O estudo tem como tema os Percursos trilhados pelas famílias para a garantia do direito à educação de crianças com necessidades especiais. Este estudo surgiu pela demanda dos integrantes do Núcleo de Estudos da Infância: Pesquisa & Extensão (NEI:P&E/UERJ), coordenado pela Prof Dr Vera Vasconcellos, em compreender como ocorreu a trajetória de escolarização de crianças acompanhadas em dois estudos realizados em creches do município do Rio de Janeiro, em 2009, após a saída delas das referidas instituições. Os estudos foram: i) Crianças focais: a triangulação educação-família-saúde na creche, realizado em 2008 e 2009 na Creche Institucional Dr. Paulo Niemeyer; e ii) Infância, Educação e Inclusão: um estudo de caso, realizado em 2009 na Creche Municipal de Odetinha Vidal de Oliveira. A pesquisa atual tem como proposta um estudo de follow-up, onde demos continuidade às duas anteriores, a partir da análise do percurso de três (3) famílias (mãe) na tentativa de garantir uma educação inclusiva de qualidade para seus filhos. Inicialmente, foi realizado um levantamento bibliográfico e documental sobre o tema. Em seguida voltou-se às famílias das crianças com o objetivo de investigar de que modo à escolarização foi sendo propiciadas a estas crianças e como suas dificuldades de aprendizagem têm sido entendidas nos espaços educacionais que frequentam. Adotamos o Estudo de Caso como proposta metodológica. Foram realizadas duas entrevistas com as mães das crianças, respectivamente em 2012 e 2013 e solicitado que elas respondessem um questionário (Caracterização Familiar), que delineava o perfil das mesmas destacando suas características sóciodemograficas. Os dados produzidos foram sistematizados através da abordagem de Análise de Conteúdo por temáticas, com ênfase nas trajetórias das crianças e suas famílias em prol da garantia ao direito à Educação. A pesquisa conclui que as crianças do estudo não encontraram espaço no sistema regular de educação, público e/ou privado, em contraste ao que garante os documentos nacionais e municipais. As trajetórias e experiências foram repletas de inseguranças e expectativas negativas por parte das escolas quanto ao desenvolvimento e escolarização das crianças. Conclui também que não é suficiente conhecer os direitos à educação da criança com necessidades especiais, as instituições precisam reconhecer os familiares como parceiros privilegiados na construção de alternativas para a produção de conhecimentos das crianças com necessidades especiais. Os dados demonstraram a importância social das escolas especiais no atendimento especializado de crianças com necessidades especiais. Os lugares ocupados por essas instituições são reconhecido pelas famílias como fundamental rede de apoio e suporte às crianças e famílias no processo de educação e inclusão escolar.

Background: Despite being the third largest tobacco producer in the world, Brazil has developed a comprehensive tobacco control policy that includes a broad restriction on both advertising and smoking in indoor public places, compulsory pictorial warning labels, and a menthol cigarette ban. However, tax and pricing policies have been developed slowly and only very recently were stronger measures implemented. This study investigated the expected responses of smokers to hypothetical price increases in Brazil.Methods: We analyzed smokers' responses to hypothetical future price increases according to sociodemographic characteristics and smoking conditions in a multistage sample of Brazilian current cigarette smokers aged >= 14 years (n = 500). Logistic regression analysis was used to examine the relationship between possible responses and different predictors.Results: in most subgroups investigated, smokers most frequently said they would react to a hypothetical price increase by taking up alternatives that might have a positive impact on health, i.e., they would try to stop smoking (52.3%) or smoke fewer cigarettes (46.8%). However, a considerable percentage responded that they would use alternatives that would reduce the effect of price increases, such as the same brand with lower cost (48.1%). After controlling for sex age group (14-19, 20-39, 40-59, and >= 60 years), schooling level (>= 9 versus <= 9 years), number of cigarettes per day (>20 versus <= 20), and stage of change for smoking cessation (precontemplation, contemplation, and preparation), lower levels of dependence were positively associated with the response I would try to stop smoking (odds ratio [OR], 2.19). Young age was associated with I would decrease the number of cigarettes (OR, 3.44). A low schooling level was strongly associated with all responses.Conclusions: Taxes and prices increases have great potential to stimulate cessation or reduction of cigarette consumption further among two important vulnerable populations of smokers in Brazil: young smokers and those of low educational level. the results from the present study also suggest that seeking illegal products may reduce the impact of increased taxes, but does not eliminate it.

