La santé des enfants demeure une question prioritaire en Afrique sub-saharienne. Les disparités en matière de mortalité entre pays et au sein des pays persistent et se sont fortement accrues durant la dernière décennie. En dépit de solides arguments théoriques voulant que les variables contextuelles soient des déterminants importants de la santé des enfants, ces facteurs, et particulièrement les influences du contexte local, ont été étudiées beaucoup moins souvent que les caractéristiques individuelles. L'objectif principal de la présente thèse est d’identifier les déterminants individuels et contextuels associés à la mortalité des enfants de moins de 5 ans en Afrique sub-saharienne. L’analyse systématique est basée sur les données les plus récentes des enquêtes démographiques et de santé (DHS/EDS). Deux questions spécifiques sont examinées dans cette thèse. La première évalue la mesure dans la quelle le contexte local affecte la mortalité infanto-juvénile, net des caractéristiques individuelles. La seconde question est consacrée à l’examen de l’effet du faible poids à la naissance sur le risque de décès avant 5 ans. Par rapport à la première question, les analyses multi-niveaux confirment pour plusieurs pays étudiés l’importance simultanée de l’environnement familial et du contexte local de résidence dans l’explication des différences de mortalité infanto-juvénile. Toutefois, par comparaison au contexte familial, l’ampleur de l’effet de l’environnement local paraît assez modeste. Il apparaît donc que le contexte familial reste un puissant déterminant de la mortalité des enfants de moins de 5 ans en Afrique sub-saharienne. Les résultats indiquent en outre que certains attributs du contexte local de résidence influencent le risque de décès des enfants avant 5 ans, au-delà des facteurs individuels dans plusieurs pays. Cette thèse confirme l’effet contextuel de l’éducation sur la mortalité des enfants. Cet effet s’ajoute, dans certains pays, à l'effet positif du niveau individuel d’éducation de la mère sur la survie de l'enfant. Les résultats montrent aussi que le degré d’homogénéité ethnique de la localité influence fortement la probabilité de mourir avant 5 ans dans certains pays. Globalement, les résultats de cette thèse suggèrent que le défi de réduire la mortalité des enfants va au-delà des stratégies visant uniquement les facteurs individuels, et nécessite une meilleure compréhension de l’influence des facteurs contextuels. Par rapport à la deuxième question, les résultats montrent également que les facteurs individuels restent aussi très importants dans l’explication des différences de mortalité des enfants dans plusieurs pays étudiés. Nos résultats indiquent que les différences de mortalité selon le poids à la naissance sont significatives dans tous les pays inclus dans l’analyse. Les enfants nés avec un faible poids (moins de 2500 grammes) courent presque 2 à 4 fois plus de risques de mourir au cours des cinq premières années de vie que les enfants de poids normal, même après correction pour l’hétérogénéité non observée. Ce résultat suggère qu’en plus des mesures visant à réduire la pauvreté et les inégalités de revenus, la réduction de l’incidence du faible poids à la naissance pourrait apporter une contribution majeure aux Objectifs du Millénaire pour le développement; spécialement comme une stratégie efficace pour réduire le niveau de mortalité parmi les enfants de moins de cinq ans.

