β-amyloid1-42 (Aβ1-42) is a major endogenous pathogen underlying the aetiology of Alzheimer's disease (AD). Recent evidence indicates that soluble Aβ oligomers, rather than plaques, are the major cause of synaptic dysfunction and neurodegeneration. Small molecules that suppress Aβ aggregation, reduce oligomer stability or promote off-pathway non-toxic oligomerization represent a promising alternative strategy for neuroprotection in AD. MRZ-99030 was recently identified as a dipeptide that modulates Aβ1-42 aggregation by triggering a non-amyloidogenic aggregation pathway, thereby reducing the amount of intermediate toxic soluble oligomeric Aβ species. The present study evaluated the relevance of these promising results with MRZ-99030 under pathophysiological conditions i.e. against the synaptotoxic effects of Aβ oligomers on hippocampal long term potentiation (LTP) and two different memory tasks. Aβ1-42 interferes with the glutamatergic system and with neuronal Ca2+ signalling and abolishes the induction of LTP. Here we demonstrate that MRZ-99030 (100–500 nM) at a 10:1 stoichiometric excess to Aβ clearly reversed the synaptotoxic effects of Aβ1-42 oligomers on CA1-LTP in murine hippocampal slices. Co-application of MRZ-99030 also prevented the two-fold increase in resting Ca2+ levels in pyramidal neuron dendrites and spines triggered by Aβ1-42 oligomers. In anaesthetized rats, pre-administration of MRZ-99030 (50 mg/kg s.c.) protected against deficits in hippocampal LTP following i.c.v. injection of oligomeric Aβ1-42. Furthermore, similar treatment significantly ameliorated cognitive deficits in an object recognition task and under an alternating lever cyclic ratio schedule after the i.c.v. application of Aβ1-42 and 7PA2 conditioned medium, respectively. Altogether, these results demonstrate the potential therapeutic benefit of MRZ-99030 in AD.

A doença de Alzheimer (DA) é uma desordem neurodegenerativa progressiva patologicamente caracterizada pela presença de placas de amilóide (placas senis) insolúveis e também pela presença de tranças neurofibrilhares,formadas pela proteína Tau hiperfosforiladada. O principal constituinte das placas senis é o peptídeo beta-amilóide (Ab), que deriva do processamento proteolítico da proteína precursora de amilóide de Alzheimer (APP). Embora Ab exista como um agregado pouco solúvel nas placas senis, ele é secretado pelas células como uma molécula solúvel. O Ab “per se” pode afectar o metabolismo da APP. Alguns autores sugerem que o Ab exerce o seu efeito alterando o processamento ou catabolismo da APP, outros sugerem que ele também induz a transcrição da APP, onde aumentando os níveis da APP pode estar a contribuir para a sua própria produção (mecanismo de “feedback” positivo). Assim sendo, torna-se difícil consolidar todas estas observações e identificar as potenciais funções fisiológicas do Ab “in vivo”, ou as consequências da sua produção. Neste trabalho caracterizaram-se os efeitos do Ab no metabolismo da APP. Os nossos estudos revelaram que um dos mecanismos induzidos pelo Ab é a acumulação intracelular do fragmento neuroprotector sAPP (isAPPa) em estruturas com características vesiculares associadas ao citosqueleto. Estudos adicionais em culturas primárias revelaram que o Ab estava a exercer o seu efeito ao nível da secreção vesicular, provavelmente interferindo com o transporte de APP/sAPP ao longo da rede do citosqueleto. Esta hipótese é sustentada pelo facto do Ab estar a afectar a estabilidade e a polimerização de proteínas envolvidas na dinâmica do citosqueleto. Contrariamente a publicações anteriores o Ab não induziu a transcrição da APP, na verdade em culturas primárias neuronais foi observado uma diminuição nos níveis de expressão da APP. Isto foi acompanhado por um aumento nos fragmentos C-terminais da APP (CTFs) e uma diminuição na localização nuclear do seu domínio intracelular (AICD), sugerindo alterações na sinalização nuclear da APP. O Ab pode afectar outras vias de sinalização, particularmente alterando o balanço entre as actividades das proteínas cinases e fosfatases, o que pode ter consequências para o desenvolvimento da doença. Os dados obtidos indicam que o Ab é capaz de inibir a actividade da proteína fosfatase1, a sua importância numa perspectiva de futuras terapias é discutida. Devido à relevância da agregação do Ab para a sua toxicidade, a formação de complexos com proteínas que promovem a sua desagregação/degradação e o seu efeito no processamento da APP foi avaliado. Na presença destes complexos observou-se uma reversão da acumulação isAPP, demonstrando o potencial terapêutico destas proteínas como moduladores do metabolismo da APP. Este trabalho permitiu compreender melhor os mecanismos envolvidos nos efeitos do Ab no processamento da APP e descobrir algumas moléculas que podem ser relevantes numa perspectiva de diagnóstico e terapia na DA.

