A previously healthy 75-year-old white male dentist presented with a 6-month history of low-back pain treated with chronic steroid therapy had a Nocardia farcinica infection diagnosed by aspirate of thyroid abscess and six blood cultures. Despite the treatment with parenteral combination of trimethoprim/sulfamethoxazole, the patient failed to respond and died after two days of therapy. Autopsy revealed disseminated nocardiosis, involving lungs with pleural purulent exudate in both sides, heart, thyroid, kidneys, brain, bones, and lumbosacral soft tissue with destruction of L2-L4.

A case of acute abdomen disease caused by abdominal angiostrongyliasis is reported. A 42-year-old otherwise healthy patient presented with a complaint of nine days of abdominal pain, constipation, disury, fever and right iliac fossa palpable mass. Exploratory laparotomy was performed. After surgical treatment the patient presented serious complications.

The aim of this investigation was to determine nitric oxide metabolite levels in saliva samples from hepatitis C virus-positive patients in an attempt to test the hypothesis if increased levels of nitric oxide metabolites correlates with the presence of HCV-RNA in saliva. Saliva of 39 HCV-positive patients and 13 HCV-negative patients, without clinical or laboratorial evidence of liver disease were tested for nitric oxide metabolites. HCV-RNA was detected in serum and saliva by a RT-PCR method and nitric oxide level was determined by evaluation of its stable degradation products, nitrate and nitrite. No differences were found between the concentration of nitrite in saliva from HCV patients and controls, in despite of the presence or not of HCV RNA in saliva. Patients with HCV and cirrhosis had higher concentrations of nitrite but not significantly different from the control group or the groups of anti-HCV patients without cirrhosis. Increased levels of nitrite were not detected in anti-HCV positive patients, an indirect indication that chronic sialoadenitis are infrequent in these patients or occurs with low intensity not sufficient to increase nitric oxide metabolite levels in saliva.

In order to understand the determinants of human infection by Leishmania chagasi in an urban area, a cross-sectional population based study was conducted using molecular and serologic methods to identify infection. Participants were interviewed using a pre-coded questionnaire. Two criteria were tested to identify risk factors: Model 1- including all participants positive in hybridization by Leishmania donovani complex probe; Model 2- including all participants positive for hybridization and at least one serologic test. In Model 1, the variables associated with infection were: ownership of birds, time spent outside house between 6:00-10:00 PM and garbage not collected. In Model 2, the variables associated with infection were: family with knowledge of the vector, garbage not collected, garbage not removed or buried, ownership of birds and eroded areas in the neighborhood. The risk factors identified were associated with household conditions, presence of animals and the likelihood of contact with phlebotomine sandflies.

Lesions involving the intra-hepatic biliary ducts in schistosomiasis have been reported in the literature, both in mice and man, but there are no data concerning their quantitative, evolutionary or post curative chemotherapeutic aspects on record. In order to obtain such data an investigation on this subject was attempted. Mice infected with 50 Schistosoma mansoni cercariae were submitted to a liver biopsy at the 9th week post-infection, and treated with 400mg/bw praziquantel immediately afterwards. Infected and non-infected controls were submitted to the same procedures. By 19 weeks from cercarial exposure all surviving animals were sacrificed. The biliary ducts were counted on histological sections and the results were expressed as biliary ducts/portal spaces. This quantitative evaluation was compared with that from normal controls and revealed hyperplasia as the main biliary duct change (p<0.007) in schistosomiasis. Hyperplastic changes underwent only mild partial and not statistically significant regression after specific chemotherapy (p>0.05). Infected and untreated animals presented ductal changes that did not differ from those of the treated group. Measurements of serum bilirrubin (total and direct), and gamma-glutamyl-transpeptidase (gamma-GT) did not reveal significant differences when animals from the several groups were compared. Thus, bile ducts exhibit a proliferative response in relation to neighboring S. mansoni injury to portal areas, but although these lesions are histopathologically impressive, they lack a functional or prognostic significance.

