Substance misuse in people with psychosis presents significant problems, but trials of treatments to address it show little sustained advantage over control conditions. An examination of mechanisms underpinning unassisted improvements may assist in the refinement of co-morbidity treatments. This study reviewed existing research on natural recovery from substance misuse in people with psychosis. To address this issue, a systematic search identified only 7 articles that fulfilled the criteria. Their results suggest that people with psychosis report similar reasons to change as do non-psychotic groups, although they did not clarify whether the relative frequencies or priority orders were the same. Differences involved issues relating to the disorder and the functional problems faced by this group: receipt of treatment for mental health difficulties, worsening of mental health difficulties, and homelessness. The current research on reasons for change in people with psychosis is sparse and has significant limitations, and as yet it offers little inspiration for new treatments. A more fertile source may prove to be a detailed investigation of successful substance control strategies that are used in self-management by this group.

Background: The Queensland East Coast Otter Trawl Fishery (ECOTF) for penaeid shrimp fishes within Australia's Great Barrier Reef World Heritage Area (GBRWHA). The past decade has seen the implementation of conservation and fisheries management strategies to reduce the impact of the ECOTF on the seabed and improve biodiversity conservation. New information from electronic vessel location monitoring systems (VMS) provides an opportunity to review the interactions between the ECOTF and spatial closures for biodiversity conservation. Methodology and Results: We used fishing metrics and spatial information on the distribution of closures and modelled VMS data in a geographical information system (GIS) to assess change in effort of the trawl fishery from 2001-2009 and to quantify the exposure of 70 reef, non-reef and deep water bioregions to trawl fishing. The number of trawlers and the number of days fished almost halved between 2001 and 2009 and new spatial closures introduced in 2004 reduced the area zoned available for trawl fishing by 33%. However, we found that there was only a relatively minor change in the spatial footprint of the fishery as a result of new spatial closures. Non-reef bioregions benefited the most from new spatial closures followed by deep and reef bioregions. Conclusions/Significance: Although the catch of non target species remains an issue of concern for fisheries management, the small spatial footprint of the ECOTF relative to the size of the GBRWHA means that the impact on benthic habitats is likely to be negligible. The decline in effort as a result of fishing industry structural adjustment, increasing variable costs and business decisions of fishers is likely to continue a trend to fish only in the most productive areas. This will provide protection for most benthic habitats without any further legislative or management intervention.

Aim: To develop approaches to the evaluation of programmes whose strategic objectives are to halt or slow weed spread. Location: Australia. Methods: Key aspects in the evaluation of weed containment programmes are considered. These include the relevance of models that predict the effects of management intervention on spread, the detection of spread, evidence for containment failure and metrics for absolute or partial containment. Case studies documenting either near-absolute (Orobanche ramosa L., branched broomrape) or partial (Parthenium hysterophorus (L.) King and Robinson, parthenium) containment are presented. Results: While useful for informing containment strategies, predictive models cannot be employed in containment programme evaluation owing to the highly stochastic nature of realized weed spread. The quality of observations is critical to the timely detection of weed spread. Effectiveness of surveillance and monitoring activities will be improved by utilizing information on habitat suitability and identification of sites from which spread could most compromise containment. Proof of containment failure may be difficult to obtain. The default option of assuming that a new detection represents containment failure could lead to an underestimate of containment success, the magnitude of which will depend on how often this assumption is made. Main conclusions: Evaluation of weed containment programmes will be relatively straightforward if containment is either absolute or near-absolute and may be based on total containment area and direct measures of containment failure, for example, levels of dispersal, establishment and reproduction beyond (but proximal to) the containment line. Where containment is only partial, other measures of containment effectiveness will be required. These may include changes in the rates of detection of new infestations following the institution of interventions designed to reduce dispersal, the degree of compliance with such interventions, and the effectiveness of tactics intended to reduce fecundity or other demographic drivers of spread. © 2012 Blackwell Publishing Ltd.

