Introdução: O efeito de Pulfrich é um fenómeno psicofísico em que o movimento lateral/pendular de um objecto num plano bidimensional pode ser interpretado pelo córtex visual como um movimento tridimensional devido à diferença relativa do tempo de latência entre os dois olhos. Objectivo: Determinar se a neuropatia óptica desmielinizante unilateral ou assimétrica origina o fenómeno de Pulfrich. Métodos: Pesquisámos em 22 doentes com o diagnóstico de neuropatia óptica desmielinizante a percepção do fenómeno através de uma imagem pendular gerada por computador. Avaliámos as acuidades visuais corrigidas, realizámos Potenciais Evocados Visuais (PEV) para quantificação do tempo de latência da onda P100 e recorremos à interposição de um filtro polarizado de 70% para verificar a sua anulação. Resultados: Dez dos 22 doentes observaram o fenómeno e este foi abolido após interposição de um filtro polarizado. Conclusões: A neuropatia óptica pode originar o efeito de Pulfrich e explicar algumas queixas visuais aparentemente inespecíficas, sem alterações evidentes das acuidades visuais ou da estereopsia.

The morphology and distribution of local-circuit neurons (interneurons) were examined, by calbindin D-28k and parvalbumin immunocytochemistry, in the frontal cortex (area 8) in two patients with frontal lobe dementia of non-Alzheimer type associated with classical amyotrophic lateral sclerosis (ALS), and in seven normal cases. The density of calbindin D-28k immunoreactive cells was dramatically reduced in ALS patients, but the density of parvalbumin-immunoreactive neurons was preserved. Decreased density of calbindin D-28k-immunoreactive neurons, which are mainly located in the upper cortical layers, may interfere with the normal processing of cortico-cortical connections, whereas integrity of parvalbumin-immunoreactive cells may be associated with the preservation of the major inhibitory intracortical circuits in patients with frontal lobe dementia.

The objective of this study was to investigate the association of functional variants of the human CX3CR1 gene (Fractalkine receptor) with the risk of Amyotrophic Lateral Sclerosis disease (ALS), the survival and the progression rate of the disease symptoms in a Spanish ALS cohort. 187 ALS patients (142sporadic [sALS] and 45 familial) and 378 controls were recruited. We investigated CX3CR1 V249I (rs3732379) and T280M (rs3732378) genotypes and their haplotypes as predictors of survival, the progression rate of the symptoms (as measured by ALSFRS-R and FVC decline) and the risk of suffering ALS disease. The results indicated that sALS patients with CX3CR1 249I/I or 249V/I genotypes presented a shorter survival time (42.21±4.82) than patients with 249V/V genotype (67.48±7.28; diff=-25.27 months 95%CI [-42.09,-8.45]; p=0.003). The survival time was shorter in sALS patients with spinal topography and CX3CR1 249I alleles (diff -29.78 months; 95%CI [-49.42,-10.14]; p=0.003). The same effects were also observed in the spinal sALS patients with 249I-280M haplotype (diff=-27.23 months; 95%CI [-49.83,-4.64]; p=0.018). In the sALS group, the CX3CR1 249I variant was associated with a faster progression of the disease symptoms (OR= 2.32; 95IC%[1.24, 4.33]; p=0.007). There was no evidence for association of these two CX3CR1 variants with ALS disease risk. The association evidenced herein is clinically relevant and indicates that CX3CR1 could be a disease-modifying gene in sALS. The progression rate of the disease"s symptoms and the survival time is affected in patients with one or two copies of the CX3CR1 249I allele. The CX3CR1 is the most potent ALS survival genetic factor reported to date. These results reinforce the role of the immune system in ALS pathogenesis.

