The oceans have proved to be an interminable source of new and effective drugs. Innumerable studies have proved that specific compounds isolated from marine organisms have great nutritional and pharmaceutical value. Polyunsaturated fattyacids (PUFA) in general are known for their dietary benefits in preventing and curing several critical ailments including Coronary heart disease (CHD) and cancers of various kinds. Eicosapentaenoic Acid (EPA) and Docosahexaenoic Acid (DHA) are two PUFA which are entirely marine in origin – and small Clupeoid fishes like sardines are known to be excellent sources of these two compounds. In this study, we selected two widely available Sardine species in the west coast, Sardinella longiceps and Sardinella fimbriata, for a comparative analysis of their bioactive properties. Both these sardines are known to be rich in EPA and DHA, however considerable seasonal variation in its PUFA content was expected and these variations studied. An extraction procedure to isolate PUFA at high purity levels was identified and the extracts obtained thus were studied for anti-bacterial, anti-diabetic and anti-cancerous properties.Samples of both the sardines were collected from landing centre, measured and their gut content analysed in four different months of the year – viz. June, September, December and March. The fish samples were analyzed for fattyacid using FAME method using gas chromatography to identify the full range of fattyacids and their respective concentration in each of the samples. The fattyacids were expressed in mg/g meat and later converted to percentage values against total fatty acids and total PUFA content. Fattyacids during winter season (Dec-Mar) were found to be generally higher than spawning season (June-Sept). PUFA dominated the profiles of both species and average PUFA content was also higher during winter. However, it was found that S. longiceps had proportionately higher EPA as compared to S. fimbriata which was DHA rich. Percentage of EPA and DHA also varied across months for both species – the spawning season seemed to show higher EPA content in S. longiceps and higher DHA content in S. fimbriata. Gut content analysis indicate that adult S. fimbriata is partial to zooplanktons which are DHA rich while adult S. longiceps feed mainly on EPA rich phytoplankton. Juveniles of both species, found mainly in winter, had a gut content showing more mixed diet. This difference in the feeding pattern reflect clearly in their PUFA profile – adult S. longiceps, which dominate the catch during the spawn season, feeding mostly on phytoplankton is concentrated with EPA while the juveniles which are found mostly in the winter season has slightly less EPA proportion as compared to adults. The same is true for S. fimbriata adults that are caught mostly in the spawning season; being rich in DHA as they feed mainly on zooplankton while the juveniles caught during winter season has a relatively lower concentration of DHA in their total PUFA.Various extraction procedures are known to obtain PUFA from fish oil. However, most of them do not give high purity and do not use materials indicated as safe. PUFA extracts have to be edible and should not have harmful substances for applying on mice and human subjects. Some PUFA extraction procedures, though pure and non-toxic, might induce cis-trans conversions during the extraction process. This conversion destroys the benefits of PUFA and at times is harmful to human body. A method free from these limitations has been standardized for this study. Gas Chromatography was performed on the extracts thus made to ensure that it is substantially pure. EPA: DHA ratios for both samples were derived - for S. longiceps this ratio was 3:2, while it was 3:8 for S. fimbriata.Eight common strains of gram positive and gram negative bacterial strains were subjected to the PUFA extracts from both species dissolved in acetone solution using Agar Well Diffusion method. The activity was studied against an acetone control. At the end of incubation period, zones of inhibition were measured to estimate the activity. Minimum inhibitory concentration for each of the active combinations was calculated by keeping p < 0.01 as significant. Four of the bacteria including multi-resistant Staphylococcus aureus were shown to be inhibited by the fish extracts. It was also found