During June 1997-June 1999 rotavirus infection was screened in infants aged up to 2 years and hospitalised with acute diarrhoea in São LuÃs, Northeastern Brazil. Altogether, 128 stool samples were collected from diarrhoeic patients and additional 122 faecal specimens from age- and- temporal matched inpatients without diarrhoea were obtained; rotavirus positivity rates for these groups were 32.0% (41/128) and 9.8% (12/122), respectively (p < 0.001). Both electropherotyping and serotyping could be performed in 42 (79.2%) of the 53 rotavirus-positive stool samples. Long and short electropherotypes were detected at similar rates - 38.1% and 40.5% of specimens, respectively. Overall, a G serotype could be assigned for 35 (83.3%) of specimens, the majority of them (66.7%) bearing G1-serotype specificity. Taking both electropherotypes and serotypes together, G1 rotavirus strains displaying long and short RNA patterns accounted for 30.9% and 19.0% of tested specimens, respectively; all G2 strains had short electropherotype. Rotavirus gastroenteritis was detected year-round and, in 1998, the incidence rates tended to be higher during the second semester than in the first semester: 45.2% and 26.1% (p = 0.13), respectively. Rotavirus infections peaked at the second semester of life with frequencies of 30.1% and 13.5% for diarrhoeic children and controls, respectively. While the six rotavirus strains bearing G2-type specificity were circulating throughout the whole study period, G1 serotypes (n = 27) emerged as from June 1998 onwards, 20 (74.1%) of which clustering in 1998. These data underscore the importance of rotaviruses in the aetiology of severe infantile gastroenteritis in Northeastern Brazil and sustain the concept that a future vaccine should confer protection against more than one serotype.

Speech interfaces for Assistive Technologies are not common and are usually replaced by others. The market they are targeting is not considered attractive and speech technologies are still not well spread. Industry still thinks they present some performance risks, especially Speech Recognition systems. As speech is the most elemental and natural way for communication, it has strong potential for enhancing inclusion and quality of life for broader groups of users with special needs, such as people with cerebral palsy and elderly staying at their homes. This work is a position paper in which the authors argue for the need to make speech become the basic interface in assistive technologies. Among the main arguments, we can state: speech is the easiest way to interact with machines; there is a growing market for embedded speech in assistive technologies, since the number of disabled and elderly people is expanding; speech technology is already mature to be used but needs adaptation to people with special needs; there is still a lot of R&D to be done in this area, especially when thinking about the Portuguese market. The main challenges are presented and future directions are proposed.

Em 1914,0 Prof. Francisco Gentil, então Enfermeiro-Mor dos Hospitais Civis de Lisboa, propos que fossem criadas no Hospital de D. Estefânia duas Enfermarias independentes, uma para doenças pediátricas médicas e outra para doenças pediátricas Cirúrgicas/Ortopédicas. Isto tornou-se realidade em 9 de Julho de 1918, através do Decreto 4.563, ficando a direcção dos dois Serviços a cargo de Jaime Ernesto Salazar de Sousa. Após a sua morte, em 1940, Abel Pereira da Cunha tornou-se o novo Director, seguindo-se-lhe Eduardo Rosado Pinto, em 1959. É então que o único Serviço existente, o Serviço 5, se divide em dois, o 3 e o 4, sob a Direcção rcspectivamente de José Rosado Pinto e Luciano José de Carvalho. Em 1983 Fernando Gabriel Pinto Coelho Afonso torna-se Director do Serviço 3 e em 1986 Antonio Genlil do Silva Martins torna-se Director do Serviço 4. Em 1994 Fernando Afonso torna-se Director do Departamento de Cirurgia (durante 1 mês, até aposentação) seguindo-se-lhe António Gentil Martins, jubilado em 2000. Em 29 de Junho de 1974, no Hospital de D. Estefânia, é fundada a Sociedade Portuguesa de Cirurgiões Pediatras, realizando-se os primeiros Congressos Internacioais em 1971 e 1972 (Luso-Brasileiro e Luso-Espanhol). Em 1986 o Hospital associa-se a Faculdade das Ciências Médicas da Universidade Nova de Lisboa, para o ensino pré-graduado de Cirurgia Pediátrica, sendo António Gentil Martins nomeado Professor Associado Convidado. Mais de 50% de todos os Cirurgiões Pediatras Portugueses fizeram o seu treino no Hospital dc D. Esteffinia e através deles a Especialidade estendeu-se a Coimbra, Viseu, Évora, Setúbal,Almada, Sintra, Montemor-o-Novo e Funchal (assim cobrindo cerca de 3/4 de toda a populacao portuguesa).

