The objective of this work was to evaluate the effect of pond management on fish feed, growth, yield, survival, and water and effluent quality, during tambaqui (Colossoma macropomum) juvenile production. Fish were distributed in nine 600 m² earthen ponds, at a density of 8 fish per m²; the rearing period was 60 days. Three different pond management were applied: limed and fertilized (LimFer), limed (Lim), and natural (Nat). Fish were fed with a commercial ration containing 34% crude protein three times daily. There were no significant differences in fish growth or yield. Three main items found in tambaqui stomach were insect, zooplankton and ration, without a significant difference among treatments in proportion. Alkalinity, hardness, and CO2 were greater in LimFer and Lim ponds. Chlorophyll a, transparency, ammonia, nitrite, temperature, and dissolved oxygen of pond water were not significantly different among treatments. Biochemical oxygen demand, total phosphorus, orthophosphate, ammonia, and nitrite were significantly greater in effluents from LimFer ponds. Pond fertilization should be avoided, because growth and yield were similar among the three pond management systems tested; besides, it produces a more impacting effluent.

OBJETIVO: Avaliar os aspectos ultra-sonográficos da glândula tireóide em pacientes portadores de tireoidites. MATERIAIS E MÉTODOS: Num período de nove meses, foram estudados 38 pacientes atendidos no Serviço de Ultra-sonografia do Instituto de Radiologia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, com diagnóstico prévio de tireoidite. Trinta e seis deles eram do sexo feminino, cujas idades variaram entre 17 e 78 anos. As variáveis observadas e estudadas foram: o volume glandular, a ecogenicidade e a ecotextura, e a presença de linfonodos na cadeia VI cervical (pré-traqueal, o sítio de drenagem linfática preferencial da tireóide) e suas dimensões. RESULTADOS: Treze pacientes apresentavam bócio. A análise ao ultra-som mostrou que 37 das 38 glândulas apresentavam ecotextura heterogênea com hipoecogenicidade difusa. Todos os pacientes apresentavam alterações nos exames laboratoriais compatíveis com tireoidites. Foram encontrados linfonodos na cadeia cervical VI (pré-traqueal) em 28 pacientes, todos de aspecto reacional. Destes, dez foram submetidos a punção aspirativa por agulha fina e o resultado citológico foi de reação inflamatória. Não foram encontrados casos de tireoidites focais que pudessem simular nódulos. CONCLUSÃO: Podemos inferir que para auxiliar no diagnóstico das tireoidites os achados ultra-sonográficos de heterogeneidade e hipoecogenicidade glandular, associados aos linfonodos na cadeia cervical VI, são de grande importância quando correlacionados aos exames clínicos e laboratoriais.

Objetivo: Avaliar a dose glandular média utilizando um detector de estado sólido e a qualidade da imagem de um sistema de radiografia direta a partir de objetos simuladores. Materiais e Métodos: Irradiações foram realizadas utilizando o controle automático de exposição e placas de polimetilmetacrilato com diferentes espessuras para o cálculo da dose glandular média. A qualidade da imagem foi avaliada por meio das estruturas visualizadas nas imagens dos objetos simuladores. Resultados: Considerando a incerteza das medições, os resultados de dose glandular média estão de acordo com os valores fornecidos pelo equipamento e com os níveis de referência adotados internacionalmente. Os resultados obtidos a partir das imagens dos objetos simuladores estavam em conformidade com os valores de referência. Conclusão: Este trabalho contribui para verificar a conformidade do equipamento em relação a dose e qualidade da imagem.

Abstract Objective: Derive filtered tungsten X-ray spectra used in digital mammography systems by means of Monte Carlo simulations. Materials and Methods: Filtered spectra for rhodium filter were obtained for tube potentials between 26 and 32 kV. The half-value layer (HVL) of simulated filtered spectra were compared with those obtained experimentally with a solid state detector Unfors model 8202031-H Xi R/F & MAM Detector Platinum and 8201023-C Xi Base unit Platinum Plus w mAs in a Hologic Selenia Dimensions system using a direct radiography mode. Results: Calculated HVL values showed good agreement as compared with those obtained experimentally. The greatest relative difference between the Monte Carlo calculated HVL values and experimental HVL values was 4%. Conclusion: The results show that the filtered tungsten anode X-ray spectra and the EGSnrc Monte Carlo code can be used for mean glandular dose determination in mammography.