L’asthme est l’une des pathologies chroniques les plus fréquemment rencontrées durant la grossesse, affectant environ 8% des femmes enceintes. Les lignes directrices pour le traitement de l’asthme affirment que le risque d’un développement non optimal du fœtus dû à un asthme mal maîtrisé est supérieur au risque associé à la prise de médicaments pour le traitement de l’asthme durant la grossesse. Des questions persistent par contre sur l’innocuité des hautes doses de corticostéroïdes inhalés (CSI) et très peu de données sont publiées pour les bêta2-agonistes à longue action (BALA). Un programme de recherche en deux volets a été développé afin de répondre à certaines de ces questions. Dans un premier volet, une cohorte de femmes asthmatiques accouchant au Québec de 1998 à 2008 a été assemblée à partir des bases de données de la Régie de l’assurance maladie du Québec et de MED-ÉCHO afin d’évaluer l’impact de la prise de CSI ou de BALA sur la prévalence de faible poids à la naissance (FPN), de prématurité et de bébé petit pour l’âge gestationnel (PAG). La cohorte était composée de 7376 grossesses dont 56,9% étaient exposées aux CSI et 8,8% aux BALA. Dans cette cohorte, l’utilisation de BALA n’était pas associée à des prévalences plus élevées de FPN (OR=0,81, IC95%:0,58–1,12), prématurité (OR=0,84, IC95%:0,61–1,15) ou PAG (OR=0,92, IC95%:0,70–1,20). Lors de la comparaison des BALA (salmétérol comparé au formotérol comme référence) la différence la plus importante était pour le PAG (OR=1,16, IC95%:0,67–2,02). Pour les CSI, une tendance à une augmentation de FPN, prématurité et PAG a été observée avec l’augmentation des doses. Le OR le plus élevé était pour une dose > 500 ug/jour (équivalent fluticasone) pour le FPN: (OR=1,57, IC95%:0,86–2,87). La comparaison des CSI les plus utilisés (fluticasone comparé au budésonide comme référence) montre des différences non statistiquement significatives avec la différence maximale observée pour le PAG (OR=1,10, IC95%:0,85–1,44). Dans un second volet, une sous-cohorte de femmes asthmatiques avec visites médicales pour exacerbation d’asthme au Centre hospitalier universitaire de Sherbrooke (CHUS) a été constituée pour comparer le traitement des exacerbations durant et hors grossesse. Les résultats montrent que le traitement par CS était moins fréquent et différé pour les femmes enceintes comparées aux femmes non-enceintes. Le traitement de maîtrise de l’asthme (CSI et/ou BALA) dans l’année précédant l’exacerbation était sous-optimal. Les résultats présentés dans cette thèse démontrent l’innocuité des BALA et des doses faibles à modérées de CSI pendant la grossesse pour les issues de FPN, prématurité et PAG alors que des études supplémentaires sont nécessaires afin d’évaluer l’innocuité des hautes doses de CSI. Une innocuité comparable entre les CSI (budésonide, fluticasone) et les BALA étudiés (formotérol, salmétérol) a également été démontrée. Les résultats montrent également un recours moindre aux CS pour le traitement des exacerbations d’asthme durant la grossesse comparativement à hors grossesse. Ces résultats sont un ajout important aux évidences permettant aux cliniciens et aux femmes enceintes asthmatiques de faire les meilleurs choix pour optimiser le traitement pharmacologique durant la grossesse.

Il existe actuellement de nombreuses preuves démontrant que des facteurs génétiques et environnementaux interagissent pendant des périodes spécifiques du développement pour rendre une personne vulnérable aux troubles psychologiques via diverses adaptations physiologiques. Cette thèse porte sur l'impact de l’adversité prénatale (représentée par le petit poids à la naissance, PPN) et de l’adversité postnatale précoce (symptômes dépressifs maternels et comportements maternels négatifs), sur le développement du cerveau, particulièrement les régions fronto-limbiques impliquées dans le traitement des émotions, pendant l'enfance et l'adolescence. Des jumeaux monozygotes (MZ) sont utilisés, lorsque possible, afin de contrôler pour les effets génétiques. Les chapitres 1 et 2 présentent les résultats de la vérification de l'hypothèse que l’adversité prénatale et postnatale précoce sont associées à une altération du fonctionnement des régions fronto-limbique tels que l’amygdale, l’hippocampe, l’insula, le cortex cingulaire antérieur et le cortex préfrontal, en réponse à des stimuli émotifs chez des enfants et des adolescents. On observe que les symptômes dépressifs maternels sont associés à une activation plus élevée des régions fronto-limbiques des enfants en réponse à la tristesse. Les résultats de l’étude avec des adolescents suggèrent que le PPN, les symptômes dépressifs et les comportements maternels négatifs sont associés à une fonction altérée des régions fronto-limbiques en réponse à des stimuli émotionnels. Chez les jumeaux MZ on observe également que la discordance intra-paire de PPN et de certains comportements maternels est associée à une discordance intra-paire du fonctionnement du cerveau et que ces altérations diffèrent selon le sexe. Le chapitre 3 présente les résultats de la vérification de l'hypothèse que l’adversité prénatale et postnatale précoce sont associées à un volume total réduit du cerveau et de l’hypothèse que les comportements maternels peuvent servir de médiateur ou de modérateur de l'association entre le PPN et le volume du cerveau. Avec des jumeaux MZ à l’adolescence on observe a) que le PPN est effectivement associé à une diminution du volume total du cerveau et b) que la discordance intra-paire de PPN est associée à une discordance du volume du cerveau. En somme, cette thèse présente un ensemble de résultats qui soutiennent deux hypothèses importantes pour comprendre les effets de l’environnement sur le développement du cerveau : que l’environnement prénatal et postnatal précoce ont un impact sur le développement du cerveau indépendamment du code génétique et que les mécanismes impliqués peuvent différer entre les garçons et les filles. Finalement, l’ensemble de ces résultats sont discutés à la lumière des autres travaux de recherche dans ce domaine et des avenues à explorer pour de la recherche ultérieure sont proposées.