Apresenta-se uma casuÃstica de nove doentes com litÃase renal, com idades compreendidas entre os 5 e os 15 anos, que foram submetidas a um tratamento com litotrÃcia. Em todos eles o diagnóstico de infecção urinária antecedeu o de litÃase renal. Em duas erianças havia antecedentes familiares de litÃase renal e, utilizando exames laboratoriais e de imagiologia, foram diagnosticados 4 doentes com hipercalciúria, 2 com hiperoxalúria, 1 com cistinúria e só num caso foi diagnosticado alterações estruturais (estenose ureteral justavesical bilateral). Sete destes doentes já tinham sido submetidos a intervenções cirúrgicas anteriores. A nossa experiência com estes 9 doentes demonstrou que a litotrÃcia é uma técnica de tratamento segura, não tendo sido registado qualquer tipo de complicação em 11 sessões. Houve 1 caso de insucesso, 5 doentes ficaram com os cálculos fragmentados mesmo após 20 meses de «follow-up» e os outros 3 ficaram curados. Por fim recorda-se que esta técnica, apesar do seu sucesso, e só uma parte do tratamento completo da litÃase renal, pois esta é um problema complexo que necessita sempre de uma avaliação metabólica e anatómica.

Introduction: Hepatic capillariosis, caused by Capillaria hepatica (Calodium hepaticum) (Bancroft, 1893), Travassos, 1915 (Nematoda, Trichinelloidea, Capillariidae), is a common zoonosis in rodents but is rare in humans. Seventy-two cases in humans have been reported worldwide since the first case was described by MACARTHUR in 192417,27. This study aimed to determine the prevalence of Capillaria hepatica in humans and rodents in an urban area of Porto Velho, the capital of Rondônia, in Brazil. Methods: After conducting a census of the area, 490 residents were randomly selected, and, after signing a term of consent, provided blood samples that were screened for anti-Capillaria hepatica antibodies. Simultaneously, rats were captured to assess the prevalence of this parasite in rodents by histopathological examination in liver sections. Results: A prevalence of 1.8% was found among residents who had specific antibodies at a dilution of 1:150, indicating exposure to parasite eggs; 0.8% of the subjects also had positive titers at a dilution of 1:400, indicating true infection. The prevalence in rats was 2%. Conclusions: The prevalence of infection with this parasite among humans and rats was low. While the prevalence encountered among humans was within the limits reported in the literature, the prevalence among rodents was much lower.

Background and Objective: Drug-induced anaphylaxis is an unpredictable and potentially fatal adverse drug reaction. The aim of this study was to identify the causes of drug-induced anaphylaxis in Portugal. Methods: During a 4-year period a nationwide notification system for anaphylaxis was implemented, with voluntary reporting by allergists. Data on 313 patients with drug anaphylaxis were received and reviewed. Statistical analysis included distribution tests and multiple logistic regression analysis to investigate significance, regression coefficients, and marginal effects. Results: The mean (SD) age of the patients was 43.8 (17.4) years, and 8.3% were younger than 18 years. The female to male ratio was 2:1. The main culprits were nonsteroidal anti-inflammatory drugs (NSAIDs) (47.9% of cases), antibiotics (35.5%), and anesthetic agents (6.1%). There was a predominance of mucocutaneous symptoms (92.2%), followed by respiratory symptoms (80.4%) and cardiovascular symptoms (49.0%). Patients with NSAID-induced anaphylaxis showed a tendency towards respiratory and mucocutaneous manifestations. We found no significant associations between age, sex, or atopy and type of drug. Anaphylaxis recurrence was observed in 25.6% of cases, and the risk was higher when NSAIDs were involved. Conclusions: NSAIDs were the most common cause of anaphylaxis in this study and were also associated with a higher rate of recurrence. We stress the need for better therapeutic management and prevention of recurring episodes of drug-induced anaphylaxis.

