Enquadramento: A dor do parto resulta de complexas interações, de caráter inibitório e excitatório e, apesar de os seus mecanismos serem análogos aos da dor aguda, há fatores próprios do trabalho de parto de natureza neurofisiológica, obstétrica, psicológica e sociológica que intervêm no seu limiar. Neste sentido, os métodos não farmacológicas podem auxiliar a parturiente no alívio da dor, reduzir os níveis de stresse e ansiedade e, consequentemente promover uma maior satisfação. Objetivos: Demonstrar evidência científica de se os métodos não farmacológicos usados no alívio da dor no trabalho de parto são mais eficazes quando comparados com a não utilização de nenhum método, placebo ou qualquer outro método; verificar o uso de medidas não farmacológicas no alívio da dor do parto; averiguar quais as variáveis sociodemográficas, variáveis contextuais da gravidez e contextuais do parto que interferem no recurso a medidas não farmacológicas no alívio da dor do parto. Métodos: No estudo empírico I seguiu-se a metodologia de revisão sistemática da literatura. Foi realizada uma pesquisa na EBSCO, PubMed, Scielo e RCAAP, de estudos publicados entre 1 janeiro de 2010 e 2 de janeiro de 2015. Os estudos encontrados foram avaliados de acordo com os critérios de inclusão previamente estabelecidos e, posteriormente, foi feita uma apreciação da qualidade dos estudos, por dois revisores, utilizando a “Grelha para avaliação crítica de um artigo descrevendo um ensaio clínico prospetivo, aleatorizado e controlado” (Bugalho & Carneiro, 2004). No final, foram incluídos no corpus do estudo 4 artigos. O estudo empírico II enquadra-se num estudo quantitativo, transversal, descritivo e retrospetivo, desenvolvido no serviço de Obstetrícia do Centro Hospitalar Cova da Beira, segundo um processo de amostragem não probabilística por conveniência (n = 382). A recolha de dados efetuou-se através da consulta de processos clínicos de mulheres com idade ≥ 18 anos, que tiveram um parto vaginal com feto vivo após as 37 semanas de gestação. Resultados: Os métodos não farmacológicos usados no alívio da dor no trabalho de parto são mais eficazes quando comparados com a não utilização de nenhum método, placebo ou qualquer outro método. Numa amostra constituída por 382 mulheres, com uma idade média de 30,95 anos (±5,451 anos), em 34,6% dos casos foram aplicadas as medidas não farmacológicas no alívio da dor no trabalho de parto, sobressaindo a respiração e o relaxamento (86,3%). Em alguns casos, ainda que em menor número, foi aplicada a hidroterapia isolada (6,9%) ou associada à respiração e ao relaxamento (5,3%), bem como a hipnose (0,8%) e a associação entre respiração e massagem (0,8%). Conclusão: De acordo com os resultados obtidos e com base na evidência científica disponível, importa referir que é essencial que os cuidados não farmacológicos de alívio da dor no parto sejam explorados, por serem mais seguros e acarretarem menos intervenções. Desta feita, os profissionais de saúde que prestam assistência às mulheres durante o trabalho de parto e parto têm de ter acesso ao conhecimento acerca desses métodos e os seus potenciais benefícios, a fim de os poderem aplicar com maior segurança, o que resultará indiscutivelmente numa maior humanização do parto, caminhando-se no sentido de uma maternidade mais segura. Introdução: Os acidentes vasculares cerebrais (AVC) são causa de epilepsia e perturbações do neurodesenvolvimento. Têm tido uma incidencia crescente atribuível em parte à melhoria das tecnicas de imagens cerebrais. Alguns diagnósticos são efectuados retrospectivamente após o periodo neonatal. O objectivo da presente serie de casos clínicos foi identificar factores comuns que possam facilitar o diagnóstico no período neonatal. Material e métodos: Estudo retrospectivo efectuado com base na consulta dos processos clinicos dos recém-nascidos, com diagnóstico de AVC entre um de Janeiro de 2003 e 31 de Dezembro de 2013. Resultados: Foram identificados onze casos de AVC perinatal, num total 28382 nados-vivos. Dois casos foram diagnosticados no periodo fetal e nove no periodo neonatal. As convulsões foram a manifestação clinica mais frequente (8 em 11 casos). A mediana da idade de diagnóstico foi um dia e variou entre um e nove dias. A ecografia transfontanelar mostrou alterações em sete casos. A ressonancia magnetica nuclear cranio-encefalica mostrou alterações em todos os casos. Cinco AVC foram arteriais isquemicos, quatro hemorragicos e dois tromboses dos seios venosos. Em seis casos foram identificadas possiveis causas. Foram observadas complicações e sequelas em quatro dos casos. Discussão: As convulsões foram a manifestação clinica mais frequentemente encontrada.Em recém-nascidos de termo com convulsões sem historia de asfixia intraparto o AVC perinatal deverá ser diagnóstico de exclusão, mesmo na ausencia de alterações na ecografia transfontanelar.

