Tradicionalmente el manejo postoperatorio de los pacientes sometidos a circulación extracorpórea ha sido muy debatido. Tras la controversia coloides versus cristaloides ha surgido la discusión sobre el volumen de perfusión en las primeras fases del postoperatorio. La recomendación más extendida es la de restringir los ritmos de infusión basta un 50% de las necesidades basales. En este trabajo se realiza un estudio retrospectivo para ver los requerimientos basales en las primeras horas del postoperatorio evaluando las complicaciones. Del análisis de nuestra experiencia se deduce que la mayoría de pacientes finalizaron este período del postoperatorio con un aporte líquido ligeramente superior a las necesidades basales, sin que tenga repercusiones patológicas graves. Los ritmos de infusión deben ser muy variables adecuándose puntualmente a las necesidades hemodinámicas del paciente que se muestra muy dinámico en esta fase del postoperatotio.

Cardiac surgery involving ischemic arrest and extracorporeal circulation is often associated with alterations in vascular reactivity and permeability due to changes in the expression and activity of isoforms of nitric oxide synthase and cyclooxygenase. These inflammatory changes may manifest as systemic hypotension, coronary spasm or contraction, myocardial failure, and dysfunction of the lungs, gut, brain and other organs. In addition, endothelial dysfunction may increase the occurrence of late cardiac events such as graft thrombosis and myocardial infarction. These vascular changes may lead to increased mortality and morbidity and markedly lengthen the time of hospitalization and cost of cardiac surgery. Developing a better understanding of the vascular changes operating through nitric oxide synthase and cyclooxygenase may improve the care and help decrease the cost of cardiovascular operations.

The purpose of the present report is to demonstrate the long-term efficacy and safety of heparin-induced extracorporeal lipoprotein precipitation (HELP) of LDL-c and fibrinogen in the management of familial hypercholesterolemia. From June 1992 to June 1998 a 22-year-old young male patient with familial hypercholesterolemia (double heterozygote for C660X and S305C) resistant to medication and diet and with symptomatic coronary artery disease (angina) was treated weekly with 90-min sessions of the HELP system. The patient had also been previously submitted to right coronary artery angioplasty. The efficacy of the method was evaluated by comparing the reduction of total cholesterol, LDL-c and fibrinogen before and after the sessions and before and after initiation of the study (data are reported as averages for each year). During the study, angina episodes disappeared and there were no detectable adverse effects of the treatment. Total cholesterol (TC), fibrinogen, and LDL-c decreased significantly after each session by 59.6, 66.1 and 64%, respectively. HDL-c showed a nonsignificant reduction of 20.4%. Comparative mean values pre- and post-treatment values in the study showed significant differences: TC (488 vs 188 mg/dl), LDL-c (416.4 vs 145 mg/dl), and fibrinogen (144.2 vs 57.4 mg/dl). There was no significant change in HDL-c level: 29.4 vs 23 mg/dl. These data show that the HELP system, even for a long period of time, is a safe and efficient mode of treatment of familial hypercholesterolemia and is associated with disappearance of angina symptoms.

In the present study we standardized an experimental model of parabiotic circulation of isolated pig heart. The isolated heart was perfused with arterial blood from a second animal as support and submitted to regional ischemia for 30 min, followed by total ischemia for 90 min and reperfusion for 90 min. Parameters for measurement of ventricular performance using different indices measured directly or indirectly from intraventricular pressure were defined as: maximum peak pressure, final diastolic pressure, pressure developed, first derivative of maximum pressure (dP/dt max), first derivative of minimum pressure (dP/dt min), systolic stress of the left ventricle (sigmas), and maximum elastance of the left ventricle. Isolated hearts subjected to regional and global ischemia presented significant worsening of all measured parameters. Less discriminative parameters were dP/dt max and dP/dt min. Elastance was the most sensitive parameter during the reperfusion period, demonstrating an early loss of ventricular function during reperfusion. The model proved to be stable and reproducible and permitted the study of several variables in the isolated heart, such as ischemia and reperfusion phenomena, the effects of different drugs, surgical interventions, etc. The model introduces an advantage over the classical models which use crystalloid solutions as perfusate, because parabiotic circulation mimics heart surgery with extracorporeal circulation.

