This qualitative study offers insight into the experiences, expectations, perceptions and beliefs that may lead to laparoscopic adjustable gastric band patients’ failure to achieve expected weight loss and seek revisional bariatric surgery. The 23 participants from two sites were interviewed and data were analysed from a grounded theory methodology in order to build a causal model. Analysis of participants’ reports identified ‘unrealistic expectations of the LAGB’ as the core category. Additionally, the restriction of the band had a negative impact on participants’ social interactions, leading to feelings of deprivation and, thus, to a desire for reward from food choices and consequently an increase of consumption of high-calorie-dense foods. These foods were chosen because of their specific texture or ability to provide reward. The resulting increase in weight or failure to achieve excess weight loss, led to feelings of shame and loneliness and emotional eating resulting in increased the consumption of rewarding foods. Thus, identifying unrealistic expectations of laparoscopic adjustable gastric band (LAGB) and emotional eating behaviours are important in those who are present initially for primary bariatric and revisional bariatric surgery, as they may contribute specifically to these patients’ weight regain and consequent failure to achieve excess weight loss.

Functional Imagery Training (FIT) is a new theory-based, manualized intervention that trains positive goal imagery. Multisensory episodic imagery of proximal personal goals is elicited and practised, to sustain motivation and compete with less functional cravings. This study tested the impact of a single session of FIT plus a booster phone call on snacking. In a stepped-wedge design, 45 participants who wanted to lose weight or reduce snacking were randomly assigned to receive a session of FIT immediately or after a 2-week delay. High-sugar and high-fat snacks were recorded using timeline follow back for the previous 3 days, at baseline, 2 and 4 weeks. At 2 weeks, snacking was lower in the immediate group than in the delayed group, and the reduction after FIT was replicated in the delayed group between 2 and 4 weeks. Frequencies of motivational thoughts about snack reduction rose following FIT for both groups, and this change correlated with reductions in snacking and weight loss. By showing that FIT can support change in eating behaviours, these findings show its potential as a motivational intervention for weight management.

The study sought to contextualize the physical, social and emotional adjustments that are faced by oesophageal cancer patients following surgery. Semi-structured interviews were conducted with five survivors, guided by the principles of Interpretative Phenomenological Analysis (IPA). Participants’ accounts encompassed descriptions of personal, social and medical relationships, illness and treatment experiences, eating behaviours, and spiritual and religious perspectives, representing myriad challenges to the self-concept. Surviving patients may have a role in addressing patient expectations about eating. The importance of attempts to nurture and maintain a sense of self should be recognized by those providing care.

The incentive sensitisation model of obesity suggests that modification of the dopaminergic associated reward systems in the brain may result in increased awareness of food-related visual cues present in the current food environment. Having a heightened awareness of these visual food cues may impact on food choices and eating behaviours with those being most aware of or demonstrating greater attention to food-related stimuli potentially being at greater risk of overeating and subsequent weight gain. To date, research related to attentional responses to visual food cues has been both limited and conflicting. Such inconsistent findings may in part be explained by the use of different methodological approaches to measure attentional bias and the impact of other factors such as hunger levels, energy density of visual food cues and individual eating style traits that may influence visual attention to food-related cues outside of weight status alone. This review examines the various methodologies employed to measure attentional bias with a particular focus on the role that attentional processing of food-related visual cues may have in obesity. Based on the findings of this review, it appears that it may be too early to clarify the role visual attention to food-related cues may have in obesity. Results however highlight the importance of considering the most appropriate methodology to use when measuring attentional bias and the characteristics of the study populations targeted while interpreting results to date and in designing future studies.