Lignocellulosic biomass is probably the best alternative resource for biofuel production and it is composed mainly of cellulose, hemicelluloses and lignin. Cellulose is the most abundant among the three and conversion of cellulose to glucose is catalyzed by the enzyme cellulase. Cellulases are groups of enzymes act synergistically upon cellulose to produce glucose and comprise of endoglucanase, cellobiohydrolase and β-glucosidase. β -glucosidase assumes great importance due to the fact that it is the rate limiting enzyme. Endoglucanases (EG) produces nicks in the cellulose polymer exposing reducing and non reducing ends, cellobiohydrolases (CBH) acts upon the reducing or non reducing ends to liberate cellobiose units, and β - glucosidases (BGL) cleaves the cellobiose to liberate glucose completing the hydrolysis. . β -glucosidases undergo feedback inhibition by their own product- β glucose, and cellobiose which is their substrate. Few filamentous fungi produce glucose tolerant β - glucosidases which can overcome this inhibition by tolerating the product concentration to a particular threshold. The present study had targeted a filamentous fungus producing glucose tolerant β - glucosidase which was identified by morphological as well as molecular method. The fungus showed 99% similarity to Aspergillus unguis strain which comes under the Aspergillus nidulans group where most of the glucose tolerant β -glucosidase belongs. The culture was designated the strain number NII 08123 and was deposited in the NII culture collection at CSIR-NIIST. β -glucosidase multiplicity is a common occurrence in fungal world and in A.unguis this was demonstrated using zymogram analysis. A total 5 extracellular isoforms were detected in fungus and the expression levels of these five isoforms varied based on the carbon source available in the medium. Three of these 5 isoforms were expressed in higher levels as identified by the increased fluorescence (due to larger amounts of MUG breakdown by enzyme action) and was speculated to contribute significantly to the total _- β glucosidase activity. These isoforms were named as BGL 1, BGL3 and BGL 5. Among the three, BGL5 was demonstrated to be the glucose tolerant β -glucosidase and this was a low molecular weight protein. Major fraction was a high molecular weight protein but with lesser tolerance to glucose. BGL 3 was between the two in both activity and glucose tolerance.121 Glucose tolerant .β -glucosidase was purified and characterized and kinetic analysis showed that the glucose inhibition constant (Ki) of the protein is 800mM and Km and Vmax of the enzyme was found to be 4.854 mM and 2.946 mol min-1mg protein-1respectively. The optimumtemperature was 60°C and pH 6.0. The molecular weight of the purified protein was ~10kDa in both SDS as well as Native PAGE indicating that the glucose tolerant BGL is a monomeric protein.The major β -glucosidase, BGL1 had a pH and temperature optima of 5.0 and 60 °C respectively. The apparent molecular weight of the Native protein is 240kDa. The Vmax and Km was 78.8 mol min-1mg protein-1 and 0.326mM respectively. Degenerate primers were designed for glycosyl hydrolase families 1, 3 and 5 and the BGL genes were amplified from genomic DNA of Aspergillus unguis. The sequence analyses performed on the amplicons results confirmed the presence of all the three genes. Amplicon with a size of ~500bp was sequenced and which matched to a GH1 –BGL from Aspergillus oryzae. GH3 degenerate primers producing amplicons were sequenced and the sequences matched to β - glucosidase of GH3 family from Aspergillus nidulans and Aspergillus acculateus. GH5 degenerate primers also gave amplification and sequencing results indicated the presence of GH5 family BGL gene in the Aspergillus unguis genomic