This prospective study evaluated the incidence of Mycobacterium tuberculosis infection among nursing students at the Federal University of Espírito Santo, using the purified protein derivative test. In parallel, we evaluated whether knowledge on tuberculosis transmission mechanisms had any impact on nursing students' attitudes in relation to the risk of tuberculosis infection. The incidence of tuberculin conversion was 10.5% per year (p=0.035, 95% CI = 3.63-17.43), whereas the expected conversion rate in the overall population in Brazil is 0.5% per year. These results indicate nursing students as a group at risk for TB infection. Among the risk factors studied only the use of NIOSH95 masks was associated with protection against infection (RR=0.2). Furthermore there was no statistical difference among students that PPD converted and those who remained negative regarding disease knowledge and the existence of adequate facilities for patient hospitalization. Our data reinforces the need for implementing special biosafety programs targeting this professional group.

Com o objetivo de avaliar aspectos epidemiológicos, clínicos e parasitológicos da doença de Chagas crônica, em pacientes do Hospital Universitário da Universidade Federal de Mato Grosso do Sul, realizamos um estudo seccional envolvendo 120 chagásicos e 120 controles não-chagásicos, de ambos os sexos, com idades de 16 a 82 anos. Os aspectos epidemiológicos foram avaliados por questionário, a cardiopatia por exame clínico, eletrocardiograma convencional, radiologia e ecodopplercardiograma e a presença de Trypanosoma cruzi no sangue por xenodiagnóstico e teste da reação em cadeia da polimerase. Os resultados mostraram predominância de alóctones com baixa escolaridade e referência de contato prévio com triatomíneos entre os chagásicos. Abortamento espontâneo foi mais freqüente nas mulheres chagásicas. A cardiopatia devido ao componente chagásico foi estimada em 20,2%. Apresentou-se com 7,5% de cardiomegalia, 6,2% de aneurisma de ventrículo esquerdo e com predominância de dispnéia, palpitações e hipertensão arterial. O xenodiagnóstico foi positivo em 26,1% dos chagásicos enquanto a PCR foi positiva em 53,7%. A análise dos resultados indicou que a doença de Chagas no grupo estudado apresenta características clínicas e parasitológicas que revelam peculiaridades regionais.

No Estado do Ceará (1992 a 2002), 16 casos de envenenamento com o Thalassophyne nattereri ocorreram no litoral, a maioria (87,5%) em praias de Fortaleza e 12,5% do interior. Noventa e quatro por cento eram do sexo masculino e 6% feminino. Com relação à idade, 75% estavam na faixa etária de 21 a 40 anos, 19% entre 41 e 60 anos e 6% entre 1 a 10 anos. O tempo de exposição foi de 1 a 5 horas (4), 6 a 12 (3), mais de 12 horas (4), 5 pacientes não informaram o tempo decorrido entre o acidente e o atendimento. Manifestações clínicas observadas foram dor, edema local, isquemia transitória, parestesia, equimose e sensação de queimação local. O tratamento consistiu de antiinflamatórios e analgésicos. Em alguns casos, foram usados anestésicos, água morna, debridamento cirúrgico e anti-histamínicos. Em 75% dos casos, observou-se cura confirmada e em 12% a cura não foi confirmada, em dois a evolução foi ignorada. Provavelmente, o número de acidentes ocorridos é maior do que o encontrado devido a subnotificação.

This study assessed the number of CD4 T lymphocytes, the parasitemia and serum levels of interferon gamma (IFN-gamma), tumor necrosis factor alpha (TNF-alpha), interleukin-1 (IL-1), IL-4 and IL-10 of patients infected by human immunodeficiency virus (HIV) and human immunodeficiency virus/Chagas' disease coinfection. CD4 T lymphocytes were low in the two groups of patients, although significantly lower in patients without Chagas' disease. Serum levels of IFN-gamma, IL-4 and TNF-alpha were significantly higher in patients with HIV/Chagas' disease. IL-4/IFN-gamma ratios were higher in patients with HIV/Chagas' disease, which showed a clear balance in favor of Th2-like cytokines in this group of patients. This Th2 balance was higher in patients with detectable parasitemia. We conclude that, although immunosuppression was observed, with CD4 T lymphocytes bellow 200/µm³, these patients did not display reactivation of T. cruzi infection and that a balance favorable to Th2 was associated with the presence of parasitemia.
Marcadores sorológicos das hepatites B e C em doadores de sangue do Hemocentro de Ribeirão Preto, SP