BACKGROUND: Monogamy, together with abstinence, partner reduction, and condom use, is widely advocated as a key behavioral strategy to prevent HIV infection in sub-Saharan Africa. We examined the association between the number of sexual partners and the risk of HIV seropositivity among men and women presenting for HIV voluntary counseling and testing (VCT) in northern Tanzania. METHODOLOGY/ PRINCIPAL FINDINGS: Clients presenting for HIV VCT at a community-based AIDS service organization in Moshi, Tanzania were surveyed between November 2003 and December 2007. Data on sociodemographic characteristics, reasons for testing, sexual behaviors, and symptoms were collected. Men and women were categorized by number of lifetime sexual partners, and rates of seropositivity were reported by category. Factors associated with HIV seropositivity among monogamous males and females were identified by a multivariate logistic regression model. Of 6,549 clients, 3,607 (55%) were female, and the median age was 30 years (IQR 24-40). 939 (25%) females and 293 (10%) males (p<0.0001) were HIV seropositive. Among 1,244 (34%) monogamous females and 423 (14%) monogamous males, the risk of HIV infection was 19% and 4%, respectively (p<0.0001). The risk increased monotonically with additional partners up to 45% (p<0.001) and 15% (p<0.001) for women and men, respectively with 5 or more partners. In multivariate analysis, HIV seropositivity among monogamous women was most strongly associated with age (p<0.0001), lower education (p<0.004), and reporting a partner with other partners (p = 0.015). Only age was a significant risk factor for monogamous men (p = 0.0004). INTERPRETATION: Among women presenting for VCT, the number of partners is strongly associated with rates of seropositivity; however, even women reporting lifetime monogamy have a high risk for HIV infection. Partner reduction should be coupled with efforts to place tools in the hands of sexually active women to reduce their risk of contracting HIV.

Background

Little is known about the quality of life (QoL) of disabled children. We describe self-reported QoL of children with cerebral palsy, factors that influence it, and how it compares with QoL of the general population.

Methods

1174 children aged 8–12 years were randomly selected from eight population-based registers of children with cerebral palsy in six European countries and 743 (63%) agreed to participate; one further region recruited 75 children from multiple sources. Researchers visited these 818 children. 318 (39%) with severe intellectual impairment could not self-report; 500 (61%) reported their QoL using KIDSCREEN, an instrument with scores in ten domains, each with SD=10. Multivariable regression was used to relate QoL to impairments, pain, and sociodemographic characteristics. Comparisons were made with QoL data from the general population.

Findings

Impairments were not significantly associated with six KIDSCREEN domains. Comparison of least and most able groups showed that severely limited self-mobility was significantly associated with reduced mean score for physical wellbeing (7·6, 95% CI 2·7–12·4); intellectual impairment with reduced mean for moods and emotions (3·7, 1·5–5·9) and autonomy (3·3, 0·9–5·7); and speech difficulties with reduced mean for relationships with parents (4·5, 1·9–7·1). Pain was common and associated with lower QoL on all domains. Impairments and pain explained up to 3% and 7%, respectively, of variation in QoL. Children with cerebral palsy had similar QoL to children in the general population in all domains except schooling, in which evidence was equivocal, and physical wellbeing, in which comparison was not possible.

Interpretation

Parents can be reassured that most children aged 8–12 years with cerebral palsy will have similar QoL to other children. This finding should guide social and educational policy to ensure that disabled children participate fully in society. Because of its association with QoL, children's pain should be carefully assessed.

Objectives
To evaluate how involvement in life situations (participation) in children with cerebral palsy varies with type and severity of impairment and to investigate geographical variation in participation.

Design
Cross sectional study. Trained interviewers visited parents of children with cerebral palsy; multilevel multivariable regression related participation to impairments, pain, and sociodemographic characteristics.