Introdução: A anafilaxia a fármacos constitui uma situação potencialmente fatal e imprevisÃvel, desconhecendo -se a real prevalência em diferentes grupos populacionais e os factores de risco relacionados. Objectivo: Contribuir para o melhor conhecimento epidemiológico da anafilaxia induzida por fármacos no nosso paÃs. Métodos: Durante um perÃodo de 4 anos (Janeiro de 2007 a Dezembro de 2010) foi implementado um sistema de notificação nacional de anafilaxia, focalizado na notificação voluntária por clÃnicos com diferenciação em patologia imunoalérgica. Foram recebidas e analisadas notificações de anafilaxia a fármacos de 313 doentes. No estudo estatÃstico foram aplicados testes de distribuição e análise de regressão logÃstica múltipla para obter significância e coeficientes de regressão e efeitos marginais. Resultados: A média de idade foi de 43,8 ±17,4 anos, sendo 8% de idade inferior a 18 anos. A relação género feminino/masculino foi de 2/1. A média de idade do primeiro episódio foi de 39 ±18,2 anos. Nove doentes apresentaram mais que uma causa de anafilaxia, correspondendo a um total de 322 notificações de grupos de fármacos envolvidos. As principais causas da anafilaxia a fármacos foram os anti -inflamatórios não esteróides (AINEs), os antibióticos e os agentes anestésicos, com respectivamente 48%, 36% e 6% dos casos. Outros fármacos implicados foram citostáticos, corticosteróides, inibidores da bomba de protões e meios de contraste iodados, entre outros. Houve predomÃnio de manifestações mucocutâneas (92%), seguido de respiratórias (81%) e de cardiovasculares (49%). Os doentes com anafilaxia a AINEs apresentaram aumento significativo da associação de manifestações mucocutâneas e respiratórias. Não foram observadas diferenças significativas em idade, género ou antecedentes de atopia entre os diferentes grupos de fármacos envolvidos. As reacções ocorreram em ambiente hospitalar em 45% dos casos. Em 53% nos 15 minutos após a administração do fármaco e 35% motivaram internamento. A recorrência da anafilaxia foi observada em 26% e o risco foi significativamente mais elevado nos casos de anafilaxia a AINEs. Apenas 48% dos doentes receberam tratamento com adrenalina e somente em 9% dos casos foi prescrito dispositivo para auto -administração de adrenalina. Conclusões: Neste estudo os AINEs foram os fármacos mais frequentes e os mais associados a recorrência de anafilaxia. Destaca -se o sub -tratamento com adrenalina e a necessidade de serem tomadas medidas no sentido do tratamento eficaz e da prevenção da recorrência de anafilaxia a fármacos.

Oral busulfan is the historical backbone of the busulfan+cyclophosphamide regimen for autologous stem cell transplantation. However intravenous busulfan has more predictable pharmacokinetics and less toxicity than oral busulfan; we, therefore, retrospectively analyzed data from 952 patients with acute myeloid leukemia who received intravenous busulfan for autologous stem cell transplantation. Most patients were male (n=531, 56%), and the median age at transplantation was 50.5 years. Two-year overall survival, leukemia-free survival, and relapse incidence were 67±2%, 53±2%, and 40±2%, respectively. The non-relapse mortality rate at 2 years was 7±1%. Five patients died from veno-occlusive disease. Overall leukemia-free survival and relapse incidence at 2 years did not differ significantly between the 815 patients transplanted in first complete remission (52±2% and 40±2%, respectively) and the 137 patients transplanted in second complete remission (58±5% and 35±5%, respectively). Cytogenetic risk classification and age were significant prognostic factors: the 2-year leukemia-free survival was 63±4% in patients with good risk cytogenetics, 52±3% in those with intermediate risk cytogenetics, and 37 ± 10% in those with poor risk cytogenetics (P=0.01); patients ≤50 years old had better overall survival (77±2% versus 56±3%; P<0.001), leukemia-free survival (61±3% versus 45±3%; P<0.001), relapse incidence (35±2% versus 45±3%; P<0.005), and non-relapse mortality (4±1% versus 10±2%; P<0.001) than older patients. The combination of intravenous busulfan and high-dose melphalan was associated with the best overall survival (75±4%). Our results suggest that the use of intravenous busulfan simplifies the autograft procedure and confirm the usefulness of autologous stem cell transplantation in acute myeloid leukemia. As in allogeneic transplantation, veno-occlusive disease is an uncommon complication after an autograft using intravenous busulfan.