The practices developed in the everyday life of obstetric services are sometimes out of step with the recommendations of the public health policies. Accordingly, this research had the objective of assessing the quality of the care provided to women and children during cases of natural childbirth in municipal public maternity wards of the city of Natal/RN, Brazilian Northeast. We developed a cross-sectional and quantitative study in two maternity wards that provide care actions to pregnant women at regular risk (maternity wards A and B). The participants were 314 puerperal women who were treated during the period between April and July 2014, whose children were born alive, through transpelvic way, with spontaneous or induced beginning of labor and that showed physical and emotional conditions to respond to the proposed questions. The data collection instrument was constructed on the basis of the recommendations of the World Health Organization focused on the care of normal childbirth and validated by skilled judges, and the final version has obtained optimum agreement (k = 0,96; IVC = 0,99). Associated with these recommendations, we used three indicators: percentage of women with induced labor or subjected to elective cesarean section (Indicator A); percentage of women served by a qualified health professional during labor and childbirth (Indicator B); and Bologna Index (Indicator C). The research obtained a favorable opinion of the Research Ethics Committee from the Federal University of Rio Grande do Norte, under the nº 562.313 and Certificate of Presentation for Ethics Appreciation: 25958513.0.0000.5537. The analysis of categories related to the recommendations of the World Health Organization was conducted by means of absolute and relative frequency and the Chi-square Pearson’s and Fisher’s exact tests made the comparison of the differences observed between the two maternity wards. Furthermore, we calculated the percentage of the indicators A and B and with the results of the Indicator C, the quality was assessed as follows: the closer to 5, the better will be the quality, and the closer to 0, the worst will be the quality, and the Mann-Whitney U test was used to compare the differences of the obtained averages. The significance level of 5% was considered in all statistical tests. The differences between the maternity wards were identified with regard to the provision of liquids orally (p=0,018), stimulus for non-supine positions (p=0,002), existence of partograph (p=0,001), support or welcoming by health professionals (p= 0,047), intravenous infusion (p<0,001), supine position (p<0,001), use of oxytocin (p<0,001), food and liquid restriction (p= 0,002) and, lastly, the fact of the touch is performed by more than 1 examiner (p=0,011). The indicators A and B showed percentages of 13,09% and 100%, respectively. The overall average of the Indicator C was equal to 2,07 (± 0,74). There was a statistically significant difference between the averages of the maternity wards (p<0,001). The care actions provided during the process of labor and childbirth is inappropriate, especially in the maternity ward B. It is necessary to implement improvements and redesign the obstetric model in force

Currently, the diagnostic ultrasound is inserted in various areas of medical action and carried out by many medical professionals, from which militate directly in the image area, such as radiologists and sonographers, but also by gynecologists, pediatricians, neurologists, general practitioners, endocrinologists, angiologists, orthopedists, rheumatologists, urologists, general and vascular surgeons. It is well known that the medical professional, for the exercise of its mission, requires a broad set of skills, competencies and attitudes developed and exercised during their training period. Living with medical students over nearly 20 years in hospital environment, I noticed gaps in the learning process by the students about what is diagnostic ultrasound and its applications, demonstrating failures as understanding the basic acoustic ultrasound, difficulties in identifying of anatomical structures in ultrasound images and inability in requests examinations and interpretations of images and reports. Based on these findings, it was developed in this Professional Masters a multimedia digital book that exposes what the ultrasound as a diagnostic modality imaging, dealing with its historiography and its physical/acoustic concepts, relating the process of formation of the ultrasound image, discussing about the features of sonographic equipments and their embedded technologies and highlighting its diagnostic applications , the latter presented through videos which will be described aspects of captured ultrasound images. This book will be available for access in digital format, serving as a teaching tool in medical education since the beginning of the course, so that can be used in conjunction with the discipline of Gross Anatomy, offered in the basic cycle of the Medicine Undergraduate Course of the Federal University of Rio Grande do Norte (UFRN).