This study aimed to evaluate the effect of preoperative imaging techniques on the success and complication rates of ureteroscopy. We performed a retrospective analysis of 736 patients (455 males and 281 females), with a mean age of 45.5±15.2 years (range, 1-88 years), who underwent rigid ureteroscopic procedures for removal of ureteral stones. Patients were divided into 4 groups according to the type of imaging modality used: group I, intravenous urography (n=116); group II, computed tomography (n=381); group III, computed tomography and intravenous urography (n=91), and group IV, ultrasonography and abdominal plain film (n=148). Patients’ demographics, stone size and location, prior shock wave lithotripsy, lithotripsy technique, operation time, success rate, and rate of intraoperative complications were compared among the groups. There were no significant differences in success and complication rates among the groups. The stone-free rate after primary ureteroscopy was 87.1% in group I, 88.2% in group II, 96.7% in group III, and 89.9% in group IV (P=0.093). The overall incidence of intraoperative complications was 11.8%. According to the modified Satava classification system, 6.1% of patients had grade 1, 5.1% had grade 2, and 0.54% had grade 3 complications. Intraoperative complications developed in 12.1% of patients in group I, 12.6% of patients in group II, 7.7% of patients in group III, and 12.2% of patients in group IV (P=0.625). Our findings clearly demonstrate that ureteroscopic treatment of ureteral stones can be safely and effectively performed with no use of contrast study imaging, except in doubtful cases of anatomical abnormalities.

Cardiopulmonary bypass (CPB) with extracorporeal circulation produces changes in the immune system accompanied by an increase in proinflammatory cytokines and a decrease in anti-inflammatory cytokines. We hypothesize that dexmedetomidine (DEX) as an anesthetic adjuvant modulates the inflammatory response after coronary artery bypass graft surgery with mini-CPB. In a prospective, randomized, blind study, 12 patients (4 females and 8 males, age range 42-72) were assigned to DEX group and compared with a conventional total intravenous anesthesia (TIVA) group of 11 patients (4 females and 7 males). The endpoints used to assess inflammatory and biochemical responses to mini-CPB were plasma interleukin (IL)-1, IL-6, IL-10, interferon (INF)-γ, tumor necrosis factor (TNF)-α, C-reactive protein, creatine phosphokinase, creatine phosphokinase-MB, cardiac troponin I, cortisol, and glucose levels. These variables were determined before anesthesia, 90 min after beginning CPB, 5 h after beginning CPB, and 24 h after the end of surgery. Endpoints of oxidative stress, including thiobarbituric acid reactive species and delta-aminolevulinate dehydratase activity in erythrocytes were also determined. DEX+TIVA use was associated with a significant reduction in IL-1, IL-6, TNF-α, and INF-γ (P<0.0001) levels compared with TIVA (two-way ANOVA). In contrast, the surgery-induced increase in thiobarbituric acid reactive species was higher in the DEX+TIVA group than in the TIVA group (P<0.01; two-way ANOVA). Delta-aminolevulinate dehydratase activity was decreased after CPB (P<0.001), but there was no difference between the two groups. DEX as an adjuvant in anesthesia reduced circulating IL-1, IL-6, TNF-α, and INF-γ levels after mini-CPB. These findings indicate an interesting anti-inflammatory effect of DEX, which should be studied in different types of surgical interventions.

Chronic kidney disease is characterized by a progressive reduction of glomerular filtration rate and/or the appearance of proteinuria, and subsequently the progressive retention of organic waste compounds called uremic toxins (UT). Over the last decades, a large number of such compounds have been identified and their effects on organs and tissues, especially the cardiovascular system, has been demonstrated. In this review, we present the current classification of UT, as proposed by the EUTox Group, and the effects of some of the probably most important UTs, such as phosphate, FGF-23, PTH, AGEs, indoxyl sulfate and para-cresyl sulfate. We provide an overview on therapeutic approaches aimed to increase their extracorporeal removal via convective and/or adsorptive strategies and to lower their intestinal production/ absorption via dietetic and pharmacological interventions. The recognition that multiple toxins contribute to the uremia supports the need for new therapeutic targets, with a potentially positive impact on CKD progression and survival.

Laser produced plasma from silver is generated using a Q-switched Nd:YAG laser. Optical emission spectroscopy is used to carry out time of flight (TOF) analysis of atomic particles. An anomalous double peak profile in the TOF distribution is observed at low pressure. A collection of slower species emerge at reduced pressure below 4 X lO-3 mbar and this species has a greater velocity spread. At high pressure the plasma expansion follows the shockwave model with cylindrical symmetry whereas at reduced pressure it shows unsteady adiabatic expansion (UAE). During UAE the species show a parabolic increases in the expansion time with radial distance whereas during shock wave expansion the exponent is less than one. The angular distribution of the ablated species in the plume is obtained from the measurement of optical density of thin films deposited on to glass substrates kept perpendicular to the plume. There is a sharp variation in the film thickness away from the film centre due to asymmetries in the plume.