A Doença de Alzheimer (AD) é a maior doença neurodegenerativa a nível mundial, e a principal causa de demência na população idosa. O processamento da proteína precursora de amilóide (APP) pelas β- e g- secretases origina o peptídeo Aβ, que agrega em oligómeros neurotóxicos e em placas senis. Estes são eventos-chave na patogénese da DA que levam à rutura da neurotransmissão sináptica, morte neuronal e inflamação neuronal do hipocampo e córtex cerebral, causando perda de memória disfunção cognitiva geral. Apesar dos grandes avanços no conhecimento do papel do processamento da APP na DA, a sua função fisiológica ainda não foi totalmente elucidada. Os mapas de interações proteína-proteína (PPI) humanos têm desempenhado um papel importante na investigação biomédica, em particular no estudo de vias de sinalização e de doenças humanas. O método dois-híbrido em levedura (YTH) consiste numa plataforma para a produção rápida de redes de PPI em larga-escala. Neste trabalho foram realizados vários rastreios YTH com o objetivo de identificar proteínas específicas de cérebro humano que interagissem com a APP, ou com o seu domínio intracelular (AICD), tanto o tipo selvagem como com os mutantes Y687F, que mimetizam o estado desfosforilado do resíduo Tyr-687. De facto, a endocitose da APP e a produção de Aβ estão dependentes do estado de fosforilação da Tyr-687. Os rastreios YTH permitiram assim obter de redes proteínas que interagem com a APP, utilizando como “isco” a APP, APPY687F e AICDY687F. Os clones positivos foram isolados e identificados através de sequenciação do cDNA. A maior parte dos clones identificados, 118, correspondia a sequências que codificam para proteínas conhecidas, resultando em 31 proteínas distintas. A análise de proteómica funcional das proteínas identificadas neste estudo e em dois projetos anteriores (AICDY687E, que mimetiza a fosforilação, e AICD tipo selvagem), permitiram avaliar a relevância da fosforilação da Tyr-687. Três clones provenientes do rastreio YTH com a APPY687F foram identificados como um novo transcrito da proteína Fe65, resultante de splicing alternativo, a Fe65E3a (GenBank Accession: EF103274), que codifica para a isoforma p60Fe65. A p60Fe65 está enriquecida no cérebro e os seus níveis aumentam durante a diferenciação neuronal de células PC12, evidenciando o potencial papel que poderá desempenhar na patologia da DA. A RanBP9 é uma proteína nuclear e citoplasmática envolvida em diversas vias de sinalização celulares. Neste trabalho caracterizou-se a nova interação entre a RanBP9 e o AICD, que pode ser regulada pela fosforilação da Tyr-687. Adicionalmente, foi identificada uma nova interação entre a RanBP9 e a acetiltransferase de histonas Tip60. Demonstrou-se ainda que a RanBP9 tem um efeito de regulação inibitório na transcrição mediada por AICD, através da interação com a Tip60, afastando o AICD dos locais de transcrição ativos. O estudo do interactoma da APP/AICD, modelado pela fosforilação da Tyr-687, revela que a APP poderá estar envolvida em novas vias celulares, contribuindo não só para o conhecimento do papel fisiológico da APP, como também auxilia a revelar as vias que levam à agregação de Aβ e neurodegeneração. A potencial relevância deste trabalho relaciona-se com a descoberta de algumas interações proteicas/vias de sinalização que podem que podem ser relevantes para o desenvolvimento de novas estratégias terapêuticas na DA.

Alzheimer’s disease is a chronic progressive neurodegenerative disease and is the most common form of dementia (estimated 50−60% of all cases), associated with loss of memory (in particular episodic memory), cognitive decline, and behavioural and physical disability, ultimately leading to death. Alzheimer’s disease is a complex disease, mostly occurring sporadically with no apparent inheritance and being the age the main risk factor. The production and accumulation of amyloid-beta peptide in the central nervous system is a key event in the development of Alzheimer’s disease. This project is devoted to the synthesis of amyloid-beta ligands, fluorophores and blood brain barrier-transporters for diagnosis and therapy of Alzheimer’s disease. Different amyloid-beta ligands will be synthesized and their ability to interact with amyloid-beta plaques will be studied with nuclear magnetic resonance techniques and a process of lead optimization will be performed. Many natural and synthetic compounds able to interact as amyloid-beta ligands have been identified. Among them, a set of small molecules in which aromatic moieties seem to play a key role to inhibit amyloid-beta aggregation, in particular heteroaromatic polycyclic compounds such as tetracyclines. Nevertheless tetracyclines suffer from chemical instability, low water solubility and possess, in this contest, undesired anti-bacterial activity. In order to overcome these limitations, one of our goals is to synthesize tetracyclines analogues bearing a polycyclic structure with improved chemical stability and water solubility, possibly lacking antibacterial activity but conserving the ability to interact with amyloid-beta peptides. Known tetracyclines have in common a fourth cycle without an aromatic character and with different functionalisations. We aim to synthesize derivatives in which this cycle is represented by a sugar moiety, thus bearing different derivatisable positions or create derivatives in which we will increase or decrease the number of fused rings. In order to generate a potential drug-tool candidate, these molecules should also possess the correct chemical-physical characteristics. The glycidic moiety, not being directly involved in the binding, it assures further possible derivatizations, such as conjugation to others molecular entities (nanoparticles, polymeric supports, etc.), and functionalization with chemical groups able to modulate the hydro/lipophilicity. In order to be useful such compounds should perform their action within the brain, therefore they have to be able to cross the blood brain barrier, and to be somehow detected for diagnostic purposes.

