OBJECTIVE: The aim of this study was to conduct a statistical analysis to determine the outcome of conservative treatment after delivery of a first fetus in multiple pregnancy and thus define new prognostic factors. STUDY DESIGN: Multicentre retrospective study involving 12 centers over a 10-year period. RESULTS: Twenty-eight twin pregnancies and seven triplet pregnancies which were managed conservatively. In twin pregnancies, 79% of the delayed-delivery fetuses survived; only 7% of the first delivered fetuses survived. The mean interval between deliveries was 47 days. No statistical difference was found concerning cerclage, antibiotic therapy, tocolysis and hospitalization. Earlier delivery of the first twin and premature rupture of membranes for the second twin were significantly related to a longer interval between deliveries. CONCLUSION: Delayed delivery in multifetal pregnancies can be successful if there are no contraindications and these pregnancies are managed in a tertiary perinatal center. Publications limited to successful cases have undoubtedly introduced some bias in assessment.

El dolor pélvico crónico es un motivo frecuente de consulta, que está presente hasta en un 40% de las mujeres, y que afecta de manera considerable su calidad de vida. El síndrome de congestión pélvica asociado a varices constituye un factor etiológico de esta patología; sin embargo, existe poca evidencia que describa la eficacia y seguridad de la técnica quirúrgica abierta. Este estudio evalúa los resultados clínicos de la cirugía abierta para el manejo de las várices pélvicas. Metodología: se realizó un estudio descriptivo de corte transversal con 94 pacientes con varices gonadales, sometidas a cirugía abierta. Se evaluaron los hallazgos del método diagnóstico, los hallazgos intraoperatorios, y la evolución post operatoria. Resultados: El 85% de las pacientes presentó reflujo bilateral, y 15% reflujo unilateral. Más del 70% reportó presencia de dolor pélvico mayor a 6 meses, dispareunia, dismenorrea, y síntomas urinarios. Se demostraron diferencias estadísticamente significativas cuando se compararon los diámetros reportados en el dúplex, con los medidos intra operatoriamente, específicamente en las mediciones de la vena gonadal derecha, tanto en pacientes con reflujo unilateral (p=0.022) como bilateral (p= 0.017). El 92% de las pacientes presentó mejoría de los síntomas posterior al tratamiento quirúrgico. Conclusiones: La ligadura de las venas ováricas por cirugía abierta es una alternativa terapéutica para el tratamiento de las várices pélvicas, con resultados que demuestran mejoría sintomática importante. El dúplex constituye un método diagnóstico útil, aunque el diámetro de las venas gonadales puede ser subestimado. Introducción: La aparición de vasodilatación aislada de la arteria cerebral media en fetos pequeños para la edad gestacional sin otros cambios en el doppler puede interpretarse como fisiológica o podría ser la manifestación inicial de una restricción de crecimiento intrauterino de inicio tardío. Se pretende evaluar la asociación de la disminución del índice de pulsatibilidad de la arteria cerebral media, como predictor de desenlaces perinatales adversos, en fetos con bajo peso para edad gestacional. Metodología: Se realizó un estudio de cohorte analítica de temporalidad histórica para determinar si el hallazgo de disminución de la pulsatibilidad en el doppler de arteria cerebral media se asocia con el pronóstico perinatal adverso, en fetos pequeños para edad gestacional mediante un muestreo no probabilístico. Resultados: Se recolectaron un total de 325 flujometría doppler de fetos pequeños para edad gestacional. El riesgo de parto pretérmino fue RR 2.6 IC95% 1.6-4.1, de hospitalización fue RR 1.4 IC95%1.1-1.9 y de muerte fue 2.1 IC95%1.5-3.2 cuando hay índice de pulsatilidad alterada en la arteria cerebral media. La regresión logística mostró que el riesgo de desenlaces desfavorables con alteraciones en la arteria cerebral media fue de RR 4.2 IC95% 2.5-7.1 ajustado por edad materna, edad gestacional y bajo peso al nacer. Discusión Los pacientes expuestos presentan mayor riesgo de desenlaces desfavorables con diferencias significativas, no así en otros estudios publicados. El presente estudio muestra asociaciones significativas que debe ser evaluada con estudios más amplios.