A doença de Charcot-Marie-Tooth (CMT) é a mais freqüente das neuropatias hereditárias, mas a sua história natural é pouco conhecida. Apesar da maioria dos pacientes apresentarem uma evolução com características mais “benignas”, é associado à doença com um comprometimento funcional, o que pode ser acompanhado de limitações e incapacitações. O presente estudo teve como objetivo avaliar a qualidade de vida de pacientes afetados pela CMT1A. Considerando que a qualidade de vida pode sofrer influência de variáveis sócio-culturais, um estudo neste contexto mostra-se oportuno. Nenhum estudo de nosso conhecimento realizou tal avaliação no contexto brasileiro. Fizeram parte deste estudo pacientes com CMT decorrente da duplicação 17p11.2-p12 (CMT1A) que foram avaliados quanto ao comprometimento funcional por meio da aplicação do SF-36. Esse questionário é utilizado internacionalmente para aferir a qualidade de vida dos indivíduos. Os resultados obtidos foram submetidos à análise estatística. Como resultados não se observaram associações entre o gênero e os domínios avaliados para a qualidade de vida. Os dados obtidos revelaram que a qualidade de vida dos pacientes com CMT1A se mostrou comprometida principalmente quanto aos seus Aspectos Sociais (p = 0,02) e Aspectos emocionais (p = 0,04). Adicionalmente, houve uma tendência ao comprometimento da Capacidade Funcional (p = 0,08), sendo que os outros índices de avaliação de comprometimento físico se mostraram preservados. A presença ou ausência de ocupação parece influenciar os Aspectos Sociais apresentando uma tendência de diferença significativa (p = 0,07), possuindo o grupo com ocupação valores superiores. Apesar da CMT1A apresentar piora clínica ao longo dos anos, a idade mais avançada não implicou em uma pior percepção da qualidade de vida no grupo de pacientes estudados no presente trabalho... Our objective was to investigate spinal cord (SC) atrophy in amyotrophic lateral sclerosis (ALS) patients, and to determine whether it correlates with clinical parameters. Forty-three patients with ALS (25 males) and 43 age- and gender-matched healthy controls underwent MRI on a 3T scanner. We used T1-weighted 3D images covering the whole brain and the cervical SC to estimate cervical SC area and eccentricity at C2/C3 level using validated software (SpineSeg). Disease severity was quantified with the ALSFRS-R and ALS Severity scores. SC areas of patients and controls were compared with a Mann-Whitney test. We used linear regression to investigate association between SC area and clinical parameters. Results showed that mean age of patients and disease duration were 53.1 ± 12.2 years and 34.0 ± 29.8 months, respectively. The two groups were significantly different regarding SC areas (67.8 ± 6.8 mm² vs. 59.5 ± 8.4 mm², p < 0.001). Eccentricity values were similar in both groups (p = 0.394). SC areas correlated with disease duration (r = - 0.585, p < 0.001), ALSFRS-R score (r = 0.309, p = 0.044) and ALS Severity scale (r = 0.347, p = 0.022). In conclusion, patients with ALS have SC atrophy, but no flattening. In addition, SC areas correlated with disease duration and functional status. These data suggest that quantitative MRI of the SC may be a useful biomarker in the disease.

Lateral pterygoid muscle (LPM) plays an important role in jaw movement and has been implicated in Temporomandibular disorders (TMDs). Migraine has been described as a common symptom in patients with TMDs and may be related to muscle hyperactivity. This study aimed to compare LPM volume in individuals with and without migraine, using segmentation of the LPM in magnetic resonance (MR) imaging of the TMJ. Twenty patients with migraine and 20 volunteers without migraine underwent a clinical examination of the TMJ, according to the Research Diagnostic Criteria for TMDs. MR imaging was performed and the LPM was segmented using the ITK-SNAP 1.4.1 software, which calculates the volume of each segmented structure in voxels per cubic millimeter. The chi-squared test and the Fisher's exact test were used to relate the TMD variables obtained from the MR images and clinical examinations to the presence of migraine. Logistic binary regression was used to determine the importance of each factor for predicting the presence of a migraine headache. Patients with TMDs and migraine tended to have hypertrophy of the LPM (58.7%). In addition, abnormal mandibular movements (61.2%) and disc displacement (70.0%) were found to be the most common signs in patients with TMDs and migraine. In patients with TMDs and simultaneous migraine, the LPM tends to be hypertrophic. LPM segmentation on MR imaging may be an alternative method to study this muscle in such patients because the hypertrophic LPM is not always palpable.

Acute disseminated encephalomyelitis (ADEM) is a widespread monophasic inflamatory disease affecting the central nervous system, that usually follows an infection or vaccination. In this study, we present an analysis of magnetic resonance imaging (MRI), cerebrospinal fluid (CSF) and clinical aspects in four patients with clinical diagnosis of ADEM. The presence of MRI demyelinating lesions was crucial, but not in itself sufficient for definitive diagnosis. Clinical and MRI follow up, in order to exclude new lesions and to reevaluate the former ones, as well as CSF, were important for the differential diagnosis with other demyelinating diseases, particularly multiple sclerosis. In addition, we have shown that early treatment with methylprednisolone after the initial symptoms was effective for improving clinical manifestations as well as for reducing MRI lesions.