Summary Secondary lymphoid organs are sites of antigen presentation, clonal expansion of B and lymphocytes, and affinity maturation of B lymphocytes. In the intestine, these immune functions occur mainly in Peyer's patches (PP). PP develop through the interplay of two main cell types, haematopoietic cells and meserichyrnal cells. One particular haematopoietic cell type was identified as the inductive cell type in the formation of both PP and lymph nodes and was therefore designated as lymphoid tissue inducer cell. For a successful PP organogenesis, the crucial molecular components involved in the crosstalk of inducer cells and their mesenchymal target cells are adhesion molecules, lymphotoxin (LT) family members, and cytokines. In particular, the interleukin 7 receptor (IL-7R) expressed on inducer cells is absolutely required. To investigate the contribution of the ligand for the IL-7R. the cytokine IL-7, in the process of PP formation, we analyzed double transgenic (TG) mice. These mice resulted from an interbreeding of an IL-7TG mouse strain where the transgene is under the control of the MHC class II promoter with a second transgenic mouse strain, which overexpresses a transactivator for MHC class II genes. Double TG offsprings revealed higher levels of IL-7 mRNA occuring earlier in embryogenesis. Consequently, double TG mice showed a striking phenotype with a 3- to 5-fold increase in PP numbers compared to single IL-7TG or control littermates. Analysis of embryonic double TG intestines demonstrated that the process of PP development was already elevated during development as early as the embryonic day 16.5. Importantly, inducer cells were significantly increased in numbers in these embryonic intestines. Furthermore, the expression of LT? mRNA, which at this early time point is exclusively expressed by inducer cells, was also increased in double TG animals. These data clearly indicate a direct influence of IL-7 on the expansion of lymphoid tissue inducer cells and on the availability of LT? leading to a higher frequency of developing PP in fetal life. Interestingly, in addition to an enhanced frequency of PP development, in double TG mice, three additional phenotypic differences were observed. i) Lymphocyte infiltration in various non-lymphoid organs, such as stomach, salivary gland, and liver. Subsequent analysis demonstrated that B lymphocytes were predominant within these tertiary lymphoid structures. ii) Ectopic lymph node-like structures containing both B and T lymphocytes were found near the inguinal lymph node. iii) Double TG mice had a severe bone resorption syndrome most likely as a consequence of the pro-osteoclastic effect of IL-7. Taken together, these results show that IL-7 plays a key role in the homeostasis of inducer cells, in the generation of PP in the gut, in the formation of ectopic lymphoid tissue, and in bone resorption. Résumé Les organes lymphoïdes secondaires sont les lieux de présentation des antigènes aux lymphocytes, permettant l'expansion des lymphocytes B et T et la maturation d'affinité des lymphocytes B. Dans l'intestin, ces fonctions immunitaires se déroulent dans les plaques de Peyer (PP). Ces plaques se développent grâce à l'interaction des cellules hématopoïétiques avec des cellules mésenchymales. Un type particulier de cellules hématopoïétiques a été identifié comme cellule inductrice dans la formation des PP et des ganglions lymphatiques et de ce fait a été désigné cellule inductrice des tissus lymphoïdes. Durant l'organogénèse des PP, les composants moléculaires cruciaux impliqués dans l'interaction des cellules inductrices et des cellules mésenchymales sont les molécules d'adhésion, les membres de la famille des lymphotoxines (LT) et les cytokines. En particulier, le récepteur de l'interleukine 7 (IL-7R) exprimé par les cellules inductrices est absolument nécessaire. Pour étudier le rôle du ligand de l'IL-7R, l'interleukine IL-7, dans la formation des PP, nous avons croisé une lignée de souris transgénique (TG) surexprimant IL-7 sous contrôle du promoteur MHC class Il avec une lignée de souris transgénique surexprimant un transactivateur des genes MHC class II. Les souris doubles TG présentent une concentration élevée d'ARNm de l'IL-7 durant l'embryogénèse, ce qui résulte en une augmentation du nombre de PP de 3 à 5 fois en comparaison aux souris ayant seul le transgène IL-7 et aux souris contrôles. L'analyse des intestins des souris doubles TG démontre que le processus de développement des PP était élevé dès le jour 16.5 du développement embryonnaire. L'augmentation du nombre des cellules inductrices dans ces intestins embryonnaires est signilicative. De plus l'expression de l'ARNm LT?, qui à ce stade précoce est exclusivement exprimé dans les cellules inductrices, est également augmenté dans les doubles TG. Ces résultats indiquent clairement une influence directe d'IL-7 sur l'expansion des cellules inductrices des tissues lymphoïdes et sur la synthèse de LT? induisant une augmentation des PP se développant durant la vie foetale. En plus du développement accru des PP dans les souris doubles TG, trois différences phénotypiques ont été observées. i) L'infiltration lymphocytaire dans différents organes non-lymphoïdes, comme l'estomac, les glandes salivaires et le foie. Des analyses complémentaires ont demontré que les lymphocytes B étaient prédominants dans ces structures lymphoïdes tertiaires. ii) Des structures de ganglions lymphatiques ectopiques contenant des lymphocytes B et T ont été trouvées près des ganglions lymphatiques inguinaux. iii) Les souris doubles TG présentent un syndrome de résorption osseuse sévère probablement dû à l'effet pro-osteoclaste d'IL-7. Globalement, ces résultats montrent que IL-7 joue un rôle clé dans l'homéostasie des cellules inductrices dans la génèse de PP de l'intestin, dans la formation des tissus lymphoïdes ectopiques et dans la résorption osseuse.

