Trypanosoma cruzi is the etiological agent of Chagas` disease, a pathogenesis that affects millions of people in Latin America. Here, we report the crystal structure of dihydroorotate dehydrogenase (DHODH) from T cruzi strain Y solved at 2.2 angstrom resolution. DHODH is a flavin mononucleotide containing enzyme, which catalyses the oxidation Of L-dihydroorotate to orotate, the fourth step and only redox reaction in the de novo biosynthesis of pyrimidine nucleotides. Genetic studies have shown that DHODH is essential for T cruzi survival, validating the idea that this enzyme can be considered an attractive target for the development of antichagasic drugs. In our work, a detailed analysis of T cruzi DHODH crystal structure has allowed us to suggest potential sites to be further exploited for the design of highly specific inhibitors through the technology of structure-based drug design. A three-phase LPME (liquid-phase microextraction) method for the enantioselective analysis of venlafaxine (VF) metabolites (O-desmethylvenlafaxine (ODV) and N-desmethylvenlafaxine (NDV) in microsomal preparations is described for the first time. The assay involves the chiral HPLC separation of drug and metabolites using a Chiralpak AD column under normal-phase mode of elution and detection at 230 nm. The LPME procedure was optimized using multifactorial experiments and the following optimal condition was established: sample agitation at 1,750 rpm, 20 min of extraction, acetic acid 0.1 mol/L as acceptor phase, 1-octanol as organic phase and donor phase pH adjustment to 10.0. Under these conditions, the mean recoveries were 41% and 42% for (-)-(R)-ODV and (+)-(S)-ODV, respectively, and 47% and 48% for (-)-( R)-NDV and (+)-( S)-NDV, respectively. The method presented quantification limits of 200 ng/mL and it was linear over the concentration range of 200-5,000 ng/mL for all analytes. The validated method was employed to study the in vitro biotransformation of VF using rat liver microsomal fraction. The results demonstrated the enantioselective biotransformation of VF.

Objectives: To develop an index for the ratio of metalloproteinase 2 (MMP-2) to its tissue inhibitor (TIMP-2) in immunostained medullary thyroid carcinoma specimens and to correlate it with clinical and pathologic prognostic factors. Metalloproteinases, enzymes related to the degradation of the extracellular matrix, take part in carcinogenesis and have been associated with the prognosis of neoplasias. Nevertheless, medullary carcinoma is rarely considered in research analysis. Researchers tend to favor the ratio of enzymes to their inhibitors over the absolute concentrations of these enzymes. Design: Retrospective study of surgical samples. Setting: Head and Neck Surgery and Endocrinology Departments, Universidade de Sao Paulo Medical School Hospital. Patients: Surgical specimens from 33 patients who had been observed for a mean of 76.8 months (range, 4-201 months) were immunohistochemically stained for MMP-2 and TIMP-2. Only patients whose clinical and pathologic data were complete and whose specimens were preserved were included in the study. Main Outcome Measures: The ratio between the expressions of MMP-2 and TIMP-2 was based on a staining index (immunostaining extent and intensity) of each of the markers. Results: Proportionally large expressions of TIMP-2 over MMP-2 correlated with low occurrences of positive findings on initial cervical examination for the presence of thyroid nodules and/or lymphadenopathy (P = .02) and cervical lymph node metastases (P < .001), conditions correlated with prognosis. A correlation with cure at the end of follow-up (P = .01) was also observed. (P < .05 was considered statistically significant.) Conclusion: The ratio of MMP-2 to TIMP-2 expression is an additional and novel prognostic predictor of the outcome of medullary carcinoma treated surgically.

Notified cases of dengue infections in Singapore reached historical highs in 2004 (9459 cases) and 2005 (13 817 cases) and the reason for such all increase is still to be established. We apply a mathematical model for dengue infection that takes into account the seasonal variation in incidence, characteristic of dengue fever, and which mimics the 2004-2005 epidemics in Singapore. We simulated a set of possible control strategies and confirmed the intuitive belief that killing adult mosquitoes is the most effective strategy to control an ongoing epidemic. On the other hand, the control of immature forms was very efficient ill preventing the resurgence of dengue epidemics. Since the control of immature forms allows the reduction of adulticide, it seems that the best strategy is to combine both adulticide and larvicide control measures during an outbreak, followed by the maintenance of larvicide methods after the epidemic has subsided. In addition, the model showed that the mixed strategy of adulticide and larvicide methods introduced by the government seems to be very effective in reducing the number of cases in the first weeks after the start of control.