Background: Increased hospital readmission and longer stays in the hospital for patients with type 2 diabetes and cardiac disease can result in higher healthcare costs and heavier individual burden. Thus, knowledge of the characteristics and predictive factors for Vietnamese patients with type 2 diabetes and cardiac disease, at high risk of hospital readmission and longer stays in the hospital, could provide a better understanding on how to develop an effective care plan aimed at improving patient outcomes. However, information about factors influencing hospital readmission and length of stay of patients with type 2 diabetes and cardiac disease in Vietnam is limited. Aim: This study examined factors influencing hospital readmission and length of stay of Vietnamese patients with both type 2 diabetes and cardiac disease. Methods: An exploratory prospective study design was conducted on 209 patients with type 2 diabetes and cardiac disease in Vietnam. Data were collected from patient charts and patients' responses to self-administered questionnaires. Descriptive statistics, bivariate correlation, logistic and multiple regression were used to analyse the data. Results: The hospital readmission rate was 12.0% among patients with both type 2 diabetes and cardiac disease. The average length of stay in the hospital was 9.37 days. Older age (OR= 1.11, p< .05), increased duration of type 2 diabetes (OR= 1.22, p< .05), less engagement in stretching/strengthening exercise behaviours (OR= .93, p< .001) and in communication with physician (OR= .21, p< .001) were significant predictors of 30-dayhospital readmission. Increased number of additional co-morbidities (β= .33, p< .001) was a significant predictor of longer stays in the hospital. High levels of cognitive symptom management (β= .40, p< .001) significantly predicted longer stays in the hospital, indicating that the more patients practiced cognitive symptom management, the longer the stay in hospital. Conclusions: This study provides some evidence of factors influencing hospital readmission and length of stay and argues that this information may have significant implications for clinical practice in order to improve patients' health outcomes. However, the findings of this study related to the targeted hospital only. Additionally, the investigation of environmental factors is recommended for future research as these factors are important components contributing to the research model.

Background Prevention of foot ulcers in patients with diabetes is extremely important to help reduce the enormous burden of foot ulceration on both patient and health resources. A comprehensive analysis of reported interventions is not currently available, but is needed to better inform caregivers about effective prevention. The aim of this systematic review is to investigate the effectiveness of interventions to prevent first and recurrent foot ulcers in persons with diabetes who are at risk for ulceration. Methods The available medical scientific literature in PubMed, EMBASE, CINAHL and the Cochrane database was searched for original research studies on preventative interventions. Both controlled and non-controlled studies were selected. Data from controlled studies were assessed for methodological quality by two independent reviewers. Results From the identified records, a total of 30 controlled studies (of which 19 RCTs) and another 44 non-controlled studies were assessed and described. Few controlled studies, of generally low to moderate quality, were identified on the prevention of a first foot ulcer. For the prevention of recurrent plantar foot ulcers, multiple RCTs with low risk of bias show the benefit for the use of daily foot skin temperature measurements and consequent preventative actions, as well as for therapeutic footwear that demonstrates to relieve plantar pressure and that is worn by the patient. To prevent recurrence, some evidence exists for integrated foot care when it includes a combination of professional foot treatment, therapeutic footwear and patient education; for just a single session of patient education, no evidence exists. Surgical interventions can be effective in selected patients, but the evidence base is small. Conclusion The evidence base to support the use of specific self-management and footwear interventions for the prevention of recurrent plantar foot ulcers is quite strong, but is small for the use of other, sometimes widely applied, interventions and is practically nonexistent for the prevention of a first foot ulcer and non-plantar foot ulcer.

A recuperação de fábricas falidas pelos próprios trabalhadores, que assumem de alguma maneira os meios de produção e retomam as atividades da empresa, é um fenômeno que se tornou recorrente no Brasil a partir década de 90, no contexto de abertura do mercado e crise econômica. Esta investigação parte da experiência da FACIT, em Juiz de Fora/MG, considerando-a no decorrer de sua história, antes e depois da autogestão, em busca dos significados atribuídos pelos trabalhadores à experiência que vivenciaram e suas trajetórias profissionais e de grupo. A compreensão dessa experiência referencia-se na história mais geral do trabalho e de suas transformações no capitalismo contemporâneo, bem como das experiências de autogestão e autonomia dos trabalhadores no interior desse modo de produção. Considera-se o trabalho categoria central na estruturação das trajetórias e identidade dos trabalhadores, sendo a iniciativa de retomarem a produção de empresas falidas uma saída que se impôs com a crise da sociedade salarial, enquanto tentativa de conquista coletiva dos meios de vida. Nesta nova situação, quando se veem proprietários das instalações e reiniciam a produção sem os antigos patrões ou seus representantes imediatos, os trabalhadores trazem consigo suas trajetórias pretéritas forjadas no reino da heterogestão, dificultando a compreensão precisa do que representa a autogestão da empresa. Habituados ao chão-da-fábrica, lugar que até então podiam ocupar na divisão do trabalho da empresa, encontram-se frente à tarefa de assumirem também a gestão da empresa. Entre o chão e a gestão da fábrica, uma nova trajetória se ensaia, com suas contradições e ambiguidades.