Abstract The purpose of this study is to unravel the geodynamic evolution of Thailand and, from that, to extend the interpretation to the rest of Southeast Asia. The methodology was based in a first time on fieldwork in Northern Thailand and Southernmost Myanmar, using a multidisciplinary approach, and then on the compilation and re-interpretation, in a plate tectonics point of view, of existing data about the whole Southeast Asia. The main results concern the Nan-Uttaradit suture, the Chiang Mai Volcanic Belt and the proposition of a new location for the Palaeotethys suture. This led to the establishment of a new plate tectonic model for the geodynamic evolution of Southeast Asia, implying the existence new terranes (Orang Laut and the redefinition of Shan-Thai) and the role of the Palaeopacific Ocean in the tectonic development of the area. The model proposed here considers the Palaeotethys suture as located along the Tertiary Mae Yuam Fault, which represents the divide between the Cimmerian Sibumasu terrane and the Indochina-derived Shan-Thai block. The term Shan-Thai, previously used to define the Cimmerian area (when the Palaeotethys suture was thought to represented by the Nan-Uttaradit suture), was redefined here by keeping its geographical location within the Shan States of Myanmar and Central-Northern Thailand, but attributing it an East Asian Origin. Its detachment from Indochina was the result of the Early Permian opening of the Nan basin. The Nan basin closed during the Middle Triassic, before the deposition of Carnian-Norian molasse. The modalities of the closure of the basin imply a first phase of Middle Permian obduction, followed by final eastwards subduction. The Chiang Mai Volcanic Belt consists of scattered basaltic rocks erupted at least during the Viséan in an extensional continental intraplate setting, on the Shan-Thai part of the Indochina block. The Viséan age was established by the dating of limestone stratigraphically overlying the basalts. In several localities of the East Asian Continent, coeval extensional features occur, possibly implying one or more Early Carboniferous extensional events at a regional scale. These events occurred either due to the presence of a mantle plume or to the roll-back of the Palaeopacific Ocean, subducting beneath Indochina and South China, or both. The Palaeopacific Ocean is responsible, during the Early Permian, for the opening of the Song Ma and Poko back-arcs (Vietnam) with the consequent detachment of the Orang Laut Terranes (Eastern Vietnam, West Sumatra, Kalimantan, Palawan, Taiwan). The Late Triassic/Early Jurassic closure of the Eastern Palaeotethys is considered as having taken place by subduction beneath its southern margin (Gondwana), due to the absence of Late Palaeozoic arc magmatism on its northern (Indochinese) margin and the presence of volcanism on the Cimmerian blocks (Mergui, Lhasa). Résumé Le but de cette étude est d'éclaircir l'évolution géodynamique de la Thaïlande et, à partir de cela, d'étendre l'interprétation au reste de l'Asie du Sud-Est. La méthodologie utilisée est basée dans un premier temps sur du travail de terrain en Thaïlande du nord et dans l'extrême sud du Myanmar, en se basant sur une approche pluridisciplinaire. Dans un deuxième temps, la compilation et la réinterprétation de données préexistantes sur l'Asie du Sud-est la été faite, dans une optique basée sur la tectonique des plaques. Les principaux résultats de ce travail concernent la suture de Nan-Uttaradit, la « Chiang Mai Volcanic Belt» et la proposition d'une nouvelle localité pour la suture de la Paléotethys. Ceci a conduit à l'établissement d'un nouveau modèle pour l'évolution géodynamique de l'Asie du Sud-est, impliquant l'existence de nouveaux terranes (Orang Laut et Shan-Thai redéfini) et le rôle joué par le Paléopacifique dans le développement tectonique de la région. Le modèle présenté ici considère que la suture de la Paléotethys est située le long de la faille Tertiaire de Mae Yuam, qui représente la séparation entre le terrain Cimmérien de Sibumasu et le bloc de Shan-Thai, d'origine Indochinoise. Le terme Shan-Thai, anciennement utilise pour définir le bloc Cimmérien (quand la suture de la Paléotethys était considérée être représentée par la suture de Nan-Uttaradit), a été redéfini ici en maintenant sa localisation géographique dans les états Shan du Myanmar et la Thaïlande nord-centrale, mais en lui attribuant une origine Est Asiatique. Son détachement de l'Indochine est le résultat de l'ouverture du