OBJECTIVE: to identify a profile of the main causes of inappropriate referrals from primary care to specialized services, as strategy for the curriculum development of core competencies related to maternal health. METHODS: a cross-sectional study was performed using document analysis of all referrals of pregnant women from primary care to the high-risk pregnancy service, state of Rio Grande do Norte, Brazil. All pregnant women referred from June to December 2014 (n = 771) were included. According to their causes the referrals were categorized as adequate, inadequate or inconclusive. RESULTS: a total of 188 referrals were classified as inadequate (24.4%) and 93 inconclusive (12.1%) totalizing 36.5% of inappropriate referrals. The main causes identified in these inappropriate referrals were: low-risk pregnancy (12.8%), unconfirmed hypertension (12.1%), risk of abortion (8.9%), teenage pregnancy (7.1%) , toxoplasmosis (5.3%), Rh incompatibility (4.6%) and urinary tract infection (4.3%). These data contributed to the formulation of the following products: 1) a continuing education program for health professionals working in primary care, undergraduate students and residents; and 2) development of a virtual platform to support professionals who need to refer patients to high-risk pregnancy service. CONCLUSION: the results of this study are relevant in the current context of education of health professionals, with potential for positively impact not only in the development of skills related to maternal health in undergraduate and graduate education, as well as contributing for improvement of the health care of the population.

The research aimed to construct and validate a data collection instrument of pregnant couple as part of primary care. It was considered hypothesis that level of agreement from 70% among participants to validate the expert panel. The document has been based on the Theory of Human Needs by Horta and adjusted by Garcia and Cubas. It is a study of methodological type developed in four stages: identification of empirical indicators to pregnant women through an integrative literature review; evaluation of empirical indicators and their relation to human needs by focus group; structuring of the second version of instrument by categorization of indicators and appearance and content validation of the third version of instrument by judges, by use of Delphi technique. The collection of data was the first stage in months from August to October 2014 in the Journal of Midwifery and Women's Health and Scopus, PubMed, Lilacs, CINAHL, Cochrane databases. The remaining steps were carried out from November 2014 to February 2015. For the focus group was counted with participation of six experts through two meetings. As for the judges, it was obtained a population of 63 and final sample of 51 judges divided into 46 basic health units of Municipal City Health Natal/RN, Brazil. The study was approved by the Research Ethics Committee of Universidade Federal do Rio Grande do Norte, under Protocol number 876.200. For data analysis of the first stage it was used descriptive statistics and results are presented in tables and charts. At that stage were identified 162 empirical indicators and, when they were related to human needs, 64 by them were on psycobiological, 97 on psychosocial and one (1) on psychospiritual needs. Regarding the second and third stages, data were treated by process of categorizing and analyzing the Content Validity Index. The indicators obtained a 100% validation index. In appearance and content validation phase of instrument non-validated items were excluded and other items obtained index above 70%. Furthermore, it obtained 99% content validity index in the second version and 95.7% in the third version of the instrument as a whole and, therefore, validated tool. This instrument contains Health Institution, pregnant woman and her partner identification data, information on the human needs of the pregnant and items pertaining to systematize the collection of pregnant couple data during prenatal care. By the conclusion of the study, nurse shall have an instrument to collect the data of pregnant couple in primary care innovative by considering psychobiological, psychosocial and psychospiritual pregnant needs and insert health and sociodemographic data of pregnant partner in the context of pregnancy. Also, the document will serve as a tool for teaching and research in obstetrical nursing.

The post-menopause stage is characterized by hormonal and organic alterations of ovarian failure. One of the most important of these is muscles alterations of the pelvic floor (MPF). According to current literature, in young women gynecological and obstetric factors, as well as lifestyles and habits influence that loss of function. However, there is still uncertainty about the influence of those variables in the MPF functions in post-menopause women. Thus, this study aimed at seeing if there is an influence from number of births, the type of birth and the level of physical activity on the MPF of post-menopause women. Another objective of this study was to compare MPF force in women who had had vaginal births with those who had been subjected to cesarean sections, those with different levels of physical activity and those with artificial and natural menopause in the initial and latter stages. Furthermore, the test of muscular force was compared to perineometry. Using observational, analytical and transversal observations, 100 women in the post-menopausal stage of life, between the ages of 45 and 65, were examined. They were divided according to the menopausal stage into three groups: women who had undergone hysterectomies, those in the initial stages of postmenopause and those in the late stage of postmenpause. The patients were questioned about social, demographic, gynecological and obstetric factors. All the volunteers were submitted to a physical examination where their height and weight were measured to arrive at the corporal mass index and their waist measurements were taken. The evaluation of the pelvic floor was conducted with muscular force tests and perineometry. These results were analyzed with statistical description and ANOVA statistical tests, multiple regression and Kolmogorov-Smirnov evaluations. The results showed homogeneity with regard to social demographic and anthropometric characteristics among the women in the final test sample (n=85). It was also seen that most of the women in all three groups were married (p=0.51) and catholic (p=0.13). The average per capital income varied between $R585.47 (+/-466.67) and $R1,271.83 (+/-1,748.95), with no significant difference between the groups (p=0.05). The G>6 group presented an average age between 58.95 (+/-3.96) which was significantly greater that the G<6 group´s average age (53.21+/- 3.88) (p=0.000). There was no difference between the groups´ anthropometric characteristics of weight (p=0.32), height (p=0.72) and corporal mass index (p=0.34), nor in the waist measurements (p=0.33). Furthermore, no significant difference was noted in the MPF function of women who had had normal births, cesarean sections or a combination of the two (TFM p=0.897; perineum measurement p=0.502). Likewise, no differences were seen in the MPF function of women who had one, two to three or four or more births (TFM p=0.28, perineum measurement p=0.13). Finally, no difference was perceived among those with different levels of physical activity (TFM p=0.663; perineum measurement p=0.741). Therefore, we found that the type of delivery, number of births and physical activity had no influence on the muscular function of the pelvic floor among the women studied. It is believed that decline in muscular function in post-menopause women is fundamentally related to the process of aging.

