The aim of this study was to evaluate the epidemiological, clinical and laboratorial profile of patients with Baggio-Yoshinari Syndrome (BYS), who underwent internment at the Instituto de Infectologia Emilio Ribas in São Paulo, Brazil, during the period from July 1990 to July 2006. BYS is a new Brazilian tick-borne disease caused by Borrelia burgdorferi sensu lato microorganisms that resembles features of Lyme disease (LD), except for its epidemiological, clinical and laboratorial particularities. From 60 patients' records with positive serology to B. burgdorferi done by ELISA and Western-blotting methods, 19 cases were diagnosed as having BYS, according to criteria adopted at LIM-17 HCFMUSP, the Brazilian Reference Laboratory for the research of BYS. The other 41 remaining patients displayed miscellaneous infections or auto-immune processes. The beginning of symptoms in BYS group varied from one day to six years, from the onset of the disease. Four of 19 patients were included in acute disease stage, and 15 in latent. General unspecific symptoms were identified in almost all cases, with high frequencies of fever (78.9%) and lymphadenomegaly (36.8%). Six patients had skin lesions (31.5%); six arthralgia or arthritis (31.5%) and eight neurological symptoms (42%). Interestingly, two patients showed antibodies directed to B. burgdorferi exclusively in cerebrospinal fluid. Since BYS is a new emergent Brazilian zoonosis and its diagnosis is sometimes complex, all the new knowledge about BYS must be scattered to Brazilian Medical specialists, aiming to teach them how to diagnose this amazing tick-borne disease and to avoid its progression to chronic irreversible sequels

Relatório EPE - Relatório de Estágio em Educação Pré-Escolar: Para aprender a ensinar é necessário passar por diversas etapas, assim sendo, o presente relatório é o culminar de um primeiro momento de estágio, do primeiro contacto com a profissão com todas as dinâmicas e especificidades de que dela fazem parte. Pretendendo-se ser um educador reflexivo e investigador, em que o desejo de saber o porquê daquilo que observa, o leva a descobrir novas coisas e a refletir sempre sobre elas, é fulcral que se conheçam alguns pressupostos teóricos que sirvam de suporte à prática a realizar. Desta forma, é feita uma contextualização teórica em que se aborda o papel do Educador de Infância, agente tão importante na educação das crianças, uma vez que educa os cidadãos de amanhã. Ao mesmo tempo, são ainda abordados alguns tópicos pertinentes relativamente à educação de infância, como a organização do ambiente educativo, a intencionalidade educativa, assim como modelos e perspetivas teóricas que fundamentam toda a prática desenvolvida. Com a realização deste relatório, inicia-se a construção do profissional reflexivo e investigador. Neste sentido, reflete-se acerca de toda ação pedagógica desenvolvida, assim como dos aspetos que se devem melhorar no futuro e que objetivos anteriormente definidos foram já alcançados. Na certeza de que o período de prática pedagógica referenciada foi o primeiro de muitos momentos de autodescoberta e realização pessoal, espera-se que o mesmo seja capaz de transparecer toda a experiência e enriquecimento para quem o lê. Relatório EPE - Relatório de Estágio em Educação Pré-Escolar:In order to learn how to teach, one needs to follow several stages, and on that path, the present report is the final step of a first stage in the trainee program, one that marked the first contact with the teaching profession and with all its dynamics and specifics. The aim is to be a reflexive educator and researcher, one that desires to understand the reason and motive of its object, one that is led to the discovery of new things and to a reflection on them. On that line, it is important to know some theoretical concepts to support the practice. With that goal in mind, this paper is a theoretical contextualization relating to the role of the infant teacher, a very important one, as it concerns the education of future citizens. At the same time, some relevant topics are addressed in what concerns the work of the infant teacher, such as the arrangement of the educational environment, the educational intentionality, and also theoretical models and perspectives, fundamental to the developed work. With this report begins the construction of the reflexive professional and researcher. In doing so, it is a reflection on several issues, namely, on all the pedagogical actions developed throughout the stage, on the features to improve in the future, and what previously defined goals were already achieved. Without a doubt, this stage of educational action is the first of several moments of self-discovery and personal achievement. Hopefully, this report will be able to reflect all the experience and to be an enriching experience to the reader.

No presente relatório de estágio a formanda reflete criticamente o seu percurso de formação respeitante às unidades curriculares de Prática Pedagógica Supervisionada na Educação Pré-Escolar e Prática Pedagógica Supervisionada no 1.º Ciclo do Ensino Básico, sendo parte integrante do Mestrado em Educação Pré-Escolar e Ensino do 1.º Ciclo do Ensino Básico. Assim, relevam-se os processos formativos em questão e evidencia-se a metodologia de investigação-ação como suporte metodológico da construção de saberes inerentes à profissionalidade docente, sendo que a prática pedagógica constituiu, neste âmbito, um momento ímpar na formação e transformação de representações do que é ser professor, na medida em que permitiu a articulação entre a teoria e a prática, em contexto real, num período de tempo suficiente para a formanda se tornar numa profissional de educação. Assim, surge como principal objetivo da prática pedagógica o desenvolvimento de competências profissionais sustentadas nos quatro pilares da educação: saber, saber-fazer, saber- ser e saber-estar para saber ensinar a aprender. Este percurso foi realizado em díade de formação, com o apoio da educadora/professora cooperantes e supervisores institucionais. Deste modo, este mestrado contribuiu fortemente para a construção de um perfil profissional que se integra no desenho de uma identidade profissional docente.