basin de Nan au Permien Inférieur. Le basin de Nan s'est fermé pendant le Trias Moyen, avant le dépôt de molasse Carnienne-Norienne. Les modalités de fermeture du basin invoquent une première phase d'obduction au Permien Moyen, suivie par une subduction finale vers l'est. La "Chiang Mai Volcanic Belt" consiste en des basaltes éparpillés qui ont mis en place au moins pendant le Viséen dans un contexte extensif intraplaque continental sur la partie de l'Indochine correspondant au bloc de Shan-Thai. L'âge Viséen a été établi sur la base de la datation de calcaires qui surmontent stratigraphiquement les basaltes. Dans plusieurs localités du continent Est Asiatique, des preuves d'extension plus ou moins contemporaines ont été retrouvées, ce qui implique l'existence d'une ou plusieurs phases d'extension au Carbonifère Inférieur a une échelle régionale. Ces événements sont attribués soit à la présence d'un plume mantellique, ou au rollback du Paléopacifique, qui subductait sous l'Indochine et la Chine Sud, soit les deux. Pendant le Permien inférieur, le Paléopacifique est responsable pour l'ouverture des basins d'arrière arc de Song Ma et Poko (Vietnam), induisant le détachement des Orang Laut Terranes (Est Vietnam, Ouest Sumatra, Kalimantan, Palawan, Taiwan). La fermeture de la Paléotethys Orientale au Trias Supérieur/Jurassique Inférieur est considérée avoir eu lieu par subduction sous sa marge méridionale (Gondwana), à cause de l'absence de magmatisme d'arc sur sa marge nord (Indochinoise) et de la présence de volcanisme sur les blocs Cimmériens de Lhassa et Sibumasu (Mergui). Résumé large public L'histoire géologique de l'Asie du Sud-est depuis environ 430 millions d'années a été déterminée par les collisions successives de plusieurs continents les uns avec les autres. Il y a environ 430 millions d'années, au Silurien, un grand continent appelé Gondwana, a commencé à se «déchirer» sous l'effet des contraintes tectoniques qui le tiraient. Cette extension a provoqué la rupture du continent et l'ouverture d'un grand océan, appelé Paléotethys, éloignant les deux parties désormais séparées. C'est ainsi que le continent Est Asiatique, composé d'une partie de la Chine actuelle, de la Thaïlande, du Myanmar, de Sumatra, du Vietnam et de Bornéo a été entraîné avec le bord (marge) nord de la Paléotethys, qui s'ouvrait petit à petit. Durant le Carbonifère Supérieur, il y a environ 300 millions d'années, le sud du Gondwana subissait une glaciation, comme en témoigne le dépôt de sédiments glaciaires dans les couches de cet âge. Au même moment le continent Est Asiatique se trouvait à des latitudes tropicales ou équatoriales, ce qui permettait le dépôt de calcaires contenant différents fossiles de foraminifères d'eau chaude et de coraux. Durant le Permien Inférieur, il y a environ 295 millions d'années, la Paléotethys Orientale, qui était un relativement vieil océan avec une croûte froide et lourde, se refermait. La croûte océanique a commencé à s'enfoncer, au sud, sous le Gondwana. C'est ce que l'on appelle la subduction. Ainsi, le Gondwana s'est retrouvé en position de plaque supérieure, par rapport à la Paléotethys qui, elle, était en plaque inférieure. La plaque inférieure en subductant a commencé à reculer. Comme elle ne pouvait pas se désolidariser de la plaque supérieure, en reculant elle l'a tirée. C'est le phénomène du «roll-back ». Cette traction a eu pour effet de déchirer une nouvelle fois le Gondwana, ce qui a résulté en la création d'un nouvel Océan, la Neotethys. Cet Océan en s'ouvrant a déplacé une longue bande continentale que l'on appelle les blocs Cimmériens. La Paléotethys était donc en train de se fermer, la Neotethys de s'ouvrir, et entre deux les blocs Cimmériens se rapprochaient du Continent Est Asiatique. Pendant ce temps, le continent Est Asiatique était aussi soumis à des tensions tectoniques. L'Océan Paléopacifique, à l'est de celui-ci, était aussi en train de subducter. Cette subduction, par roll-back, a déchiré le continent en détachant une ligne de microcontinents appelés ici « Orang Laut Terranes », séparés du continent par deux océans d'arrière arc : Song Ma et Poko. Ceux-ci sont composés de Taiwan, Palawan, Bornéo ouest, Vietnam oriental, et la partie occidentale de Sumatra. Un autre Océan s'est ouvert pratiquement au même moment dans le continent Est Asiatique : l'Océan de Nan qui, en s'ouvrant, a détaché un microcontinent appelé Shan-Thai. La fermeture de l'Océan de Nan, il y a environ 230 millions d'années a resolidarisé Shan-Thai et le continent