Tutkimuksen tavoitteena on analysoida alhaisen tuloluokan segmentille kohdennettavaa mobiilituotetarjoomaa lähtien kohdesegmenttien määrittelystä aina suositeltavien tuoteominaisuuksien rajaamiseen. Taustatutkimuksen avulla selvitetään mobiilimarkkinoiden kehitykseen vaikuttavia tekijöitä asiakaspotentiaalin ja tulevaisuudennäkymien arvioimiseksi. Haastattelututkimuksen avulla on selvitetty kohdesegmentin mobiilipalvelujen tarvetta ja kyseisten markkinoiden tarjoamia mahdollisuuksia, jotta tähän tarpeeseen voidaan kannattavasti vastata. Mobiilipalveluiden saattamiseksi myös alhaisten tuloluokkien ulottuville, on loppuasiakkaalle mobiililiittymän hankkimisesta aiheutuva kokonaiskustannus saatava laskettua huomattavasti nykyistä alhaisemmalle tasolle. Tämä edellyttää, että operaattorin on voitava karsia omia kustannuksiaan jokaisella osa-alueella, ja kehitettävä uusia liiketoimintamalleja kannattavuuden säilyttämiseksi. Pohjimmiltaan tämä tarkoittaa sujuvaa yhteistyötä verkkojen ja mobiilipuhelinten valmistajien kanssa, huolellista markkinoiden segmentointia sekä tuotetarjooman kohdentamista.

Contexte : L’hypertension artérielle (HTA) est reconnue comme un important facteur de risque des maladies cardiovasculaires et de la mortalité prématurée. Les données montrent qu’un adulte sur 4 dans le monde souffrait d’hypertension en 2000 et ce chiffre serait en augmentation. Dans les pays africains, les estimations prévoient une progression plus rapide de la prévalence de l’HTA engendrant ainsi un problème additionnel à gérer pour le système de santé qui est toujours aux prises avec les maladies transmissibles. Les progrès économiques et l’urbanisation semblent entraîner des modifications des habitudes de vie dans ces pays qui seraient associés à une émergence des maladies non transmissibles, dont l’HTA. Le Burkina Faso, pays de l’Afrique de l’Ouest classé comme un pays à faibles revenus, aurait amorcé sa transition épidémiologique et il importe de faire un état sur les maladies non transmissibles en émergence. Afin de contribuer à la connaissance des aspects épidémiologiques de l’HTA au Burkina Faso, trois objectifs spécifiques ont été retenus pour la présente recherche : 1) Estimer la prévalence de l’HTA et identifier les facteurs associés dans la population rurale et la population semi-urbaine du Système de Surveillance démographique et de Santé de Kaya (Kaya HDSS) ; 2) Déterminer la différence de prévalence de l’HTA entre la zone lotie et la zone non lotie de la ville de Ouagadougou et identifier les facteurs associés ; 3) Déterminer la détection, le traitement, le contrôle de l’HTA et estimer la fréquentation des centres de santé par les personnes hypertendues nouvellement dépistées dans la population adulte du Centre Nord du Burkina Faso. Méthodologie : Le cadre de notre recherche est le Burkina Faso. Deux sites ont fait l’objet de nos investigations. Kaya HDSS, situé dans la région du Centre Nord du Burkina Faso a servi de site pour les enquêtes ayant permis l’atteinte du premier et du troisième objectif général. Une étude transversale a été menée en fin 2012 sur un échantillon aléatoire de 1645 adultes résidents du site. Un entretien suivi de mesures anthropométriques et de la pression artérielle (PA) ont été réalisés au domicile des participants. Toutes