The purpose of this research is to identify the impact of recent organizational change upon the culture of firefighting. The experiences of female firefighters were utilized as a measure of cultural change. A purposive sample of twenty-seven male and female firefighters were interviewed in a semi-structured format about their experiences in the fire service. This research found that the culture of firefighting has adjusted to the presence of previously excluded groups by forging a division among the identities and roles of male and female firefighters. The white, male firefighters, who have traditionally constituted a majority of the workforce, have continued to identify with traditional firefighter roles and reported high levels of cohesion. In contrast, the female firefighters showed a greater variance in their identification with traditional roles and decreased levels of cohesion with the main body of the group.

The morphogen Sonic Hedgehog (SHH) plays a critical role in the development of different tissues. In the central nervous system, SHH is well known to contribute to the patterning of the spinal cord and separation of the brain hemispheres. In addition, it has recently been shown that SHH signaling also contributes to the patterning of the telencephalon and establishment of adult neurogenic niches. In this work, we investigated whether SHH signaling influences the behavior of neural progenitors isolated from the dorsal telencephalon, which generate excitatory neurons and macroglial cells in vitro. We observed that SHH increases proliferation of cortical progenitors and generation of astrocytes, whereas blocking SHH signaling with cyclopamine has opposite effects. In both cases, generation of neurons did not seem to be affected. However, cell survival was broadly affected by blockade of SHH signaling. SHH effects were related to three different cell phenomena: mode of cell division, cell cycle length and cell growth. Together, our data in vitro demonstrate that SHH signaling controls cell behaviors that are important for proliferation of cerebral cortex progenitors, as well as differentiation and survival of neurons and astroglial cells.

This article examines regulatory governance of the post-initial training market in The Netherlands. From an historical perspective on policy formation processes, it examines market formation in terms of social, economic, and cultural factors in the development of provision and demand for post-initial training; the roles of stakeholders in the longterm construction of regulatory governance of the market; regulation of and public providers; policy responses to market failure; and tripartite division of responsibilities between the state, social partners, commercial and publicly-funded providers. Historical description and analysis examine policy narratives of key stakeholders with reference to: a) influence of societal stakeholders on regulatory decision-making; b) state regulation of the post-initial training market; c) public intervention regulating the market to prevent market failure; d) market deregulation, competition, employability and individual responsibility; and, e) regulatory governance to prevent ‘allocative failure’ by the market in non-delivery of post-initial training to specific target groups, particularly the low-qualified. Dominant policy narratives have resulted in limited state regulation of the supply-side, a tripartite system of regulatory governance by the state, social partners and commercial providers as regulatory actors. Current policy discourses address interventions on the demand-side to redistribute structures of opportunity throughout the life courses of individuals. Further empirical research from a comparative historical perspective is required to deepen contemporary understandings of regulatory governance of markets and the commodification of adult learning in knowledge societies and information economies. Currently, the Division of Appeals and Hearings of the South Carolina Department of Health and Human Services has no specific presence on the agency's website to provide information or to allow for the electronic submission of appeals. This project's focus was developing an online presence for the Division on SCDHHS' website. The page will make the Division's procedures publicly available to beneficiaries, providers, and agency program staff who attend hearings. Additionally, parties will have a secure online portal through which they can file appeals and upload supporting documentation, reducing the need to send appeals via first class mail. The online appeal portal will further the agency' s goal of reducing paper.

Summary and conclusion: One of the main challenges facing the Swiss health care system remains continued fragmentation as a consequence of the federal structure and the division of responsibilities. This constitutes a major barrier for the implementation of a nationwide public health strategy and a more rational organization of health care. Much of the health reform agenda during the past decade was driven by cost-containment efforts. Despite the absence of a national regulatory framework targeting prevention and care of chronic diseases, national strategies and programmes have been developed within a growing recognition of the need to address chronic disease in the health care system. In addition, several small-scale pilot and preliminary structured care programmes have been implemented in selected localities. The latter are however still too few and limited in scope to cover the needs of the majority of patients with chronic diseases residing in Switzerland. Given the continued high satisfaction with the system among health care users, of around 80%, pressure to actually initiate larger scale changes might not yet be sufficiently strong.