A presente dissertação revela os resultados de um minucioso trabalho de revisão bibliográfica cujo objetivo consistia em buscar as diferentes concepções de autogestão presentes no pensamento dos principais teóricos que abordaram tal temática, no intervalo compreendido entre o alvorecer do século XIX e os anos 70 do século passado. Com efeito, parte-se do princípio de que a autogestão (entendida como uma nova ordem social em que predomina a total igualdade entre seus membros, ausentes quaisquer divisões entre dirigentes e dirigidos, cabendo à coletividade a tomada de decisões que afetem a vida social como um todo) é um subproduto das contradições e antagonismos inerentes a conflitiva ordem capitalista. Assim sendo, a autogestão social seria uma bandeira histórica do movimento operário e socialista, teorizada pelos principais pensadores ligados a tal movimento. Esta dissertação mostrará como as diversas concepções de autogestão nascem e refletem contextos históricos bem específicos, a partir das condições em que se dá o desenvolvimento capitalista e sua inseparável luta de classes.

O presente trabalho discute as perspectivas para a formação de professores presentes nos blogs de educadores, que buscam e transmitem conhecimentos, em uma rede colaborativa que desponta como uma nova modalidade de formação continuada. Optou-se pela abordagem metodológica qualitativa, dada as características do objeto de estudo e pela possibilidade de ampliação do diálogo entre a teorização e os dados coletados. Procedeu-se uma discursão a partir do discurso dos professores blogueiros entrevistados e da observação de seus blogs. Os 31 blogs que se teve acesso foram classificados segundo suas características preponderantes. Responderam o questionário proposto 27 blogueiros. Pode-se afirmar que os blogs dados seus aspectos de ferramentas de interação, de fácil acesso e uso e de aglutinação de pessoas em razão de interesses comuns, têm efeitos sobre a prática docente. Embora se tenha identificado nos blogs a prevalência de uma concepção compensatória, eles se mostram como ações de autogerenciamento por parte dos docentes, que não mais se isolam com suas dúvidas, incertezas e insegurança, mas, valendo-se da democratização da informática, lançam-se na internet para socializar saberes, partilhar práticas em uma rede de colaboração mútua.

Research on assessment and monitoring methods has primarily focused on fisheries with long multivariate data sets. Less research exists on methods applicable to data-poor fisheries with univariate data sets with a small sample size. In this study, we examine the capabilities of seasonal autoregressive integrated moving average (SARIMA) models to fit, forecast, and monitor the landings of such data-poor fisheries. We use a European fishery on meagre (Sciaenidae: Argyrosomus regius), where only a short time series of landings was available to model (n=60 months), as our case-study. We show that despite the limited sample size, a SARIMA model could be found that adequately fitted and forecasted the time series of meagre landings (12-month forecasts; mean error: 3.5 tons (t); annual absolute percentage error: 15.4%). We derive model-based prediction intervals and show how they can be used to detect problematic situations in the fishery. Our results indicate that over the course of one year the meagre landings remained within the prediction limits of the model and therefore indicated no need for urgent management intervention. We discuss the information that SARIMA model structure conveys on the meagre lifecycle and fishery, the methodological requirements of SARIMA forecasting of data-poor fisheries landings, and the capabilities SARIMA models present within current efforts to monitor the world’s data-poorest resources.

Esta dissertação tem como objetivo a análise da eficiência técnica das operadoras de planos de saúde vis-à-vis a criação da Agência Nacional de Saúde Suplementar - ANS, no período de 2003 a 2008. Para tanto, foi utilizada a Análise Envoltória de Dados nos dados da saúde suplementar para geração das medidas de eficiência das operadoras, agrupadas em cinco modalidades: autogestão, cooperativa médica, filantropia, medicina de grupo e seguradora. Foram propostas quatro diferentes abordagens que pretenderam visualizar as performances das operadoras nas questões econômico-financeiras e assistenciais, bem como a relação do setor de saúde suplementar com o Sistema Único de Saúde - SUS. Além disso, os dados foram desagregados em grandes regiões brasileiras proporcionando a observação das diferenças regionais. Observou-se que existem grandes diferenças entre modalidades e também entre regiões. A comparação entre as medidas de eficiência nas quatro diferentes abordagens e a evolução, ano a ano, dos normativos da ANS, ainda necessita de uma análise mais detalhada, mas indica, ainda que primariamente, que as modalidades tiveram diferentes reações à regulação.