DNA.From the partial gene sequencing results, specific as well as degenerate primers were designed for TAIL PCR. Sequencing results of the 1.0 Kb amplicon matched Aspergillus nidulans β -glucosidase gene which belongs to the GH1 family. The sequence mainly covered the N-Terminal region of the matching peptide. All the three BGL proteins ie. BGL1, BGL3 and BGL5 were purified by chromatography an electro elution from Native PAGE gels and were subjected to MALDI-TOF mass spectrometric analysis. The results showed that BGL1 peptide mass matched to . β -glucosidase-I of Aspergillus flavus which is a 92kDa protein with 69% protein coverage. The glucose tolerant β -glucosidase BGL5 mass matched to the catalytic C-terminal domain of β -glucosidase-F from Emericella nidulans, but the protein coverage was very low compared to the size of the Emericella nidulans protein. While comparing the size of BGL5 from Aspergillus unguis, the protein sequence coverage is more than 80%. BGL F is a glycosyl hydrolase family 3 protein.The properties of BGL5 seem to be very unique, in that it is a GH3 β -glucosidase with a very low molecular weight of ~10kDa and at the same time having catalytic activity and glucose 122 tolerance which is as yet un-described in GH β -glucosidases. The occurrence of a fully functional 10kDA protein with glucose tolerant BGL activity has tremendous implications both from the points of understanding the structure function relationships as well as for applications of BGL enzymes. BGL-3 showed similarity to BGL1 of Aspergillus aculateus which was another GH3 β -glucosidase. It may be noted that though PCR could detect GH1, GH3 and GH5 β-glucosidases in the fungus, the major isoforms BGL1 BGL3 and BGL5 were all GH3 family enzymes. This would imply that β-glucosidases belonging to other families may also co-exist in the fungus and the other minor isoforms detected in zymograms may account for them. In biomass hydrolysis, GT-BGL containing BGL enzyme was supplemented to cellulase and the performances of blends were compared with a cocktail where commercial β- glucosidase was supplemented to the biomass hydrolyzing enzyme preparation. The cocktail supplemented with A unguis BGL preparation yielded 555mg/g sugar in 12h compared to the commercial enzyme preparation which gave only 333mg/g in the same period and the maximum sugar yield of 858 mg/g was attained in 36h by the cocktail containing A. unguis BGL. While the commercial enzyme achieved almost similar sugar yield in 24h, there was rapid drop in sugar concentration after that, indicating probably the conversion of glucose back to di-or oligosaccharides by the transglycosylation activity of the BGl in that preparation. Compared this, the A.unguis enzyme containing preparation supported peak yields for longer duration (upto 48h) which is important for biomass conversion to other products since the hydrolysate has to undergo certain unit operations before it goes into the next stage ie – fermentation in any bioprocesses for production of either fuels or chemicals.. Most importantly the Aspergillus unguis BGL preparation yields approximately 1.6 fold increase in the sugar release compared to the commercial BGL within 12h of time interval and 2.25 fold increase in the sugar release compared to the control ie. Cellulase without BGL supplementation. The current study therefore leads to the identification of a potent new isolate producing glucose tolerant β - glucosidase. The organism identified as Aspergillus unguis comes under the Aspergillus nidulans group where most of the GT-BGL producers belong and the detailed studies showed that the glucose tolerant β -glucosidase was a very low molecular weight protein which probably belongs to the glycosyl hydrolase family 3. Inhibition kinetic