um papel preponderante no diagnóstico das porfirias, uma vez que mimetizam os sintomas clínicos de um ataque agudo de porfiria, contribuindo para subestimar esta doença, levando a um atraso no diagnóstico e diminuído o sucesso do prognóstico. Os ataques agudos de porfiria, nomeadamente na porfiria aguda intermitente, porfiria variegata, coproporfiria hereditária, e deficiência da desidratase do ácido delta-aminolevulínico, apesar de serem doenças multissistémicas, têm em comum como apresentação clínica, a dor abdominal aguda. A pesquisa de porfobilinogénio (PBG) na urina, através da realização do teste de Hoesch, é uma forma rápida e fácil de excluir a suspeita clínica de porfiria. Pretendemos com este trabalho, alertar para a necessidade de um diagnóstico laboratorial atempado, que pela sua simplicidade poderá descartar ou confirmar se a dor abdominal aguda, tão frequente nas urgências hospitalares, será ou não uma manifestação clínica de um ataque agudo de porfiria. Este estudo contribuirá não só para aumentar o nosso conhecimento acerca destas doenças, como também permitirá uma melhor compreensão dos mecanismos de patogenicidade das porfirias, o qual ainda permanece pouco conhecido.

There are complex and diverse methodological problems involved in the clinical and epidemiological study of respiratory diseases and their etiological factors. The association of urban growth, industrialization and environmental deterioration with respiratory diseases makes it necessary to pay more attention to this research area with a multidisciplinary approach. Appropriate study designs and statistical techniques to analyze and improve our understanding of the pathological events and their causes must be implemented to reduce the growing morbidity and mortality through better preventive actions and health programs. The objective of the article is to review the most common methodological problems in this research area and to present the most available statistical tools used.

O diagnóstico de doença hepática autoimune em doentes com patologia hepática implica a exclusão de outras causas de lesão hepática como vírica, alcoólica, tóxica, devido a alterações genéticas ou metabólicas, esteatose hepática não alcoólica e uma criteriosa avaliação de dados clínicos, bioquímicos, histológicos e colangiográficos especificas destas patologias (Invernizzi et al 2007) O diagnóstico e tratamento precoces destas patologias são fundamentais para a prevenção da alta morbilidade e mortalidade associada a estes doentes. O despiste de patologia hepática autoimune assenta na utilização de testes serológicos para a deteção de autoanticorpos associados a estas patologias. O conhecimento destes testes e a interpretação dos resultados obtidos revelam-se fundamentais para o diagnóstico ou exclusão destas doenças (Beuers 2005). Deste modo, foi objetivo deste trabalho a pesquisa e identificação de autoanticorpos em uso clínico: ANA, AMA, AML, ANCA, Anti-SLA/LP, anti-LKM, anti-LC1 e anti-actina F, em doentes com suspeita de HAI e CBP em que foi excluída causa vírica, alcoólica e tóxica. O trabalho incidiu particularmente na comparação dos resultados do perfil de autoanticorpos de pedidos feitos ao exterior com os resultados obtidos recorrendo à utilização de um novo kit de imunoblot, e assim determinar a relevância da introdução da pesquisa dos novos autoanticorpos e avaliar a relação custo/benefício da implementação do kit BlueDot liver da D-tek® na rotina laboratorial do serviço de Patologia Clínica do Hospital Pedro Hispano. Os resultados encontrados foram de 100% de concordância entre os métodos de imunofluorescência indireta e imunoblot, e Elisa e Imunoblot. Deste modo seria uma boa estratégia a implementação desta última técnica na rotina laboratorial uma vez que proporciona uma rápida disponibilização dos resultados para o clínico, antecipando desta forma o diagnóstico e o início rápido do tratamento em benefício do doente. Por outro lado, quando analisámos a relação custo/beneficio, seria vantajosa a implementação desta técnica uma vez que o laboratório dispõe de capacidade técnica, e o custo de aquisição do kit não excede o valor praticado atualmente correspondendo a uma poupança de 51%.

OBJECTIVE: To assess the lag structure between air pollution exposure and elderly cardiovascular diseases hospital admissions, by gender. METHODS: Health data of people aged 64 years or older was stratified by gender in São Paulo city, Southeastern Brazil, from 1996 to 2001. Daily levels of air pollutants (CO, PM10, O3, NO2, and SO2) , minimum temperature, and relative humidity were also analyzed. It were fitted generalized additive Poisson regressions and used constrained distributed lag models adjusted for long time trend, weekdays, weather and holidays to assess the lagged effects of air pollutants on hospital admissions up to 20 days after exposure. RESULTS: Interquartile range increases in PM10 (26.21 mug/m³) and SO2 (10.73 mug/m³) were associated with 3.17% (95% CI: 2.09-4.25) increase in congestive heart failure and 0.89% (95% CI: 0.18-1.61) increase in total cardiovascular diseases at lag 0, respectively. Effects were higher among female group for most of the analyzed outcomes. Effects of air pollutants for different outcomes and gender groups were predominately acute and some "harvesting" were found. CONLUSIONS: The results show that cardiovascular diseases in São Paulo are strongly affected by air pollution.

OBJECTIVE: To determine health care costs and economic burden of epidemiological changes in diseases related to tobacco consumption. METHODS: A time-series analysis in Mexico (1994-2005) was carried out on seven health interventions: chronic obstructive pulmonary diseases, lung cancer with and without surgical intervention, asthma in smokers and non-smokers, full treatment course with nicotine gum, and full treatment course with nicotine patch. According with Box-Jenkins methodology, probabilistic models were developed to forecast the expected changes in the epidemiologic profile and the expected changes in health care services required for selected interventions. Health care costs were estimated following the instrumentation methods and validated with consensus technique. RESULTS: A comparison of the economic impact in 2006 vs. 2008 showed 20-90% increase in expected cases depending on the disease (p<0.05), and 25-93% increase in financial requirements (p<0.01). The study data suggest that changes in the demand for health services for patients with respiratory diseases related to tobacco consumption will continue showing an increasing trend. CONCLUSIONS: In economic terms, the growing number of cases expected during the study period indicates a process of internal competition and adds an element of intrinsic competition in the management of preventive and curative interventions. The study results support the assumption that if preventive programs remain unchanged, the increasing demands for curative health care may cause great financial and management challenges to the health care system of middle-income countries like Mexico.

Introduction: 188Re is a promising radionuclide for metabolic therapy because of the emission of high energy beta-particles. The development of watersoluble bone-seeking polymers such as PEI-MP (polyethyleneimine, functionalised with methylphosphonate-groups) that might be labeled with 188Re are recent approaches, with a strong potential for bone cancer treatment. The aim of this study was to evaluate the efficacy of 188Re-PEI-MP, as therapeutic agent for osteosarcoma, through in vitro and in vivo models.
