Public values are moving from a research concern to policy discourse and management practice. There are, though, different readings of what public values actually mean. Reflection suggests two distinct strands of thinking: a generative strand that sees public value emerging from processes of public debate; and an institutional interpretation that views public values as the attributes of government producers. Neither perspective seems to offer a persuasive account of how the public gains from strengthened public values. Key propositions on values are generated from comparison of influential texts. A provisional framework is presented of the values base of public institutions and the loosely coupled public propositions flowing from these values. Value propositions issue from different governing contexts, which are grouped into policy frames that then compete with other problem frames for citizens’ cognitive resources. Vital democratic commitments to pluralism require public values to be distributed in competition with other, respected, frames.

This paper is aimed at those interested in the promotion of student retention in higher education; particularly those with an interest in peer mentoring as a means of student support. It critically discusses the results of an exploratory study analysing the perceptions of peer mentors and mentees within five universities in the United Kingdom. The aim of the study was to analyse how student peer mentoring can aid transition into university by focusing specifically on how senior students can support their junior counterparts in their first year at university. The paper discusses the results of a survey which was completed by 329 student peer mentors and mentees. Focusing on the benefits and outcomes of participation in Mentoring Programmes, the survey was distinctive in that it asked mentors and mentees similar questions. From a theoretical perspective, the paper contributes to debates about peer support in higher education showing that participation in such programmes can have positive outcomes from both social and pedagogic perspectives. Practically speaking, the results have important implications for Higher Education Institutions as the research highlights the importance of putting into place formally structured Peer Mentoring Programmes which facilitate student support at a time when new students are most at risk of ‘dropping out’.

The purpose of this research was to explore perceptions among 9 th through 12th grade students from Brazil, Haiti and Jamaica, with respect to their heritage languages: Portuguese, Haitian Creole, and Jamaican Patois. An additional purpose was to understand in greater detail possible variations of perception with respect to heritage language maintenance (or loss) in relation to one’s gender, first language, and place of birth. The research implemented semi-structured interviews with male and female adolescents with these heritage language backgrounds. Participants’ responses were recorded and transcribed. The transcriptions were analyzed via a categorizing of themes emerging from the data. Data were analyzed using inductive analysis. Three categories emerged from the inductive analysis of the data: (a) heritage language, (b) bilingualism, and (c) English as a second language. The analysis reveals that as participants learn English, they continue to value their heritage language and feel positively toward bilingualism, but differ in their preference regarding use of native language and English in a variety of contexts. There seems to be a mismatch between a positive attitude and an interest in learning their heritage language. Families and teachers, as agents, may not be helping students fully understand the advantages of bilingualism. Students seem to have a lack of understanding of bilingualism’s cognitive and bi-literacy benefits. Instead, employment seems to be perceived as the number one reason for becoming bilingual. Also, the students have a desire to add culture to the heritage language curriculum. The study was conducted at one of the most diverse and largest high schools in Palm Beach, in Palm Beach County, Florida. The results of this study imply that given the positive attitude toward heritage language and bilingualism, students need to be guided in exploring their understanding of heritage language and bilingualism. Implications for teaching and learning, as well as recommendations for further research, are included.

This article examines the structure and health implications of two industries, chicken and tomatoes, that play prominent roles in US food and agricultural competitiveness. Both industries have become more concentrated over time, with powerful "lead firms" driving geographical, technological, and marketing changes. Overall, a processed food revolution has taken place in agricultural products that transforms the types of food and dietary options available to consumers. The nature of contemporary food and agricultural value chains affects the strategies and policies that can be effectively employed to address major health goals such as improved nutrition, food safety, and food security.

Rapid technological advances and liberal trade regimes permit functional reintegration of dispersed activities into new border-spanning business networks variously referred to as global value chains (GVCs). Given that the gains of a country from GVCs depend on the activities taking place in its jurisdiction and their linkages to global markets, this study starts by providing a descriptive overview of China’s economic structure and trade profile. The first two chapters of this paper demonstrate what significant role GVCs have played in China’s economic growth, evident in enhanced productivity, diversification, and sophistication of China’s exports, and how these economic benefits have propelled China’s emergence as the world’s manufacturing hub in the past two decades. However, benefits from GVC participation – in particular technological learning, knowledge building, and industrial upgrading – are not automatic. What strategies would help Chinese industries engage with GVCs in ways that are deemed sustainable in the long run? What challenges and related opportunities China would face throughout the implementation process? The last two chapters of this paper focus on implications of GVCs for China’s industrial policy and development. Chapter Three examines how China is reorienting its manufacturing sector toward the production of higher value-added goods and expanding its service sector, both domestically and internationally; while Chapter Four provides illustrative policy recommendations on dealing with the positive and negative outcomes triggered by GVCs, within China and beyond the country’s borders. To the end, this study also hopes to shed some light on the lessons and complexities that arise from GVC participation for other developing countries.