Resumo: Na promoção de comportamentos alimentares saudáveis ao longo da vida. que deve iniciar-se o mais precocemente possível, a educação alimentar é um elemento chave. O jardim-de-infância é um espaço efectivo na implementação de projectos de educação alimentar, onde os educadores de infância são actores imprescindíveis. Este estudo teve como objectivo desenvolver uma metodologia apropriada para identificar as capacidades de estudantes de educação de infância e avaliar as suas aptidões no contexto da educação alimentar infantil. A população alvo do estudo foram estudantes do 4ºano da licenciatura em educação de infância de instituições públicas e privadas da área metropolitana de Lisboa (n=287). Construiu-se um videograma, integrador dos domínios da aprendizagem (cognitivo e afectivo) e dos factores associados aos comportamentos em saúde (predisponentes, capacitadores e de esforço). A construção dos questionários estruturou-se em: conhecimentos-atitudes-comportamentos.Pretendia-se que os estudantes transmitissem a sua opinião, de acordo com os questionários.Os estudantes manifestaram conhecimentos, no âmbito da alimentação infantil e da utilidade e aplicabilidade da educação alimentar.Nenhum estudante apresentou conhecimentos negativos, numa escala de zero a 18 valores, 86,9% obtiveram uma classificação entre 11 e 17 valores e 13,1% a classificação máxima.A atitude dos estudantes revelou-se consensual com o desejável, 98,5% apresentaram atitudes positivas face aos aspectos estruturadores das actividades de educação alimentar.No domínio do "saber fazer" 51,0% dos estudantes demonstraram fragilidades no seu desempenho/comportamento. Em conclusão, a utilização do questionário demonstrou ser apropriado para a medição dos conhecimentos, das atitudes e como forma de os estudantes, após visionarem o videograma, espelharem o seu desempenho/comportamento face à representação do educador de infância. A taxa de resposta nunca foi inferior a 98,9% (n=284). No domínio dos conhecimentos, os estudantes manifestaram concepções positivas no campo da alimentação infantil e da educação alimentar. As atitudes enfatizaram-se no domínio afectivo, nos aspectos relacionais e comunicacionais. No domínio dos comportamentos, 91,8% dos estudantes apresentaram dificuldades na identificação dos aspectos menos correctos do desempenho dos actores do video, o que pode sugerir lacunas no âmbito do "saber fazer". Abstract:Nutrition education is key element to promote lifelong healthy eating behaviours and it must begin since early stages of life. Nursery schools are an effective space in the implementation of nutrition education projects, where the nursery teachers are indispensable actors. The aim of this study was to develop an appropriate methodology to identify nursery education student´s skills and evaluate their attitudes in the childhood nutrition education's context. The study's population were students of the 4th degree in private and public nursery education universities in Lisbon metropolitan area (n=287). A video integrator of learning domains (cognitive and affective) and of factors associated with health behaviours (predisposing, enabling and reinforcing) was developed. Questionnaire´s construction was structured in: knowledge-attitudes-behaviours.According to the questionnaires students were asked to transmit their opinion. Students revealed knowledge, in the scope of the childhood nutrition and in utility and applicability of nutrition education. No students presented negative results in knowledge ina zero - 18 scale; 86,9% students obtained a classification between 11 and 17 and the maximum classification was obtained by 13.1% students. Student's attitude was according to win the desirable, 98,5% student´s revealed positive attitudes in the nutrition education's structure aspects of chilhood activities.In the domain of the "know to do" 51,0% of students showed fragilities in their performance/behaviour. In conclusion, the questionnaire´s utilization confirm to be appropriated in measurement knowledge and attitudes and also, after watching the video, as a way for students reflect their performance/behaviour face to the nursery teacher´s representation. Answer´s rate was never lower than 98.9% (n=284). In the knowledge's domain students demonstrated positive conceptions in the childhood nutrition and nutrition education's fields. Attitudes were emphasized in the effective domain in the communication and relational aspects. In behaviour's domain 91.8% students presented difficulties in identifying the less correct aspects of the video actor's performance what may suggest gaps in the "know to do" scope.

RESUMO - Este artigo aborda a situação do consumo alimentar em Portugal e apresenta resultados de vários estudos sobre as estratégias para influenciar os comportamentos alimentares, junto da população jovem, na escola. Aplicando os fundamentos teóricos da promoção da saúde à educação nas escolas e à gestão da rede das escolas promotoras de saúde, ilustra uma abordagem da educação alimentar coerente com esses fundamentos teóricos e sugere algumas orientações para promover uma alimentação saudável.

Both obesity and being underweight have been associated with increased mortality. Underweight, defined as a body mass index (BMI) ≤ 18.5 kg per m(2) in adults and ≤ -2 standard deviations from the mean in children, is the main sign of a series of heterogeneous clinical conditions including failure to thrive, feeding and eating disorder and/or anorexia nervosa. In contrast to obesity, few genetic variants underlying these clinical conditions have been reported. We previously showed that hemizygosity of a ∼600-kilobase (kb) region on the short arm of chromosome 16 causes a highly penetrant form of obesity that is often associated with hyperphagia and intellectual disabilities. Here we show that the corresponding reciprocal duplication is associated with being underweight. We identified 138 duplication carriers (including 132 novel cases and 108 unrelated carriers) from individuals clinically referred for developmental or intellectual disabilities (DD/ID) or psychiatric disorders, or recruited from population-based cohorts. These carriers show significantly reduced postnatal weight and BMI. Half of the boys younger than five years are underweight with a probable diagnosis of failure to thrive, whereas adult duplication carriers have an 8.3-fold increased risk of being clinically underweight. We observe a trend towards increased severity in males, as well as a depletion of male carriers among non-medically ascertained cases. These features are associated with an unusually high frequency of selective and restrictive eating behaviours and a significant reduction in head circumference. Each of the observed phenotypes is the converse of one reported in carriers of deletions at this locus. The phenotypes correlate with changes in transcript levels for genes mapping within the duplication but not in flanking regions. The reciprocal impact of these 16p11.2 copy-number variants indicates that severe obesity and being underweight could have mirror aetiologies, possibly through contrasting effects on energy balance.

