El Síndrome de Burnout, es una condición cuya detección ha venido en aumento en las últimas décadas. Sin embargo, son pocos los estudios realizados en docentes universitarios en Colombia. El objetivo del presente estudio es estimar la prevalencia y los factores asociados, en población administrativa y docente de una universidad privada.

Introducción: La enfermedad respiratoria ocupacional es causada por la exposición a diferentes agentes en el trabajo. Las pruebas objetivas realizadas en sospecha de enfermedad respiratoria de origen laboral, son importantes herramientas que permiten realizar un adecuado diagnóstico, una detección precoz de la enfermedad respiratoria ocupacional, disminuye el progreso rápido de la patología, la morbilidad de los trabajadores y el impacto negativo sobre su futuro laboral. Objetivo: Caracterizar las pruebas paraclínicas de las patologías respiratorias de trabajadores, en un centro de referencia neumológico de Bucaramanga año 2014-2016. Materiales y métodos: Se realizó un estudio descriptivo retrospectivo con datos secundarios de 96 trabajadores que laboran en diferentes actividades económicas. Se incluyeron variables sociodemográficas, laborales, ayudas imagenológicas y pruebas de función pulmonar, realizando 3 grupos de acuerdo a su patología que fueron: Asma, síndrome de disfunción reactiva de la vía aérea y neumoconiosis. En el análisis estadístico se emplearon medidas de tendencia central y dispersión. Resultados: De los 96 trabajadores 84.4% son hombres, las actividades económicas más frecuentes fueron la industria del petróleo y gas en un 27.1% y trabajadores en materiales de construcción en un 19.8%. En la caracterización paraclínica por grupo de patología, para asma predominó la obstrucción en la espirometría (46.9%) y los volúmenes pulmonares con atrapamiento aéreo (95.5%), en RADS (síndrome de disfunción de vías aéreas reactivas) los volúmenes pulmonares con atrapamiento aéreo (77%) y en las neumoconiosis para Rx de tórax (90.3%) y Tac de tórax (100%) reportaron alteraciones parenquimatosas, espirometría con obstrucción (54.8%) y volúmenes pulmonares con atrapamiento aéreo (62.5 %).Discusión y Conclusiones: Las ocupaciones de mayor riesgo para desarrollo de neumopatías de origen ocupacional fuero, , son la minería y construcción y para asma la agricultura y manufacturas. Para asma se evidenció que no hay significancia diagnóstica para estudios imagenológicos pero sí para las pruebas de función pulmonar. Para neumoconiosis el estudio imagenológico es el de mayor importancia ya que en las radiografías se presentan cambios incluso mucho antes de la afectación de la función pulmonar. Para RADS se concluyó que la realización de un test de provocación con metacolina sería el Gold estándar para el diagnóstico. Las pruebas de función respiratoria son de vital importancia para determinar la enfermedad ocupacional en trabajadores expuestos para vigilancia y detección precoz, es conveniente la realización de protocolos para la evaluación y diagnóstico de la enfermedad respiratoria de origen ocupacional. EPOC OCUPACIONAL: UNA REVISION DE LA LITERATURA (2004 - 2016) Camilo Alfredo Vanegas Clavijo, Md. Resumen La enfermedad pulmonar obstructiva crónica (EPOC) es una de las condiciones patológicas más frecuentes en la población general la cual produce una alta morbimortalidad en las personas que la padecen. A pesar de que el tabaquismo es la principal causa, los factores ocupacionales vienen incrementando su importancia para el desarrollo de esta enfermedad. Se cree que cerca del 20% de los casos de EPOC son atribuibles a la exposición ocupacional desencadenando un bajo rendimiento laboral por el deterioro de la calidad de vida. Objetivo Realizar una revisión de la literatura de los años 2004 al 2016 de los factores de riesgo ocupacional en el desarrollo de la EPOC y de las actividades que más incidencia tienen en la aparición de esta patología. Métodos Se utilizaron las bases de datos PubMed, Science Direct y Ovid para identificar artículos científicos relacionados con la exposición ocupacional, sus componentes y el desarrollo de la EPOC, y que estén publicados desde el año 2004 al 2016. Se tuvieron en cuenta solo artículos en idiomas inglés y español y se excluyeron artículos relacionados con población general y publicaciones editoriales, suplementos o publicaciones no científicas. Revisando títulos y resúmenes inicialmente con aplicación de filtros se obtuvieron en total 83 artículos. Resultados Según se describe en la mayoría de estudios y revisiones recientes, ya sea en muestras de población general o en lugares de trabajo, se indica que alrededor del 15 al 20% de todos los casos de EPOC están relacionados con el trabajo. Se documenta en varios de estos estudios una fuerte asociación entre la exposición ocupacional y un mayor riesgo de EPOC en trabajadores de la minería de carbón y oro, en las actividades relacionadas con la construcción y los trabajadores de las industrias textiles. Desestimular el tabaquismo, las medidas preventivas y la detección temprana de la enfermedad reducen la instauración y progresión de la enfermedad, mejorando la calidad de vida de los trabajadores y con ello aumentando el desempeño laboral. Conclusión En áreas como la minería, la construcción, la industria metalmecánica y la fabricación de textiles, la exposición ocupacional a polvos, gases, vapores y humos en los diversos ambientes laborales tienen una alta relación con la producción de la EPOC. Se recomienda incentivar al personal involucrado en el cuidado de la salud de los trabajadores en el desarrollo de programas de vigilancia en donde las medidas preventivas y de identificación temprana de esta enfermedad sean una prioridad. Introducción: El síndrome de agotamiento profesional o de burnout se define como una respuesta al estrés laboral crónico, afecta con frecuencia a los trabajadores del sector salud y ha sido descrito según la Organización Mundial de la Salud (OMS) como un riesgo laboral. En su desarrollo intervienen distintos factores principalmente sociodemográficos y laborales. Objetivo: Determinar la prevalencia de síndrome de agotamiento profesional y su relación con las condiciones de trabajo del personal de salud de la zona rural del Cauca. Metodología: Se realizó un estudio de corte transversal en 4 hospitales rurales nivel I, con un muestreo probabilístico aleatorio simple, para un total de 212 trabajadores, de los cuales el 74.5% fueron asistenciales y el 25.5% administrativos, Se les aplicó el cuestionario Maslash Burnout Inventory (MBI) que consta de 22 ítems y mide los 3 aspectos del síndrome: cansancio emocional, despersonalización y falta de realización personal y el cuestionario nacional de condiciones de trabajo del instituto nacional de seguridad e higiene en el trabajo de España (INSHT). Se realizó un análisis de datos en STATA versión 12. Resultados: Se encontró una prevalencia general de síndrome de agotamiento profesional de 39.7% y según cada dimensión para cansancio emocional de 21.7%, para despersonalización de 15.1% y para realización personal de 13.7%. Se halló significancia estadística con relación a la edad y baja realización personal (p=0.037), con los factores de riesgo psicolaborales por sobrecarga y cansancio emocional (p=0.020), falta de autonomía y cansancio emocional (p=0.021) entre otros. En los factores de riesgo biomecánicos por falta de iluminación y cansancio emocional (p=0.000), falta de iluminación y despersonalización (p=0.017) y falta de iluminación y síndrome en general (p=0.000). Conclusión: La prevalencia de síndrome de agotamiento profesional en el personal de salud que trabaja en zona rural del Cauca fue de 39.7%, la dimensión más determinante fue cansancio emocional seguido de despersonalización y por ultimo realización personal. Se recomienda iniciar en la institución con un programa de vigilancia epidemiológica de prevención para el agotamiento laboral e intervenir en los factores biomecánicos y factores psicolaborales.

