Chest pain in children and adolescents is a frequent observation, although potentially relevant disease is rather rare and then found in situations with acute presentation. In children with an inflammatory/infectious clinical context the differential diagnosis is oesophagitis, pleuropneumonia or pericarditis. Potentially dangerous complications may be found in youth with predisposing conditions for aortic dissection, pneumothorax or pulmonary embolism, or even in rare instances for an acute coronary complication. In these cases aggressive diagnostic work-up is mandatory. In the frequent elective outpatient evaluation of teenagers with long-lasting episodes of chest pain, relevant underlying cardiovascular disease only rarely can be found as the cause. In the elective outpatient evaluation for chest pain, usually patient history and clinical examination may be enough to track the problem, the main role of the physician is to provide reassurance with minimal but appropriate testing.

In recent years, a growing number of reports in the literature have linked osteonecrosis of the jaw bones with intravenously administered bisphosphonates prescribed for the treatment of hypercalcemia of malignancy due to bone lesions of multiple myeloma or bone metastases in patients with breast or prostate cancer. Furthermore, an association between chronic oral bisphosphonate use in patients with osteoporosis or Paget's disease, and bone necrosis in the mandible or maxilla has been demonstrated in numerous case reports and case series in the last couple of years. Therapeutically, osteonecrosis of the jaws seems to be difficult to treat surgically, often resulting in a recurring or even progressing lesion. In the present case report of a bisphosphonate-associated osteonecrosis of the maxilla in a patient with osteoporosis, the current literature will be discussed, and open research questions and potential problems for our daily dental practice routine will be addressed.

Objective: This case report describes the use of serial measurements of peripheral venous and abdominal fluid hematocrits as a guide to treatment in a dog with traumatic hemoabdomen. Material and methods: An 18-month-old, female mixed-breed dog was presented with traumatic hemoabdomen. The dog was treated with intravenous fluids, and concurrent measurements of venous and abdominal fluid hematocrits were performed. Results: The initial hematocrit values were 39% in venous blood and 42% in abdominal fluid. Following fluid therapy, the venous hematocrit decreased to 20%, whereas the abdominal hematocrit remained stable at 40%. Based on this finding severe ongoing intra-abdominal hemorrhage was ruled out, and the dog was treated conservatively. Conclusion and clinical relevance: Comparison of serial abdominal and venous hematocrits may be useful in assessing the need for surgical management in patients with traumatic hemoabdomen.