les personnes qui avaient une PA élevée (PA systolique ≥ 140 mm Hg et/ou PA diastolique ≥ 90 mm Hg) et qui n’avaient pas été diagnostiquées auparavant ont été référées à une formation sanitaire. Un second entretien a été réalisé avec ces personnes environ un mois après. Pour le second objectif général, c’est le système de surveillance démographique et de santé de Ouagadougou (Ouaga HDSS) qui a été retenu comme site. Ouaga HDSS couvre 5 quartiers de la zone nord de Ouagadougou. Une étude transversale a été réalisée en 2010 sur un échantillon aléatoire représentatif de la population adulte résidante du site (N = 2041). Des entretiens suivis de mesures anthropométriques et de la PA ont été réalisés durant l’enquête. Résultats : Notre premier article examine la prévalence de l’HTA et les facteurs associés en milieu rural et en milieu semi-urbain. Au total 1481 participants ont été dépistés et la prévalence totale pondérée était de 9,4 % (95 % IC : 7,3 % - 11,4 %) avec une grande différence entre le milieu semi-urbain et le milieu rural : un adulte sur 10 était hypertendu en milieu semi-urbain contre un adulte sur 20 en milieu rural. L’analyse multivariée nous a permis d’identifier l’âge avancé, le milieu semi-urbain et l’histoire familiale de l’HTA comme des facteurs de risque. Dans le deuxième article, nous avons déterminé la différence entre la zone lotie et la zone non lotie du milieu urbain en ce qui concerne l’HTA. Sur 2 041 adultes enquêtés, la prévalence totale est estimée à 18,6 % (95 % IC : 16,9 % - 20,3 %). Après ajustement avec l’âge, il n’y avait pas de différence de prévalence entre la zone lotie et la zone non lotie de la ville de Ouagadougou. Dans ce milieu urbain, l’obésité et l’inactivité physique sont confirmées comme des facteurs à risque. En plus des personnes âgées qui constituent un groupe à risque, les migrants venant du milieu rural et les veuves ont été identifiés comme des personnes à risque. Notre troisième article examine la détection, le traitement, le contrôle de l’HTA ainsi que la fréquentation des structures de santé par les personnes nouvellement dépistées hypertendues dans le milieu non urbain. Sur les 1481 participants à l’étude, 41 % n’avaient jamais mesuré leur PA. Sur les 123 participants (9,4 %) dépistés comme hypertendus, 26,8 % avaient déjà bénéficié d’un diagnostic et 75,8 % de ces derniers étaient sous traitement anti hypertensif. Parmi les participants sous traitement, 60 % (15 sur 25) avaient leur PA sous contrôle. Le suivi de 2 mois a permis de retrouver 72 des 90 personnes nouvellement dépistées. Seulement 50 % ont été en consultation et parmi ces derniers 76 % (28 personnes sur les 37) ont reçu une confirmation de leur hypertension. Conclusion : Notre recherche montre l’évolution de l’HTA en fonction du niveau d’urbanisation de la population. Même si les maladies infectieuses restent la priorité du système de santé, la lutte contre l’HTA doit s’inscrire dans un cadre général de lutte contre les maladies cardiovasculaires. Cette lutte doit être amorcée maintenant avant que des proportions inquiétantes ne soient atteintes. Des actions de prévention telles que la promotion de saines habitudes de vie, le dépistage de masse et un renforcement des capacités du système de soins sont à mettre en œuvre.