Background. Clinical significance of multifocal pulmonary neuroendocrine proliferations (MNEP), including tumorlets and pulmonary neuroendocrine cell hyperplasia, in association with Typical Carcinoid (TC), is still debated. Methods. A large retrospective series of TC with long-term follow-up data prospectively collected from two institutions was evaluated. Recurrence or new TC development was followed-up. Patients with TC alone and MNEP+TC were compared. Results. 234 TC patients undergone surgery were included: 41 MNEP+TC (17.5%) and 193 TC alone (82.5%). In the MNEP+TC group older age (p<0.001), peripheral tumors (p=0.0032), smaller tumor size (p=0.011) and lymph-nodal spread (p=0.02) were observed in comparison with TC group. Relapses occurred in 8 patients (19.5%) in the MNEP+TC group and in 7 (3.6%) of the TC group. The 10-years progression-free survival were 96.1% in TC and 83.8% in MNEP+TC (p<0.001). After matching, in 36 pairs of patients a significantly higher 5-years progression-free survival was calculated for TC group (p<0.01). Furthermore the odds of belonging to MNEP+TC group was higher with work-related exposure to inhalant agents (p=0.008), asthma/bronchitis (p=0.002), emphysema, fibrosis and inflammatory status (p=0.032), micronodules on the chest CT scan and respiratory insufficiency (p=0.036). Conclusions. The identification of MNEP requires careful pathological examination and postoperative follow-up. MNEP seems to be an adverse prognostic factor in patients with synchronous TC. Therefore, suspicion of MNEP during the pre-operative assessment should not be underestimated, enabling changes in the surgical strategy.