Työn tavoitteena oli tutkia, millä tavalla perusterveydenhuollon perinteisestä terveysasemamallista hyvinvointiasemamalliin siirtyminen vaikuttaa sosiaali- ja terveyspalvelujen tarjonnan kustannuksiin, mikäli vaikutus on havaittavissa. Toisaalta tavoitteena oli kartoittaa, millä keinoilla hyvinvointiasemien kustannustehokkuutta voitaisiin parantaa tulevaisuudessa ja mitä kustannushyötyjä tiiviimmällä sosiaali- ja terveystoimen yhteistyöllä on mahdollista saavuttaa. Tutkimuksessa käsiteltiin terveyden ja hyvinvoinnin edistämisen roolin merkitystä perusterveydenhuollossa, sähköistä asiointipalveluja ja terveydenhuollon ammattiryhmien välistä työnjakoa sekä edelleen, miten näillä voidaan vaikuttaa tuottavuuteen ja vaikuttavuuteen. Havaittiin, että sähköisellä asioinnilla ja työnjaolla on selkeä yhteys kustannuksiin. Työn empiirisessä osassa tarkasteltiin kolmea hyvinvointiasemamallin pilottihankkeessa mukana olevaa asemaa Lauritsalassa, Lemillä ja Ruokolahdella. Kustannusten tarkastelu keskittyi henkilöstö- ja tilakustannuksiin. Tutkittiin myös, miten sähköinen asiointi ja Eksoten alueen puhelinpalvelujen keskittäminen vaikuttaa edellä mainittuihin kustannuseriin. Tutkimuksen tuloksina havaittiin, että hyvinvointiasemamallin pilotointi ei ole tutkittavilla asemilla merkittävästi vaikuttanut kustannuksiin henkilöstön osalta; tilakustannukset olivat pienentyneet Lemillä ja Ruokolahdella. Laskelmien perusteella muodostettujen tavoitteiden mukaisesti on hyvinvointiasemilla kuitenkin pyrittävä vähintään noin 14 % tai enintään 32 % säästöön henkilöstö- ja tilakustannuksissa pilottivaiheen kustannuksiin verrattuna. Tulevaisuudessa olennaiseksi kustannusten hallinnan edellytykseksi havaittiin työnjaon muuttaminen hoitajavetoisemmaksi ja sähköisten asiointipalvelujen lisääntyvä tarjonta. Sähköisten asioinnin lisääntyvällä käytöllä on mahdollista vähentää hoitohenkilöstön tarvetta kokonaisuudessaan. Sekä sähköisen asioinnin käytön lisäämisellä että puhelinpalvelujen keskittämisellä tulevaisuudessa havaittiin olevan huomattava vaikutus henkilöstöresurssien käyttöön ja edelleen kustannustehokkuuteen.

Jusqu’à présent, la santé des personnes âgées a surtout été étudiée sous l’angle de la perte d’autonomie, des politiques de santé et des difficultés soulevées par l’allocation des ressources, et ces problèmes ont été généralement traités par des technocrates, des spécialistes et rarement par les personnes âgées elles-mêmes. En outre, on s’intéresse peu aux perceptions des personnes âgées selon le ou les points de vue qui peuvent être les leurs. L’originalité de cette thèse provient justement du fait qu’elle a pour objet l’expérience du vieillissement telle que vécue par des personnes âgées et les représentations qu’elles s’en font dans leurs rapports avec les services de santé –vécu, attentes et besoins, le tout envisagé sous l’angle des questions et enjeux éthiques de respect de l’autonomie des personnes, de justice et d’équité, de responsabilité partagée. Il ressort de trente entretiens avec des personnes âgées entre 70 et 91 ans que l’expérience du vieillissement est toujours unique, singulière, même si certaines catégorisations et modélisations peuvent être construites en fonction de l’état de santé, des ruptures et des pertes subies, des liens maintenus ou pas. La préservation de leur autonomie apparaît comme un élément important, parfois central dans les propos recueillis, qui disent les efforts consentis à cette fin, reconnaissant toutefois que, fragilisée, l’autonomie a besoin, pour pouvoir encore se vivre et s’exercer malgré les limites et les contraintes liées à l’âge ou à la maladie, d’une solidarité effective, de l’aide des proches et d’un soutien plus large. Le partage des responsabilités et des tâches entre les proches et les services sociaux est perçu comme inique. Aussi interroge-t-on, en conclusion de la thèse, la volonté de l’État et sa contribution dans la mise en œuvre de la politique de maintien à domicile et des dispositifs de pratique, et son bien-fondé en regard de l’hétérogénéité du vieillissement.