In Sri Lanka policy responses have direct impacts on rural dwellers. Over 80% of Sri Lanka’s population live in rural areas and 90% of them represent low income dwellers. Their production system may be hampered by fragmented landholding, poor economics of scale, low investment levels resulting from poor financial services as well as inappropriate or limited technology. They are vulnerable to price hikes of basic foods and food security issues due to fragmented landholding and poor financial services. Policy measures to reduce the transmission of higher international prices in domestic markets exist to protect the food security of the vulnerable population. This paper will discuss the food policy and strategies implemented by the government and outside to the above facts this paper also describes the effectiveness of the policies forwarded by the government. The objective of this study is to analyse the impact of policy responses to the food price crisis and rural food security in Sri Lanka. Outside of the above facts this study also treats the impact of policies and decisions on the nutritional condition of rural dwellers. Furthermore this study is to analyse the fluctuation of buying power with the price hikes and the relation of above facts with issues like malnutrition. This paper discusses why policy makers should pay greater attention to rural dwellers and describes the multiple pathways through which food price increases have on rural people. It also provides evidence of the impact of this crisis in particular, through hidden hunger, and discusses how current policy responses should adjust and improve to protect the rural dwellers in the short and long term.

This paper uses the data of 1338 rural households in the Northern Mountainous Region of Vietnam to examine the extent to which subsidised credit targets the poor and its impacts. Principal Component Analysis and Propensity Score Matching were used to evaluate the depth of outreach and the income impact of credit. To address the problem of model uncertainty, the approach of Bayesian Model Average applied to the probit model was used. Results showed that subsidised credit successfully targeted the poor households with 24.10% and 69.20% of clients falling into the poorest group and the three bottom groups respectively. Moreover, those who received subsidised credit make up 83% of ethnic minority households. These results indicate that governmental subsidies are necessary to reach the poor and low income households, who need capital but are normally bypassed by commercial banks. Analyses also showed that ethnicity and age of household heads, number of helpers, savings, as well as how affected households are by shocks were all factors that further explained the probability at which subsidised credit has been assessed. Furthermore, recipients obtained a 2.61% higher total income and a 5.93% higher farm income compared to non-recipients. However, these small magnitudes of effects are statistically insignificant at a 5% level. Although the subsidised credit is insufficient to significantly improve the income of the poor households, it possibly prevents these households of becoming even poorer.

Introducción: La desnutrición intrahospitalaria es variable en los estudios publicados. Tiene impacto en el curso tórpido de la enfermedad, en la prolongación de la estancia hospitalaria y en los altos costos subsecuentes. No existen estudios realizados en Colombia en relación al estado nutricional del paciente pediátrico hospitalizado. Por este motivo se decidió caracterizar el perfil nutricional de pacientes pediátricos hospitalizados en la Clínica Infantil Colsubsidio Materiales y métodos: Se desarrolló un estudio transversal en pacientes pediátricos hospitalizados en un periodo entre Junio 2014 – Marzo 2015. Se realizó descripción de las variables y análisis univariado. Resultados: Se analizaron 320 pacientes: 50% niños y 50% niñas. Edad promedio: 4.6 años, predominio del grupo de los lactantes. Media de estancia hospitalaria: 4,6 días. Baja ingesta, ayuno, dolor de moderada intensidad y factor de estrés grado I fueron las variables más frecuentemente relacionadas con desnutrición. Al ingreso a hospitalización el 57.5 % se encontraron eutróficos, el 17.19% con desnutrición crónica, el 8.44% con desnutrición aguda y el 16.88% con aumento anormal de peso. Al egreso, los eutróficos disminuyeron a 56.25 %, la desnutrición crónica y el aumento anormal de peso se mantuvieron igual y la desnutrición aguda aumento al 10%. Conclusión: Los hallazgos de este estudio están en concordancia con los hallazgos descritos en la literatura. El mayor compromiso nutricional se asoció a patologías respiratorias y gastrointestinales y a mayor estancia hospitalaria.

Recent research in Sub-Saharan Africa has revealed the importance of children’s caring roles in families affected by HIV and AIDS. However, few studies have explored young caregiving in the context of HIV in the UK, where recently arrived African migrant and refugee families are adversely affected by the global epidemic. This paper explores young people’s socio-spatial experiences of caring for a parent with HIV, based on qualitative research with 37 respondents in London and other urban areas in England. In-depth semi-structured interviews were conducted with young people with caring responsibilities and mothers with HIV, who were predominantly African migrants, as well as with service providers. Drawing on their perspectives, the paper discusses the ways that young people and mothers negotiate the boundaries of young people’s care work within and beyond homespace, according to norms of age, gender, generational relations and cultural constructions of childhood. Despite close attachments within the family, the emotional effects of living with a highly stigmatised life-limiting illness, pressures associated with insecure immigration status, transnational migration and low income undermined African mothers’ and young people’s sense of security and belonging to homespace. These factors also restricted their mobility and social participation in school/college and neighbourhood spaces. While young people and mothers valued supportive safe spaces within the community, the stigma surrounding HIV significantly affected their ability to seek support. The article identifies security, privacy, independence and social mobility as key dimensions of African young people’s and mothers’ imagined futures of ‘home’ and ‘family’.

Opportunistic land encroachment occurs in many low-income countries, gradually yet pervasively, until discrete areas of common land disappear. This paper, motivated by field observations in Karnataka, India, demonstrates that such an evolution of property rights from common to private may be efficient when the boundaries between common and private land are poorly defined, or ‘‘fuzzy.’’ Using a multi-period optimization model, and introducing the concept of stock and flow enforcement, I show how effectiveness of enforcement effort, whether encroachment is reversible, and punitive fines, influence whether an area of common land is fully defined and protected or gradually or rapidly encroached.

In-work benefits have been introduced in a number of Bismarckian welfare regimes in a context of austerity despite being targeted at politically weak constituents and representing a deviation from prevailing welfare institutions. This article addresses this puzzle by looking at the introduction in 2008 of an in-work benefit scheme in France, the Active Income of Solidarity. The analysis reveals that this reform was the result of a cross-cutting alliance between the conservative party and employers, as well as parts of the socialist party and the union movement. The alliance was possible thanks to actors’ multiple interpretations of the reform. The reform was difficult to oppose given its support by experts and public opinion and because it entailed an increase in revenues for low-income workers.