Est Asiatique et la trace de cet événement est aujourd'hui enregistrée dans la suture (la cicatrice de l'Océan) de Nan-Uttaradit. La cause de l'ouverture de l'Océan de Nan peut soit être due à la subduction du Paléopacifique, soit aux fait que la subduction de la Paléotethys tirait le continent Est Asiatique par le phénomène du « slab-pull », soit aux deux. La subduction du Paléopacifique avait déjà crée de l'extension dans le continent Est Asiatique durant le Carbonifère Inférieur (il y a environ 340-350 millions d'années) en créant des bassins et du volcanisme, aujourd'hui enregistré en différents endroits du continent, dont la ceinture volcanique de Chiang Mai, étudiée ici. A la fin du Trias, la Paléotethys se refermait complètement, et le bloc Cimmérien de Sibumasu entrait en collision avec le continent Est Asiatique. Comme c'est souvent le cas avec les grands océans, il n'y a pas de suture proprement dite, avec des fragments de croûte océanique, pour témoigner de cet évènement. Celui-ci est visible grâce à la différence entre les sédiments du Carbonifère Supérieur et du Permieñ Inférieur de chaque domaine : dans le domaine Cimmérien ils sont de type glaciaire alors que dans le continent Est Asiatique ils témoignent d'un climat tropical. Les océans de Song Ma et Poko se sont aussi refermés au Trias, mais eux ont laissé des sutures visibles

BACKGROUND: A possible strategy for increasing smoking cessation rates could be to provide smokers who have contact with healthcare systems with feedback on the biomedical or potential future effects of smoking, e.g. measurement of exhaled carbon monoxide (CO), lung function, or genetic susceptibility to lung cancer. OBJECTIVES: To determine the efficacy of biomedical risk assessment provided in addition to various levels of counselling, as a contributing aid to smoking cessation. SEARCH STRATEGY: We systematically searched the Cochrane Collaboration Tobacco Addiction Group Specialized Register, Cochrane Central Register of Controlled Trials 2008 Issue 4, MEDLINE (1966 to January 2009), and EMBASE (1980 to January 2009). We combined methodological terms with terms related to smoking cessation counselling and biomedical measurements. SELECTION CRITERIA: Inclusion criteria were: a randomized controlled trial design; subjects participating in smoking cessation interventions; interventions based on a biomedical test to increase motivation to quit; control groups receiving all other components of intervention; an outcome of smoking cessation rate at least six months after the start of the intervention. DATA COLLECTION AND ANALYSIS: Two assessors independently conducted data extraction on each paper, with disagreements resolved by consensus. Results were expressed as a relative risk (RR) for smoking cessation with 95% confidence intervals (CI). Where appropriate a pooled effect was estimated using a Mantel-Haenszel fixed effect method. MAIN RESULTS: We included eleven trials using a variety of biomedical tests. Two pairs of trials had sufficiently similar recruitment, setting and interventions to calculate a pooled effect; there was no evidence that CO measurement in primary care (RR 1.06, 95% CI 0.85 to 1.32) or spirometry in primary care (RR 1.18, 95% CI 0.77 to 1.81) increased cessation rates. We did not pool the other seven trials. One trial in primary care detected a significant benefit of lung age feedback after spirometry (RR 2.12; 95% CI 1.24 to 3.62). One trial that used ultrasonography of carotid and femoral arteries and photographs of plaques detected a benefit (RR 2.77; 95% CI 1.04 to 7.41) but enrolled a population of light smokers. Five trials failed to detect evidence of a significant effect. One of these tested CO feedback alone and CO + genetic susceptibility as two different intervention; none of the three possible comparisons detected significant effects. Three others used a combination of CO and spirometry feedback in different settings, and one tested for a genetic marker. AUTHORS' CONCLUSIONS: There is little evidence about the effects of most types of biomedical tests for risk assessment. Spirometry combined with an interpretation of the results in terms of 'lung age' had a significant effect in a single good quality trial. Mixed quality evidence does not support the hypothesis that other types of biomedical risk assessment increase smoking cessation in comparison to standard treatment. Only two pairs of studies were similar enough in term of recruitment, setting, and intervention to allow meta-analysis.