Setting
Eight European regions with population registers of children with cerebral palsy; one further region recruited children from multiple sources.

Participants
1174 children aged 8-12 with cerebral palsy randomly selected from the population registers, 743 (63%) joined in the study; the further region recruited 75 children.

Main outcome measure
Children’s participation assessed by the Life-H questionnaire covering 10 main areas of daily life. Scoring ignored adaptations or assistance required for participation.

Results
Children with pain and those with more severely impaired walking, fine motor skills, communication, and intellectual abilities had lower participation across most domains. Type of cerebral palsy and problems with feeding and vision were associated with lower participation for specific domains, but the sociodemographic factors examined were not. Impairment and pain accounted for up to a sixth of the variation in participation. Participation on all domains varied substantially between regions: children in east Denmark had consistently higher participation than children in other regions. For most participation domains, about a third of the unexplained variation could be ascribed to variation between regions and about two thirds to variation between individuals.

Conclusions
Participation in children with cerebral palsy should be assessed in clinical practice to guide intervention and assess its effect. Pain should be carefully assessed. Some European countries facilitate participation better than others, implying some countries could make better provision. Legislation and regulation should be directed to ensuring this happens.

Objectives. We investigated whether exposure to negative aspects of close relationships was associated with subsequent increase in body mass index (BMI) and waist circumference.
Methods. Data came from a prospective cohort study (Whitehall II) of 9425 civil servants aged 35 to 55 years at baseline (phase 1: 1985-1988). We assessed negative aspects of close relationships with the Close Persons Questionnaire (range 0-12) at phases 1 and 2 (1989-1990). We measured BMI and waist circumference at phases 3 (1991-1994) and 5 (1997-1999). Covariates at phase 1 included gender, age, marital status, ethnicity, BMI, employment grade, smoking, physical activity, fruit and vegetable consumption, and common mental disorder.
Results. After adjustment for sociodemographic characteristics and health behaviors, participants with higher exposure to negative aspects of close relationships had a higher likelihood of a 10% or greater increase in BMI and waist circumference (odds ratios per 1-unit increase 1.08 [95% confidence interval (CI)=1.02, 1.14; P=.007] and 1.09 [CI=1.04, 1.14; P <= .001], respectively) as well as a transition from the overweight (25 <= BMI <30) to the obese (BMI >= 30) category.
Conclusions. Adverse social relationships may contribute to weight gain.

BACKGROUND: Physical inactivity has been associated with obesity and related chronic diseases. Understanding built environment (BE) influences on specific domains of physical activity (PA) around homes and workplaces is important for public health interventions to increase population PA.

PURPOSE: To examine the association of home and workplace BE features with PA occurring across specific life domains (work, leisure, and travel).

METHODS: Between 2012 and 2013, telephone interviews were conducted with participants in four Missouri metropolitan areas. Questions included sociodemographic characteristics, home and workplace supports for PA, and dietary behaviors. Data analysis was conducted in 2013; logistic regression was used to examine associations between BE features and domain-specific PA.

RESULTS: In home neighborhoods, seven of 12 BE features (availability of fruits and vegetables, presence of shops and stores, bike facilities, recreation facilities, crime rate, seeing others active, and interesting things) were associated with leisure PA. The global average score of home neighborhood BE features was associated with greater odds of travel PA (AOR=1.99, 95% CI=1.46, 2.72); leisure PA (AOR=1.84, 95% CI=1.44, 2.34); and total PA (AOR=1.41, 95% CI=1.04, 1.92). Associations between workplace neighborhoods' BE features and workplace PA were small but in the expected direction.

CONCLUSIONS: This study offers empirical evidence on BE supports for domain-specific PA. Findings suggest that diverse, attractive, and walkable neighborhoods around workplaces support walking, bicycling, and use of public transit. Public health practitioners, researchers, and worksite leaders could benefit by utilizing worksite domains and measures from this study for future BE assessments.