Background. Clinical manifestations of dengue vary in different areas of endemicity and between specific age groups, whereas predictors of outcome have remained controversial. In Brazil, the disease burden predominantly affects adults, with an increasing trend toward progression to dengue hemorrhagic fever (DHF) noted. Methods. A cohort of adults with confirmed cases of dengue was recruited in central Brazil in 2005. Patients were classified according to the severity of their disease. Associations of antibody responses, viremia levels (as determined by real-time polymerase chain reaction [PCR]), and serotypes (as determined by multiplex PCR) with disease severity were evaluated. Results. Of the 185 symptomatic patients > 14 years of age who had a confirmed case of dengue, 26.5% and 23.2% were classified as having intermediate dengue fever (DF)/ DHF (defined as internal hemorrhage, plasma leakage, manifested signs of shock, and/ or thrombocytopenia [platelet count, <= 50,000 platelets/mm(3)]) and DHF, respectively. The onset of intermediate DF/ DHF and DHF occurred at a late stage of disease, around the period of defervescence. Patients with DHF had abnormal liver enzyme levels, with a > 3-fold increase in aspartate aminotransferase level, compared with the range of values considered to be normal. Overall, 65% of patients presented with secondary infections with dengue virus, with such infection occurring in similar proportions of patients in each of the 3 disease category groups. Dengue virus serotype 3 (DV3) was the predominant serotype, and viremia was detected during and after defervescence among patients with DHF or intermediate DF/ DHF. Conclusions. Viremia was detected after defervescence in adult patients classified as having DHF or intermediate DF/ DHF. Secondary infection was not a predictor of severe clinical manifestation in adults with infected with the DV3 serotype.

A survey was performed to estimate the frequency of Escherichia coli and Shiga toxin-producing E. coli (STEC) in carcasses obtained from an abattoir in Brazil between February 2006 and June 2007. A total of 216 beef carcasses were sampled at three stages of the slaughter process-preevisceration, postevisceration, and postprocessing-during the rain and dry seasons, respectively. Of the carcasses sampled, 58%, were preevisceration E. coli positive, 38% were postevisceration positive, and 32% postprocessing positive. At the postprocessing stage, the isolation of E. coli was twice as high in the rain season. E. coli was isolated from 85 carcasses of which only 3 (1.4%) were positive for stx-encoding genes. No E. coli O157 serogroup isolates were detected. No antimicrobial resistance was found in nine of the isolates (10% of the total). The most frequent resistances were seen against cephalothin (78%), streptomycin (38%), nalidixic acid (36%), and tetracycline (30%). Multidrug resistance (MDR) to three or more antimicrobial agents was determined in 28 (33%) E. coli isolates. The presence of STEC and MDR strains among the isolates in the beef carcasses emphasizes the importance of proper handling to prevent carcass contamination.

The complete nucleotide sequence of the mitochondrial (mt) DNA molecule of the liverfluke, Fasciola hepatica (phylum Platyhelminthes, class Trematoda, family Fasciolidae), was determined, It comprises 14462 bp, contains 12 protein-encoding, 2 ribosomal and 22 transfer RNA genes, and is the second complete flatworm (and the first trematode) mitochondrial sequence to be described in detail. All of the genes are transcribed from the same strand. Of the genes typically found in mitochondrial genomes of eumetazoans, only atp8 is absent. The nad4L and nad4 genes overlap by 40 nt. Most intergenic sequences are very short. Two larger non-coding regions are present. The longer one (817 nt) is located between trnG and cox3 and consists of 8 identical tandem repeats of 85 nt, rich in G and C, followed by 1 imperfect repeat. The shorter non-coding region (187 nt) exhibits no special features and is separated from the longer region by trnG. The gene arrangement resembles that of some other trematodes including the eastern Asian Schistosoma species (and cyclophyllidean cestode species) but it is strikingly different from that of the African schistosomes, represented by Schistosoma mansoni. The genetic code is as inferred previously for flatworms. Transfer RNA genes range in length from 58 to 70 nt, their products producing characteristic 'clover leaf' structures, except for tRNA(S-VON) and tRNA(S-AGN) lacking the DHU arm.

Pectus excavatum is the most common deformity of the thorax and usually comprises Computed Tomography (CT) examination for pre-operative diagnosis. Aiming at the elimination of the high amounts of CT radiation exposure, this work presents a new methodology for the replacement of CT by a laser scanner (radiation-free) in the treatment of pectus excavatum using personally modeled prosthesis. The complete elimination of CT involves the determination of ribs external outline, at the maximum sternum depression point for prosthesis placement, based on chest wall skin surface information, acquired by a laser scanner. The developed solution resorts to artificial neural networks trained with data vectors from 165 patients. Scaled Conjugate Gradient, Levenberg-Marquardt, Resilient Back propagation and One Step Secant gradient learning algorithms were used. The training procedure was performed using the soft tissue thicknesses, determined using image processing techniques that automatically segment the skin and rib cage. The developed solution was then used to determine the ribs outline in data from 20 patient scanners. Tests revealed that ribs position can be estimated with an average error of about 6.82±5.7 mm for the left and right side of the patient. Such an error range is well below current prosthesis manual modeling (11.7±4.01 mm) even without CT imagiology, indicating a considerable step forward towards CT replacement by a 3D scanner for prosthesis personalization.