studies helped to understand the Ki and it is the second highest among the nidulans group of Aspergilli. This has promoted us for a detailed study regarding the mechanism of glucose tolerance. The proteomic 123 analyses clearly indicate the presence of GH3 catalytic domain in the protein. Since the size of the protein is very low and still its active and showed glucose tolerance it is speculated that this could be an entirely new protein or the modification of the existing β -glucosidase with only the catalytic domain present in it. Hydrolysis experiments also qualify this BGL, a suitable candidate for the enzyme cocktail development for biomass hydrolysis

Introducción: La cesárea es un procedimiento quirúrgico considerado como inocuo por algunas personas, sin tener en cuenta las posibles consecuencias en los embarazos siguientes. Existe literatura que evidencia un aumento en el riesgo para abrupcio de placenta, acretismo placentario, placenta previa, mortalidad materna, sepsis materna, ingreso a UCI, al igual que apgar bajo, bajo peso neonatal, distress respiratorio,entre otros.El objetivo de este estudio fue evaluar el impacto del antecedente de cesárea en la morbilidad materna y neonatal mediante una revisión sistemática de literatura. Metodología Se realizó una búsqueda en diferentes bases de datos desde 1996 hasta 2011 sobre el antecedente de cesárea y sus posibles riesgos en el desenlace maternos y perinatales. Se utilizaron variaciones de términos MeSH. Resultados La búsqueda arrojó un total de 2.483 artículos, entre los cuales fueron escogidos 247 por criterios de elegibilidad.Estos fueron evaluados en su totalidad. Posteriormente se realizó el análisis metanalítico de cada uno de los desenlaces. Los resultados deben interpretarse con precaución pues la calidad metodológica de algunos estudios fue variable. En el riesgo conjunto se encontraron resultados estadísticamente significativos en placenta previa, apgar bajo, histerectomía periparto. Discusión El antecedente de cesárea presenta un riesgo aumentado para los siguientes embarazos en la morbilidad tanto materna como neonatal. Faltan más estudios analíticos para definir el verdadero papel de la cesarea.

Experimental and epidemiological studies demonstrate that fetal growth restriction and low birth weight enhance the risk of chronic diseases in adulthood. Derangements in tissue-specific epigenetic programming of fetal and placental tissues are a suggested mechanism of which DNA methylation is best understood. DNA methylation profiles in human tissue are mostly performed in DNA from white blood cells. The objective of this study was to assess DNA methylation profiles of IGF2 DMR and H19 in DNA derived from four tissues of the newborn. We obtained from 6 newborns DNA from fetal placental tissue (n = 5), umbilical cord CD34+ hematopoietic stem cells (HSC) and CD34- mononuclear cells (MNC) (n = 6), and umbilical cord Wharton jelly (n = 5). HCS were isolated using magnetic-activated cell separation. DNA methylation of the imprinted fetal growth genes IGF2 DMR and H19 was measured in all tissues using quantitative mass spectrometry. ANOVA testing showed tissue-specific differences in DNA methylation of IGF2 DMR (p value 0.002) and H19 (p value 0.001) mainly due to a higher methylation of IGF2 DMR in Wharton jelly (mean 0.65, sd 0.14) and a lower methylation of H19 in placental tissue (mean 0.25, sd 0.02) compared to other tissues. This study demonstrates the feasibility of the assessment of differential tissue specific DNA methylation. Although the results have to be confirmed in larger sample sizes, our approach gives opportunities to investigate epigenetic profiles as underlying mechanism of associations between pregnancy exposures and outcome, and disease risks in later life.