that the extracts from S. fimbriata were better than the one from S. longiceps in annihilating harmful bacteria.Four groups of mice subjects were studied to evaluate the antidiabetic properties of the PUFA extracts. Three groups were induced diabetes by administration of alloxan tetra hydrate. One group without diabetes was kept as control and another with diabetes was kept as diabetic control. For two diabetic groups, a prescribed amount of fish extracts were fed from each of the extracts. The biochemical parameters like serum glucose, total cholesterol, LDL & HDL cholesterol, triglycerides, urea and creatinine were sampled from all four groups at regular intervals of 7 days for a period of 28 days. It was found that groups fed with fish extracts had marked improvement in the levels of total LDL & HDL cholesterol, triglycerides and creatinine. Groups fed with extracts from S. fimbriata seem to have fared better as compared to S. longiceps. However, both groups did not show any marked improvement in blood glucose levels or levels of urea.Cell lines of MCF-7 (Breast Cancer) and DU-145 (Prostate Cancer) were used to analyse the cytotoxicity of the PUFA extracts. Both cell lines were subjected to MTT Assay and later the plates were read using an ELISA reader at a wavelength of 570nm. It was found that both extracts had significant cytotoxic effects against both cell lines and a peak cytotoxicity of 85-90% was apparent. IC50 values were calculated from the graphs and it was found that S. longiceps extracts had a slightly lower IC50 value indicating that it is toxic even at a lower concentration as compared to extracts from S. fimbriata.This study summarizes the bioactivity profile of PUFA extracts and provides recommendation for dietary intake; fish based nutritional industry and indigenous pharmaceutical industry. Possible future directions of this study are also elaborated.

Diabetes mellitus is a heterogeneous metabolic disorder characterized by hyperglycemia with disturbances in carbohydrate, protein and lipid metabolism resulting from defects in insulin secretion, insulin action or both. Currently there are 387 million people with diabetes worldwide and is expected to affect 592 million people by 2035. Insulin resistance in peripheral tissues and pancreatic beta cell dysfunction are the major challenges in the pathophysiology of diabetes. Diabetic secondary complications (like liver cirrhosis, retinopathy, microvascular and macrovascular complications) arise from persistent hyperglycemia and dyslipidemia can be disabling or even life threatening. Current medications are effective for control and management of hyperglycemia but undesirable effects, inefficiency against secondary complications and high cost are still serious issues in the present prognosis of this disorder. Hence the search for more effective and safer therapeutic agents of natural origin has been found to be highly demanding and attract attention in the present drug discovery research. The data available from Ayurveda on various medicinal plants for treatment of diabetes can efficiently yield potential new lead as antidiabetic agents. For wider acceptability and popularity of herbal remedies available in Ayurveda scientific validation by the elucidation of mechanism of action is very much essential. Modern biological techniques are available now to elucidate the biochemical basis of the effectiveness of these medicinal plants. Keeping this idea the research programme under this thesis has been planned to evaluate the molecular mechanism responsible for the antidiabetic property of Symplocos cochinchinensis, the main ingredient of Nishakathakadi Kashayam, a wellknown Ayurvedic antidiabetic preparation. A general introduction of diabetes, its pathophysiology, secondary complications and current treatment options, innovative solutions based on phytomedicine etc has been described in Chapter 1. The effect of Symplocos cochinchinensis (SC), on various in vitro biochemical targets relevant to diabetes is depicted in Chapter 2 including the preparation of plant extract. Since diabetes is a multifactorial disease, ethanolic extract of the bark of SC (SCE) and its fractions (hexane, dichloromethane, ethyl acetate and 90 % ethanol) were evaluated by in vitro methods against multiple targets such as control of postprandial hyperglycemia, insulin