RESUME : L'athérosclérose, pathologie inflammatoire artérielle chronique, est à l'origine de la plupart des maladies cardiovasculaires qui constituent l'une des premières causes de morbidité et mortalité en France. Les études observationnelles et expérimentales montrent que l'exercice physique prévient la mortalité cardiovasculaire. Cependant, les mécanismes précisant les bénéfices cliniques de l'exercice sur l'athérosclérose sont encore largement inconnus. Le but général de ce travail a donc été d'explorer, en utilisant un modèle expérimental d'athérosclérose, la souris hypercholestérolémique génétiquement dépourvue en apolipoprotéine E (apoE-/-), les mécanismes athéroprotecteurs de l'exercice. La dysfonction endothéliale, généralement associée aux facteurs de risque cardiovasculaire, serait l'une des étapes précoces majeures de l'athérogenèse. Elle est caractérisée par une diminution de la biodisponibilité en monoxyde d'azote (NO) avec la perte de ses propriétés vasculo-protectrices, ce qui favorise un climat pro-athérogène (stress oxydatif, adhésion et infiltration des cellules inflammatoires dans la paroi artérielle...) conduisant à la formation de la plaque athéromateuse. L'objectif de notre premier travail a donc été d'explorer les effets de l'exercice d'une part, sur le développement des plaques athéromateuses et d'autre part, sur la fonction endothéliale de la souris apoE-/-. Nos résultats montrent que l'exercice réduit significativement l'extension de l'athérosclérose et prévient la dysfonction endothéliale. L'explication pharmacologique montre que l'exercice stimule la fonction endothéliale via, notamment, une plus grande sensibilité des récepteurs endothéliaux muscariniques, ce qui active les événements signalétiques cellulaires récepteurs-dépendants à l'origine d'une bioactivité accrue de NO. Les complications cliniques graves de l'athérosclérose sont induites par la rupture de la plaque instable provoquant la formation d'un thrombus occlusif et l'ischémie du territoire tissulaire en aval. L'objectif de notre deuxième travail a été d'examiner l'effet de l'exercice sur la qualité/stabilité de la plaque. Nos résultats indiquent que l'exercice de longue durée stabilise la plaque en augmentant le nombre de cellules musculaires lisses et en diminuant le nombre de macrophages intra-plaques. Nos résultats montrent aussi que la phosphorylation de la eNOS (NO Synthase endothéliale) Akt-dépendante n'est pas le mécanisme moléculaire majeur à l'origine de ce bénéfice. Enfin, dans notre troisième travail, nous avons investigué l'effet de l'exercice sur le développement de la plaque vulnérable. Nos résultats montrent, chez un modèle murin de plaque instable (modèle d'hypertension rénovasculaire à rénine et angiotensine II élevés) que l'exercice prévient l'apparition de la plaque vulnérable indépendamment d'un effet hémodynamique. Ce bénéfice serait associé à une diminution de l'expression vasculaire des récepteurs AT1 de l'Angiotensine II. Nos résultats justifient l'importance de l'exercice comme outil préventif des maladies cardiovasculaires. ABSTRACT : Atherosclerosis, a chronic inflammatory disease, is one of the main causes of morbidity and mortality in France. Observational and experimental data indicate that regular physical exercise has a positive impact on cardiovascular mortality. However, the mechanisms by which exercise exerts clinical benefits on atherosclerosis are still unknown. The general aim of this work was to elucidate the anti-atherosclerotic effects of exercise, using a mouse model of atherosclerosis: the apolipoprotein E-deficient mice (apoE-/- mice). Endothelial dysfunction, generally associated with cardiovascular risk factors, has been recognized to be a major and early step in atherogenesis. Endothelial dysfunction is characterized by Nitric Oxide (NO) biodisponibility reduction with loss of NO-mediated vasculoprotective actions. This leads to vascular effects such as increased oxidative stress and increased adhesion of inflammatory cells into arterial wall thus playing a role in atherosclerotic plaque development. Therefore, one of the objective of our study was to explore the effects of exercise on atherosclerotic plaque extension and on endothelial function in apoE-/- mice. Results show that exercise significantly reduces plaque progression and prevents endothelial dysfunction. Pharmacological explanation indicates that exercise stimulates endothelial function by increasing muscarinic receptors sensitivity which in turn activates intracellular signalling receptor-dependent events leading to increased NO bioactivity. The clinical manifestations of atherosclerosis are the consequences of unstable plaque rupture with thrombus formation leading to tissue ischemia. The second aim of our work was to determine the effect of exercise on plaque stability. We demonstrate that long-term exercise stabilizes atherosclerotic plaques as shown by decreased macrophage and increased Smooth Muscle Cells plaque content. Our results also suggest that the Akt-dependent eNOS phosphorylation pathway is not the primary molecular mechanism mediating these beneficial effects. Finally, we assessed a putative beneficial effect of exercise on vulnerable plaque development. In a mouse model of Angiotensine II (Ang II)-mediated vulnerable atherosclerotic plaques, we provide fist evidence that exercise prevents atherosclerosis progression and plaque vulnerability. The beneficial effect of swimming was associated with decreased aortic Ang II AT1 receptor expression independently from any hemodynamic change. These findings suggest clinical benefit of exercise in terms of cardiovascular event protection.

Messages à retenir: L'identification des anomalies pleurales et parenchymateuses liées à une exposition à l'amiante est essentielle pour la reconnaissance de maladie professionnelle, avec de lourdes conséquences économiques. Le scanner thoracique est l'outil le plus fiable de ce dépistage. La détection d'anomalies dans les régions basales et sous-pleurales doit conduire à la réalisation de coupes complémentaires en procubitus sur ces régions. Le diagnostic TDM des anomalies pleurales et parenchymateuses liées à l'amiante repose sur des critères diagnostiques et un vocabulaire précis. De nombreuses images pièges peuvent mimer des plaques ou une asbestose. Une bonne connaissance de l'anatomie de l'interface pleuro-parenchymateuse et des principaux diagnostics différentiels permet souvent de redresser le diagnostic. Résumé: Le scanner multicoupe est l'outil de référence pour le dépistage des pathologies pleurales et parenchymateuses liées à l'exposition à l'amiante. Le diagnostic de plaques pleurales repose sur une surélévation quadrangulaire et nette de l'interface pleuro-parenchymateuse, dans une ou des localisation(s) évocatrice(s). Les pièges responsables de fausses images de plaques sont nombreux : structures anatomiques et variantes de la normale, opacités postéro-basales liées à des troubles ventilatoires, sarcoïdose... Les plaques doivent être différenciées des autres anomalies pleurales bénignes pouvant être rencontrées dans ce contexte, mais non spécifiques d'une exposition à l'amiante : épaississements pleuraux diffus, fibrose de la plèvre viscérale, images en pieds de corneille. La reconnaissance d'une asbestose nécessite de s'affranchir de troubles ventilatoires liés à la déclivité ou à des contraintes mécaniques focales, chroniques, telles que des ostéophytes para-vertébraux. Les coupes en procubitus permettent d'affirmer l'existence d'une pathologie interstitielle débutante dans les régions sous-pleurales et basales. Des anomalies interstitielles isolées ne peuvent être attribuées à une asbestose, de même que des anomalies interstitielles non spécifiques, associées ou non à une pathologie diffuse fibrosante.