Background The Well London programme used community engagement, complemented by changes to the physical and social neighbourhood environment, to improve physical activity levels, healthy eating and mental wellbeing in the most deprived communities in London. The effectiveness of Well London is being evaluated in a pair-matched cluster randomised trial (CRT). The baseline survey data are reported here. Methods The CRT involved 20 matched pairs of intervention and control communities (defined as UK census lower super output areas; ranked in the 11% most deprived LSOAs in London by Index of Multiple Deprivation) across 20 London boroughs. The primary trial outcomes, sociodemographic information and environmental neighbourhood characteristics were assessed in three quantitative components within the Well London CRT at baseline: a cross-sectional, interviewer-administered adult household survey; a self-completed, school-based adolescent questionnaire; a fieldworker completed neighbourhood environmental audit. Baseline data collection occurred in 2008. Physical activity, healthy eating and mental wellbeing were assessed using standardised, validated questionnaire tools. Multiple imputation was used to account for missing data in the outcomes and other variables in the adult and adolescent surveys. Results There were 4107 adults and 1214 adolescent respondents in the baseline surveys. The intervention and control areas were broadly comparable with respect to the primary outcomes and key sociodemographic characteristics. The environmental characteristics of the intervention and control neighbourhoods were broadly similar. There was greater between cluster variation in the primary outcomes in the adult population compared to the adolescent population. Levels of healthy eating, smoking and self-reported anxiety/depression were similar in the Well London population and the national Health Survey for England. Levels of physical activity were higher in the Well London population but this is likely to be due to the different measurement tools used in the two surveys. Conclusions Randomisation of social interventions such as Well London is acceptable and feasible and in this study the intervention and control arms are well balanced with respect to the primary outcomes and key sociodemographic characteristics. The matched design has improved the statistical efficiency of the study amongst adults but less so amongst adolescents. Follow-up data collection will be completed 2012.

Background Clustering of lifestyle risk behaviours is very important in predicting premature mortality. Understanding the extent to which risk behaviours are clustered in deprived communities is vital to most effectively target public health interventions. Methods We examined co-occurrence and associations between risk behaviours (smoking, alcohol consumption, poor diet, low physical activity and high sedentary time) reported by adults living in deprived London neighbourhoods. Associations between sociodemographic characteristics and clustered risk behaviours were examined. Latent class analysis was used to identify underlying clustering of behaviours. Results Over 90% of respondents reported at least one risk behaviour. Reporting specific risk behaviours predicted reporting of further risk behaviours. Latent class analyses revealed four underlying classes. Membership of a maximal risk behaviour class was more likely for young, white males who were unable to work. Conclusions Compared with recent national level analysis, there was a weaker relationship between education and clustering of behaviours and a very high prevalence of clustering of risk behaviours in those unable to work. Young, white men who report difficulty managing on income were at high risk of reporting multiple risk behaviours. These groups may be an important target for interventions to reduce premature mortality caused by multiple risk behaviours.

RESUMO - Angola é um país em desenvolvimento e dado o baixo número de profissionais na reabilitação e a centralização dos cuidados desta área na capital do país, Luanda, é pertinente caraterizar o acesso dos utentes e perceber em que medida é que a oferta destes cuidados satisfaz as necessidades atuais da população. Sendo assim, este estudo teve como objetivos, avaliar o acesso dos utentes do Centro de Referência Nacional público para Reabilitação em Angola, CMFR, e identificação de áreas prioritárias de ação para promoção de igualdade no acesso. A análise metodológica consistiu na recolha e análise de dados dos utentes que acederam ao CMFR de Setembro de 2014 a Janeiro de 2015 e pesquisa documental abrangente, não-sistemática, sobre equidade e acesso a cuidados de saúde em Reabilitação a nivel internacional e em Angola. Para o tratamento e recolha de dados foram consideradas variáveis sociodemográficas e caraterizadoras do acesso onde se fez a caraterização da população em estudo, análise das variáveis resultantes do acesso ao CMFR através de uma análise de regressão.