La dimensione giuridica del tempo, cioè il ruolo del tempo nel diritto, ha contribuito a far emergere il lavoro quale concetto legale astratto dal prestatore, funzionale alla sua alienazione. Rompere l’unitarietà del tempo della persona, per consacrarne una parte al lavoro, ha dato ai rapporti di potere tra gli individui una forma giuridica in grado di legittimarli. Il primo capitolo si sofferma sulla costruzione legale del lavoro subordinato attraverso la definizione del suo tempo. Tra gli elementi essenziali del contratto tipizzato dall’art. 2094 c.c., il tempo nella causa, oltre a mostrare la non istantaneità dello scambio e la indeterminatezza del regolamento contrattuale, non assurge a criterio discretivo dalle collaborazioni o dal lavoro autonomo. Se dalla causa si passa ad indagare l’oggetto, l’interprete si imbatte nel pudore di svelare quale sia il vero elemento su cui incide il contratto, la persona del lavoratore, che induce a prediligere la finzione di allontanare l’attività dal corpo di chi la produce: l’orario è la tecnica giuridica che rende possibile la partecipazione del lavoro ad una logica di scambio. Il secondo e il terzo capitolo si concentrano sulla regolazione e interpretazione del tempo di lavoro in chiave diacronica. La legislazione lavoristica ha trovato i propri albori nella disciplina eteronoma dell’orario, come strumento per tutelare i prestatori dagli eccessi mercantilistici. L’attuale quadro normativo è molto attento alle ragioni creditorie ma la giurisprudenza della Corte di giustizia testimonia l’irrequietudine di una materia viva e in movimento. Non è un caso che il legislatore europeo, nei piccoli passi compiuti per rafforzare il diritto sociale comune agli Stati membri, abbia assegnato al tempo un ruolo centrale, come prevedibilità delle condizioni di lavoro (direttiva n. 1152 del 2019), come equilibrio tra attività professionale e vita familiare (direttiva n. 1158 del 2019) e un domani, forse, come diritto alla disconnessione.

BACKGROUND Temporomandibular disorder (TMD) is a multifactorial syndrome related to a critical period of human life. TMD has been associated with psychological dysfunctions, oxidative state and sexual dimorphism with coincidental occurrence along the pubertal development. In this work we study the association between TMD and genetic polymorphisms of folate metabolism, neurotransmission, oxidative and hormonal metabolism. Folate metabolism, which depends on genes variations and diet, is directly involved in genetic and epigenetic variations that can influence the changes of last growing period of development in human and the appearance of the TMD. METHODS A case-control study was designed to evaluate the impact of genetic polymorphisms above described on TMD. A total of 229 individuals (69% women) were included at the study; 86 were patients with TMD and 143 were healthy control subjects. Subjects underwent to a clinical examination following the guidelines by the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Genotyping of 20 Single Nucleotide Polymorphisms (SNPs), divided in two groups, was performed by multiplex minisequencing preceded by multiplex PCR. Other seven genetic polymorphisms different from SNPs (deletions, insertions, tandem repeat, null genotype) were achieved by a multiplex-PCR. A chi-square test was performed to determine the differences in genotype and allelic frequencies between TMD patients and healthy subjects. To estimate TMD risk, in those polymorphisms that shown significant differences, odds ratio (OR) with a 95% of confidence interval were calculated. RESULTS Six of the polymorphisms showed statistical associations with TMD. Four of them are related to enzymes of folates metabolism: Allele G of Serine Hydoxymethyltransferase 1 (SHMT1) rs1979277 (OR = 3.99; 95%CI 1.72, 9.25; p = 0.002), allele G of SHMT1 rs638416 (OR = 2.80; 95%CI 1.51, 5.21; p = 0.013), allele T of Methylentetrahydrofolate Dehydrogenase (MTHFD) rs2236225 (OR = 3.09; 95%CI 1.27, 7.50; p = 0.016) and allele A of Methionine Synthase Reductase (MTRR) rs1801394 (OR = 2.35; 95CI 1.10, 5.00; p = 0.037). An inflammatory oxidative stress enzyme, Gluthatione S-Tranferase Mu-1(GSTM1), null allele (OR = 2.21; 95%CI 1.24, 4.36; p = 0.030) and a neurotransmission receptor, Dopamine Receptor D4 (DRD4), long allele of 48 bp-repeat (OR = 3.62; 95%CI 0.76, 17.26; p = 0.161). CONCLUSIONS Some genetic polymorphisms related to folates metabolism, inflammatory oxidative stress, and neurotransmission responses to pain, has been significantly associated to TMD syndrome.

