The hundredth anniversary of the outbreak of the First World War is only the first of a large number of major European historical anniversaries that will occur in the coming four years. Other twentieth-century anniversaries include that of the Russian Revolution and the Easter Uprising; notable corollaries from earlier centuries include the Battle of Bannockburn, the Hanoverian succession, the Battle of Waterloo and, perhaps most significant of all, the five hundredth anniversary of the Lutheran Reformation. Rather than commission special issues or other features to tie in to individual anniversaries centred on or relevant to German history in a manner which repeats unthinkingly the conventions of scholarly and popular culture, the editors elected to reflect more fundamentally on what might be at stake in major anniversaries for professional scholars of history. In anticipation of the major wave of scholarly and popular publications, commemorative activities and memory conflicts that each of these will generate, and in order to reflect upon the dynamics of German history, memory and commemoration in a more overtly comparative context, the editors invited a number of scholars working on different national histories to reflect on the possibilities and potential pitfalls such anniversaries offer to historians who tie their work in to such moments. They are Jörg Arnold (Nottingham), Thomas A. Brady (Berkeley), Fearghal McGarry (Queen’s University, Belfast), Tim Grady (Chester) and Dan Healey (St Antony’s College, Oxford). The questions were posed by the editors.

Background

Chronic kidney disease is now regarded as a risk factor for cardiovascular disease. The impact of occupational or non-occupational physical activity (PA) on moderate decreases of renal function is uncertain.

Objectives

We aimed to identify the potential association of PA (occupational and leisure-time) on early decline of estimated glomerular filtration rate (eGFR) and to determine the potential mediating effect of PA on the relationship between eGFR and heart disease.

Methods

From the PRIME study analyses were conducted in 1058 employed men. Energy expended during leisure, work and commuting was calculated. Linear regression analyses were used to determine the link between types of PA and moderate decrements of eGFR determined with the KDIGO guideline at the baseline assessment. Cox proportional hazards analyses were used to explore the potential effect of PA on the relationship between eGFR and heart disease, ascertained during follow-up over 10 years.

Results

For these employed men, and after adjustment for known confounders of GFR change, more time spent sitting at work was associated with increased risk of moderate decline in kidney function, while carrying objects or being active at work was associated with decreased risk. In contrast, no significant link with leisure PA was apparent. No potential mediating effect of occupational PA was found for the relationship between eGFR and coronary heart disease.

Conclusion

Occupational PA (potential modifiable factors) could provide a dual role on early impairment of renal function, without influence on the relationship between early decrease of e-GFR and CHD risk.

It is unknown how interventions aimed at increasing physical activity (PA), other than traditional pulmonary rehabilitation, are structured and whether they are effective in increasing PA in chronic obstructive pulmonary disease (COPD). The primary aim of this review was to outline the typical components of PA interventions in patients with COPD. This review followed the PRISMA guidelines. A structured literature search of relevant electronic databases from inception to April 2014 was undertaken to outline typical components and examine outcome variables of PA interventions in patients with COPD. Over 12000 articles were screened and 20 relevant studies involving 31 PA interventions were included. Data extracted included patient demographics, components of the PA intervention, PA outcome measures and effects of the intervention. Quality was assessed using the PEDro and CASP scales. There were 13 randomised controlled trials and three randomised trials (PEDro score 5-7/10) and four cohort studies (CASP score 5/10). Interventions varied in duration, number of participant/researcher contacts and mode of delivery. The most common behaviour change techniques included information on when and where (n = 26/31) and how (n = 22/31) to perform PA behaviour and self-monitoring (n = 18/31). Significant between-group differences post-intervention in favour of the PA intervention, compared to a control group or to other PA interventions, in one or more PA assessments were found in 7/16 studies. All seven studies used walking as the main type of PA/exercise. In conclusion, although the components of PA interventions were variable, there is some evidence that PA interventions have the potential to increase PA in patients with COPD

BACKGROUND: Physical inactivity has been associated with obesity and related chronic diseases. Understanding built environment (BE) influences on specific domains of physical activity (PA) around homes and workplaces is important for public health interventions to increase population PA.

PURPOSE: To examine the association of home and workplace BE features with PA occurring across specific life domains (work, leisure, and travel).

METHODS: Between 2012 and 2013, telephone interviews were conducted with participants in four Missouri metropolitan areas. Questions included sociodemographic characteristics, home and workplace supports for PA, and dietary behaviors. Data analysis was conducted in 2013; logistic regression was used to examine associations between BE features and domain-specific PA.

RESULTS: In home neighborhoods, seven of 12 BE features (availability of fruits and vegetables, presence of shops and stores, bike facilities, recreation facilities, crime rate, seeing others active, and interesting things) were associated with leisure PA. The global average score of home neighborhood BE features was associated with greater odds of travel PA (AOR=1.99, 95% CI=1.46, 2.72); leisure PA (AOR=1.84, 95% CI=1.44, 2.34); and total PA (AOR=1.41, 95% CI=1.04, 1.92). Associations between workplace neighborhoods' BE features and workplace PA were small but in the expected direction.

CONCLUSIONS: This study offers empirical evidence on BE supports for domain-specific PA. Findings suggest that diverse, attractive, and walkable neighborhoods around workplaces support walking, bicycling, and use of public transit. Public health practitioners, researchers, and worksite leaders could benefit by utilizing worksite domains and measures from this study for future BE assessments.