This article presents selected findings and lessons from a cardiovascular research and prevention program initiated in 1989 in the Republic of Seychelles, a country in demographic and epidemiological transition. Rapid and sustained aging of the population (e.g., two-fold increase of people aged 30-39 from 1979 to 1995) implies, over the next few decades, further dramatic increase of the burden of chronic diseases, particularly cardiovascular disease (CVD). Epidemiological surveillance shows high age-specific rates of CVD (particularly stroke), high prevalence of peripheral atherosclerosis (plaques in carotid and femoral arteries), high prevalence of classical modifiable risk factors in the adult population (particularly hypertension), and substantial proportions of children with overweight. Stagnant life expectancy in men and an increase in women have been observed over the last two decades; this occurred despite largely improved health services and reduced infant mortality rates, and may reflect the large CVD burden found in middle-aged men (less so in middle-aged women). A national program of prevention of CVD has been initiated since 1991, which includes a mix of interventions to reduce risk factors in the general population and in high-risk individuals. Substantial research to back the prevention program indeed shows, at the moment, epidemiological patterns in Seychelles similar to those observed in Western countries (e.g., an association between peripheral atherosclerosis [as a proxy of CVD] and low density lipoprotein-cholesterol, smoking, diabetes, and [inversely] walking). This clearly supports the view that promotion of healthy lifestyles and control of conventional risk factors should be the main targets for CVD prevention and control.

Purpose: The purpose of this study was to compare the plaque morphology between coronary and peripheral arteries using intravascular ultrasound (IVUS). Methods: IVUS was performed in 68 patients with coronary and 93 with peripheral artery lesions (29 carotid, 50 renal, and 14 iliac). Plaques were classified as fibroatheroma (VH-FA) (further subclassified as thin-capped [VH-TCFA] and thick-capped [VH-ThCFA]), fibrocalcific plaque (VH-FC) and pathological intimal thickening (VH-PIT). Results: Plaque rupture (13% of coronary, 7% of carotid, 6% of renal, and 7% of iliac arteries; P=NS) and VH-TCFA (37% of coronary, 24% of carotid, 16% of renal, and 7% of iliac arteries; P=0.02) was observed in all arteries. Compared to coronary arteries, VH-FA was less frequently observed in renal (P<0.001) and iliac arteries (P<0.006), while VH-PIT and VH-FC were prevalent in both of these peripheral arteries. Lesions with positive remodeling demonstrated more characteristics of VH-FA in coronary, carotid, and renal arteries compared to those with intermediate/negative remodeling (all P<0.01). There was positive relationship between RI and percent necrotic core area in all four arteries. Conclusions: Atherosclerotic plaque phenotypes were heterogeneous among four different arteries. In contrast, the associations of remodeling mode with plaque phenotype and composition were similar among the various arterial beds.

BACKGROUND: Screening of peripheral atherosclerosis is increasingly used, but few trials have examined its clinical impact. We aimed to assess whether carotid plaque screening helps smokers to improve their health behaviors and cardiovascular risk factors. METHODS: We randomly assigned 536 smokers aged 40 to 70 years to carotid plaque ultrasonographic screening (US group) vs no screening (control group) in addition to individual counseling and nicotine replacement therapy for all participants. Smokers with at least 1 plaque received pictures of their plaques with a 7-minute structured explanation. The outcomes included biochemically validated smoking cessation at 12 months (primary outcome) and changes in cardiovascular risk factor levels and Framingham risk score. RESULTS: At baseline, participants (mean age, 51.1 years; 45.0% women) smoked an average of 20 cigarettes per day with a median duration of 32 years. The US group had a high prevalence of carotid plaques (57.9%). At 12 months, smoking cessation rates were high, but did not differ between the US and control groups (24.9% vs 22.1%; P = .45). In the US group, cessation rates did not differ according to the presence or absence of plaques. Control of cardiovascular risk factors (ie, blood pressure and low-density lipoprotein cholesterol and hemoglobin A(1c) levels in diabetic patients) and mean absolute risk change in Framingham risk score did not differ between the groups. The mean absolute risk change in Framingham risk score was +0.6 in the US group vs +0.3 in the control group (P = .56). CONCLUSION: In smokers, carotid plaque screening performed in addition to thorough smoking cessation counseling is not associated with increased rates of smoking cessation or control of cardiovascular risk factors.