Se realizó un estudio genético – poblacional en dos grupos etarios de población colombiana con la finalidad de evaluar las diferencias genéticas relacionadas con el polimorfismo MTHFR 677CT en busca de eventos genéticos que soporten la persistencia de este polimorfismo en la especie humana debido que este ha sido asociado con múltiples enfermedades. De esta manera se genotipificaron los individuos, se analizaron los genotipos, frecuencias alélicas y se realizaron diferentes pruebas genéticas-poblacionales. Contrario a lo observado en poblaciones Colombianas revisadas se identificó la ausencia del Equilibrio Hardy-Weinberg en el grupo de los niños y estructuras poblacionales entre los adultos lo que sugiere diferentes historias demográficas y culturales entre estos dos grupos poblacionales al tiempo, lo que soporta la hipótesis de un evento de selección sobre el polimorfismo en nuestra población. De igual manera nuestros datos fueron analizados junto con estudios previos a nivel nacional y mundial lo cual sustenta que el posible evento selectivo es debido a que el aporte de ácido fólico se ha incrementado durante las últimas dos décadas como consecuencia de las campañas de fortificación de las harinas y suplementación a las embarazadas con ácido fólico, por lo tanto aquí se propone un modelo de selección que se ajusta a los datos encontrados en este trabajo se establece una relación entre los patrones nutricionales de la especie humana a través de la historia que explica las diferencias en frecuencias de este polimorfismo a nivel espacial y temporal.

Objectives: To evaluate risk factors for recurrence of carcinoma of the uterine cervix among women who had undergone radical hysterectomy without pelvic lymph node metastasis, while taking into consideration not only the classical histopathological factors but also sociodemographic, clinical and treatment-related factors. Study design: This was an exploratory analysis on 233 women with carcinoma of the uterine cervix (stages IB and IIA) who were treated by means of radical hysterectomy and pelvic lymphadenectomy, with free surgical margins and without lymph node metastases on conventional histopathological examination. Women with histologically normal lymph nodes but with micrometastases in the immunohistochemical analysis (AE1/AE3) were excluded. Disease-free survival for sociodemographic, clinical and histopathological variables was calculated using the Kaplan-Meier method. The Cox proportional hazards model was used to identify the independent risk factors for recurrence. Results: Twenty-seven recurrences were recorded (11.6%), of which 18 were pelvic, four were distant, four were pelvic + distant and one was of unknown location. The five-year disease-free survival rate among the study population was 88.4%. The independent risk factors for recurrence in the multivariate analysis were: postmenopausal status (HR 14.1; 95% CI: 3.7-53.6; P < 0.001), absence of or slight inflammatory reaction (HR 7.9; 95% CI: 1.7-36.5; P = 0.008) and invasion of the deepest third of the cervix (FIR 6.1; 95% CI: 1.3-29.1; P = 0.021). Postoperative radiotherapy was identified as a protective factor against recurrence (HR 0.02; 95% CI: 0.001-0.25; P = 0.003). Conclusion: Postmenopausal status is a possible independent risk factor for recurrence even when adjusted for classical prognostic factors (such as tumour size, depth of turnout invasion, capillary embolisation) and treatment-related factors (period of treatment and postoperative radiotherapy status). Vriesea minarum is a rupiculous bromeliad species, with naturally fragmented populations, restricted to the Iron Quadrangle, Minas Gerais, Brazil. It is a threatened species, which is suffering from habitat loss due to the growth of cities and mining activities. The knowledge of genetic variability in plant populations is one of the main branches of conservation genetics, linking genetic data to conservation strategies while the knowledge about plant reproductive biology can aid in understanding key aspects of their life story, as well as in the comprehension of their