Introducción: La segunda endocrinopatía que complica el embarazo es la enfermedad tiroidea, con una prevalencia de 2.5-5% en Estados Unidos. El hipotiroidismo en el embarazo se ha relacionado con efectos deletéreos maternos y perinatales, con evidencia escasa y controversial. Es importante conocer las diferencias en estos desenlaces en mujeres con hipotiroidismo pregestacional (HPG) y gestacional (HG). Metodología: Cohorte retrospectiva en una institución prestadora de salud (IPS) con HPG y HG, con registro de TSH en la historia clínica prenatal. Desenlaces maternos y perinatales: Amenaza de aborto, aborto espontáneo, hipertensión inducida por el embarazo, preeclamsia, abrupcio placenta, amenaza de parto pretérmino, óbito fetal, presentación podálica, cesárea, hemorragia posparto y bajo peso al nacer. Se compararon desenlaces entre pacientes con HPG versus HG y el tipo de hipotiroidismo (clínico, sub-clínico y controlado). Resultados: 337 gestantes entre Junio de 2009 y Diciembre de 2011, 67 con HPG. La TSH disminuyó del primero al tercer trimestre de embarazo en ambos grupos. El IMC en pacientes con HPG es mayor significativamente que en HG (p=0,04). Existe diferencia estadísticamente significativa entre tipo de hipotiroidismo clínico, subclínico y controlado en pacientes con HG y HPG en el primero y segundo trimestre (p<0.05). Con relación a desenlaces maternos y perinatales ninguno mostró asociación estadísticamente significativa, p>0.05. Conclusión: No se observaron diferencias significativas en los desenlaces materno-perinatales de pacientes con HG versus HPG, ambos grupos con suplencia hormonal tiroidea. Esto sugiere que independientemente del momento del diagnóstico, lo importante es tratar la enfermedad tempranamente para disminuir o evitar complicaciones.

La colonización materna por estreptococo del grupo B (SGB) en países en desarrollo es del 4-20%; el 50% de sus hijos nacen colonizados y el 1-2% desarrollan enfermedad invasiva con alto riesgo de morbimortalidad y secuelas. Objetivo: determinar los factores de riesgo materno-fetales asociados a enfermedad severa y mortalidad neonatal por SGB en una unidad de recién nacidos. Materiales y métodos: se realizó un estudio observacional analítico de cohorte histórica durante un período de dos años. Se tomaron 11 casos con enfermedad invasiva y 15 controles. Se calcularon frecuencias absolutas y relativas, y se buscaron asociaciones mediante el cálculo del estadístico chi2. Resultados: los factores maternos predictivos para enfermedad por SGB incluyeron fiebre periparto mayor a 37,5 grados centígrados (p < 0,05), corioamnionitis y ruptura de membranas mayor a 18 horas (p < 0,05). Los factores de riesgo neonatal incluyeron prematurez (< 37 semanas) y bajo peso al nacer (< 2.500 gramos) (p < 0,05). Se encontró una mortalidad de 5 (45%). Conclusiones: hubo relación estadísticamente significativa entre la corioaminionitis materna, la ruptura de membranas mayor a 18 horas, la prematurez y el peso bajo al nacer con la severidad de la enfermedad y la mortalidad. La incidencia estimada de infección neonatal en la unidad de recién nacidos fue de 1,8 casos/1.000 nacidos vivos, y la de colonización materna fue de 4,3 casos/1.000 maternas. Se deben realizar más estudios en el país para establecer la incidencia real de enfermedad neonatal por SGB y hacer investigación sobre la costo-efectividad de las medidas de prevención.