resistance, oxidative stress, pancreatic beta cell proliferation, inhibition of protein glycation, protein tyrosine phosphatase-1B (PTP-1B) and dipeptidyl peptidase-IV (DPPxxi IV). Among the extracts, SCE exhibited comparatively better activity like alpha glucosidase inhibition, insulin dependent glucose uptake (3 fold increase) in L6 myotubes, pancreatic beta cell regeneration in RIN-m5F and reduced triglyceride accumulation in 3T3-L1 cells, protection from hyperglycemia induced generation of reactive oxygen species in HepG2 cells with moderate antiglycation and PTP-1B inhibition. Chemical characterization by HPLC revealed the superiority of SCE over other extracts due to presence of bioactives (beta-sitosterol, phloretin 2’glucoside, oleanolic acid) in addition to minerals like magnesium, calcium, potassium, sodium, zinc and manganese. So SCE has been subjected to oral sucrose tolerance test (OGTT) to evaluate its antihyperglycemic property in mild diabetic and diabetic animal models. SCE showed significant antihyperglycemic activity in in vivo diabetic models. Chapter 3 highlights the beneficial effects of hydroethanol extract of Symplocos cochinchinensis (SCE) against hyperglycemia associated secondary complications in streptozotocin (60 mg/kg body weight) induced diabetic rat model. Proper sanction had been obtained for all the animal experiments from CSIR-CDRI institutional animal ethics committee. The experimental groups consist of normal control (NC), N + SCE 500 mg/kg bwd, diabetic control (DC), D + metformin 100 mg/kg bwd, D + SCE 250 and D + SCE 500. SCEs and metformin were administered daily for 21 days and sacrificed on day 22. Oral glucose tolerance test, plasma insulin, % HbA1c, urea, creatinine, aspartate aminotransferase (AST), alanine aminotransferase (ALT), albumin, total protein etc. were analysed. Aldose reductase (AR) activity in the eye lens was also checked. On day 21, DC rats showed significantly abnormal glucose response, HOMA-IR, % HbA1c, decreased activity of antioxidant enzymes and GSH, elevated AR activity, hepatic and renal oxidative stress markers compared to NC. DC rats also exhibited increased level of plasma urea and creatinine. Treatment with SCE protected from the deleterious alterations of biochemical parameters in a dose dependent manner including histopathological alterations in pancreas. SCE 500 exhibited significant glucose lowering effect and decreased HOMA-IR, % HbA1c, lens AR activity, and hepatic, renal oxidative stress and function markers compared to DC group. Considerable amount of liver and muscle glycogen was replenished by SCE treatment in diabetic animals. Although metformin showed better effect, the activity of SCE was very much comparable with this drug. xxii The possible molecular mechanism behind the protective property of S. cochinchinensis against the insulin resistance in peripheral tissue as well as dyslipidemia in in vivo high fructose saturated fat diet model is described in Chapter 4. Initially animal were fed a high fructose saturated fat (HFS) diet for a period of 8 weeks to develop insulin resistance and dyslipidemia. The normal diet control (ND), ND + SCE 500 mg/kg bwd, high fructose saturated fat diet control (HFS), HFS + metformin 100 mg/kg bwd, HFS + SCE 250 and HFS + SCE 500 were the experimental groups. SCEs and metformin were administered daily for the next 3 weeks and sacrificed at the end of 11th week. At the end of week 11, HFS rats showed significantly abnormal glucose and insulin tolerance, HOMA-IR, % HbA1c, adiponectin, lipid profile, liver glycolytic and gluconeogenic enzyme activities, liver and muscle triglyceride accumulation compared to ND. HFS rats also exhibited increased level of plasma inflammatory cytokines, upregulated mRNA level of gluconeogenic and lipogenic genes in liver. HFS exhibited the increased expression of GLUT-2 in liver and decreased expression of GLUT-4 in muscle and adipose. SCE treatment also preserved the architecture of pancreas, liver, and kidney tissues. Treatment with SCE reversed the alterations of biochemical parameters, improved insulin sensitivity by modifying gene expression in liver, muscle and adipose tissues. Overall results suggest that SC mediates the antidiabetic activity mainly via alpha glucosidase inhibition, improved insulin sensitivity, with antiglycation and antioxidant activities.