Despite numerous government projects aimed at reorganizing and qualifying obstetric and neonatal care in Brazil, it remains problematic, with repercussions for maternal and newborn mortality and humanized care of both the mother and child. The objective of this study was to analyze the care provided to women during the pregnancy-puerperium cycle, based on reports of public health service users regarding their pregnancy and delivery experiences, using comprehensiveness and humanization as reference. The study applied a qualitative approach and the methodological strategy consisted of listening to the women, in order to identify, based on the meanings of their discourse concerning their experiences with health services, continuities and discontinuities of care during the pregnancy-puerperium cycle. Study participants were women who gave birth at a municipal public maternity, residents of Natal, Brazil, who at the time of the interviews, were between 10 and 42 days postpartum. Seven women reported their pregnancy and delivery experiences at public services. As interviews and observation took place, the material produced was also analyzed, in order to achieve simultaneous production and data analysis. Using systematization, a dialogue was established between the women’s discourses and production in the field of Collective Health, with respect to concepts and discussion about obstetric and neonatal care as well as the Comprehensiveness and Humanization of such care. Participant discourses underscored aspects related to prenatal care starting at pregnancy and its repercussions as well as prenatal monitoring by health services; aspects associated with care during labor and delivery, as well as those involved in postpartum in the maternity, both with respect to newborn and maternal careç and lastly, puerperium care after discharge from the maternity. Analysis of results sought to identify lines of continuity and discontinuity in the comprehensiveness and humanization of care. Based on these lines and as final contributions of the study, the following paths were proposed to achieve comprehensive and humanized production of health care for women during the pregnancy-puerperium cycle: Path 1- Reassess care in the maternal and newborn health network, aimed at comprehensiveness in terms of guaranteeing access to the various services and technological resources available to enhance health and life. Path 2- Reorganize work processes in order to attain comprehensive and humanized care for women in the pregnancy-puerperium cycle. Path 3 – Qualify the professional-user relationship in care management during the pregnancy-puerperium cycle. Path 4 – Invest in the qualification of communication processes in the different dimensions of care during the pregnancy-puerperium cycle.

L'elaborato si inserisce in un progetto sviluppato presso l'Istituto Scientifico Romagnolo per lo Studio e la Cura dei Tumori (I.R.S.T.) di Meldola che riguarda la valutazione di parametri cardiologici che possano risultare indici predittivi di uno scompenso cardiaco in pazienti affetti da linfoma. Tutti i soggetti considerati nel progetto sono stati sottoposti a trattamenti chemioterapici facenti uso di antracicline e la cui funzionalità cardiaca è stata controllata periodicamente tramite ecografia bidimensionale e volumetrica, utilizzando il sistema VividE9 della GE Healthcare. L'obiettivo dell'analisi è quello di ricercare se oltre alla frazione di eiezione esistono parametri più predittivi di una eventuale cardiotossicità come gli strain, calcolati attraverso l'ausilio della tecnica ecografica.