Numerous studies have documented subtle but consistent sex differences in self-reports and observer-ratings of five-factor personality traits, and such effects were found to show well- defined developmental trajectories and remarkable similarity across nations. In contrast, very little is known about perceived gender differences in five-factor traits in spite of their potential implications for gender biases at the interpersonal and societal level. In particular, it is not clear how perceived gender differences in five-factor personality vary across age groups and national contexts and to what extent they accurately reflect assessed sex differences in personality. To address these questions, we analyzed responses from 3,323 individuals across 26 nations (mean age = 22.3 years, 31% male) who were asked to rate the five-factor personality traits of typical men or women in three age groups (adolescent, adult, and older adult) in their respective nations. Raters perceived women as slightly higher in openness, agreeableness, and conscientiousness as well as some aspects of extraversion and neuroticism. Perceived gender differences were fairly consistent across nations and target age groups and mapped closely onto assessed sex differences in self- and observer-rated personality. Associations between the average size of perceived gender differences and national variations in sociodemographic characteristics, value systems, or gender equality did not reach statistical significance. Findings contribute to our understanding of the underlying mechanisms of gender stereotypes of personality and suggest that perceptions of actual sex differences may play a more important role than culturally based gender roles and socialization processes.

Nous nous sommes intéressés à l’analyse et à la mise à jour d’une typologie de l’aide reçue par les personnes âgées de 65 ans et plus vivant à domicile. Cette étude secondaire s’est basée sur les données recueuillies dans deux milieux francophones, Hochelaga-Maisonneuve (HM) et Moncton (MCT). La collecte de données avait été faite par l’entremise d’un questionnaire administré par entrevue face à face. Les deux objectifs, de cette thèse sont : 1) Établir une typologie des réseaux d’aide, résultant de la combinaison des sources d’aide et des tâches accomplies ; 2) Identifier les principaux déterminants d’appartenance aux réseaux. La typologie obtenue met en relation les ressources, formelles ou informelles, utilisées par les personnes âgées et l’aide instrumentale reçue. La capacité ou l’incapacité à effectuer neuf activités de la vie quotidienne et huit de la vie domestique ont servi à évaluer l’aide reçue. Six ressources formelles et dix informelles ont été examinées selon qu’elles étaient les 1ères, 2ièmes ou 3ièmes sources d’aide utilisées par les personnes âgées. L’approche privilégiée s’est inspirée de celle des réseaux sociaux et du modèle de Pescosolido. C’est l’influence des caractéristiques sociodémographiques des personnes âgées, de leurs états de santé, de leurs habitudes de vie sur leurs réseaux qui nous ont intéressés. Les résultats sont présentés à chaque fois pour nos deux milieux séparément. Nous commençons par un descriptif des sources d’aide utilisées et des aides reçues. Puis les profils des sources d’aide utilisées et des activités accomplies sont exposés pour l’ensemble des personnes âgées. Ces profils servent de base pour obtenir notre typologie. Elle comprend cinq catégories. Ces catégories sont toutes composées de personnes âgées faisant appel à de l’aide formelle, informelle ou mixte pour accomplir des tâches uniques ou multiples. La première catégorie « Transitoire », comprend 39% (HM) et 46% (MCT) des personnes âgées qui débute un processus d’incapacité. Elles font appel à des ressources informelles pour accomplir une tâche unique. La deuxième catégorie « Personnes âgées seules » en rassemble 14% (HM et MCT), majoritairement des femmes, avec peu d’incapacités. Ces dernières utilisent de l’aide formelle pour une tâche unique. La troisième catégorie « Familiale » regroupe 12% (HM et MCT) des personnes âgées bien entourées qui ont plusieurs incapacités. Ces gens font appel à des sources d’aide informelles pour réaliser des tâches multiples. La quatrième catégorie « Très fragile » rassemble 30% (HM) et 25% (MCT) des personnes âgées peu entourées ayant beaucoup d’incapacités. Elles utilisent des ressources d’aide mixtes pour effectuer des tâches multiples. La cinquième catégorie « Pré institutionnel » comprend 4% (HM et MCT) des personnes âgées qui ont le plus d’incapacités et qui sont seules. Ces gens font appel à de l’aide formelle pour des tâches multiples. Les déterminants d’appartenance à ces catégories proviennent des blocs sociodémographiques, état de santé et réseaux sociaux de notre modèle théorique. Une des contributions importantes de cette thèse a été de pouvoir identifier cinq catégories bien distinctes composant une typologie de l’aide reçue, indépendamment du milieu, par des personnes âgées vivant à domicile.
