OBJECTIVE: Mounting evidence suggests that the limbic system is pathologically involved in cases of psychiatric comorbidities in temporal lobe epilepsy (TLE) patients. Our objective was to develop a conceptual framework describing how neuropathological and connectivity changes might contribute to the development of psychosis and to the potential neurobiological mechanisms that cause schizophrenia-like psychosis in TLE patients. METHODS: In this review, clinical and neuropathological findings, especially brain circuitry of the limbic system, were examined together to enhance our understanding of the association between TLE and psychosis. Finally, the importance of animal models in epilepsy and psychiatric disorders was discussed. CONCLUSIONS: TLE and psychiatric symptoms coexist more frequently than chance would predict. Damage and deregulation among critical anatomical regions, such as the hippocampus, amygdala, thalamus, and the temporal, frontal and cingulate cortices, might predispose TLE brains to psychosis. Studies of the effects of kindling and injection of neuroactive substances on behavior and electrophysiological patterns may offer a model of how limbic seizures in humans increase the vulnerability of TLE patients to psychiatric symptoms.

For the last 40 years, schizophrenia has been considered to be the result primarily of a dysfunction in brain dopaminergic pathways. In this review, it is described and discussed findings concerning nitric oxide-mediated neurotransmission in schizophrenia. Studies were searched in PubMed, SciELO, and LILACS using the terms schizophrenia and nitric oxide plasma levels or nitric oxide serum levels, with no time limit. The reference lists of selected articles were also hand-searched for additional articles. From 15 potential reports, 10 were eligible to be included in the review and meta-analysis. These studies included a total of 505 patients with schizophrenia and 339 healthy volunteers. No significant difference was found between patients and healthy controls regarding total nitrite plasma/serum levels (effect size g = 0.285, 95%CI = -0.205 to 0.774, p = 0.254). However, when studies with patients under antipsychotic treatment were examined separately, there was a significant difference between patients and healthy volunteers (effect size g = 0.663, 95%CI = 0.365 to 0.961, p < 0.001), showing that patients under treatment have higher levels of plasma/serum nitric oxide than controls. These results suggest that antipsychotics increase nitric oxide plasma/serum levels and that the nitrergic pathway would be a fertile target for the development of new treatments for patients with schizophrenia.

OBJECTIVE: To investigate the influence of brain-derived neurotrophic factor (BDNF) gene variations on cognitive performance and clinical symptomatology in first-episode psychosis (FEP). METHODS: We performed BDNF val66met variant genotyping, cognitive testing (verbal fluency and digit spans) and assessments of symptom severity (as assessed with the PANSS) in a population-based sample of FEP patients (77 with schizophreniform psychosis and 53 with affective psychoses) and 191 neighboring healthy controls. RESULTS: There was no difference in the proportion of Met allele carriers between FEP patients and controls, and no significant influence of BDNF genotype on cognitive test scores in either of the psychosis groups. A decreased severity of negative symptoms was found in FEP subjects that carried a Met allele, and this finding reached significance for the subgroup with affective psychoses (p < 0.01, ANOVA). CONCLUSIONS: These results suggest that, in FEP, the BDNF gene Val66Met polymorphism does not exert a pervasive influence on cognitive functioning but may modulate the severity of negative symptoms.