Introducción: Las enfermedades cardiovasculares son las primeras causas de morbilidad y mortalidad en los registros globales, teniendo en cuenta que muchas de ellas son susceptibles de manejo quirúrgico, es cada vez más importante conocer las complicaciones post operatorias neurológicas de tales procedimientos. Objetivos: Evaluar la presencia de factores predictores para el desarrollo de crisis epilépticas en el periodo pos operatorio de cirugías cardiovasculares. Materiales y métodos: Estudio de casos y controles, en pacientes sometidos a cirugías cardiovasculares en la Fundación Cardioinfantil entre los años 2008 y 2009. Resultados: Se analizaron 641 historias de pacientes, de los cuales 22 presentaron crisis durante el post operatorio y 66 pacientes se tomaron como controles. En 4 de los 22 pacientes quienes presentaron convulsiones (18.1%) no fue usado acido tranexamico durante la intervención teniendo esto una significancia estadística (p= 0.003). Conclusión: El uso de ácido tranexámico en los equipos de circulación extra-corpórea durante las citadas cirugías cardiovasculares podría ser un factor protector para la presentación de crisis epilépticas en el post operatorio. Los valores de creatinina elevados podrían ser un factor de riesgo para el desarrollo de convulsiones pero es necesario realizar nuevos estudios con un tamaño probable de muestra más grande y dirigida a evidenciar la relación entre estos.

La cirugía cardiaca con circulación extracorpórea tiene riesgo de lesión del sistema nervioso central que impacta negativamente el desenlace postoperatorio. Su espectro es variable siendo lo más frecuente la lesión isquémica. Sin embargo esta no es la única complicación neurológica, las convulsiones también tienen un impacto significativo sobre la morbimortalidad y se asocian a un incremento de la estancia hospitalaria. Mientras que los eventos isquémicos tras cirugía cardiaca están bien caracterizados, existen pocos datos acerca de las convulsiones postoperatorias de origen no isquémico. Dentro de las teorías postuladas para explicar estos fenómenos llama la atención el potencial epileptogénico de los medicamentos antifibrinolíticos, ampliamente utilizados con el objetivo de minimizar el sangrado y reducir la exposición a hemoderivados. Numerosos reportes sugieren que el ácido tranexámico puede inducir actividad convulsiva en el postoperatorio. El objetivo de este estudio retrospectivo de casos y controles fue examinar los factores asociados con convulsiones postoperatorias en pacientes sometidos a cirugía cardiaca con circulación extracorpórea que recibieron ácido tranexámico. Todos los pacientes sometidos a cirugía cardiaca con circulación extracorpórea entre Enero de 2008 y Diciembre de 2009 fueron identificados. Los casos se definieron como aquellos pacientes que convulsionaron y requirieron inicio de anticonvulsivantes dentro de las 48 horas post-quirúrgicas. Los criterios de exclusión incluyeron epilepsia preexistente y pacientes con alguna lesión isquémica nueva en neuroimágen. Los controles fueron seleccionados aleatoriamente a partir de la cohorte inicial. Se analizaron un total de 28 casos y 112 controles. Se evidenció una asociación entre disfunción renal, valores elevados de creatinina preoperatoria y convulsiones postoperatorias.

Objetivo: Reportar la experiencia obtenida con la realización de Nefrectomía Parcial para el tratamiento de masas renales en la Fundación Santa Fe de Bogotá y Fundación Cardioinfantil. Materiales y Métodos: Se revisó el registro de procedimientos quirúrgicos de las dos instituciones entre enero de 2005 y marzo de 2011. Se incluyeron los pacientes llevados a nefrectomía parcial y se revisaron las historias clínicas. Se excluyeron pacientes operados por patología no tumoral. Se registraron variables preoperatorias, intraoperatorias y postoperatorias. Se creó una base de datos en Excel y se elaboró un análisis descriptivo de las variables utilizando el paquete estadístico Stata 10.0. Resultados: Se realizaron un total de 63 nefrectomías parciales. Se analizaron 59 que fueron realizadas por sospecha de patología tumoral (quistes complejos o cáncer). El promedio de edad fué 60.7 años. En los pacientes con sospecha de tumor renal, la principal indicación para el procedimiento conservador fue el tamaño de la masa (82.53%). La creatinina preoperatoria fue 1.01 mg/dl en promedio. El abordaje utilizado con mayor frecuencia fue la lumbotomía (89.8%). En 79.6% de los casos se realizó isquemia fría. El sangrado fue 354 cc en promedio. En 6.77 % de los pacientes fue necesario ampliar el margen. El diagnóstico definitivo más frecuente fue carcinoma de células claras en el 72.8% de los casos. La creatinina postoperatoria fue de 1.14mg/dl en promedio. Un 98.3% de los pacientes permanecen libres de recaída con un seguimiento promedio de 27 meses. Conclusiones: La nefrectomía parcial por abordaje a cielo abierto es un procedimiento oncológicamente efectivo, con baja morbimortalidad y que permite mantener la función renal, en manos experimentadas. Es considerada el patrón de oro para el tratamiento de masas renales en estado T1a, tumores bilaterales, en pacientes con falla renal o en aquellos que tengan enfermedades que potencialmente la afecten.