Countless cities are rapidly developing across the globe, pressing the need for clear urban planning and design recommendations geared towards sustainability. This article examines the intersections of Jane Jacobs’ four conditions for diversity with low-carbon and low-energy use urban systems in four cities around the world: Lyon (France), Chicago (United-States), Kolkata (India), and Singapore (Singapore). After reviewing Jacobs’ four conditions for diversity, we introduce the four cities and describe their historical development context. We then present a framework to study the cities along three dimensions: population and density, infrastructure development/use, and climate and landscape. These cities differ in many respects and their analysis is instructive for many other cities around the globe. Jacobs’ conditions are present in all of them, manifested in different ways and to varying degrees. Overall we find that the adoption of Jacobs' conditions seems to align well with concepts of low-carbon urban systems, with their focus on walkability, transit-oriented design, and more efficient land use (i.e., smaller unit sizes). Transportation sector emissions seems to demonstrate a stronger influence from the presence of Jacobs' conditions, while the link was less pronounced in the building sector. Kolkata, a low-income, developing world city, seems to possess many of Jacobs' conditions, while exhibiting low per capita emissions - maintaining both of these during its economic expansion will take careful consideration. Greenhouse gas mitigation, however, is inherently an in situ problem and the first task must therefore be to gain local knowledge of an area before developing strategies to lower its carbon footprint.

What explains cross-national variation in wage inequality? Research in comparative political economy stresses the importance of the welfare state and wage coordination in reducing not only disposable income inequality but also gross earnings inequality. However, the cross-national variation in gross earnings inequality between median and low income workers is at odds with this conventional wisdom: the German coordinated market economy is now more unequal in this type of inequality than the UK, a liberal market economy. To solve this puzzle, I argue that non-inclusive coordination benefits median but not bottom income workers and is as a result associated with higher – rather than lower - wage inequality. I find support for this argument using a large N quantitative analysis of wage inequality in a panel of Western European countries. Results are robust to the inclusion of numerous controls, country fixed effects, and also hold with a sample of OECD countries. Taken together these findings force us to reconsider the relationship between coordination and wage inequality at the bottom of the income distribution.

A mandioca (Manihot esculenta Crantz) é uma planta cultivada predominantemente para a subsistência dos trabalhadores rurais sendo uma das principais fontes de carboidratos disponíveis aos estratos sociais de baixa renda e tem importante participação na geração de emprego e renda. O Estado do Pará destaca-se pelo volume de produção de mandioca, mas o cultivo ainda está voltado essencialmente para a subsistência sendo pouco explorado como matéria prima em derivados industriais e insumo para enfrentar problemas globais como a fome, a degradação ambiental e a produção de energia, gerando emprego e renda para comunidades locais e rurais. Este estudo tem como objetivo avaliar como está se desenvolvendo a organização de produtores rurais de mandioca da Associação de Desenvolvimento Comunitário e Rural Bom Jesus, através do modelo de gestão de cadeia produtiva e compreender em que medida se justifica economicamente o cultivo de mandioca. A pesquisa é de natureza exploratória e descritiva e por meio do questionário e entrevista aplicado a 17 produtores rurais juntamente com a fundamentação teórica sobre o entendimento contemporâneo de cadeia produtiva foram percebidos evidências que justificam economicamente, em parte, o cultivo da mandioca. A geração de postos de trabalhos para as mulheres, a utilização de resíduos como incremento no arranjo produtivo, a apoio da extensão e do crédito rural, e as vantagens advindas da organização dos produtores rurais proporcionando renda e produção constantes no campo, foram os fatores considerados. Porém, a necessidade de inovações tecnológicas, parcerias com os setores industriais, arranjos produtivos mais rentáveis, mecanização do plantio e da colheita, os problemas ambientais (resíduos tóxicos e desmatamento) e de acesso à pesquisa, o nível de escolaridade, e a cooperação entre os agricultores, são desafios reais a serem enfrentados e o fator decisivo é o apoio governamental. Este estudo demonstra a realidade de uma sociedade quase sempre marginalizada e esquecida pelas políticas públicas. Além da variável econômica, a avaliação das culturas produtivas deve passar pelos filtros de análise social e ambiental, pois assim, nos permite com maior exatidão compreender os problemas reais da nossa sociedade, em especial da vida na Amazônia.