A través del tiempo las técnicas anestésicas basadas en opioides y especialmente en fentanyl han sido las más frecuentemente utilizadas en cirugía cardiaca pediátrica con circulación extracorpórea (CEC). La población pediátrica y en especial los niños menores de 10 kilos de peso tienen mayor susceptibilidad a las complicaciones postoperatorias, una de las razones para esta especial susceptibilidad es la respuesta al estrés y la reacción inflamatoria sistémica que desencadena la CEC en estos pacientes. La utilización de nuevos opioides (Remifentanil) más potentes, puede tener un impacto importante en la disminución del estrés metabólico desencadenado por la cirugía cardiaca. Mediante un estudio de cohortes realizado con los pacientes menores de 10 kilogramos de peso llevados a cirugía cardiaca con CEC se expusieron a dos diferentes técnicas anestésicas: la técnica “tradicional” basada en Fentanyl y otra basada en Remifentanil, se midieron las covariables que pudieran provocar altos niveles de estrés metabólico y se tomaron niveles basales y postoperatorios de tres metabolitos relacionados con el estrés (Lactato, Glucosa y Cortisol). Los resultados del estudio muestran grupos comparables en las mediciones basales de metabolitos de estrés y características clínicas, además de una relación, estadísticamente significativa, entre una menor producción de glucosa durante la CEC en los pacientes que son anestesiados con infusiones de remifentanil, los demás metabolitos no tuvieron relación con la técnica anestésica. Estos resultados abren caminos para la realización de estudios clínicos con el objetivo de establecer terapéuticas que permitan la disminución de la morbi-mortalidad asociada a cirugía cardiaca.

La cirugía cardiaca con circulación extracorpórea tiene riesgo de lesión del sistema nervioso central que impacta negativamente el desenlace postoperatorio. Su espectro es variable siendo lo más frecuente la lesión isquémica. Sin embargo esta no es la única complicación neurológica, las convulsiones también tienen un impacto significativo sobre la morbimortalidad y se asocian a un incremento de la estancia hospitalaria. Mientras que los eventos isquémicos tras cirugía cardiaca están bien caracterizados, existen pocos datos acerca de las convulsiones postoperatorias de origen no isquémico. Dentro de las teorías postuladas para explicar estos fenómenos llama la atención el potencial epileptogénico de los medicamentos antifibrinolíticos, ampliamente utilizados con el objetivo de minimizar el sangrado y reducir la exposición a hemoderivados. Numerosos reportes sugieren que el ácido tranexámico puede inducir actividad convulsiva en el postoperatorio. El objetivo de este estudio retrospectivo de casos y controles fue examinar los factores asociados con convulsiones postoperatorias en pacientes sometidos a cirugía cardiaca con circulación extracorpórea que recibieron ácido tranexámico. Todos los pacientes sometidos a cirugía cardiaca con circulación extracorpórea entre Enero de 2008 y Diciembre de 2009 fueron identificados. Los casos se definieron como aquellos pacientes que convulsionaron y requirieron inicio de anticonvulsivantes dentro de las 48 horas post-quirúrgicas. Los criterios de exclusión incluyeron epilepsia preexistente y pacientes con alguna lesión isquémica nueva en neuroimágen. Los controles fueron seleccionados aleatoriamente a partir de la cohorte inicial. Se analizaron un total de 28 casos y 112 controles. Se evidenció una asociación entre disfunción renal, valores elevados de creatinina preoperatoria y convulsiones postoperatorias.

El Switch arterial se ha convertido en la primera opción quirúrgica en pacientes con Transposición de grandes arterias (DTGA). El objetivo de este estudio es presentar la experiencia de la Fundación Cardioinfantil desde el año 2003 hasta julio del 2011, y determinar los factores de riesgo asociados a bajo gasto y mortalidad. Materiales y métodos: Estudio de caso - cohorte. Se revisaron las historias clínicas de los pacientes para obtener la información de las variables. El análisis se realizó mediante métodos estándar para variables continuas y variables categóricas. Los casos y sus controles se compararon y solo las variables con valores de p < 0.05 se analizaron como factores de riesgo. Resultados: 58 pacientes operados. Mediana de edad 12 días (RIQ 7 - 34), El 70.7% (41/58) de los pacientes fueron hombres. La DTGA con septum integro fue la anomalía más frecuente 53,5% (31/58). La mortalidad fue 15,2% (9/58) y la frecuencia de bajo gasto fue 55,2% (32/58). Entre el grupo de controles y casos no hubo diferencias estadísticamente significativas entre las variables preoperatorias y la presencia de bajo gasto y muerte, excepto el tiempo prolongado de circulación extracorpórea que fue factor de riesgo independiente para mortalidad OR 1.1 IC 95% (1 – 1,2) p de 0.002, y para bajo gasto cardiaco el score de vasoactivos-inotrópicos elevado OR 1.2 IC 95% (1.1 – 1,4) p de 0.001. Conclusiones: La frecuencia de las características de los pacientes operados en nuestra población es similar a lo reportado en la literatura mundial, sin embargo los factores de riesgo para mortalidad y bajo gasto no tuvieron el mismo impacto en nuestra población excepto por el tiempo de circulación extracorpórea y el uso de inotrópicos.