El pronóstico de la Neumonía Adquirida en la Comunidad Severa (NAC-S) depende de decisiones terapéuticas instauradas tempranamente. Los cambios fisiológicos ocurridos en las primeras horas pueden ser difíciles de detectar. No existe ningún modelo para la determinación temprana del éxito de la terapia instaurada en NAC-S. Metodología: Descripción de la totalidad de los pacientes con NAC-S hospitalizados en la Unidad de Cuidado Intensivo de la Fundación Cardioinfantil entre los años 2008 y 2012 haciendo comparaciones entre grupos (muertos vs. supervivientes) y entre momentos (0, 24 y 48 horas desde el ingreso a la UCI) y realizando regresión logística binaria. Resultados: Entre los pacientes que fallecieron la necesidad de soporte vasoactivo fue mayor en todos los momentos evaluados (sig=0.001), en la línea de base tuvieron mayores requerimientos de la Fracción Inspirada de O2 (mediana 0.55% vs. 0.50%, sig=0.011), a las 24 horas tuvieron pH (mediana 7.345 vs.7.370, sig=0.025) y tensión arterial diastólica (mediana 58.5mmHg vs.61.0mmHg, sig =0.049) menores, y a las 48 horas glicemia (mediana 157mg/dL vs.142mg/dL, sig =0.026) creatinina (mediana 1.1mg/dL vs.0.7mg/dL, sig =0.062) y nitrógeno ureico (mediana 35mg/dL vs. 22mg/dL, sig =0.003) mayores comparados con los pacientes que sobrevivieron. Entre los pacientes supervivientes hubo una disminución de la frecuencia cardiaca entre las 0 y 24 horas (mediana 97lpm vs. 86lpm, sig =0.000) y entre las 0 y las 48 horas (mediana 97lpm vs. 81lpm, sig=0.000) y una disminución de los neutrófilos entre las 0 y las 48 horas (mediana 9838 vs. 8617, sig=0.062). Conclusiones: Nuestros hallazgos sugieren la existencia de una secuencia de fenómenos fisiopatológicos que al ser reconocida temprana y claramente permitiría establecer un plan de reanimación más especifico y eficaz. Estas diferencias se pueden plantear en el contexto de un modelo mixto predictivo

Introducción: la insuficiencia renal crónica IRC ha aumentado su prevalencia en los últimos años pasando de 44.7 pacientes por millón en 1993 a 538.46 pacientes por millón en 2010, los pacientes quienes reciben terapia de remplazo renal hemodiálisis en Colombia cada vez tienen una mayor sobrevida. El incremento de los pacientes y el incremento de la sobrevida nos enfocan a mejorar la calidad de vida de los años de diálisis. Metodología: se comparó la calidad de vida por medio del SF-36 en 154 pacientes con IRC estadio terminal en manejo con hemodiálisis, 77 pacientes incidentes y 77 pacientes prevalentes, pertenecientes a una unidad renal en Bogotá, Colombia. Resultados: se encontró una disminución de la calidad de vida en los componentes físicos (PCS) y metales (MCS) de los pacientes de hemodiálisis en ambos grupos. En el modelo de regresión logística la incapacidad laboral (p=0.05), el uso de catéter (p= 0,000), el bajo índice de masa corporal (p=0.021), la hipoalbuminemia (p=0,033) y la anemia (p=0,001) fueron factores determinantes en un 78,9% de baja calidad de vida de PCS en los pacientes incidentes con respecto a los prevalentes. En el MCS de los pacientes incidentes vs. Prevalentes se encontró la hipoalbuminemia (p=0.007), la anemia (p=0.001) y el acceso por catéter (p=0.001) como factores determinantes en un 70.6% de bajo MCS Conclusiones: la calidad de vida de los pacientes de diálisis se encuentra afectada con mayor repercusión en el grupo de los pacientes incidentes, se debe mejorar los aspectos nutricionales, hematológicos y de acceso vascular en este grupo.