A menudo los científicos secuencian el ADN de un gran número de personas con el objetivo de determinar qué genes se asocian con determinadas enfermedades. Esto permite meóon del genoma humano. El precio de un perfil genómico completo se ha posicionado por debajo de los 200 dólares y este servicio lo ofrecen muchas compañías, la mayor parte localizadas en EEUU. Como consecuencia, en unos pocos a~nos la mayoría de las personas procedentes de los países desarrollados tendrán los medios para tener su ADN secuenciado. Alrededor del 0.5% del ADN de cada persona (que corresponde a varios millones de nucleótidos) es diferente del genoma de referencia debido a variaciones genéticas. Así que el genoma contiene información altamente sensible y personal y representa la identidad biológica óon sobre el entorno o estilo de vida de uno (a menudo facilmente obtenible de las redes sociales), sería posible inferir el fenotipo del individuo. Multiples GWAS (Genome Wide Association Studies) realizados en los últimos a~nos muestran que la susceptibilidad de un paciente a tener una enfermedad en particular, como el Alzheimer, cáncer o esquizofrenia, puede ser predicha parcialmente a partir de conjuntos de sus SNP (Single Nucleotide Polimorphism). Estos resultados pueden ser usados para medicina genómica personalizada (facilitando los tratamientos preventivos y diagnósticos), tests de paternidad genéticos y tests de compatibilidad genética para averiguar a qué enfermedades pueden ser susceptibles los descendientes. Estos son algunos de los beneficios que podemos obtener usando la información genética, pero si esta información no es protegida puede ser usada para investigaciones criminales y por compañías aseguradoras. Este hecho podría llevar a discriminaci ón genética. Por lo que podemos concluir que la privacidad genómica es fundamental por el hecho de que contiene información sobre nuestra herencia étnica, nuestra predisposición a múltiples condiciones físicas y mentales, al igual que otras características fenotópicas, ancestros, hermanos y progenitores, pues los genomas de cualquier par de individuos relacionados son idénticos al 99.9%, contrastando con el 99.5% de dos personas aleatorias. La legislación actual no proporciona suficiente información técnica sobre como almacenar y procesar de forma segura los genomas digitalizados, por lo tanto, es necesaria una legislación mas restrictiva ---ABSTRACT---Scientists typically sequence DNA from large numbers of people in order to determine genes associated with particular diseases. This allows to improve the modern healthcare and to provide a better understanding of the human genome. The price of a complete genome profile has plummeted below $200 and this service is ofered by a number of companies, most of them located in the USA. Therefore, in a few years, most individuals in developed countries will have the means of having their genomes sequenced. Around 0.5% of each person's DNA (which corresponds to several millions of nucleotides) is diferent from the reference genome, owing to genetic variations. Thus, the genome contains highly personal and sensitive information, and it represents our ultimate biological identity. By combining genomic data with information about one's environment or lifestyle (often easily obtainable from social networks), could make it possible to infer the individual's phenotype. Multiple Genome Wide Association Studies (GWAS) performed in recent years have shown that a patient's susceptibility to particular diseases, such as Alzheimer's, cancer, or schizophrenia, can be partially predicted from sets of his SNPs. This results can be used for personalized genomic medicine (facilitating preventive treatment and diagnosis), genetic paternity tests, ancestry and genealogical testing, and genetic compatibility tests in order to have knowledge about which deseases would the descendant be susceptible to. These are some of the betefts we can obtain using genoma information, but if this information is not protected it can be used for criminal investigations and insurance purposes. Such issues could lead to genetic discrimination. So we can conclude that genomic privacy is fundamental due to the fact that genome contains information about our ethnic heritage, predisposition to numerous physical and mental health conditions, as well as other phenotypic traits, and ancestors, siblings, and progeny, since genomes of any two closely related individuals are 99.9% identical, in contrast with 99.5%, for two random people. The current legislation does not ofer suficient technical information about safe and secure ways of storing and processing digitized genomes, therefore, there is need for more restrictive legislation.

Reconhecendo, a partir da constatação empírica, a multiplicidade de escolhas de crenças no Mundo e em particular na periferia urbana paulistana, reconhecemos, também, a emergência criativa de novas possibilidades de crer e não crer. Tal amplitude não apenas aponta para o crer (segundo as ofertas de um sem número de religiões) e o não crer (ateu e agnóstico), mas para uma escolha que poderia vir a ser silenciada e esquecida, neste binômio arcaico e obsoleto, quando alguém se dá à liberdade crer sem ter religião. Reconhecer interessadamente os sem-religião nas periferias urbanas paulistanas é dar-se conta das violências a que estes indivíduos estão submetidos: violência econômica, violência da cidadania (vulnerabilidade) e proveniente da armas (grupos x Estado). Tanto quanto a violência do esquecimento e silenciamento. A concomitância espaço-temporal dos sem-religião nas periferias, levou-nos buscar referências em teorias de secularização e de laicidade, e, a partir destas, traçar uma história do poder violento, cuja pretensão é a inelutabilidade, enquanto suas fissuras são abertas em espaços de resistências. A história da legitimação do poder que se quer único, soberano, de caráter universal, enquanto fragmenta a sociedade em indivíduos atomizados, fragilizando vínculos horizontais, e a dos surgimentos de resistências não violentas questionadoras da totalidade trágica, ao reconhecer a liberdade de ser com autonomia, enquanto se volta para a produção de partilha de bens comuns. Propomos reconhecer a igual liberdade de ser (expressa na crença da filiação divina) e de partilhar o bem comum em reconhecimentos mútuos (expressa pela ação social), uma expressão de resistência não violenta ao poder que requer a igual abdicação da liberdade pela via da fragmentação individualizante e submissão inquestionável à ordem totalizante. Os sem-religião nas periferias urbanas, nossos contemporâneos, partilhariam uma tal resistência, ao longo da história, com as melissas gregas, os profetas messiânicos hebreus, os hereges cristãos e os ateus modernos, cuja pretensão não é o poder, mas a partilha igual da liberdade e dos bens comuns. Estes laicos, de fato, seriam agentes de resistências de reconhecimento mútuos, em espaços de multiplicidade crescente, ao poder violento real na história.