Drawing on an empirical study of public transport, this paper studies interactive value formation at the provider—customer interface, from a practice—theory perspective. In contrast to the bulk of previous research, it argues that interactive value formation is not only associated with value co-creation but also with value co-destruction. In addition, the paper also identifies five interaction value practices — informing, greeting, delivering, charging, and helping — and theorizes how interactive value formation takes place as well as how value is intersubjectively assessed by actors at the provider—customer interface. Furthermore, the paper also distinguishes between four types of interactive value formation praxis corresponding with four subject positions which practitioners step into when engaging in interactive value formation.

Single agent chemotherapy of advanced lung cancer is still unsatisfactory. The most encouraging results have been obtained with high dose intermittent treatment with cyclophosphamide, methotrexate or adriamycin. Combination chemotherapy is probably more efficacious than single agent chemotherapy, especially in oat cell carcinoma. The best treatment schedules give a response rate of approximately 80%. The value of adjuvant chemotherapy is not yet established, but new controlled clinical trials are indicated considering the poor results achieved by surgery and radiotherapy.

Cette recherche a pour objectif de mesurer la valeur marginale estimée par les investisseurs canadiens à la suite de l’ajout de 1 dollar d’encaisse, pour la période trimestrielle s’étalant de 2001 à 2013. Autrement dit, nous tentons d’évaluer si l’investisseur déprécie, estime à sa juste valeur nominale ou apprécie la valeur de 1 dollar de trésorerie, et ce à travers son impact sur la valeur marchande de l’équité. Parallèlement, nous tentons d’analyser l’influence des variables liées à la rentabilité, le financement, la croissance et le risque sur la valeur que place l’investisseur à l’ajout de 1 dollar d’encaisse. L’effet sectoriel sera notamment exploré sur l’ensemble de ces périodes. Des études passées ont démontré qu’il existe une asymétrie entre la valeur nominale de 1 dollar d’encaisse et celle estimée par les investisseurs. Plus précisément que l’ajout de 1 dollar de liquidité est évalué à prime. Somme toute, ces analyses affinent leurs résultats en tenant compte de l’influence des autres variables telles que mentionnées plus haut, afin de déterminer celles qui entraînent une appréciation de l’encaisse de celles qui conduisent à sa dépréciation. Nous avons conclu qu’en moyenne la valeur marginale de 1 dollar de liquidité est évaluée à 2,13$ par les investisseurs. Nous avons également pu prouver que les anticipations des investisseurs et le risque total influencent significativement cette estimation. Pour tenter de mesurer la valeur que place l’investisseur à l’ajout de 1 dollar d’encaisse, nous avons utilisé la méthodologie de Pinkowitz et Williamson (2002). Ce faisant, nous avons dans un premier temps mesuré l’impact net de l’encaisse sur la valeur marchande de l’équité en élimant du modèle les variables tenant compte des variations trimestrielles t-2 et t+2. L’hypothèse à vérifier en ce qui concerne l’impact net de l’encaisse est qu’il existe une asymétrie significative entre la valeur de 1 dollar et celle estimée par l’investisseur qui n’est pas uniquement provoquée par l’influence des variations passées et futures de l’encaisse. Les résultats obtenus nous ont permis de prouver que la valeur de 1 dollar d’encaisse est appréciée par l’investisseur, et ce malgré la suppression ou la présence des variables tenant compte des variations. Dans la même ligné, nous avons pu démontrer que, conjointement à l’encaisse au temps t, seules les variations liées aux anticipations des investisseurs impactaient significativement la valeur marchande de l’équité. En ce qui concerne le risque dont l’analyse s’est effectuée en dissociant le risque systématique du risque systémique. L’hypothèse à vérifier est qu’une augmentation du risque systématique entraîne une augmentation de la valeur de l’encaisse estimée par l’investisseur. Les résultats obtenus viennent appuyer cette affirmation. Quant au risque systémique, l’hypothèse à vérifier tente à affirmer qu’en période de crise l’encaisse est sous-estimée par l’investisseur et son impact sur la VME non significatif. Nos résultats nous conduisent à démontrer que pour la crise financière de 2006, cette hypothèse est justifiée, mais nous ne permettent pas de la vérifier pour chaque crise. Finalement, nous tenions à vérifier que dépendamment du secteur étudié, l’encaisse pouvait être plus appréciée par les investisseurs. Or, nos résultats ont montré qu’en présence de la liquidité aucun secteur ne se distingue significativement de ses pairs.