O presente trabalho, trata-se de um estudo de Investigação-Acção desenvolvido numa instituição vocacionada para o ensino e para a reabilitação de crianças e jovens surdocegos, situada na cidade de Lisboa. Nesta pesquisa procuramos averiguar se a construção e dinamização de um jardim sensorial poderá constituir-se como um espaço de aprendizagem e de lazer para crianças e jovens surdocegos. De modo a encontrar respostas para esta problemática, procedemos à revisão de literatura considerada pertinente para esta temática e ao levantamento de informação de modo a caracterizar o meio ambiente, quer a instituição onde o projecto foi implementado. Para a concretização deste estudo procedeu-se a uma recolha dos dados considerados relevantes. Posteriormente, com recurso à Investigação-Acção, uma metodologia qualitativa, procedeu-se a uma recolha de dados diversificada: observações participantes, questionários dirigidos à equipa educativa da instituição e aos educadores que participaram na implementação do projecto e análise documental. Para a sua concretização recorremos ao emprego de técnicas e instrumentos de pesquisa. A avaliação final dos resultados de implementação do Projecto indicou que os utentes apreciaram a frequência do local nas suas várias valências, quer enquanto local de lazer, frequentando-o nos momentos de pausa; quer enquanto local de aprendizagens, onde puderam usufruir do contacto com a natureza, vivenciando os diferentes ciclos e onde os utentes que frequentaram o Curso de Jardinagem puderam aplicar os conhecimentos adquiridos no referido Curso. A implementação deste Projecto contribuiu também para a valorização dos espaços exteriores da instituição, e sua requalificação. Paralelamente, este Projecto proporcionou trabalho em equipa, quer durante a fase de construção, com a angariação de materiais, o fornecimento de contactos e em alguns casos ou o prolongamento do horário de trabalho de modo a garantir que as metas eram atingidas; quer na dinamização das actividades desenvolvidas, com a concretização das propostas de actividades apresentadas e com a tomada de iniciativa para a dinamização de outras actividades, em contexto de sala/atelier ou nos momentos de lazer.

OBJETIVO: Avaliar a sensibilidade e a especificidade de diferentes protocolos de medida da pressão arterial para o diagnóstico da hipertensão em adultos. MÉTODOS: Estudo transversal com amostra não probabilística de 250 funcionários de ambos os sexos de instituição pública na faixa etária de 35 a 74 anos em Vitória, ES, entre 2008 e 2010. Os participantes tiveram suas pressões arteriais aferidas por três métodos: medida clínica, automedida e medida ambulatorial por 24 horas. Foram coletados dados antropométricos (peso, estatura e circunferências) e realizada entrevista. Os métodos foram confrontados e analisados em relação à medida ambulatorial 24 h (padrão-ouro). Foram calculadas as medidas de desempenho diagnóstico: sensibilidade, especificidade, valores preditivos positivos e negativos e acurácia. A metodologia de Bland & Altman foi utilizada para verificar a concordância entre a variabilidade pressórica na medida ambulatorial (desvio-padrão relativo ao período diurno) e a variabilidade pressórica na automedida (desvio-padrão das quatro medidas). Adotou-se nível de significância de 5% para todos os testes. RESULTADOS: A automedida apresentou maior sensibilidade (S = 84%; IC95%: 75;93) e acurácia global (0,817; p < 0,001) no diagnóstico da hipertensão que a medida clínica (S = 79%; IC95%: 73;86 e AG = 0,815; p < 0,001). Apesar da forte correlação com o método de medida ambulatorial durante a vigília (r = 0,843; p = 0,000), a automedida não mostrou boa concordância com o referido método para a medida sistólica (viés = 5,82; IC95%: 4,49;7,15). Foram identificados sete (2,8%) indivíduos com hipertensão do avental branco, 26 (10,4%) com hipertensão mascarada e 46 (18,4%) com efeito do avental branco. CONCLUSÕES: Os resultados sugerem que a automedida apresenta sensibilidade superior à medida clínica para identificar verdadeiros hipertensos na população. Os valores de predição negativa encontrados confirmam a superioridade da automedida em relação à medida clínica no que tange à capacidade do teste em apontar os indivíduos verdadeiramente normotensos. Contudo, não pode substituir a medida clínica, que ainda é o método mais fidedigno.