Introdução: As fístulas vesico-vaginais são, de todas as fístulas do aparelho urinário, as mais frequentes. A etiologia deste tipo de fístulas, varia de acordo com a região do globo. Nos países em vias de desenvolvimento, a principal causa de fístulas vesico vaginais corresponde a complicações obstétricas. A pressão exercida pelo feto, nas paredes da vagina, bexiga e uretra proximal leva muitas vezes à necrose dos tecidos. Esta necrose pode levar a um conjunto de complicações que vão desde as fístulas vesico vaginais, estenose vaginal, atresia rectal, infertilidade secundária, ou mesmo à incompetência do esfíncter anal e osteíte púbica. Apresentamos 3 casos clínicos de fístulas vesico vaginais de causa obstétrica bem como uma revisão dos aspectos teóricos e técnicos do diagnóstico e tratamento desta patologia. Materiais e Métodos: Descrevem-se 3 casos clínicos de fístulas vesico vaginais de causa obstétrica tratadas em 2012. O primeiro caso foi abordado por via vaginal reparado em três planos, utilizando-se o retalho de Martius. Os outros dois casos devido à localização e complexidade da fístula foram abordados por via abdominal com interposição de epíploon. Resultados: No primeiro caso a doente foi seguida noutra instituição, nos outros dois casos não houve recidiva da fístula num follow up de 6 e 12 meses respectivamente. Relativamente à correção cirúrgica obedeceu-se aos seguintes princípios: uma boa exposição do trajecto fistuloso, desbridamento do tecido desvitalizado e isquémico, utilização de retalhos bem vascularizados e preenchimento do espaço livre. O encerramento foi realizado em vários planos, sem sobreposição das suturas, sempre sem tensão. Discussão: As fístulas vesico-vaginais permanecem um enorme desafio do ponto de vista cirúrgico, necessitando de uma abordagem multidisciplinar, bem como da criação de centros de referência.

Se realizó un estudio clínico descriptivo en 80 pacientes en labor de parto, en el Hospital Vicente Corral Moscoso de la ciudad de Cuenca en 1995, con el objeto de evaluar ecográficamente, durante la labor de parto, la estática, antropometría, el grado de bienestar fetal y determinar el perfil neuroconductal. Se establecieron 2 grupos; un grupo control conformado por 40 pacientes en labor de parto en las que se realizó el control clínico y un grupo de pacintes [grupo de estudio] en las que además de la valoración se realizó el control ecográfico, considerando principalmente como parámetros de control agudos de movimientos respiratorios, movimientos oculares, movimientos corporales y bucales, el tono fetal y la estimulación vibroacústica tipo 1 [reflejo de sobresalto]. Se encontró que el perfil neuroconductual planteado en el trabajo tiene una sensibilidad del 100 por ciento, con una especificidad del 82.6 por ciento, un valor predictivo de la prueba negativa del 100 por ciento. La exactitud es de 90 por ciento y el índice de Kappa de 0.75 por ciento. La prueba de Fisher, al comparar cada uno de los parámetros con la valoración del Apgar es significativa con una probabilidad de p menos 0.01. La escala de puntuación utilizada incluyó; 10, feto sano; 8, sospecha de hipoxia fetal; 6, hipoxia fetal leve; 4 hipoxia fetal moderada; y 0-2, hipoxia fetal grave. Se observó correlación entre puntaje del perfil neuroconductual y el exámen clínico del recién nacido. Durante la labor de parto el diámetro biparietal para la estimación del peso fetal no tiene utilidad clínica. Se acompaña una revisión bibliográfica exhaustiva, se exponen las conclusiones y se realizan las recomendaciones pertinentes

Introducción: El estado nutricional materno tiene un efecto determinante sobre la morbimortalidad obstétrica y perinatal. Las embarazadas obesas representan un reto para el anestesiólogo ya que habitualmente presentan otras patologías asociadas, mayor riesgo de nacimiento por cesárea y dificultades técnicas asociadas tanto con la anestesia regional como general. La Organización de las Naciones Unidas para la Agricultura y la Alimentación (FAO), la Organización Mundial de la Salud (OMS) y American College of Obstetricians and Gynecologists (ACOG) recomienda usar el índice de masa corporal (IMC) para evaluar el estado nutricional de la mujer embarazada. Material y métodos: Realizamos un estudio prospectivo, descriptivo que incluyó 893 pacientes sometidas a cesárea con anestesia raquídea en el Centro Hospitalario Pereira Rossell desde octubre de 2014 a octubre del 2015. Resultados: Se obtuvo un promedio de IMC 29.8 ± 0.2 Kg. /m2, con un mínimo y un máximo en 16.01 y 51.56 respectivamente. Con una incidencia de obesidad del 43,2%. Conclusiones: Los resultados de nuestro estudio muestran una alta prevalencia de obesidad, consideramos que estos datos pueden ser representativos de la realidad obstétrica de nuestro país ya que hasta el momento no hemos encontrado un estudio nacional en el que se determine la incidencia de obesidad en la población obstétrica.