Esta dissertação apresenta estudo sobre o soerguimento de atividades em crise através da constituição de sociedades cooperativas por ex-empregados à luz dos princípios da Economia Social, dentro da área de concentração Pensamento Jurídico e Relações Sociais, na linha de pesquisa Empresa e Atividades Econômicas. Diante do crescente desemprego e precarização dos postos de trabalho, formas organizacionais autogestionárias, notadamente através de cooperativas, tem sido uma alternativa viável em muitos casos. A fim de demonstrar a viabilidade e utilidade de tais iniciativas, esta dissertação traçará uma evolução histórica das cooperativas e fará uma análise de sua estrutura legal (Lei n. 5.764/71) a fim de demonstrar que este tipo societário é a forma jurídica que mais se adéqua à proposta da Economia Social. A fim de atingir tal intento, a empregar-se-á o método indutivo, bem como revisão bibliográfica e análise de leis e projetos de lei na seara do Direito Societário e Direito do Trabalho, por se tratar de proposta interdisciplinar. Além disso, haverá pesquisa qualitativa e quantitativa de informações oriundas de bases de dados governamentais e de pesquisas do outros ramos do conhecimento. Ao enfrentar questões delicadas atinentes ao tema proposto buscar-se-á oferecer soluções a fim de contribuir para a maximização de tais experiências.

Psychological factors play a major role in exacerbating chronic pain. Effective self-management of pain is often hindered by inaccurate beliefs about the nature of pain which lead to a high degree of emotional reactivity. Probabilistic models of perception state that greater confidence (certainty) in beliefs increases their influence on perception and behavior. In this study, we treat confidence as a metacognitive process dissociable from the content of belief. We hypothesized that confidence is associated with anticipatory activation of areas of the pain matrix involved with top-down modulation of pain. Healthy volunteers rated their beliefs about the emotional distress that experimental pain would cause, and separately rated their level of confidence in this belief. Confidence predicted the influence of anticipation cues on experienced pain. We measured brain activity during anticipation of pain using high-density EEG and used electromagnetic tomography to determine neural substrates of this effect. Confidence correlated with activity in right anterior insula, posterior midcingulate and inferior parietal cortices during the anticipation of pain. Activity in the right anterior insula predicted a greater influence of anticipation cues on pain perception, whereas activity in right inferior parietal cortex predicted a decreased influence of anticipatory cues. The results support probabilistic models of pain perception and suggest that confidence in beliefs is an important determinant of expectancy effects on pain perception.

随着Internet的飞速发展,符合J2EE规范的Web容器已经成为部署企业Web应用的主流平台.同时,企业Web应用的多样化和复杂性的增长,迫切地要求Web容器提供差分服务的支持.然而,传统的Web容器采用尽力而为的服务模型,无法提供差分服务支持.已有的研究采用准入控制、优先级调度等方法来为Web容器提供差分服务支持,但是它们只能提供单调、静态的差分服务策略.提出了一个基于自管理单元的Web容器DSWC,它能够根据SLA定义提供两级差分服务支持.此外,提出了一个自适应差分服务策略选择算法,它能够根据动态变化的运行环境自适应地选择差分服务策略.原型系统的实验结果显示,DSWC能够有效地为请求提供符合SLA定义的细粒度的差分服务支持.

Distributed applications are being deployed on ever-increasing scale and with ever-increasing functionality. Due to the accompanying increase in behavioural complexity, self-management abilities, such as self-healing, have become core requirements. A key challenge is the smooth embedding of such functionality into our systems. Natural distributed systems such as ant colonies have evolved highly efficient behaviour. These emergent systems achieve high scalability through the use of low complexity communication strategies and are highly robust through large-scale replication of simple, anonymous entities. Ways to engineer this fundamentally non-deterministic behaviour for use in distributed applications are being explored. An emergent, dynamic, cluster management scheme, which forms part of a hierarchical resource management architecture, is presented. Natural biological systems, which embed self-healing behaviour at several levels, have influenced the architecture. The resulting system is a simple, lightweight and highly robust platform on which cluster-based autonomic applications can be deployed.