distribution and survival strategies. Thus, the study of diversity, richness, and genetic structure, as well as the reproductive biology of populations of V. minarum can contribute to the development of conservation actions. Chapter 1 presents the transferability of 14 microsatellite loci for V. minarum. Among the results of this chapter, we highlight the successful transferability of 10 microsatellite loci described for other species of Bromeliaceae, all of which are polymorphic. In Chapter 2, we present the genetic analyses of 12 populations of V. minarum that are distributed throughout the Iron Quadrangle. We used the 10 microsatellite loci tested in Chapter 1. The results show a low population structuring (Fst = 0.088), but with different values of genetic richness (mean = 2.566) and gene diversity (mean = 0.635) for all populations; and a high inbreeding coefficient (Gis = 0.376). These may be the result of pollinators action and/or efficient seed dispersal, thus allowing a high connectivity among populations of naturally fragmented outcrops. The reproductive biology and floral morphology of a population of V. minarum, located in the Parque Estadual da Serra do Rola-Moça, are studied in Chapter 3. This reserve is the only public environmental protection area where the species occurs. As a result of field experiments and observations, we found that the species has its flowering period from January to March, with flowers that last for two days and that it has a mixed pollination syndrome. It is primarily alogamous, but also has the capacity to be self-ferilized. It is expected that data obtained in chapters 1, 2 and 3 serve as basis for other studies with species from the ferruginous rocky fields, since until now, to our knowledge, there are no other survey of endemic species from the Iron Quadrangle, seeking to merge the genetic knowledge, with the data of the reproductive biology, with the ultimate aim of biodiversity conservation. Considering the great habitat loss for the species by mining, it becomes crucial to analyze the creation of new protected areas for its conservation

Objective: To evaluate whether intracytoplasmic morphologically selected sperm injection (IMSI) could influence early paternal effects by observing embryo quality at day 2.Study design: The study included 30 couples with at least one of the following criteria: male factor infertility, at least 2 previous failures of implantation or previous miscarriages after IVF/ICSI. Sibling oocytes of each patient were randomly assigned to either the ICSI group or the IMSI group. For IMSI, spermatozoa were selected at 8400x magnification through an inverted microscope equipped with Nomarski differential interference contrast optics, Uplan Apo 100x oil/1.35 objective lens and variable zoom lens. For conventional ICSI, spermatozoa were selected at 400x magnification. An embryo was defined as top quality if there were four identical blastomeres on day 2 with no fragments or multinucleation of blastomeres. Data were analysed using the Wilcoxon and chi-squared tests. The significance level was set at P < 0.05. The variables were analysed in relation to the general population and the subpopulations with or without male factor.Results: A total of 331 MII oocytes (30 oocyte retrievals) were selected and injected by the ICSI (n: 172) or IMSI (n: 159) procedure. For IMSI, only spermatozoa classified as morphologically normal at high magnification were used. No differences (P > 0.05) in fertilisation rate (ICSI: 70.9%; IMSI: 70.4%), early embryo cleavage rate (ICSI: 66.9%; IMSI: 60.4%) or cleavage rate (ICSI: 99.2%; IMSI: 99.1%) were observed. on day 2, as compared to ICSI, IMSI provided a similar proportion of top quality embryos (ICSI: 57.8%; IMSI: 52.2%; P > 0.05). These results were not influenced by the presence or absence of male factor.Conclusion: In terms of embryo quality at day 2, IMSI had the same performance as conventional ICSI. However, we cannot exclude the possibility that IMSI effects occur only as a positive later paternal effect. (C) 2010 Elsevier B.V. All rights reserved.

