BACKGROUND: Research into the methods which caregivers use to encourage children to eat vegetables is limited, with minimal evidence about what the barriers are to offering these foods. Vegetable consumption in children is typically low, and so gaining information on these factors is vital in order to develop further caregiver-centred interventions to increase children's vegetable consumption. This study aimed to investigate the methods caregivers use to offer vegetables to preschool aged children, as well as the factors which influence whether and how caregivers present vegetables to their children. METHOD: Seventeen caregivers with a preschool aged child participated in focus groups to assess these questions. RESULTS: Thematic analysis indicated that caregivers use a range of methods to offer their children vegetables, with these methods falling into three broad categories: behavioural/active methods, passive methods and food manipulations. Influences on caregiver offering which emerged from the focus groups formed four categories: information, cost, parent factors and child factors. CONCLUSIONS: Together with large-scale quantitative data, this information can be used to shape future interventions aiming to increase children's vegetable intake as well as to tailor advice given to caregivers striving to achieve a healthful diet for their children.

Universal screening for prostate cancer (Pca) using prostate-specific antigen-based testing is not recommended, as the potential harms of screening (overdiagnosis and overtreatment) outweigh potential benefits. The case for Pca screening requires a paradigm shift, which emphasizes the risks of screening over the risks of undetected cancer. Physicians are encouraged to use shared decision making with patients who express an interest in Pca screening, taking into account both the patient's screening preferences and individual risk profile. New models of care informed by the Patient Protection and Affordable Care Act are intended to assist clinicians in providing recommended preventive services. © 2014 Elsevier Inc.

The article examines developments in the marketisation and privatisation of the English National Health Service, primarily since 1997. It explores the use of competition and contracting out in ancillary services and the levering into public services of private finance for capital developments through the Private Finance Initiative. A substantial part of the article examines the repeated restructuring of the health service as a market in clinical services, initially as an internal market but subsequently as a market increasing opened up to private sector involvement. Some of the implications of market processes for NHS staff and for increased privatisation are discussed. The article examines one episode of popular resistance to these developments, namely the movement of opposition to the 2011 health and social care legislative proposals. The article concludes with a discussion of the implications of these system reforms for the founding principles of the NHS and the sustainability of the service.

Objective: Using an Importance-Performance Analysis (IPA), this paper examined a Radiotherapy and Oncological Patient and Non-patient perceived importance and performance of ten Humanization Volunteer Program selection factors in the General Hospital in Spain.Methods: The authors identified a list of seven items from the Health-marketing literature reviews, and each item was rated using a 5-point Likert scale. Responses were obtained of 148 usable interviews.Results: The importance-performance patient grid was corrected with literature recommendations and, in the patient sample, shows three items fall in the “Keep up the good work” quadrant, four items fall into the “Low priority” quadrant, zero items fall into the “Possible overkill” quadrant, and no items fall in the “Concentrate here” quadrant. Non-patient (n=80) factors means shows statistical differences with patient (n=68) means in catering (t=-2.38; df.=146; p<0.05), and information (t=2.16; df146; p<0,05).Conclusions: The results are useful in identifying areas for strategic focus to help Health Services managers develop humanization programs with volunteer workers and different program users. Implication to volunteer programs managers and researchers were discussed.

Speech and language ability is not a unitary concept; rather, it is made up of multiple abilities such as grammar, articulation and vocabulary. Young children from socio-economically deprived areas are more likely to experience language difficulties than those living in more affluent areas. However, less is known about individual differences in language difficulties amongst young children from socio-economically deprived backgrounds. The present research examined 172 four-year-old children from socio-economically deprived areas on standardised measures of core language, receptive vocabulary, articulation, information conveyed and grammar. Of the total sample, 26% had difficulty in at least one area of language. While most children with speech and language difficulty had generally low performance in all areas, around one in 10 displayed more uneven language abilities. For example, some children had generally good speech and language ability, but had specific difficulty with grammar. In such cases their difficulty is masked somewhat by good overall performance on language tests but they could still benefit from intervention in a specific area. The analysis also identified a number of typically achieving children who were identified as having borderline speech and language difficulty and should be closely monitored