Background: Selective serotonin reuptake inhibitors (SSRIs) are first-line treatments for posttraumatic stress disorder (PTSD). Serotonergic (5HT) attenuation of stress sensitivity is postulated from SSRIs` effects in other anxiety disorders, and we studied this in PTSD. Methods: Ten patients with PTSD fully recovered on SSRIs (Clinical Global Impression Scale-I 1 and 2) were enrolled in the study. Patients were tested on two occasions I week apart; in each session, they received a drink containing large neutral amino acids (LNAAs) either with (sham tryptophan depletion [STD], control) or without (acute tryptophan depletion [ATD]) tryptophan. At 5.5 hours after the drink, subjects were exposed to a trauma-related exposure challenge. Self-reports of PTSD (visual analogue scales [VAS] and the Davidson Trauma Scale [DTSI), anxiety (Spielberger State Inventory [STAI] Form Y-1), and mood (Profile of Mood States [POMS]) were obtained. Heart rate (HR), systolic (SBP) and diastolic (DBP) blood pressure were also measured. Results: The trauma-related exposure challenge induced anxiety on both days, with more marked responses on the ATD day according to VAS, DTS, POMS, and DBP (p < .05). A trend of significance (.1 > p >.05) was observed for STAI Form Y-1, HR, and SBP. Conclusions: These data demonstrate that ATD accentuates responses to trauma-related stimuli in SSRI-recovered PTSD. They also suggest that SSRI-induced increases in serotonin function restrain PTSD symptoms, especially under provocation, supporting a role for serotonin in mediating stress resilience.

Purpose: As reported by several authors, angiotensin II (AngII) is a proinflammatory molecule that stimulates the release of inflammatory cytokines and activates nuclear factor kappa B (NF kappa B), being also associated with the increase of cellular oxidative stress. Its production depends on the activity of the angiotensin converting enzyme (ACE) that hydrolyzes the inactive precursor angiotensin I (AngI) into AngII. It has been suggested that AngII underlies the physiopathological mechanisms of several brain disorders such as stroke, bipolar disorder, schizophrenia, and disease. The aim of the present work was to localize and quantify AngII AT1 and AT2 receptors in the cortex and hippocampus of patients with temporal lobe epilepsy related to mesial temporal sclerosis (MTS) submitted to corticoamygdalohippocampectomy for seizure control. Method: Immunohistochemistry, Western blot, and real-time PCR techniques were employed to analyze the expression of these receptors. Results: The results showed an upregulation of AngII AT1 receptor as well as its messenger ribonucleic acid (mRNA) expression in the cortex and hippocampus of patients with MTS. In addition, an increased immunoexpression of AngII AT2 receptors was found only in the hippocampus of these patients with no changes in its mRNA levels. Discussion: These data show, for the first time, changes in components of renin-angiotensin system (RAS) that could be implicated in the physiopathology of MTS.

Aberrant movement patterns and postures are obvious to clinicians managing patients with musculoskeletal pain. However, some changes in motor function that occur in the presence of pain are less apparent. Clinical and basic science investigations have provided evidence of the effects of nociception on aspects of motor function. Both increases and decreases in muscle activity have been shown, along with alterations in neuronal control mechanisms, proprioception, and local muscle morphology. Various models have been proposed in an attempt to provide an explanation for some of these changes. These include the vicious cycle and pain adaptation models. Recent research has seen the emergence of a new model in which patterns of muscle activation and recruitment are altered in the presence of pain (neuromuscular activation model). These changes seem to particularly affect the ability of muscles to perform synergistic functions related to maintaining joint stability and control. These changes are believed to persist into the period of chronicity. This review shows current knowledge of the effect of musculoskeletal pain on the motor system and presents the various proposed models, in addition to other shown effects not covered by these models. The relevance of these models to both acute and chronic pain is considered. It is apparent that people experiencing musculoskeletal pain exhibit complex motor responses that may show some variation with the time course of the disorder. (C) 2001 by the American Pain Society.