Objective: The objective was to analyze the effect of raloxifene oil the vaginal epithelium of postmenopausal women.Study design: In this non-randomized clinical trial, 80 women (mean age = 60.6 years) were prospectively studied. Forty patients received 60 mg/day of raloxifene (RG), and 40 women constituted it non-treated control group (CG), paired by age and time since menopause. The treated group consisted of patients with osteoporosis of the lumbar spine. Those with a diagnosis of infection ill the lower genital tract and using hormone therapy (HT) up to 6 months prior to the study were excluded. Vaginal smears were collected at baseline and after 6 months of intervention. The vaginal maturation value (VMV) was determined, and counts of superficial, intermediate and parabasal cells were performed. Smears were analyzed by only one cytopathologist who was blinded to patient data. The t-test, Wilcoxon test, and Chi-Squared test were used in the statistical analysis.Results: The study groups were homogeneous regarding age, time since menopause, parity, HT use, smoking, and body mass index. No statistically significant differences were observed in VMV median values (RG, 39.7 and 35.7; CG, 50.0 and 50.0, respectively) or in the percentage of superficial, intermediate and parabasal cells between the groups at baseline and after 6 months (p > 0.05). There was no significant correlation between VMV and age, time since menopause, previous HT use, or body mass index, in either of the groups.Conclusion: Treatment with raloxifene for 6 months has no effect on the maturation of the vaginal epithelium ill postmenopausal women with osteoporosis. Objectives: To investigate the occurrence and severity of lymphoedema of the lower extremities (LLE), quality of life (QoL), and urinary and sexual dysfunction in women with vulvar cancer submitted to surgical treatment.Study design: Twenty-eight patients with vulvar cancer submitted to vulvectomy and inguinofemoral lymphadenectomy and 28 healthy, age-matched women (control group) were evaluated. The occurrence and severity of LLE were determined by Miller's Clinical Evaluation. QoL, urinary function and sexual function were assessed by the EORTC QLQ-C30, SF-ICIQ and FSFI questionnaires, respectively. The differences between groups and correlations were assessed using Student's r-test, Chi-squared test, Mann-Whitney U-test and Spearman's rho test.Results: The groups were similar in terms of marital status, educational status, menopausal status, hormone therapy and height. The occurrence and severity of LLE were higher in women with vulvar cancer compared with the control group (p < 0.001 and p = 0.003, respectively). A significant association was found between the severity of LLE and advanced age (p = 0.04), and the severity of LLE and higher body mass index (BMI; p = 0.04) in patients with vulvar cancer. In the patients with vulvar cancer, there was a significant correlation between the severity of LLE and worse QoL in the following domains: physical, cognitive, emotional, social, fatigue, pain, sleep and financial questions (p < 0.05). There was no difference in urinary function between the two groups (p = 0.113). Age and number of deliveries were the only variables associated with the occurrence of urinary incontinence (p = 0.01). Urinary incontinence was present in women with a mean age of 74.9 +/- 4.6 years and a mean of 7.3 +/- 1.3 normal deliveries. There was no difference between the groups in terms of the sexual function. Multivariate analysis showed an association between sexual function and age (p = 0.01), and sexual function and being in a stable relationship (p = 0.02).Conclusion: Patients submitted to vulvectomy or inguinofemoral lymphadenectomy for vulvar cancer are at higher risk of developing LLE compared with healthy, age-matched women. This has a negative effect on QoL, but does not interfere with urinary or sexual function. (C) 2012 Elsevier B.V. All rights reserved.

Objective: To evaluate levels of proinflammatory cytokines and sialidase activity in aerobic vaginitis (AV) in relation to normal vaginal flora and bacterial vaginosis (BV). Study design: In this cross-sectional study, a total of 682 consecutive non-pregnant women attending the gynecology service were assessed and 408 women were included. Vaginal rinsing samples were collected from 223 women with microscopic finding of BV (n = 98), aerobic vaginitis (n = 25) and normal flora (n = 100). Samples were tested for interleukin (IL)-1β, IL-6, IL-8, tumor necrosis factor (TNF)-α, and sialidase activity. Results: Compared to women with normal flora, vaginal levels of IL-1β were highly increased in both BV and AV (p < 0.0001). Significantly higher vaginal IL-6 was detected in AV (p < 0.0001) but not in BV, in relation to normal flora. Women with AV also presented increased IL-8 levels (p < 0.001), while those with BV presented levels similar to normal flora. Sialidase was increased in BV and AV compared with the normal group (p < 0.0001) but no difference in sialidase activity was observed between BV and AV. Conclusion: A more intense inflammatory host response occurs for AV than for BV when compared with normal flora. Furthermore, the increased sialidase activity in AV and BV indicates that both abnormal vaginal flora types can be harmful to the maintenance of a healthy vaginal environment. © 2012 Elsevier B.V.