Bakgrund: All sjukvård i Sverige styrs av Hälso-och sjukvårdslagen [HSL]. Under vissa omständigheter kan denna lag behövas kompletteras med Lagen om psykiatrisk tvångsvård [LPT]. Dessa personer vårdas inom psykiatrisk slutenvård. Omvårdnadens syfte är att patienten samtycker till vården och tvångsvården kan över gå till frivillig vård. Dessa personers problematik är dock ofta komplex och omvårdnaden upplevs innebära många svårigheter för sjuksköterskorna. Syfte: Att beskriva sjuksköterskors erfarenheter och upplevelse av omvårdnad i samband med LPT-vård inom sluten psykiatrisk vård. Metod: Empirisk kvalitativ studie med deskriptiv design. Studien baseras på sju semistrukturerade intervjuer med sjuksköterskor. Dessa analyserades med manifest innehållsanalys. Resultat: Resultatet visade att en förutsättning för att uppnå målet med tvångsvården genom omvårdnad var att skapa en allians med patienten. Kommunikation som innefattade individuellt anpassad information var en nyckelfaktor till alliansskapande. Omvårdnadsarbetet innefattade samverkan och sjuksköterskorna upplevde ett behov av kompetensutveckling. Slutsats: Sjuksköterskor upplever att omvårdnad enligt LPT är en utmaning och sammankopplat med flertalet svårigheter. Erhålls kompetens, tid för reflektion och trygghet i arbetsgruppen ses dock inte omvårdnaden som en omöjlighet utan kan upplevas som positivt för både den som vårdar och för den som vårdas.

Objetivou-se compreender a humanização do atendimento à criança na Atenção Básica na visão dos profissionais. Estudo qualitativo, realizado em uma Unidade de Saúde da Família de Natal-RN, Brasil. Dezesseis profissionais responderam a um formulário contendo questões referentes ao atendimento à criança, à humanização e às práticas realizadas para humanizar o atendimento. Os dados foram categorizados por temas e analisados a partir dos princípios da Política Nacional de Humanização. Para os profissionais, humanizar o atendimento envolve acolher, escutar, aconselhar sobre o que está sendo realizado com a criança, valorizar a família, e tornar o sujeito ativo no atendimento, mesmo que de forma incipiente. A maioria dos profissionais descreveu atendimento que valorizava parte dos princípios da política de humanização, mesmo com dificuldades para implementá-los na rotina. Requer, portanto, estímulos e atualização dos profissionais para uma postura autocrítica sobre o atendimento