Binge eating occurs primarily on highly palatable food (PF) suggesting that the reward value of food has an important role in this behaviour. Bingeing also leads to reward dysfunction in rats and humans. The rewarding effect of binge eating may involve opioid mechanisms as opioid antagonists reduce PF consumption in animals that binge eat and binge eating produces neuroadaptations of opioid receptors in rodents. We tested this hypothesis by using the conditioned place preference (CPP) paradigm. First we established a sucrose CPP in male and female Long-Evans rats (n=8 for each group) using 1%, 5%, 15%, or 30% sucrose solution. Next, rats underwent the sucrose bingeing model in which separate groups of rats (n=8 for each group) received 12hr and 24hr access to 10% sucrose solution and chow, 12hr access to 0.1% saccharin solution and chow, or 12hr access to chow only every day for 28 days. Immediately following these sessions, rats were conditioned and tested in the CPP paradigm using a 15% sucrose solution. Finally, we examined whether the sucrose bingeing model altered morphine reward in female rats. Rats (n=8 for each group) received 12hr and 24hr access to 10% sucrose solution and chow every day for 28 days. Immediately following this access period, rats were conditioned to morphine (6mL/kg) or saline solution in the CPP paradigm and tested for a CPP. In all experiments, rats drank more sucrose solution than water during conditioning sessions. Male rats did not develop a CPP to any concentration of sucrose solution and females developed a CPP to 15% sucrose solution only. Following the sucrose bingeing protocol, sucrose CPP was attenuated in male rats that binged on sucrose and in all female rats. Sucrose bingeing in females did not affect the development of a CPP to morphine. These results suggest that sucrose consumption and sucrose CPP are measures of different psychological components of reward. Furthermore, sucrose bingeing reduces the rewarding effect of sucrose, but not morphine, suggesting that opioid reward is still intact.

The present work, where additional value-creating processes in existing combined heat and power (CHP) structures have been examined, is motivated by a political- and consumer-driven strive towards a bioeconomy and a stagnation for the existing business models in large parts of the CHP sector. The research is based on cases where the integration of flash pyrolysis for co-production of bio-oil, co-gasification for production of fuel gas and synthetic biofuels as well as leaching of extractable fuel components in existing CHP plants have been simulated. In particular, this work has focused on the CHP plants that utilize boilers of fluidized bed (FB) type, where the concept of coupling a separate FB reactor to the FB of the boiler forms an important basis for the analyses. In such dual fluidized bed (DFB) technology, heat is transferred from the boiler to the new rector that is operating with other fluidization media than air, thereby enabling other thermochemical processes than combustion to take place. The result of this work shows that broader operations at existing CHP plants have the potential to enable production of significant volumes of chemicals and/or fuels with high efficiency, while maintaining heat supply to external customers. Based on the insight that the technical preconditions for a broader operation are favourable, the motivation and ability among the incumbents in the Swedish CHP sector to participate in a transition of their operation towards a biorefinery was examined. The result of this assessment showed that the incumbents believe that a broader operation can create significant values for their own operations, the society and the environment, but that they lack both a strong motivation as well as important abilities to move into the new technological fields. If the concepts of broader production are widely implemented in the Swedish FB based CHP sector, this can substantially contribute in the transition towards a bioeconomy.

‘Urban identity’ is high on the policy agenda and pervades the discourse of the planning community on the value of historical city centres. Unfortunately, there seems to be, until today, no proposal in scholarly literature of any unified conceptual framework or any tools to make identity operational. ‘Tourism’ takes advantage of this process, by seeking the qualities of the place, its authenticity and its perceived uniqueness that is grounded on the physical features as well as on the presence of local communities – their way of living and investing in the place. The interdependence between identity as perceived by tourists (external observer) and the identity of the residents rooted in the relationship with the place (in-group) are key to addressing the identity of historic urban areas. These issues are addressed in the context of the growing attractiveness of Lisbon, Portugal, using a historic neighbourhood as a case study. The findings, which are on a set of interviews with different groups of users, showed the points of convergence and divergence between the different groups’ views of the neighbourhood’s identity. This actor-oriented approach is pivotal to understanding the process and to produce knowledge for informed action.

To compare time and risk to biochemical recurrence (BR) after radical prostatectomy of two chronologically different groups of patients using the standard and the modified Gleason system (MGS). Cohort 1 comprised biopsies of 197 patients graded according to the standard Gleason system (SGS) in the period 1997/2004, and cohort 2, 176 biopsies graded according to the modified system in the period 2005/2011. Time to BR was analyzed with the Kaplan-Meier product-limit analysis and prediction of shorter time to recurrence using univariate and multivariate Cox proportional hazards model. Patients in cohort 2 reflected time-related changes: striking increase in clinical stage T1c, systematic use of extended biopsies, and lower percentage of total length of cancer in millimeter in all cores. The MGS used in cohort 2 showed fewer biopsies with Gleason score ≤ 6 and more biopsies of the intermediate Gleason score 7. Time to BR using the Kaplan-Meier curves showed statistical significance using the MGS in cohort 2, but not the SGS in cohort 1. Only the MGS predicted shorter time to BR on univariate analysis and on multivariate analysis was an independent predictor. The results favor that the 2005 International Society of Urological Pathology modified system is a refinement of the Gleason grading and valuable for contemporary clinical practice.