O estado do Maranhão possui um grande potencial pesqueiro; entretanto, a pesca foi excluída das prioridades governamentais e científicas. Considerando as lacunas de conhecimento existentes, este trabalho realizou um diagnóstico dos dados pretéritos e atuais disponíveis sobre a pesca no Estado, abordando o estado da arte, caracterização das embarcações e artes de pesca, além da análise de produção, espécies de valor comercial e socioeconomia dos atores sociais envolvidos. A partir do quadro visualizado foi possível delimitar a existência de 21 unidades, aqui denominadas de Sistema de Produção Pesqueira, por meio de um processo de subdivisões sucessivas da atividade pesqueira de acordo com a frota, prática ou arte de pesca, recursos explorados, ambiente, residência, relação de trabalho e renda do pescador e grau de isolamento da área de pesca. Para tanto, foram aplicados questionários com os diferentes atores sociais e vivência em campo. Os sistemas foram caracterizados segundo os aspectos econômicos, sociais, tecnológicos, ecológico e manejo, evidenciando-se uma ampla variedade de práticas e frotas, que atuam, predominantemente, em ambientes costeiros, com pequenas embarcações e artes de pescas simples, capturando diferentes espécies-alvo, principalmente das famílias Scianidae e Aridae. O quadro socioeconômico dos pescadores é de pobreza e abandono, com baixa organização social e pequena renda, precárias condições de moradia e nível educacional e acesso à saúde limitado. O mercado e a legislação têm propiciado o livre acesso aos recursos e práticas predatórias, comprometendo os recursos pesqueiros, que são explotados sem qualquer preocupação com a sustentabilidade, demonstrando insuficiências nas ações de manejo e gerenciamento. Na tentativa de detectar indicadores que estimem o estado de “saúde” dos sistemas utilizou-se a metodologia do Rapfish, através de um conjunto de atributos agrupados em cinco áreas temáticas: ecológica, econômica, social, tecnológica e manejo. Os resultados destacaram como bons indicadores: organização social, número de pescadores explorando o sistema; grau de escolaridade; uso de petrechos destrutivos; medidas de manejo governamentais e tradicionais. A relação de trabalho e renda foram bons critérios para diferenciar três tendências na finalidade das pescarias: subsistência, intermediárias e “semi-indutrial”. Alguns sistemas se destacaram como menos sustentável a exemplo das capturas de siris, que tem declinado por falta de manejo, deficiente organização social e a comercialização de fêmeas ovadas; e das pescarias de lagosta, que utilizam artes consideradas destrutivas. O sistema que envolve a captura de caranguejo sobressaiu-se pela existência de medidas de manejo tradicional e melhor gerenciamento do recurso pelos órgãos públicos. Assim, este estudo permitiu o uso de um sistema de referência para análise e monitoramento da sustentabilidade das pescarias regionais, com em indicadores científicos e/ou etnoconhecimento, que induziu ao surgimento de propostas de manejo norteadas pelo gerenciamento da pesca, organização social e educação ambiental. A seguir, foi realizado estudo de caso do sistema de produção que utilizam as embarcações de médio porte nas pescarias de Cynoscion acoupa utilizando como arte o malhão, para entrar em detalhes de um dos sistemas de produção. Essa escolha teve como base, os grandes volumes de captura, a grande abrangência da área de atuação desse sistema em todo o litoral do Estado, além do grande número de pescadores envolvidos nele, representando importante fonte de renda para o Estado. O estudo de caso prestou especial atenção aos saberes tradicionais da população no uso e na manutenção do recurso, complementados com estudos sobre a pesca e biologia reprodutiva de Cynoscion acoupa, capturadas na região da baía de São Marcos e adjacências. As capturas das pescadas-amarela ocorreram durante todo o ano com safra no início do período chuvoso, e produção estadual estimada em 10.600.00 kg/ano. Verificou-se que este sistema vem sofrendo intensa e desordenada exploração, sendo possível inferir pelas características biológicas da espécie, que o crescente nível de esforço não é compatível com a capacidade de suporte ambiental nem como as necessidades dos pescadores. Quanto aos parâmetros reprodutivos, verificou-se que o comprimento médio de primeira maturação sexual (L50) para os machos foi de 39,9 cm e para as fêmeas a primeira maturação sexual ocorreu com tamanho ligeiramente superior, 41,6 cm de comprimento total. A proporção sexual foi de 1:1,4 favorável aos machos. Constatou-se que a espécie em questão completa todo o seu ciclo de vida na área estudada; o processo reprodutivo ocorre durante todo o ano, com dois picos de desova, um no bimestre novembro/dezembro e outro em março/maio. Acredita-se que por meio das informações obtidas é possível subsidiar melhores propostas e ações de sustentabilidade desta pescaria, combinando o etno-conhecimento e o conhecimento científico deste sistema.