The abdominal compartment syndrome (ACS) is the result of various physiological alterations produced by an abnormal increase of the intra-abdominal pressure. Some of these patients will undergo a surgical procedure for its management. Methods: This is a retrospective case series of 28 patients with ACS who required surgical treatment at the Hospital Occidente de Kennedy between 1999 and 2003. We assessed retrospectively the behavior of McNelis’s equation for prediction of the development of the ACS. Results: The leading cause of ACS in our study was intraabadominal infection (n=6 21,4%). Time elapsed between diagnosis and surgical decompression was less than 4 hours in 75% (n=21) of the cases. The variables that improved significantly after the surgical decompression were CVP (T: 4,0 p: 0,0001), PIM (T: 2,7; p: 0,004), PIA (T1,8; p:0,034) and Urine Output (T:-2,4; p:0,02). The values of BUN, Creatinine and the cardiovascular instability did not show improvement. The ICU and hospital length of stay were 11 days (SD: 9) and 18 days (SD13) respectively. Global mortality was 67,9% (n=19) and mortality directly attributable to the syndrome was 30% (n=8). The behavior of the McNelis’s equation was erratic. Conclusions: The demographic characteristics as well as disease processes associated with ACS are consistent with the literature. The association between physiological variables and ACS is heterogeneous between patients. Mortality rates attributable to ACS in our institution are within the range described world-wide. The behavior of the McNelis’s equation seems to depend greatly upon fluid balance.

INTRODUCCIÓN. La mediastinitis posterior a cirugía de revascularización miocárdica es una infección infrecuente, pero potencialmente fatal. En la Fundación Cardioinfantil se ha observado una tendencia al incremento de la misma en los últimos años, obligando a un cambio en las medidas de profilaxis antimicrobiana, pasando de cefalosporinas a vancomicina – gentamicina, sin embargo no se conoce aún el impacto de estas medidas. OBJETIVO: Determinar si el cambio de la profilaxis antibiótica en pacientes sometidos a revascularización miocárdica influye en una disminución de la incidencia de mediastinitis durante los años 2012 – 2013. METODOLOGÍA: Estudio de cohortes retrospectivo, evaluando la incidencia de mediastinitis post revascularización miocárdica, en pacientes expuestos a 2 diferentes tipos de profilaxis antimicrobiana (cefalosporinas vs vancomicina-gentamicina). Se describieron los patrones de susceptibilidad y resistencia de los patógenos encontrados en mediastinitis y la mortalidad de esta patología. RESULTADOS: Los patógenos más frecuentemente aislados en la mediastinitis fueron Staphylococcus aureus y Klebsiella pneumoniae, en la mayoría monomicrobiano. Se encontraron patógenos con perfiles de resistencia como betalactamasas de espectro extendido en Gram negativos y resistencia a la meticilina en cocos Gram positivos. El RR de mediastinitis del grupo expuesto a vancomicina-gentamicina respecto al grupo de cefalosporinas fue de 0,9 con IC 95% 0,28 – 3,28. CONCLUSIÓN: la epidemiologia microbiana de la mediastinitis no difiere de la reportada en otras series. La profilaxis antimicrobiana con vancomicina - gentamicina en pacientes sometidos a revascularización miocárdica, no redujo la incidencia de mediastinitis. Se propone regresar a la terapia de profilaxis con cefalosporinas.