This study aimed at evaluating whether human papillomavirus (HPV) groups and E6/E7 mRNA of HPV 16, 18, 31, 33, and 45 are prognostic of cervical intraepithelial neoplasia (CIN) 2 outcome in women with a cervical smear showing a low-grade squamous intraepithelial lesion (LSIL). This cohort study included women with biopsy-confirmed CIN 2 who were followed up for 12 months, with cervical smear and colposcopy performed every three months. Women with a negative or low-risk HPV status showed 100% CIN 2 regression. The CIN 2 regression rates at the 12-month follow-up were 69.4% for women with alpha-9 HPV versus 91.7% for other HPV species or HPV-negative status (P < 0.05). For women with HPV 16, the CIN 2 regression rate at the 12-month follow-up was 61.4% versus 89.5% for other HPV types or HPV-negative status (P < 0.05). The CIN 2 regression rate was 68.3% for women who tested positive for HPV E6/E7 mRNA versus 82.0% for the negative results, but this difference was not statistically significant. The expectant management for women with biopsy-confirmed CIN 2 and previous cytological tests showing LSIL exhibited a very high rate of spontaneous regression. HPV 16 is associated with a higher CIN 2 progression rate than other HPV infections. HPV E6/E7 mRNA is not a prognostic marker of the CIN 2 clinical outcome, although this analysis cannot be considered conclusive. Given the small sample size, this study could be considered a pilot for future larger studies on the role of predictive markers of CIN 2 evolution.

Phase I trials use a small number of patients to define a maximum tolerated dose (MTD) and the safety of new agents. We compared data from phase I and registration trials to determine whether early trials predicted later safety and final dose. We searched the U.S. Food and Drug Administration (FDA) website for drugs approved in nonpediatric cancers (January 1990-October 2012). The recommended phase II dose (R2PD) and toxicities from phase I were compared with doses and safety in later trials. In 62 of 85 (73%) matched trials, the dose from the later trial was within 20% of the RP2D. In a multivariable analysis, phase I trials of targeted agents were less predictive of the final approved dose (OR, 0.2 for adopting ± 20% of the RP2D for targeted vs. other classes; P = 0.025). Of the 530 clinically relevant toxicities in later trials, 70% (n = 374) were described in phase I. A significant relationship (P = 0.0032) between increasing the number of patients in phase I (up to 60) and the ability to describe future clinically relevant toxicities was observed. Among 28,505 patients in later trials, the death rate that was related to drug was 1.41%. In conclusion, dosing based on phase I trials was associated with a low toxicity-related death rate in later trials. The ability to predict relevant toxicities correlates with the number of patients on the initial phase I trial. The final dose approved was within 20% of the RP2D in 73% of assessed trials.

The objectives of the study were to evaluate the performance of sentinel lymph node biopsy (SLNB) in detecting occult metastases in papillary thyroid carcinoma (PTC) and to correlate their presence to tumor and patient characteristics. Twenty-three clinically node-negative PTC patients (21 females, mean age 48.4 years) were prospectively enrolled. Patients were submitted to sentinel lymph node (SLN) lymphoscintigraphy prior to total thyroidectomy. Ultrasound-guided peritumoral injections of (99m)Tc-phytate (7.4 MBq) were performed. Cervical single-photon emission computed tomography and computed tomography (SPECT/CT) images were acquired 15 min after radiotracer injection and 2 h prior to surgery. Intra-operatively, SLNs were located with a gamma probe and removed along with non-SLNs located in the same neck compartment. Papillary thyroid carcinoma, SLNs and non-SLNs were submitted to histopathology analysis. Sentinel lymph nodes were located in levels: II in 34.7 % of patients; III in 26 %; IV in 30.4 %; V in 4.3 %; VI in 82.6 % and VII in 4.3 %. Metastases in the SLN were noted in seven patients (30.4 %), in non-SLN in three patients (13.1 %), and in the lateral compartments in 20 % of patients. There were significant associations between lymph node (LN) metastases and the presence of angio-lymphatic invasion (p = 0.04), extra-thyroid extension (p = 0.03) and tumor size (p = 0.003). No correlations were noted among LN metastases and patient age, gender, stimulated thyroglobulin levels, positive surgical margins, aggressive histology and multifocal lesions. Sentinel lymph node biopsy can detect occult metastases in PTC. The risk of a metastatic SLN was associated with extra-thyroid extension, larger tumors and angio-lymphatic invasion. This may help guide future neck dissection, patient surveillance and radioiodine therapy doses.