Las alteraciones en el sistema inmune innato y en la respuesta inflamatoria, ya sean causadas por factores endógenos o exógenos, han sido implicadas como un componente fundamental en la fisiopatología de las enfermedades psiquiátricas. Esto es de gran relevancia en la esquizofrenia, una enfermedad incurable de gran impacto socioeconómico que afecta al 1% de la población mundial, caracterizada por alteraciones en la neurotransmisión, cuyos tratamientos antipsicóticos sólo engloban parte de la sintomatología de la enfermedad, tienen importantes efectos adversos, y una gran proporción de los pacientes muestran resistencia al tratamiento y baja adherencia. Uno de los principales factores de riesgo ambiental de esta enfermedad es la exposición al estrés. El estrés es capaz de activar indirectamente, mediante el daño celular o la permisividad ante patógenos, uno de las principales agentes de la inmunidad innata, los receptores tipo Toll (TLR). TLR3 actúa principalmente como centinela frente a infecciones virales al reconocer las dobles hebras de RNA viral, en cambio, TLR4 detecta principalmente el LPS, un componente estructural de las paredes de bacterias Gram negativas. Además, ambos TLRs son capaces de detectar patrones moleculares asociados a daño (DAMPs). Los principales coordinadores de la respuesta celular inmune son las citoquinas, las cuales inducen una respuesta Th1, Th2 o Th reguladora, también son las encargadas de activar la microglía en reposo, y una vez más son las que orquestan la polarización M1 o M2 según las citoquinas presentes en el ambiente. Por otro lado, el daño celular y la acumulación de mediadores proinflamatorios oxidativo/nitrosativos activan vías antioxidantes como la del factor NRF2, que a su vez también incide en la regulación de la activación microglial. Otro factor que interviene en el proceso, y que se relaciona directamente con la enfermedad es el sistema endocannabinoide...

Objective: Evaluate the work structure and process in Psychos ocial Care Centers (CAPS) and the professionals profile, the satisfaction, conditions and work overload. Methods: Cross - sectional study conducted in five CAPS in Campina Grande city. The study sample consisted of five coordinators, 42 graduate professional s, 26 mid - level (technical and auxiliary nurses, and caregivers), and the medical records pertaining to 413 users followed up. Data were collected using validated questionnaires (CAPSUL - rating CAPS in southern Brazil) and adapted to the study, between July and October 2014. The questionnaires were double entered and submitted to validation in the sub - program “Validate Epi Info 3.5.4” , used along with the “SPSS 17.0” for processing the statistical analyzes. Measures of central tendency and dispersion were ap plied to the descriptive analyzes; “Fisher's” exact test to check the CAPS impact on hospital admissions and the “Bonferroni” adjusted to verify the diagnoses according to sex. 5% significance level was adopted. The study was approved by the Ethics Committ ee of the Rio Grande do Norte Federal University (UFRN), protocol 719.435, of 05.30.2014. Results: From the structure analysis were identified contextual factors that influenced the work process of CAPS professionals, such as: deficiencies with regard to h uman resources; forms of health professionals employment and qualifications; temporary contract existence. As to process dimension, it was found that the home visits performance by health professionals shows to be ineffective, given its insufficiency and i rregularity, which can be explained by the high demand, reduced staff and transportation lack. It was low coverage of items inherent to Therapeutic Individual Project, as the income generation program, insertion at work and home visit. The reference and co unter reference flow are still not satisfactorily organized. There was statistically significant difference for the diagnosis, with a predominance of mood disorders related to stress among women and those related to alcohol and other drugs among men (p <0. 05). There was an association between the degree of health professionals satisfaction and working conditions, overload and factors related to the content and working conditions, the security measures, comfort and CAPS appearance, contact between the teams and users, families treatment by the teams, temporary employment relationship. Conclusion: The data collected indicate the need for the CAPS organization through increased investments in the sector in order to enhance the infrastructure as potentiating el ement of practices with a view to changing the care model for mental health proposed by the Psychiatric Reform. It is hoped therefore that this research will contribute to better planning in CAPS unit management, with another tool to improve the dimensions involving the structure and the professional work process and improve this mental health care model.

In the central nervous system (CNS) of mammalian, fast synaptic transmission between nerve cells is performed primarily by α-amino-3-hydroxy-5-methyl-4- isoxazolepropionic acid (AMPA) receptors, an ionotropic glutamate receptor that is related with learning, memory and homeostasis of the nervous system. Impairments in their functions are correlated with development of many brain desorders, such as epilepsy, schizophrenia, autism, Parkinson and Alzheimer. The use of willardiine analogs has been shown a powerful tool to understanding of activation and desensitization mechanisms of this receptors, because the modification of a single ligand atom allows the observation of varying levels of efficacy. In this work, taking advantage of Fluorine Willardiine (1.35Å), Hydrogen Willardiine (1.65Å), Bromine Willardiine (1.8Å) and Iodine Willardiine (2.15Å) structures co-crystalized with GluA2 with codes 1MQI, 1MQJ, 1MQH and 1MQG, we attempted to energetically differentiate the four ligands efficacy. The complexes were submitted to energetic calculations based on density functional theory (DFT), under the optics of molecular fractionation with conjugate caps (MFCC) method. Obtained results show a relationship between the energetic values and willardiines efficacy order (FW> HW > BrW > IW), also show the importance of E705, R485, Y450, S654, T655, T480 e P478 as the amino acids that contribute most strongly with the interaction of four partial agonists. Furthermore, we outlined the M708 behaviour, attracted by FW and HW ligands, and repels by BrW and IW. With the datas reported on this work, it is possible for a better understanding of the AMPA receptor, which can serve as an aid in the development of new drugs for this system.