Tiivistelmä TURUN YLIOPISTO Oikeustieteellinen tiedekunta Perhe-oikeus VALJAKKA EEVA: Vain lakiko lasta suojelee? Väitöskirja, 219 s. Kesäkuu 2016 Tutkimuksessa tarkastellaan suomalaisen lastensuojelulainsäädännön pitkän ai- kavälin kehitystä 1800-luvun lopulta 2010-luvulle. Tavoitteena on selvittää, mil- lä tavoin lasten suojelun tarvetta on lastensuojelulakeja säädettäessä määritelty ja millaisin keinoin julkinen valta on suojellut lapsia. Tutkimuksessa luodaan myös yleisemmin kuvaa siitä, millaista keskustelua lastensuojelun yhteiskunnallisesta tehtävästä, yksityisen ja julkisen hyvinvointivastuun jakautumisesta sekä julkisen vallan perheen yksityisyyteen puuttuvien toimien tarpeellisuudesta ja oikeutukses- ta on eri aikoina käyty. Tutkimuksessa lastensuojelun käsitettä käytetään sekä laa- jemmassa (Child Welfare) että suppeammassa (Child Protection) merkityksessä. Tutkimus on lapsi- ja oikeuspoliittinen puheenvuoro, jossa lastensuojelun institutionaalisia kehityskulkuja tarkastellaan yhteiskunnassa sosioekonomisesti heikoimmassa asemassa olevan lapsiväestön ja lastensuojelun asiakkaiksi valikoi- tuvien lasten näkökulmasta. Tutkimusote on ongelmalähtöinen ja yhteyshakuinen siten, että tutkimuskysymyksiä tarkastellaan monitieteisesti hallinto- ja sosiaalioi- keudellisessa viitekehyksessä. Tutkimus osoittaa, että tarkasteltaessa lastensuojelua käsitteen suppeam- massa merkityksessä lastensuojeluinstituution rakenteet kuten lasten suojelutar- peen määrittely, suojelun keinot ja lastensuojelulain julkilausutut tavoitteet ovat pysyneet lähes samankaltaisina aina ensimmäisen lastensuojelulain (1936) säätä- misestä lukien. Tarkasteltaessa lastensuojelua käsitteen laajemmassa merkityksessä voidaan lastensuojelussa nähdä tapahtuneen muutoksia. Vähitellen hyvinvointivaltion laajentuessa lastensuojelua koskeva ymmärrys muuttui yhä moniulotteisemmak- si. Lastensuojelu merkitsi 1960-luvulle tultaessa laajaa lapsiväestön hyvinvointia turvaavaa yhteiskuntapoliittista ohjelmaa. Myös sosiaalipolitiikan suunnanmuutos 1990-luvulla ja sen myötä julkisen vallan vastuun kaventaminen merkitsivät muu- tosta lastensuojelussa. Monille yhteiskunnan eri osa-aluille ulottuvien perheiden ja lasten hyvinvointia tavoittelevien politiikkaohjelmien ja niiden toteuttamispyrki- mysten sijaan lastensuojelu kaventui niin käsitteenä kuin käytännön toimintanakin tarkoittamaan lähes yksinomaan sosiaalihuollon erityispalvelua eli lapsi- ja perhe- kohtaista lastensuojelua. A terapia analítico-comportamental infantil, embasada pelos pressupostos do Behaviorismo Radical e pelos princípios da Análise do Comportamento, é caracterizada por atendimento da criança em consultório e orientação de pais. Estudos têm apontado dificuldades encontradas pelos psicólogos em relação aos resultados clínicos no que se refere à generalização de ganhos terapêuticos. Apesar das investigações concluírem que os programas de intervenção com pais têm apresentado resultados satisfatórios, a prática clínica relata problemas em relação à generalização, principalmente quando avaliados ao longo do tempo. A presente pesquisa teve como objetivo compreender o processo de orientação de pais a partir de relações de elementos descritos pela literatura como influentes no processo de generalização de ganhos terapêuticos. Foi aplicado questionário, construído pela autora e sua orientadora a partir da bibliografia de referência da área estudada, com análise de especialistas e análise cognitiva dos itens (aplicação a 5 indivíduos com características semelhantes às dos nossos participantes), em 38 pais de crianças atendidas por psicólogos analistas do comportamento na cidade de Fortaleza-Ce, Brasil. Os dados foram avaliados a partir de uma análise descritiva (Tendência Central e Dispersão). Os itens do questionário foram classificados em nove categorias que foram compreendidas a partir da análise de correlação de Spearman por meio do programa estatístico SPSS. Os resultados constataram correlações positivas e significativas entre o processo de generalização e as categorias estudadas (Relação terapêutica; Ocorrência da orientação de pais; Ocorrência da generalização - Percepção dos pais; Treino de habilidades sociais para pais; Dinâmica familiar - Dificuldades no contexto familiar; Dinâmica familiar - Participação do cônjuge nos processos de orientação/generalização; Estabelecimento de regras funcionais pelos pais; Dificuldades pessoais dos pais), indicando que o psicólogo precisa avaliar e intervir nestas variáveis com o objetivo de contribuir para melhores efeitos no processo terapêutico.