Ergonomic interventions such as increased scheduled breaks or job rotation have been proposed to reduce upper limb muscle fatigue in repetitive low-load work. This review was performed to summarize and analyze the studies investigating the effect of job rotation and work-rest schemes, as well as, work pace, cycle time and duty cycle, on upper limb muscle fatigue. The effects of these work organization factors on subjective fatigue or discomfort were also analyzed. This review was based on relevant articles published in PubMed, Scopus and Web of Science. The studies included in this review were performed in humans and assessed muscle fatigue in upper limbs. 14 articles were included in the systematic review. Few studies were performed in a real work environment and the most common methods used to assess muscle fatigue were surface electromyography (EMG). No consistent results were found related to the effects of job rotation on muscle activity and subjective measurements of fatigue. Rest breaks had some positive effects, particularly in perceived discomfort. The increase in work pace reveals a higher muscular load in specific muscles. The duration of experiments and characteristics of participants appear to be the factors that most have influenced the results. Future research should be focused on the improvement of the experimental protocols and instrumentation, in order to the outcomes represent adequately the actual working conditions. Relevance to industry: Introducing more physical workload variation in low-load repetitive work is considered an effective ergonomic intervention against muscle fatigue and musculoskeletal disorders in industry. Results will be useful to identify the need of future research, which will eventually lead to the adoption of best industrial work practices according to the workers capabilities.

Introduction: There are more than 300,000 extractors using the babaçu coconut as a source of income in the States of Maranhão, Pará, Tocantins and Piauí, and this activity is associated with fungal infections. The objective of this study was to examine the occurrence of emergent fungi in the conjunctiva, nails and surface and subcutaneous injuries of female coconut breakers in Esperantinópolis, Maranhão. Additionally, soil samples and palm structures were collected. Methods: The obtained samples were cultured in Petri dishes containing potato-dextrose-agar and chloramphenicol. The etiological agent was confirmed by a direct mycological exam and growth in culture. Results: In total, 150 domiciles were visited, and samples were collected from 80 patients. From the ground, the most frequently isolated fungus was Aspergillus niger (53. 8%). the most frequently detected fungus in babaçu coconut was Aspergillus niger (66.7%). Conjunctival fungal growth occurred in 76.3% of the women. The ocular fungal microbiota consisted of filamentous fungi (80.6%), and yeasts were present in 19.4% of cases. Onychomycosis was diagnosed in 44% (11/25) of the women. Conclusions: The identification of the genera Neosartorya, Rhizopus and Curvularia in onychomycoses shows that emergent filamentous fungi can be isolated. Aspergillus sp., Penicillium sp. and Scedosporium sp. were the predominant genera found in the babaçu coconut. From ocular conjunctiva, Candida spp. were the most prevalent species isolated, and Fusarium sp. was present only in one woman. The nearly permanent exposure of coconut breakers to the external environment and to the soil is most likely the reason for the existence of a mycotic flora and fungal infections, varying according to the individual's practices and occupation.

The aim of the present study is to explore obsessive-compulsive disorder (OCD)-related abnormalities in white matter connectivity in OCD for a core region associated with inhibitory control [i.e. inferior frontal gyrus (IFG)]. Fifteen patients with OCD (11 men) and 15 healthy controls (nine men) underwent diffusion tensor imaging scanning to study four diffusivity indexes of white matter integrity [fractional anisotropy, mean diffusivity (MD), axial diffusivity and radial diffusivity (RD)]. The results showed that persons with OCD manifested significantly lower fractional anisotropy levels in the bilateral IFG as well as its parcellations in the pars opercularis, pars triangularis, and pars orbitalis. Significantly higher levels of MD, RD were evident for the OCD group in the IFG as a whole as well as in the bilateral subregions of the pars triangularis and pars opercularis (for MD and RD), the right side of the pars orbitalis (for RD), and the left side of the pars triangularis and right side pars opercularis (for axial diffusivity). Overall, the results suggest significant alterations in structural connectivity, probably associated with myelination and axonal abnormalities in the IFG of OCD patients.