O presente Relatório de Estágio, desenvolvido na Unidade Curricular Prática Pedagógica Supervisionada (PPS), inserida no Mestrado em Educação Pré- Escolar e Ensino do 1.º Ciclo do Ensino Básico, pretende expor, de forma crítica e reflexiva, todo o percurso formativo da mestranda. Assim, realça que, o trabalho desenvolvido, em ambos os contextos, promoveu a aquisição de aprendizagens e de competências que se espelharam ao nível pessoal, profissional e social. Releva o processo formativo sustentado no construtivismo e socio construtivismo e, ainda, na metodologia de investigação ação (I-A), sendo que esta última orientou a prática educativa com o objetivo de a transformar melhorando-a. Neste sentido, a formanda passou por etapas estruturais - observação, planificação, ação, avaliação e reflexão - que contribuíram para a construção do saber, saber-fazer, saber-ser e saber-estar, pilares fundamentais da educação, nomeadamente para quem quer aprender a ensinar a aprender. Paralelamente, toda a PPS foi desenvolvida tendo em conta o trabalho colaborativo, em que a construção de saberes foi proporcionada por vários intervenientes, como o par pedagógico, os professores supervisores, os professores cooperantes e as crianças. ABSTRACT - It is the purpose of the present thesis to emphasize, through a series of examples, the need and value of appropriate pre-analysis of the impact of health care regulation. Specifically, the thesis presents three papers on the theme of regulation in different aspects of health care provision and financing. The first two consist of economic analyses of the impact of health care regulation and the third comprises the creation of an instrument for supporting economic analysis of health care regulation, namely in the field of evaluation of health care programs. The first paper develops a model of health plan competition and pricing in order to understand the dynamics of health plan entry and exit in the presence of switching costs and alternative health premium payment systems. We build an explicit model of death spirals, in which profitmaximizing competing health plans find it optimal to adopt a pattern of increasing relative prices culminating in health plan exit. We find the steady-state numerical solution for the price sequence and the plan’s optimal length of life through simulation and do some comparative statics. This allows us to show that using risk adjusted premiums and imposing price floors are effective at reducing death spirals and switching costs, while having employees pay a fixed share of the premium enhances death spirals and increases switching costs. Price regulation of pharmaceuticals is one of the cost control measures adopted by the Portuguese government, as in many European countries. When such regulation decreases the products’ real price over time, it may create an incentive for product turnover. Using panel data for the period of 1997 through 2003 on drug packages sold in Portuguese pharmacies, the second paper addresses the question of whether price control policies create an incentive for product withdrawal. Our work builds the product survival literature by accounting for unobservable product characteristics and heterogeneity among consumers when constructing quality, price control and competition indexes. These indexes are then used as covariates in a Cox proportional hazard model. We find that, indeed, price control measures increase the probability of exit, and that such effect is not verified in OTC market where no such price regulation measures exist. We also find quality to have a significant positive impact on product survival. In the third paper, we develop a microsimulation discrete events model (MSDEM) for costeffectiveness analysis of Human Immunodeficiency Virus treatment, simulating individual paths from antiretroviral therapy (ART) initiation to death. Four driving forces determine the course of events: CD4+ cell count, viral load resistance and adherence. A novel feature of the model with respect to the previous MSDEMs is that distributions of time to event depend on individuals’ characteristics and past history. Time to event was modeled using parametric survival analysis. Events modeled include: viral suppression, regimen switch due virological failure, regimen switch due to other reasons, resistance development, hospitalization, AIDS events, and death. Disease progression is structured according to therapy lines and the model is parameterized with cohort Portuguese observational data. An application of the model is presented comparing the cost-effectiveness ART initiation with two nucleoside analogue reverse transcriptase inhibitors (NRTI) plus one non-nucleoside reverse transcriptase inhibitor(NNRTI) to two NRTI plus boosted protease inhibitor (PI/r) in HIV- 1 infected individuals. We find 2NRTI+NNRTI to be a dominant strategy. Results predicted by the model reproduce those of the data used for parameterization and are in line with those published in the literature.