The gender wage gap is well studied in developed countries; however, recently it has generated much interest in developing countries. This thesis addresses three issues regarding the gender wage gap in Bangladesh. Firstly, it explores the wage determinants for formal public and private sector employees in Bangladesh and examines the gender pay gap. This is the first time different decomposition methods have been used to compare the sources of the gender wage gap as well as any potential discrimination effect in the formal sector of the Bangladeshi labour market. These decomposition methods are: the original Oaxaca (1973) and Blinder (1973) decomposition methods, the Neumark (1988), Cotton (1988), and Reimers (1983) methods, and the extended Oaxaca method including both the employment selection and the double selection correction in the wage equation. In addition to mentioned methods, to quantify the gender wage gap in monetary terms, a recently developed ‘simulated change’ approach by Olsen and Walby (2004) is also applied for the first time to the Bangladeshi data. By using the Labour Force Survey 2005-06, BBS (LFS 2005-06) data results show formal sector female employees earn about 32.1 per cent less than their male counterparts. Without considering the selection correction, a large range of human capital, demographic and labour market related variables are explained less than half of the total gender wage gap (21 to 46 per cent of the total wage gap) and the major part of the wage gap is unexplained (54 to 79 per cent of the total wage gap). This could partly be attributed to discrimination. Using the double selection correction method, the decomposition results changed where a small part of the wage gap was explained by the measured characteristics (only nine per cent of the total wage gap) and a major part is attributed to the discrimination and selection effect. The selection effect also reveals that exclusion of the double selection correction might lead to an overestimation of the gender wage gap in the formal sector of Bangladesh. In addition, results based on the Olsen and Walby (2004) simulation method show that, if the other characteristics of male and female employees were similar, ‘being female’ is sufficient, to generate significantly lower wages than males in the formal sector. If females in the workplace are treated as males, without considering any other endowment increases, females could increase their earnings by 4095.3 Taka1 per year. Results also indicate that not only endowment differences in human capital and work experience related variables were important, but discrimination appears to play a significant role in the total wage gap throughout the formal sector of the Bangladeshi labour market. Secondly, the study investigates whether public sector employees enjoyed a wage premium or not, compared to the private sector and whether the gender wage gap is greater in the public sector. In addition, the research considered whether there was an impact from the inclusion of the different selection correction terms in the wage equation. In Bangladesh, public sector employees have, on average, a 60 per cent wage premium over the private sector. Using both the original Oaxaca and the extended Oaxaca methods, where selection effect is partly captured by both explained and unexplained components, and using the public sector wage structure as the basis of the non-discriminatory wage structure, these methods revealed a considerably larger portion of explained (72 - 93 per cent of the total wage gap) and a smaller portion of unexplained part of the wage gap. However, if the selection correction is considered as another component of the decomposition outcome then the major portion of the total public and private sectors wage gap is justified (explained) by the effect of the selection correction and unexplained factors. Furthermore, a large part of the wage premium exists in favour of public sector female employees compared to males and the gender wage gap is lower in the public sector than the private sector. Finally, this study compares the gender wage gap of five different occupations. The gender wage gap is associated with labour market rigidities where one of the important factors is occupational segregation where females are disproportionately distributed in occupations resulting in lower earnings. The largest gender wage gap was found in agriculture, forestry, fisheries, production and transport labour jobs (56.4 per cent) and the lowest in the professional, technical administrative and managerial jobs (22.1 per cent). Substantial differences are found in the size of the endowment gap across occupations and larger variations occurred in the adjusted wage gap which varied from 21.4 per cent in sales and service occupations, to 100 per cent in professional and technical jobs (the highest). This too can be partly explained by discrimination. A reduction in the gender wage gap is expected not only to increase national income, but also to reduce poverty and lead to better outcomes for future generations. National policy should aim to reduce the gender wage gap and achieve gender wage equality in the formal sector; for example through a targeted program to remove the gender differences in education and to reduce the skill difference, with a better child care policy to encourage labour force retention and increased labour market experience for female employees, with anti-discriminatory policies and the enforcement of existing antidiscrimination policies, and a more equal distribution of males and females across occupations.