Los pacientes con enfermedad renal crónica (ERC) terminal se caracterizan por presentar alteraciones nutricionales que se perpetúan independiente de la modalidad de la Terapia de Reemplazo Renal (TRR). En la diálisis peritoneal, tanto APD (automated peritoneal dialysis) como la CAPD (continuous ambulatory peritoneal dialysis) existe un alto riesgo de pérdida de albúmina por el filtrado peritoneal, sin diferencias claras en el estatus nutricional. El presente estudio caracteriza el estado nutricional de los pacientes incidentes en diálisis peritoneal de RTS Bogotá Regional 1, con un seguimiento de un año, para conocer los cambios en las variables nutricionales. Se realizó un estudio de cohortes retrospectivo, analizando 2 grupos según el tipo de diálisis peritoneal escogida (APD o CAPD), con un análisis análisis descriptivo trimestral de las características nutricionales y posteriormente una comparación entre las 2 modalidades de TRR. Encontrando un promedio de edad de 60,8 años, la mayoría hombres y etiología principal nefropatía diabética. La mayoría de las variables nutricionales permanecieron sin cambios durante seguimiento. El Test de equilibrio peritoneal para glucosa y creatinina mostró valores promedio bajo y promedio alto y la mayoría de pacientes un rango nutricional normal. Al comparar APD y CAPD, solo se encontraron de forma aislada, diferencias significativas en algunas variables aisladas. Este es el primer estudio en Colombia que evalúa diferentes aspectos nutricionales en diálisis peritoneal. Introducción: El uso de sustancias químicas como protección contra plagas es una práctica común en las floriculturas, la aplicación de los conceptos de manejo integrado de éstas resguarda los cultivos asegurando la cantidad y la calidad necesaria del producto neto requerido para satisfacer la demanda internacional. El biomonitoreo se ha utilizado en una variedad de estudios ocupacionales y ambientales para determinar exposiciones a plaguicidas. Materiales y Métodos: Se realizó un estudio de corte transversal en 300 trabajadores vinculados a 15 empresas, con el objetivo de caracterizar la exposición al plaguicida Metamidofos en trabajadores de empresas de flores en la Sabana de Bogotá (Cundinamarca) y Rionegro (Antioquia). Se aplicó una encuesta, la cual incluyó las variables socio demográficas, ocupacionales y toxicológicas y se recolectaron muestras de orina para determinar el plaguicida metamidofos. Resultados: Se observó que la fuerza laboral pertenecía en su gran mayoría a la zona urbana (88.5%) y se resalta la participación del género femenino en edades de entre 19 y 58 años con un promedio de 36,7 años. El oficio en el que se encontraba la mayor parte de los trabajadores fue cultivo y corte con el 52.5%. El tiempo de trabajo en el sector floricultor reportado fue mínimo de 12 (3.3%) y máximo de 22 años (6.1%). En cuanto a los elementos de protección personal, el 96.1% (344) afirma utilizarlos en su trabajo. El promedio de metamidofos al inicio de la jornada fue de 29,12 μg/l y al finalizar 15,70 μg/l. Conclusiones: Este trabajo permitió conocer el panorama de la exposición al plaguicida Metamidofos y constituye un aporte para continuar investigando sobre el tema y a la vez hacer seguimiento a los trabajadores por medio de su inclusión en programas de vigilancia epidemiológica.