Este estudo envolveu 25.891 doadores de sangue que compareceram pela primeira vez ao Hemocentro de Ribeirão Preto, entre 23/06/1996 e 22/06/2001. Seu objetivo foi estudar a positividade de marcadores sorológicos das hepatites B e C em testes de triagem e estimar a prevalência de infecção atual ou pregressa pelos vírus de ambas as hepatites, através da análise dos resultados de testes confirmatórios. Estudaram-se dados do Hemocentro e do Hospital das Clínicas da Faculdade de Medicina, coletando-se informações referentes ao doador e aos resultados dos testes sorológicos. A população foi composta majoritariamente por homens (83,6%) de 26 a 45 (64%) anos de idade. Os valores de positividade nos testes da triagem foram 0,6% (IC95%: 0,54 0,72) para o HBsAg e 1,2% (IC95%: 1,02 1,28), para o anti-HCV. Os valores da prevalência, nos testes confirmatórios, foram 0,2% (IC95%: 0,16 0,28), para a hepatite B, e 0,3% (IC95%: 0,24 0,38) para a hepatite C.

O laboratório do Pólo da Alta Sorocabana, Presidente Prudente, SP e Instituto Biológico de São Paulo, SP, realizaram avaliação do diagnóstico laboratorial da raiva no período de 1996 a 2003 na região oeste do Estado de São Paulo. Para tal, se fez uso dos testes de imunofluorescência direta e prova biológica (inoculação em camundongos) em 4.950 amostras encaminhadas para análise envolvendo as espécies canina, felina, bovina, quiróptera (morcego) e outras (eqüina, caprina, suína e roedores). Detectou-se a presença de 74 amostras positivas, sendo que destas, 58 (78,4%) foram referentes a quirópteros não hematófagos e 16 (21,6%) para bovinos. O presente estudo epidemiológico constatou que, apesar do alto índice de positividade nos quirópteros, não houve um surto de raiva nestas espécies na região de Presidente Prudente no período estudado, porque o aumento no índice de positividade foi decorrente do significativo aumento de amostras de quirópteros encaminhadas ao laboratório para pesquisa do vírus rábico.

The objective of the present study was to analyze HCV serological and virological parameters from hemophiliacs in the State of Bahia. Anti-HCV was investigated by ELISA in a cohort of 268 hemophiliacs A/B who were followed-up in a reference unit for hemotherapy in the State of Bahia. HCV viremia and genotypes were also determined from a subset of 66 anti-HCV seropositive hemophiliacs. Seroprevalence among hemophiliacs was 42.2% (95% CI 36.5-48.1) and was significantly higher (p<0.05) according to age >10 years, presence of factor VIII/IX inhibitory antibodies and other infection markers. None of the hemophiliacs less than 5 years of age were anti-HCV seropositive. Viremia was detectable in 77.3% (51/66). HCV genotype 1 (74%) was the most prevalent followed by genotype 3 (22%) and genotype 2 (4%). Our results indicate that HCV prevalence is still high among hemophiliacs, although HCV transmission was not observed in young hemophiliacs.

Em abril/2004, um grupo de pessoas realizou passeio a um sítio em cidade no baixo sul da Bahia e em seguida três adolescentes de uma mesma família apresentaram septicemia grave com óbito de dois destes. Objetivando identificar o evento, o Serviço de Vigilância procedeu à investigação epidemiológica, clínica, laboratorial e ambiental, identificando a Chromobacterium violaceum em material biológico de um dos pacientes que evoluíram para óbito e na água e solo do local do passeio. Esta é o primeira descrição desta doença na Bahia.
