Nurses globally are required and expected to report nursing errors. As is clearly demonstrated in the international literature, fulfilling this requirement is not, however, without risks. In this discussion paper, the notion of ‘nursing error’, the practical and moral importance of defining, distinguishing and disclosing nursing errors and how a distinct definition of ‘nursing error’ fits with the new ‘system approach’ to human-error management in health care are critiqued. Drawing on international literature and two key case exemplars from the USA and Australia, arguments are advanced to support the view that although it is ‘right’ for nurses to report nursing errors, it will be very difficult for them to do so unless a non-punitive approach to nursing-error management is adopted.

The use of practical learning experience in undergraduate degree programs offers students the opportunity to apply their knowledge and receive feedback in a supportive environment before entering the workplace or undertaking further study. Traditional laboratory based instruction allows students this opportunity; however, it tends to provide limited opportunity for students to explore creative solutions to problem solving. The University of Waikato recently established undergraduate degree programs in engineering and has aimed to incorporate flexible learning opportunities for students, as part of their degree and as extra-curricular activities. This paper presents some of the practical project based opportunities that have been adopted and examines the role these have played in a developing engineering program.

A review of ethical literature demonstrates that the material presented to date is largely based upon theoretical and empirical research. While this information has contributory value, the information produced is largely observational rather than practical. Managers are anxious to receive assistance with the mechanisms by which ethics can be integrated into their organisations. Utilising the recent experience of the author with a large utility company in Asia committed to developing an ethical programme to enhance ethical awareness in their organisation, this paper intends to review current systems and procedures available to managers for integrating ethics into business. In addition to reviewing mechanisms for promoting an ethical climate, where appropriate, reference will be made to prior research and specific organisations where these practices have been used successfully. The paper concludes with a set of summary recommendations for managers embarking on the introduction of an ethical programme to their organisation.

The decision-making process in business frequently involves ethical considerations. Although ethics often come down to personal decisions, those decisions ultimately affect the corporate image of an organisation. It has been shown that sound ethics are good for business, and therefore it is important that managers encourage their staff to recognise and to implement the company's ethical priorities. This article looks at practical ways for managers to establish ethical priorities at three levels: an individual level, a group level and an organisational level. Each of these levels is explored, and the benefits and disadvantages of different action-related strategies for encouraging an ethical awareness will vary among companies, industries and cultures – no single method is universally appropriate. However, all managers should consider how ethical standards can best be introduced and communicated throughout their organisation. Ethics is not just a “flavour of the month” – it is here to stay, and presents a challenge to all managers.

While most people agree that the inculcation of ethical awareness is desirable, the means of stimulating this awareness vary among companies, industries and cultures. The fundamental question surrounding the difference between social responsibility and ethics is addressed. Guidelines for establishing ethical priorities from both the individual, group and organisational perspectives are provided.

Explains the tradition of Christian mystical theology and ecndevours to highlight its contemporary importance and relevance. Michael Ramsey, Archbishop of Canterbury, 1961-1974, was seriously concerned with mystical theology and argued for its greater appreciation within the Church in the West.

This paper presents laboratory experiments to test a bottom up approach to production control and supply chain management. Built upon the successful traditional kanban (Card) system, the new intelligent system associates a kanban agent to each physical kanban. Instead of relying on demand forecast and planning, kanban agents reason about their own movements to adapt to changing demands. After previous simulations results of the intelligent system showed significant performance improvements over the traditional system, we further use the Auto-ID Laboratory at Cambridge University to test the feasibility of the idea in a realistic manufacturing environment. The results from the experiments demonstrated the superiority on several performance measures of the intelligent system compared to the traditional system used as a benchmark. Moreover, the implementation of the experiments exposed several real world constraints not shown in the simulation study and practical solutions were adopted to address these.

The roles, patterns of use and emphases of practical activities depend fundamentally on teachers' backgrounds and beliefs. Students' intense engagement with real objects was found to encourage exploration and enjoyment, and support recall. The study also identified the variety and value of science conducted outside the normal classroom.

The nature, consequences and benefits of practice-based learning in architecture are explored nationally / transnationally from both student and practitioner perspectives. The findings suggest that the strengths of learning in practice contexts should be utilised and better integrated with formal learning at university for Australian architectural education in the 21st century.

Practical lithium-metal batteries are the ultimate goal of battery researchers. The addition of a zwitterionic compound (see Figure) to an ionic liquid electrolyte doped with a lithium salt results in a 100% enhancement of the current densities achieved in the cycling of a lithium-metal cell. This phenomenon arises due to increased lithium-ion mobility or a reduced solid electrolyte interphase layer resistance.

This article documents an action research (AR) project aimed at identifying the practical steps needed to become an agile manufacturer through a combination of the theory of constraints (TOC) and resource- based view (RBV) approaches in a small to medium enterprise (SME) in the Australian manufacturing sector. To date, lean production has been highlighted as a possible catalyst for creating an agile manufacturer, despite the evidence suggesting that lean manufacturing lacks the responsiveness and adaptability to effectively handle a rapidly changing market place and only works well in a stable environment. A more flexible system of production is required to fully encompass the agile characteristics needed to attain a competitive advantage. This research provides empirical evidence that the TOC perspective can be used as a practical approach for becoming an agile manufacturer. The study provides a workable approach for small firms to achieve ‘Agility’ in practice.