The hexane and methanolic extracts from pheromonal glands of Castnia licus (Drury) virgin females have been studied. Analyses by gas chromatography and mass spectrometry allowed us to determine the major constituents present in the hexane extract as n-alkanes C21 to C30, (Z)-9-hexadecenoic acid (C16), and (Z)-9-octadecenoic acid (C18) and hexadecanoic acid (C16). Aldehyds, alkenes and acetates were also detected in low concentrations in the extracts. Female pheromone glands were analysed for pheromone precursors using the methanolic extract. In addition to the compounds methyl hexadecanoate and methyl (Z)-9-octadecenoate, the glandular tissue contains a homologous series of methyl esters from C12 to C24. The hexane extract of the female abdomenal glands elicited activity from males in a behavioural bioassay.

The construction and analytical evaluation of a coated graphite Al(III) ion-selective electrode, based on the ionic pair formed between the Al(F)n3-n anion and tricaprylylmethylammonium cation (Aliquat 336S) incorporated on a poly(vinylchloride) (PVC) matrix membrane are described. A thin membrane film of this ionic pair and dibutylphthalate (DBPh) in PVC was deposited directly on a cylindric graphite rod (2 cm length x 0.5 cm diameter) attached to the end of a glass tube using epoxy resin. The membrane solution was prepared by dissolving 40% (m/m) of PVC in 10 mL of tetrahydrofuran following addition of 45% (m/m) of DBPh and 15% (m/m) of the ionic pair. The effect of membrane composition, fluoride concentration, and several concomitants as potential interferences on the electrode response were investigated. The aluminium(III) ion-selective electrode showed a linear response ranging from 1.4 x 10-4 to 1.0 x 10-2 mol L-1, a detection limit of 4.0 x 10-5 mol L-1, aslope of -54.3±0.2mV dec-1 and a lifetime of more than 1 year (over 3000 determinations for each membrane). The slope indicates that the ion-selective electrode responds preferentially to the Al(F)4- species. Application of this electrode for the aluminium(III) determination in stomach anti-acid samples is reported.

The Baccharis genus is represented by more than 500 species distributed mainly in the tropical areas of South America. Many of them are extensively used in folk medicine in the treatment or prevention of anemias, inflammations, diabetes and stomach, liver and prostate diseases. Phytochemical and biological investigations in about 120 species resulted mainly in the isolation of clerodane and labdane diterpenes and flavonoid aglicones with the flavone unit being the most frequent.

By mid 2003, the Brazilian people accompanied astonished, in the press, the news about the death of more than 20 persons due to ingestion of a pharmaceutical product containing a suspension of barium sulfate (Celobar®) commonly used as a radiological contrast. Analysis of the product indicated the presence of barium carbonate (about 13% weight/weight) which reacts easily with the hydrochloric acid in the stomach liberating barium ions, a severe poison. In this article, we briefly discuss the possible economic, personal and technical causes that led to this disaster.

Nitrite, which is present in preserved meat and can be produced in the oral cavity by reduction of nitrate taken from vegetables, could react in stomach with nitrosatable drugs, giving genotoxic-carcinogenic N-nitroso compounds (NOC). The mutagenicity of reaction mixtures formed by sodium nitrite and selected sulfa-drugs (sulfathiazole, HST; phtalylsulfathiazole, PhST; complex Co(II)-sulfathiazole, Co(II)-ST) in acidic medium was evaluated using the Salmonella typhimurium reverse mutation assay (Ames test), with TA98 and TA 100 strains. The reactions were carried out at room temperature, with a mole ratio [nitrite]/[sulfa-drug] > 1. The three reaction mixtures showed mutagenic effects in the considered range.