Cross-cultural studies have much to teach clinicians and researchers alike about psychopathology in general and about social anxiety disorder (SAD) in particular. Unfortunately, little is known about the degree and the mechanisms through which cultural environment may influence clinical manifestations of SAD. OBJECTIVE: Our objective was to identify culture-related clinical patterns in SAD and related disorders. METHODS: We described socio-demographic and clinical characteristics of a sample of 62 adult outpatients with SAD seen at a university clinic for anxiety and depressive disorders in Rio de Janeiro, Brazil, and compared them with those reported in clinical samples from North America, Europe, Asia and Oceania identified through a systematic review in Medline, PsychINFO, and LILACS. RESULTS: Our comparison of trans-cultural features of SAD lends partial support to Heimberg's (1997) contention that the majority of socio-demographic features and symptoms of this disorder are relatively independent of geographic and cultural differences. CONCLUSION: Patients with SAD were almost universally characterized by: 1) a predominance of males in clinical samples; 2) early onset of the disorder; 3) high educational attainment; and 4) great frequency of comorbidities.

This is a project to develop a document for teaching graduate econometrics that is "open source", specifically, licensed as GNU GPL. That is, anyone can access the document in editable form, and can modify it, as long as they make their modifications available. This allows for personalization, as well as a simple way to make contributions and error corrections. The hope is that people preparing to teach econometrics for the first time might find it useful, and eventually be motivated to contribute back to the project. The central document is something between a set of lecture notes and a text book. It's not as terse as lecture notes, but not as complete or well-referenced as a text book. Of course, the document is constantly evolving, and you are welcome to modify it as you like. The document contains (at least when viewed in HTML or PDF form) hyperlinks to example programs written using the GNU/Octave language. The document itself is written using the LyX word processor. LyX documents can be exported as LaTeX, so the system is quite portable.

En primer lloc els objectius que s’estableixen en aquest estudi són: Valorar la importància de la melodia, dels accents i la musicalitat del text en les cançons traduïdes analitzades. Plantejar la necessitat de la traducció de cançons infantils. Analitzar les traduccions existents de l’anglès al català del repertori de cançons infantils que es canten a les corals infantils. Relacionar poesia infantil i cançó. Visualitzar l’ús de la cançó infantil per a l’ensenyament dels infants, tant pel que fa a la transmissió de valors com per a l’ensenyament de llengües. La metodologia utilitzada fa referència a dos aspectes. El mètode qualitatiu amb la lectura d’articles i la bibliografia seleccionada i el mètode quantitatiu amb l’anàlisi de tres entrevistes i vint-i-una enquestes a responsables de corals infantils. En aquest estudi ofereixo l’estat de la qüestió sobre la traducció de cançons, amb especial atenció a la traducció de cançons per ser cantades. Després s’aborda la traducció de cançons infantils per ser cantades, per acabar especificant el tema d’aquest treball d’investigació en concret: la traducció de cançons infantils per ser cantades, amb l’estudi de traduccions de l’anglès al català de cançons cantades per corals infantils de Catalunya, i se n’ubica el context: el moviment coral infantil a Catalunya. Per a la confecció d’aquest estudi s’ha fet un recull de vint-i-set cançons traduïdes (anglès-català) que canten les corals infantils de Catalunya aplegades en el Secretariat de Corals Infantils de Catalunya (SCIC). Se n’ha seleccionat les cançons traduïdes de l’anglès al català, incloent-hi la partitura, la lletra original i la traducció. Aquest recull permet establir un lligam entre la traducció, la música i la pedagogia musical a Catalunya, els tres eixos principals d’aquest treball. D’aquestes cançons se n’han analitzat tres seguint els criteris establerts per autors com Peter Low, per la seva àmplia experiència en la traducció de música vocal. Els criteris són referents al ritme, rima, cantabilitat, naturalitat i sentit. Per tal de fer aquest estudi es van portar a terme tres entrevistes a traductors de cançons infantils: Enriqueta Anglada, Maria Martorell i Josep Vigo. Per valorar l’ús de cançons traduïdes dins del moviment coral infantil en l’actualitat i de manera més global, el treball inclou una enquesta, adreçada a directors de cors infantils, resposta per 21 cors. Aquestes enquestes palesen els canvis produïts amb el temps respecte a la necessitat de traduir cançons, ja que actualment la traducció no és tant important tot i que es continua valorant la seva importància, sobretot per afavorir la comprensió del que es canta. Però alhora es referma el desig de cantar en l’idioma original (especialment en el cas de l’anglès), quan aquest idioma l’entén qui dirigeix o interpreta la cançó, quan aquest fet es veu com a repte, o quan es prioritza la relació original entre música i text.

This paper presents an analysis of motor vehicle insurance claims relating to vehicle damage and to associated medical expenses. We use univariate severity distributions estimated with parametric and non-parametric methods. The methods are implemented using the statistical package R. Parametric analysis is limited to estimation of normal and lognormal distributions for each of the two claim types. The nonparametric analysis presented involves kernel density estimation. We illustrate the benefits of applying transformations to data prior to employing kernel based methods. We use a log-transformation and an optimal transformation amongst a class of transformations that produces symmetry in the data. The central aim of this paper is to provide educators with material that can be used in the classroom to teach statistical estimation methods, goodness of fit analysis and importantly statistical computing in the context of insurance and risk management. To this end, we have included in the Appendix of this paper all the R code that has been used in the analysis so that readers, both students and educators, can fully explore the techniques described