The purpose of this study was to examine the influence of family support on diabetes education behavioural outcomes, specifically in relation to diet, exercise, and blood glucose monitoring in adult individuals with Type 2 diabetes. Fifty-three individuals attending diabetes education for the first time were followed approximately 1 month. The findings for the influence of family support were mixed. Family attending diabetes class with participants had a positive influence with respect to diet. This is consistent with Carl Rogers (1969) who espouses setting a positive climate for learning and that learning new attitudes or information comes when external barriers are at a minimum. However family attending class with participants had no influence with respect to exercise or blood glucose monitoring. The family support action of encouraging with respect to diet overall did not influence healthy eating behaviours except for decreased skipped meals and scheduled snacks. In fact, in the areas of family willing to make healthy choices along with participant, the less the family was involved in encouraging, the better the participant did. Exercise on the other hand was influenced positively by family encouragement. This is consistent with Bandura's theory that enhancement of self-confidence and self-efficacy can lead to desired behaviour changes. Family encouragement however did not appear to influence blood glucose monitoring behaviours. This study has implications for practice in that diabetes education programs can encourage family to attend classes or get involved in encouraging the person with diabetes, so that it may help to increase healthy eating behaviours and exercise. As time is necessary to implement changes in behaviour, future research can look at the influence of family support over a 6-month, I-year, or greater period.

Introduction : L’adolescence est une période cruciale dans le développement des saines habitudes alimentaires et pour le maintien de celles-ci à l’âge adulte. Les choix alimentaires sont guidés par de multiples facteurs individuels, environnementaux et sociaux qui, durant la période de l’adolescence, se butent à une quête identitaire. Objectifs : Explorer et mettre en relief les principaux déterminants jouant un rôle dans le processus décisionnel des choix alimentaires chez de jeunes adolescents québécois tout en évaluant la méthode de recherche qualitative utilisée. Méthodologie : Issue du projet de recherche « Les dimensions socioculturelles des pratiques alimentaires et d'activité physique des jeunes : une enquête qualitative auprès d'adolescents québécois de 12 à 14 ans », cette étude repose sur l’analyse de 30 entrevues semi-dirigées utilisant des photographies en lien avec l’alimentation. Résultats : Les facteurs individuels et ceux liés aux environnements sociaux se démarquent principalement. Parmi les facteurs individuels, le goût et les perceptions sensorielles, la représentation ou l’impression personnelle des aliments et les connaissances nutritionnelles sont très influents. La famille ainsi que les activités sociales associées à l’aliment ou au lieu de consommation dominent au chapitre des facteurs sociaux. Enfin, l’utilisation de la photographie s’avère un outil de collecte de données très intéressant afin d’obtenir des propos complémentaires en situation d’entrevue. Conclusion : Cette étude confirme l’importance de certains déterminants (surtout individuels et sociaux) qui influencent les comportements alimentaires des jeunes adolescents québécois. Les connaissances acquises dans le cadre de ce projet seront utiles pour les divers intervenants appelés à développer ou à mettre en œuvre des programmes d’intervention adaptés pour la promotion de la santé et de saines habitudes chez les jeunes adolescents québécois. Mudanças notáveis nos hábitos alimentares, acompanhadas de práticas fisioculturistas intensas, são a tônica dos dias atuais, com registros de carências nutricionais e de obesidade bastante preocupantes, do ponto de vista da saúde pública, entre crianças, jovens e adultos. Mas o que conhecem as crianças e os adolescentes sobre o processo de digestão-nutrição, os conceitos básicos envolvidos e as condutas alimentares adequadas à boa saúde humana? Como é desenvolvido esse tema nas escolas públicas e particulares de ensino? Tais questionamentos desencadearam um estudo sobre a natureza das práticas desenvolvidas por professores de ciências e biologia e o conhecimento apresentado por alunos de escolas públicas e particulares. Os resultados revelaram inadequação no tratamento metodológico de ensino do processo de digestão e conceitos envolvidos nesse tema, que levam os alunos ao desinteresse e a manterem praticamente inalterados os conhecimentos ordinários que possuem. O processo de digestão e nutrição, bem como suas implicações para a saúde, configuraram-se como fenômenos desvinculados do aluno, à semelhança do que observamos nos livros didáticos por eles utilizados. A dinâmica das inter-relações alimentares entre seres vivos são superficialmente consideradas em ecologia e passam ao largo das adaptações comportamentais, morfológicas e fisiológicas envolvidas. Considerando esses resultados, propõe-se conteúdo baseado em abordagem ecológica, voltado para determinadas atividades experimentais, jogos e interações coevolutivas de seres vivos - aspectos biológicos e sociais, para o despertar de posturas reflexivas e críticas diante das transformações sociais em curso e de nossas necessidades biológicas no que se refere à alimentação e saúde.