Adenocarcinomas of the cardia and gastroesophageal junction are peculiar entities with three different origins, which differ somewhat from other adenocarcinomas of the stomach in their clinical presentation and pathogenesis, and have a poorer prognosis. In this article the authors reviewed definitions, incidence and epidemiology, etiologic factors, genetic implications, clinical presentation, diagnosis, staging and treatment, with emphasis on the surgical approach, discussing the current management of these cancers. The prognostic factors related specifically to the cardia cancers are: esophageal invasion greater than 3cm, microscopic residual tumor and wall penetration (>T2). Preoperative workup should include computed tomography, and endoscopic ultrasonography and laparoscopy when available. Preoperative recognition of T3/ T4/N2 lesions should indicate inclusion in neo-adjuvant protocols whenever possible. The authors present the results of 46 resected cases of adenocarcinomas of the cardia and GE junction of the Instituto Nacional do Câncer- Brazil (1981-1995). Cure was intended in 29 and palliation in 17 patients. The most common type of resection was total gastrectomy with abdominal esophagectomy (28 cases). Morbidity (major and minor) occurred in 50% of the patients. The main causes were of respiratory origin and fistulas (19.6% each). Death occurred in 44% of the patients with fistula. Postoperative death until the 30th day occurred in 17.24% of the curative cases and in 23.52% of the palliative ones. The median survival time was 68.5 months for stage I, 25 months for stage II, 31 months for stage III and 12.5 months for stage IV diseases. The median survival time was 8 months for palliation and 28.5 months for cure. No long-term survival was obtained with the palliative group, whereas 25% survived five years of more in the curative group. The authors conclude that the surgical approach should be the one the surgeon feels more comfortable with. Complete removal of the disease proved by frozen section, splenectomy and D2 lymphadenectomy should be the standard therapy with curative intent.

The authors present a rare case of pathology from spleen, a nonparasitic, non-neoplastic splenic cyst, in an adult male patient without a previous history of abdominal trauma. Principal clinical findings were pain in left upper quadrant, nausea, regurgitation and precocious satiation. On investigation, ultrasonography and CT scan showed an expansive lesion in the spleen, compressing the stomach. Surgical procedure was splenectomy and a microscopical examination revealed a non- parasitic, non-neoplastic splenic cyst. Splenic cysts are usually benigns, but surgical remotion is necessary when they are symptomatic or exist risk of rupture. In this report is presented the classification of splenic cysts, clinical manifestations, diagnostics methods and discussed their surgical treatment.

Gastric bezoars are impactations offoreign material in lhe stomach. When they are caused by hail; they are named tricho- bezoars. The complications oftrichobezoars are very rare. In this papel; we describe a case of a 16-year-old girl that had a previous history oftricophagia, and had an acute abdominal pain with a pneumoperitoneum in the abdomen radiography. An operation was performed and a gastric perforation was founded associated with a giant trichobezoa7: The trichobezoar was removed by traction through a gastrostomy which was performed in order to remove lhe trichobezoa7: Some fragments of the ulcer were obtained to histological study. The gastrostomy was treated by a gastrorraphy confection. In lhe post- operative period a left subfrenic abscess was revealed and has been drained by laparatomy 15 days after the fisrt operation. After the second surgical procedure the patient had a good evolution, and left the hospital in good health conditions.

Bezoars are uncommon foreign bodies found in the stomach and intestines. They are usually secundary to "strange" or "weird" alimentary habits. The contents may include hait; stones, vegetal fibers and others. Diagnosis is generally made due to complications, mainly parcial or complete obstruction of the segment affected. Bleeding and peiforation may also occur: This paper describes a case of a 14-years-old female patient, who presented herself to lhe Emergency Room and was diagnosed as having a gastric peiforation due to a trichobezoar that was 15 cm long and weighted 900g. A review of lhe literature and comments about diagnosis and management are presented.