A margem dos programas habitacionais empreendidos por organismos estatais, por demais burocratizados e ínfimos se comparados à demanda que deveriam suprir, os loteamentos periféricos empreendidos por particulares sempre existiram nos arredores das grandes cidades.

Esta pesquisa objetivou compreender o comportamento de compra de vestuário de jovens de baixa renda, residentes em três bairros periféricos da cidade de São Paulo. A metodologia utilizada baseou-se em entrevistas em profundidade e amostra de 30 jovens mulheres, de 18 a 24 anos, com renda familiar mensal entre 3 e 7 salários mínimos. Além disso, realizou-se uma simulação de compra, com a pesquisadora acompanhando a visita a uma loja escolhida pelas jovens e verificando seu comportamento de compra. A perspectiva de análise foi a do interacionismo simbólico, que busca identificar os significados simbólicos, subjacentes às escolhas das marcas e dos produtos de consumo. Os resultados da pesquisa indicam uma riqueza de significados simbólicos associados às marcas de vestuário, que envolvem as decisões e escolhas de consumo das jovens. Também mostrou que além das marcas preferidas, as jovens escolhem as lojas pela localização, sortimento de produtos, preço e atendimento.

Brazilian public policy entered in the so-called new social federalism through its conditional cash transfers. States and municipalities can operate together through the nationwide platform of the Bolsa Familia Program (BFP), complementing federal actions with local innovations. The state and the city of Rio de Janeiro have created programs named, respectively, Renda Melhor (RM) and Família Carioca (FC). These programs make use of the operational structure of the BFP, which facilitates locating beneficiaries, issuing cards, synchronizing payment dates and access passwords and introducing new conditionalities. The payment system of the two programs complements the estimated permanent household income up to the poverty line established, giving more to those who have less. Similar income complementation system was subsequently adopted in the BFP and the Chilean Ingreso Ético Familiar, which also follow the principle of estimation of income used in the FC and in the RM. Instead of using the declared income, the value of the Rio cash transfers are set using the extensive collection of information obtained from the Single Registry of Social Programs (Cadastro Único): physical configuration of housing, access to public services, education and work conditions for all family members, presence of vulnerable groups, disabilities, pregnant or lactating women, children and benefits from other official transfers such as the BFP. With this multitude of assets and limitations, the permanent income of each individual is estimated. The basic benefit is defined by the poverty gap and priority is given to the poorest. These subnational programs use international benchmarks as a neutral ground between different government levels and mandates. Their poverty line is the highest of the first millennium goal of the United Nations (UN): US$ 2 per person per day adjusted for the cost of living. The other poverty line of the UN, US$ 1.25, was implicitly adopted as the national extreme poverty line in 2011. The exchange of methodologies between federal entities has happened both ways. The FC began with the 575,000 individuals living in the city of Rio de Janeiro who were on the payroll of the BFP. Its system of impact evaluation benefited from bi-monthly standardized examinations. In the educational conditionalities, the two programs reward students' progress, a potential advantage for those who most need to advance. The municipal program requires greater school attendance than that of the BFP and the presence of students’ parents at the bimonthly meetings held on Saturdays. Students must achieve a grade of 8 or improve at least 20% in each exam to receive a bi-monthly premium of R$50. In early childhood, priority is given to the poor children in the program Single Administrative Register (CadÚnico) to enroll in kindergarten, preschools and complementary activities. The state program reaches more than one million people with a payment system similar to the municipal one. Moreover, it innovates in that it transfers awards given to high school students to savings accounts. The prize increases and is paid to the student, who can withdraw up to 30% annually. The total can reach R$3,800 per low-income student. The State and the city rewarded already education professionals according to student performance, now completing the chain of demand incentives on poor students and their parents. Increased performance is higher among beneficiaries and the presence of their guardians at meetings is twice compared to non beneficiaries; The Houston program, also focuses on aligning the incentives to teachers, parents and students. In general, the plan is to explore strategic complementarities, where the whole is greater than the sum of its parts. The objective is to stimulate, through targets and incentives, synergies between social actors (teachers, parents, students), between areas (education, assistance, work) and different levels of government. The cited programs sum their efforts and divide labor so as to multiply interactions and make a difference in the lives of the poor.