As doenças cardíacas são um achado frequente na prática clínica. Saber se pacientes assintomáticos com doença mixomatosa da válvula mitral necessitam ou não de medicação numa fase inicial da doença não é tarefa fácil. A ativação neuro-hormonal, apesar de ter um efeito benéfico compensatório a curto prazo, torna-se deletéria a longo prazo, sendo que para isso é necessária intervenção farmacológica para inibir a sua atividade O SRAA (sistema renina angiotensina aldosterona) tem importantes mecanismos patofisiológicos implicados no desenvolvimento da insuficiência cardíaca congestiva e tem como produto final a aldosterona, que contribui para a remodelagem cardíaca. Neste trabalho verifiquei a inexistência de uma diferença significativa de valores de aldosterona sérica de cães assintomáticos com Doença Mixomatosa Valvular Mitral (estadio B2 da classificação ACVIM) e os valores do intervalo de referência desta hormona. Concluí também não haver relação entre a idade, ureia, creatinina, rácio Proteina-Creainina, Pressões arteriais sistólica, média e diastólica, frequência de pulso, parâmetros de remodelagem cardíaca e padrão do fluxo transmitral com os valores da aldosterona medidos. É através da evidência destes achados que sugiro a não instituição de um IECA neste tipo de pacientes. O uso precoce de IECA pode não só não trazer vantagens terapêuticas nesta fase da doença como também vai promover o aparecimento precoce de fenómenos de “escape da aldosterona”.

A Miopatia atípica é uma doença emergente em alguns países europeus, que afecta cavalos de pasto, caracterizada por fraquez súbita, decúbito prolongado e degeneração muscular aguda. Tem-se verificado recentemente um aumento do número de surtos, incluindo em países em que a doença nunca fora diagnosticada anteriormente, o que justifica o presente estudo, que tem como objectivo a contribuição para a descrição do perfil epidemológico da MA, com base na descrição de dados geográficos, demográficos e clínicos de casos confirmados e suspeitos de MA, observados num Hospital Equino de referência, na Bélgica, durante o Outono de 2010. Foram incluídos na análise retrospectiva da série de casos clínicos estudados tanto a identificação fenotípica de cada indivíduo, assim como as respectivas práticas de maneio, incluindo condições de pastoreio, história clínica e achados bioquímicos e, sempre que possível, histopatológicos. Os sinais clínicos ocorreram em condições climáticas desfavoráveis. Os sintomas mais frequentes incluíram fraqueza e recusa em mover-se. As concentrações de creatinina quinase, CK, variaram entre 20360 UI/L a 213350 UI/L nos casos confirmados, CC e casos de elevada probabilidade, CEP para miopatia atípica, MA. A associação de MA com as condições climáticas sugere que o maneio das pastagens é de extrema importância na prevenção da condição.

Covariation in the structural composition of the gut microbiome and the spectroscopically derived metabolic phenotype (metabotype) of a rodent model for obesity were investigated using a range of multivariate statistical tools. Urine and plasma samples from three strains of 10-week-old male Zucker rats (obese (fa/fa, n = 8), lean (fal-, n = 8) and lean (-/-, n = 8)) were characterized via high-resolution H-1 NMR spectroscopy, and in parallel, the fecal microbial composition was investigated using fluorescence in situ hydridization (FISH) and denaturing gradient gel electrophoresis (DGGE) methods. All three Zucker strains had different relative abundances of the dominant members of their intestinal microbiota (FISH), with the novel observation of a Halomonas and a Sphingomonas species being present in the (fa/fa) obese strain on the basis of DGGE data. The two functionally and phenotypically normal Zucker strains (fal- and -/-) were readily distinguished from the (fa/fa) obese rats on the basis of their metabotypes with relatively lower urinary hippurate and creatinine, relatively higher levels of urinary isoleucine, leucine and acetate and higher plasma LDL and VLDL levels typifying the (fa/fa) obese strain. Collectively, these data suggest a conditional host genetic involvement in selection of the microbial species in each host strain, and that both lean and obese animals could have specific metabolic phenotypes that are linked to their individual microbiomes.

The effects of diet and breed on the concentration of water-soluble flavour precursors, namely sugars, free amino acids, ribonucleotides, creatinine, carnosine and creatine, were studied in beef longissimus hanborum muscle. Diet had a significant effect on the concentration of free amino acids, with animals fed on grass silage having higher free amino acid levels than animals fed on a concentrate diet, whereas animals fed concentrates had a higher total reducing sugar content. Differences between a beef breed (Aberdeen Angus x Holstein-Friesian) and a dairy breed (Holstein-Friesian) were generally small. (C) 2007 Elsevier Ltd. All rights reserved.