Ginecomastia é o aumento da mama masculina que pode acometer até 65% dos indivíduos deste sexo na fase infanto-puberal, compreendida entre 13 e 16 anos. Tem como principais causas hepatite ou cirrose hepática, carcinoma ou doenças inflamatórias pulmonares crônicas, carcinomas ou disfunções testiculares, tumores glandulares (pituitária, supra-renal), alterações dos níveis séricos de testosterona, síndromes genéticas (síndrome de Klinefelter, p.ex.), uso de drogas como heroína, maconha ou anabolizantes e hanseníase. Podemos classificar a ginecomastia quanto ao volume, quanto aos tecidos que a compõem (gordurosa ou pseudoginecomastia, glandular e mista), ou quanto ao tratamento necessário para sua correção cirúrgica (pequena, moderada e grave). O tratamento das formas mais graves de ginecomastia é muito diferente daquele aplicado às formas mais suaves, pois nas formas graves, além da ressecção dos tecidos gorduroso e glandular, existe a necessidade de ressecção da pele em excesso e o reposicionamento do complexo aréolo-mamilar. O objetivo deste trabalho é descrever uma técnica cirúrgica específica para estes pacientes portadores de formas graves de ginecomastia, através de dois pedículos dermogordurosos, um lateral e um medial, com aproximadamente 2cm de espessura, mantendo assim a nutrição do complexo aréolo-mamilar. Esses pedículos são delimitados entre as bissetrizes dos quadrantes súpero-lateral e ínfero-lateral, e súpero-medial e ínfero-medial, tendo o mamilo como vértice. Na área de pele excessiva periareolar, obtida através do pinçamento interdigital, é realizada a desepidermização dos pedículos lateral e medial e ressecção de toda pele e tecido celular subcutâneo até a fáscia peitoral nas regiões superior e inferior aos pedículos; a síntese é realizada em dois planos, sendo periareolar a cicatriz resultante. Foram operados com esta técnica vinte pacientes com forma grave de ginecomastia, com média etária de 23,3 anos; sendo seis pacientes da raça negra. O bom posicionamento do complexo aréolo-mamilar e uma cicatriz periareolar resultante, bem como a retirada de conteúdo suficiente, foram as principais vantagens observadas. Como complicações, tivemos assimetria das placas aréolo-mamilares em dois casos, nos quais havia acentuada diferença entre os dois lados na avaliação pré-operatória; cicatrização hipertrófica em um paciente da raça negra, cuja cicatriz foi atenuada com injeções intracicatriciais de triancinolona; necrose parcial de aréola em um caso, cuja ferida cicatrizou por segunda intenção, dispensando qualquer tratamento local posterior; deiscência de sutura periareolar em um caso, no qual foi feita a ressutura, com bom resultado, e quatro pacientes apresentaram coleção sero-hemática subcutânea, que foram drenadas e não apresentaram recidiva.

Laparoscopic techniques have provided a new dimension to correct functional disorders of the esophagus, which has stimulated some investigators to recently report the use of laparoscopic cardiomyotomy in the treatment of esophageal achalasia. Now, a new instrument has been added to the current laparoscopic technique to offer a safer and easier method to proceed complete myotomy. After the dissection of the esophagogastric junction, a special catheter is introduced reaching the stomach. lt has an illuminated 10 cm extremity connected to a light source. lts withdrawal allows to visualize every muscle circular fiber by transillumination withan improved view provide by the laparoscopic optic system lens. This condition modifies the operative surgeon s attitude offering a better controlled situation over the procedure. The use of transillumination o fthe esophagogastric junction provides a good identification of the mucosa e submucosa avoiding the risk of esophageal perforation. It also helps to perform a complete myotomy preventing the ocurrence of persistent disphagia in the postoperative period. Cardiomyotomy with parcial fundoplication is possible by videolaparoscopic approach, now made easier with transillumination. This technique is safe and the functional results are similar to those observed in the literature for conventional open procedures, with the obvious advantages of the minimally invasive approach.