Introdução: A atividade física (AF) parece ter um impacto positivo na saúde física e mental durante a gravidez, nascimento e puerpério, sendo que os programas de preparação para a parentalidade (PPP) poderão ser fundamentais para o seu suporte e estimulação. Objetivos: O objetivo do presente estudo foi avaliar a relação entre a preparação para a parentalidade no 3º trimestre de gravidez e os níveis de AF da gestante. Especificamente pretendeu-se verificar a relação entre as caraterísticas sociodemográficas das gestantes, os níveis de ansiedade e o PPP. Métodos: Efetuou-se um estudo transversal analítico onde se utilizaram duas amostras. Uma constituída por 42 gestantes que frequentaram o PPP no CHTS (GPP) e outra por 41 gestantes do HSJ que não frequentou (GNPP). A cada gestante foi pedido que preenchessem 3 questionários (caraterização sociodemográfica e saúde obstétrica, Questionário de Atividade Física para gestantes-PPAQ e Escala de ansiedade de Zung), administrados, individualmente, por um Fisioterapeuta. Resultados: Não se verificaram diferenças entre os grupos relativamente ao score da AF total (p=0,615), contudo, o GPP apresentou um número superior de gestantes que praticava desporto organizado durante a gravidez (p=0,016) comparativamente ao GNPP. Relativamente à intensidade da AF, verificou-se que o GPP apresentava uma prática maior de AF vigorosa (p=0,023). No que diz respeito ao tipo de AF, o GPP apresentou um número superior de gestantes a praticar AF desportiva (p<0,001) enquanto no GNPP se verificou uma maior AF ocupacional (p=0,002). Relativamente às caraterísticas sociodemográficas verificaram-se diferenças estatisticamente significativas entre os dois grupos relativamente à idade (p<0,001), paridade (p<0,001) e nível educacional (p<0,001). No que respeita aos níveis de ansiedade não se verificaram diferenças estatisticamente significativas entre os grupos (GPP vs GNPP p=0,916). Conclusão: No GPP um maior número de gestantes praticava atividade física desportiva e de intensidade vigorosa. Verificaram-se diferenças entre os dois grupos no que diz respeito à idade, paridade e nível educacional. Não se verificou associação entre o PPP e os níveis de ansiedade durante este período.

Introdução: A gravidez e a prática da atividade física (AF) são temas ainda pouco abordados na nossa sociedade. Muitas mulheres sofrem de ansiedade no período pré-natal e não conhecem os benefícios que a prática da AF tem sobre a mesma. Estudos demonstram que a AF está associada à diminuição da ansiedade, contudo poucos avaliam este efeito na gravidez. Objetivo(s): Os objetivos deste estudo foram classificar o nível de AF das grávidas; avaliar os sintomas de ansiedade e analisar a associação entre os níveis de ansiedade e a AF das gestantes. Métodos: Trata-se de um estudo longitudinal prospetivo, realizado numa amostra de 56 grávidas com idades entre os 18 e os 37 anos (29,14 ±4,641 anos). As participantes foram avaliadas em dois momentos, T1 (10-12 semanas gestacionais) e T2 (20-22 semanas gestacionais), tendo sido mensurada a AF com acelerómetros GTX3 e a ansiedade com a escala de Zung. Resultados: A média da AF leve e moderada não apresentou diferenças entre os dois momentos de avaliação; contudo, verificou-se uma tendência para a AF leve diminuir do primeiro para o segundo trimestre (1283,88 ± 530,37 vs 1098,16 ± 489,72 counts/min total semana; p=0,459) e a AF moderada aumentar (2470,62 ± 404,50 vs 2528,55 ± 493,05 counts/min total semana; p=0,459). A maioria das grávidas não cumpriu as recomendações da ACSM nos dois momentos de avaliação (T1: 72,1% e T2: 73,2%) (p=0,06). Não se verificaram diferenças no estado de ansiedade das grávidas de T1 para T2. 10,7% das grávidas apresentaram ansiedade em T1 e 7,1% em T2 (p=0,063). Cumprir ou não as recomendações da ACSM não esteve associado ao estado de ansiedade. Conclusão: Neste estudo verificámos que a maioria das gravidas não cumpre as recomendações da AF segundo ACSM, os níveis de ansiedade não diferiram ao longo da gravidez, verificámos ainda que o cumprimento das recomendações da AF não está associado aos níveis de ansiedade.

Objective Public health organizations recommend that preschool-aged children accumulate at least 3 h of physical activity (PA) daily. Objective monitoring using pedometers offers an opportunity to measure preschooler's PA and assess compliance with this recommendation. The purpose of this study was to derive step-based recommendations consistent with the 3 h PA recommendation for preschool-aged children. Method The study sample comprised 916 preschool-aged children, aged 3 to 6 years (mean age = 5.0 ± 0.8 years). Children were recruited from kindergartens located in Portugal, between 2009 and 2013. Children wore an ActiGraph GT1M accelerometer that measured PA intensity and steps per day simultaneously over a 7-day monitoring period. Receiver operating characteristic (ROC) curve analysis was used to identify the daily step count threshold associated with meeting the daily 3 hour PA recommendation. Results A significant correlation was observed between minutes of total PA and steps per day (r = 0.76, p < 0.001). The optimal step count for ≥ 3 h of total PA was 9099 steps per day (sensitivity (90%) and specificity (66%)) with area under the ROC curve = 0.86 (95% CI: 0.84 to 0.88). Conclusion Preschool-aged children who accumulate less than 9000 steps per day may be considered Insufficiently Active.

Na presente dissertação apresentam-se os resultados dos trabalhos de arqueologia efetuados na colina do Vimeiro durante o ano de 2014, tendo em vista a análise de parte do campo de Batalha do Vimeiro. O estudo do espólio recolhido no que diz respeito à sua tipologia e localização espacial tem como objetivos finais compreender e identificar os vários momentos do combate da Colina do Vimeiro. Os dados recolhidos arqueologicamente serão em última análise uma visão aproximada da veracidade dos acontecimentos.