The aim of this study was to explore how the structure of mealtimes within the family setting is related to children's fussy eating behaviours. Seventy-five mothers of children aged between 2 and 4 years were observed during a typical mealtime at home. The mealtimes were coded to rate mealtime structure and environment as well as the child's eating behaviours (food refusal, difficulty to feed, eating speed, positive and negative vocalisations). Mealtime structure emerged as an important factor which significantly distinguished children with higher compared with lower levels of food fussiness. Children whose mothers ate with their child and ate the same food as their child were observed to refuse fewer foods and were easier to feed compared with children whose mothers did not. During mealtimes where no distractors were used (e.g. no TV, magazines or toys), or where children were allowed some input into food choice and portioning, children were also observed to demonstrate fewer fussy eating behaviours. Findings of this study suggest that it may be important for parents to strike a balance between structured mealtimes, where the family eats together and distractions are minimal, alongside allowing children some autonomy in terms of food choice and intake.

Grandparents play a valuable role in the socialisation of young children, and as many as 36% of British parents use grandparents as their main form of childcare. Research has begun to explore how grandparents impact the social and cognitive development of children, but very little research has evaluated their contribution to child feeding. The present study explores whether there are differences between parents and grandparents in terms of their feeding practices, and whether grandparents' feeding practices are related to the number of hours that they spend caring for grandchildren. Results indicate that grandparents reported using significantly more maladaptive feeding practices such as using food to regulate emotions and restricting food, but more positive practices such as providing a healthy food environment. The more hours that grandparents spent caring for children the more their feeding practices resembled those broadly reported by parents. Results suggest that grandparents can have a measurable impact on child feeding behaviour which in turn is likely to predict the eating behaviours of their grandchildren. © 2014 Elsevier Ltd.

Introdução e objectivos: A saúde oral em atletas é parte essencial para a saúde geral, sendo um factor determinante para a qualidade de vida e desempenho desportivo. Logo, um bom estado de saúde só existirá se a cavidade oral se encontrar ausente de patologias. Os desequilíbrios nutricionais possuem efeitos sobre a cavidade oral, condicionando assim a qualidade de vida e desempenho do atleta. Desta forma, o excesso de ingestão de alguns alimentos podem ser factores de risco para a saúde, tendo conta em que, a etiologia da cárie dentária está relacionada com a ação de microorganismos orais que produzem ácidos orgânicos, a partir do metabolismo dos hidratos de carbono. O objectivo deste estudo foi avaliar a saúde oral bem como os hábitos alimentares e o uso de protetores bucais durante a prática desportiva. Participantes e Métodos: Foram observados 55 atletas de voleibol entre os 15 e 18 anos de ambos os géneros, do clube de voleibol Academia José Moreira e Leixões. Tratou-se de um estudo transversal, no qual foi realizado exame clínico intraoral (índice de cárie CPOd, indice de erosão dentária BEWE) e preenchimento de questinário, em que os indivíduos foram caracterizados em 5 componentes: dados sociodemográficos (idade, peso e estatura), dados sobre perceção de saúde, dados sobre comportamentos de saúde oral, dados sobre prática desportiva e dados sobre comportamentos alimentares (questionário semi-quantitativo de frequência alimentar). A análise estatística descritiva e inferencial dos dados recolhidos foi realizada com o auxílio do programa informático SPSS, versão 23.0. Resultados: Os hábitos de saúde oral não são os mais adequados e a percentagem de atletas que visita o médico dentista é elevada para “só quando tem dores” ou “ocasionalmente”. A média do CPOD geral foi de 4,22 ± 4,55. Não houve diferenças estatisticamente significativas (p>0,05) entre o CPOD geral e o IMC. Nenhum dos atletas usa protetor bucal durante a prática desportiva. Os alimentos mais consumidos foram a carne, fruta, leite, peixe e, biscoitos, bolos e bolachas; e os menos consumidos foram as bebidas alcoólicas, mel ou compotas e café. Conclusão: Os hábitos de higiene oral são um melhor indicador do que o IMC para a presença de cárie. Não há relação direta entre índice CPOD e IMC. Seria importante prestar mais informação sobre vantagens do uso de protetores bucais junto dos atletas e de treinadores bem como, esclarecer que a consistência e as propriedades sensoriais ligadas à textura e à consistência dos alimentos na superfície dentária interferem com a cárie.