Background: Isometric grip strength, evaluated with a handgrip dynamometer, is a marker of current nutritional status and cardiometabolic risk and future morbidity and mortality. We present reference values for handgrip strength in healthy young Colombian adults (aged 18 to 29 years). Methods: The sample comprised 5.647 (2.330 men and 3.317 women) apparently healthy young university students (mean age, 20.6±2.7 years) attending public and private institutions in the cities of Bogota and Cali (Colombia). Handgrip strength was measured two times with a TKK analogue dynamometer in both hands and the highest value used in the analysis. Sex- and age-specific normative values for handgrip strength were calculated using the LMS method and expressed as tabulated percentiles from 3 to 97 and as smoothed centile curves (P3, P10, P25, P50, P75, P90 and P97). Results: Mean values for right and left handgrip strength were 38.1±8.9 and 35.9±8.6 kg for men, and 25.1±8.7 and 23.3±8.2 kg for women, respectively. Handgrip strength increased with age in both sexes and was significantly higher in men in all age categories. The results were generally more homogeneous amongst men than women. Conclusions: Sex- and age-specific handgrip strength normative values among healthy young Colombian adults are defined. This information may be helpful in future studies of secular trends in handgrip strength and to identify clinically relevant cut points for poor nutritional and elevated cardiometabolic risk in a Latin American population. Evidence of decline in handgrip strength before the end of the third decade is of concern and warrants further investigation

La vulnerabilidad de la población a condiciones ambientales adversas ha tenido especial relevancia para la literatura de inequidad en los últimos tiempos. De hecho, el concepto de justicia ambiental nace a partir de las disparidades que los individuos enfrentan en la calidad del ambiente. Este trabajo es una aproximación a este concepto ya que considera las actividades mineras como posibles generadoras de pasivos ambientales, que a su vez, pueden afectar las condiciones bajo las cuales los individuos se desarrollan. Desde la crisis financiera del 2008, los precios del oro experimentaron alzas signicativas en relación a periodos anteriores y generaron un aumento de las actividades mineras de oro. En este sentido, el objetivo del trabajo es investigar el impacto de las actividades mineras del oro sobre la salud de los recién nacidos en Colombia durante el periodo de boom en el precio de los minerales en la pasada década. Con este n, se usa información sobre el potencial minero, los precios internacionales del oro y las estadísticas vitales de Colombia. Las estimaciones indican que mayores niveles actividad minera implican un incremento en la tasa de bebés nacidos antes de las 27 semanas de gestación y en la tasa de bebés de bajo peso (nacidos con menos de 2.500 gramos). Adicionalmente se encuentra que las actividades mineras no tienen un efecto sobre la tasa de defunciones fetales. Los resultados son robustos a diferentes medidas de minera, que incluyen presencia de minera ilegal, titulación minera y volumen de producción de oro.

Obesity is an escalating threat of pandemic proportions and has risen to such unrivaled prominence in such a short period of time that it has come to define a whole generation in many countries around the globe. The burden of obesity, however, is not equally shared among the population, with certain ethnicities being more prone to obesity than others, while some appear to be resistant to obesity altogether. The reasons behind this ethnic basis for obesity resistance and susceptibility, however, have remained largely elusive. In recent years, much evidence has shown that the level of brown adipose tissue thermogenesis, which augments energy expenditure and is negatively associated with obesity in both rodents and humans, varies greatly between ethnicities. Interestingly, the incidence of low birth weight, which is associated with an increased propensity for obesity and cardiovascular disease in later life, has also been shown to vary by ethnic background. This review serves to reconcile ethnic variations in BAT development and function with ethnic differences in birth weight outcomes to argue that the variation in obesity susceptibility between ethnic groups may have its origins in the in utero programming of BAT development and function as a result of evolutionary adaptation to cold environments.

In the past three decades, Brazil has undergone rapid changes in major social determinants of health and in the organisation of health services. In this report, we examine how these changes have affected indicators of maternal health, child health, and child nutrition. We use data from vital statistics, population censuses, demographic and health surveys, and published reports. In the past three decades, infant mortality rates have reduced substantially, decreasing by 5.5% a year in the 1980s and 1990s, and by 4.4% a year since 2000 to reach 20 deaths per 1000 livebirths in 2008. Neonatal deaths account for 68% of infant deaths. Stunting prevalence among children younger than 5 years decreased from 37% in 1974-75 to 7% in 2006-07. Regional differences in stunting and child mortality also decreased. Access to most maternal-health and child-health interventions increased sharply to almost universal coverage, and regional and socioeconomic inequalities in access to such interventions were notably reduced. The median duration of breastfeeding increased from 2.5 months in the 1970s to 14 months by 2006-07. Official statistics show stable maternal mortality ratios during the past 10 years, but modelled data indicate a yearly decrease of 4%, a trend which might not have been noticeable in official reports because of improvements in death registration and the increased number of investigations into deaths of women of reproductive age. The reasons behind Brazil`s progress include: socioeconomic and demographic changes (economic growth, reduction in income disparities between the poorest and wealthiest populations, urbanisation, improved education of women, and decreased fertility rates), interventions outside the health sector (a conditional cash transfer programme and improvements in water and sanitation), vertical health programmes in the 1980s (promotion of breastfeeding, oral rehydration, and immunisations), creation of a tax-funded national health service in 1988 (coverage of which expanded to reach the poorest areas of the country through the Family Health Program in the mid-1990s); and implementation of many national and state-wide programmes to improve child health and child nutrition and, to a lesser extent, to promote women`s health. Nevertheless, substantial challenges remain, including overmedicalisation of childbirth (nearly 50% of babies are delivered by caesarean section), maternal deaths caused by illegal abortions, and a high frequency of preterm deliveries.

Background: We evaluated the outcome of newborns admitted in the neonatal Intensive Care Unit (ICU) in Diadema, Brazil. Methods: We evaluated 72 newborns, data were extracted from research forms, newborns` hospital records, mothers interviews, domiciliary inquiry made with the responsible for the newborn care, and paediatric accompaniment cards. Results: 48.93% presented low birth weight, 48% were considered to have normal birth weight and 2% had a birth weight higher than 4000g. Concerning gestational age, 57.44% were younger than 37 weeks old. During hospitalisation, newborn had appointments with doctors from other specialties (inter-appointments), around 40% were cardiologists. After hospital discharge 82.98% were referred to local primary health care units, and the main specialities were cardiology and neurology. Among the newborns evaluated 85.11% were accompanied by paediatric health care units. Conclusion: The implementation of a specialised newborn health accompaniment program in Brazil after ICU discharge is important for positive outcomes regarding newborns growth and development.

Epidemiological studies suggest that glucocorticoid excess in the fetus may contribute to the pathophysiology of cardiovascular diseases in adulthood. However, the impact of maternal glucocorticoid on the cardiovascular system of the offspring has not been much explored in studies involving humans, especially in childhood. The objective of this study was to assess the influence of maternal cortisol concentrations on child arterial elasticity. One hundred and thirty pregnant women followed from 1997 to 2000, and respective children 5-7 years of age followed from 2004 to 2006 were included in the study. Maternal cortisol was determined in saliva by an enzyme immunoassay utilizing the mean concentration of nine samples of saliva. Arterial elasticity was assessed by the large artery elasticity index (LAEI; the capacitive elasticity of large arteries) by recording radial artery pulse wave, utilizing the equipment HDI/PulseWave CR-2000 Cardiovascular Profiling System (R). The nutritional status of the children was determined by the body mass index (BMI). Insulin concentration was assessed by chemiluminescence, and insulin resistance by the homeostasis model assessment. Blood glucose, total cholesterol and fractions (LDL-c and HDL-c) and triglyceride concentrations were determined by automated enzymatic methods. The association between maternal cortisol and child arterial elasticity was assessed by multivariate linear regression analysis. There was a statistically significant association between maternal cortisol and LAEI (P=0.02), controlling for birth weight, age, BMI and HDL-c of the children. This study suggests that exposure to higher glucocorticoid concentrations in the prenatal period is associated to lower arterial elasticity in childhood, an earlier cardiovascular risk marker.

Low birth weight has been associated with increased obesity in adulthood. It has been shown that dietary salt restriction during intrauterine life induces low birth weight and insulin resistance in adult Wistar rats. The present study had a two-fold objective: to evaluate the effects that low salt intake during pregnancy and lactation has on the amount and distribution of adipose tissue; and to determine whether the phenotypic changes in fat mass in this model are associated with alterations in the activity of the renin-angiotensin system. Maternal salt restriction was found to reduce birth weight in male and female offspring. In adulthood, the female offspring of dams fed the low-salt diet presented higher adiposity indices than those seen in the offspring of dams fed a normal-salt diet. This was attributed to the fact that adipose tissue mass (retroperitoneal but not gonadal, mesenteric or inguinal) was greater in those rats than in the offspring of dams fed a normal diet. The adult offspring of dams fed the low-salt diet, compared to those dams fed a normal-salt diet, presented the following: plasma leptin levels higher in males and lower in females; plasma renin activity higher in males but not in females; and no differences in body weight, mean arterial blood pressure or serum angiotensin-converting enzyme activity.
