In this postgraduate thesis the focus is on two documentary films, Renaud Brothers's Dope Sick Love (2005) and Joonas Neuvonen's Reindeerspotting (2010). In both of these films, addicts are on limelight. The directors of both films follow addicts and their everyday lives on streets with handheld cameras. This thesis will analyze and compare the narratives of these addicts. Do these documentaries affirm to the existing conventions, according to which representations of addicts and addiction are always either miraculous survival stories or stories, which always end up in utter decadence. The main questions this thesis proposes, are how addicts and their narratives are being handled in the two case study films, and how the drug cultures depicted in the films differ from each other. What changes between New York City and Rovaniemi, Northern Finland, and what remains the same. The academic and theoretical framework of this thesis consists of Susanna Helke's doctoral thesis Nanookin jälki, where the history of documentary films' methods and approaches are discussed. Additionally, the thinking is heavily informed by such poststructuralist writers as Roland Barthes, Michel Foucault, Louis Althusser, Stuart Hall and Chris Weedon.

As human action, drinking alcohol or smoking cigarettes, for instance, are phenomena that are familiar to most people. We recognise when people use drugs or gamble whether that happens in the television or around us. Sometimes we call these actions addictions. Addictive actions are actions that puzzle us. This puzzlement has raised different kinds of views on addiction that describe the phenomenon in different ways. The proponents of the views pick features that they consider to be sufficient in capturing the phenomenon. The disease view emphasises that addicted individuals are not in control over their own actions, whereas the choice view highlights addicted individuals’ capacity to act according to their own preferences. Some see addiction as a defect of will and addictive action is a manifestation of that. Sometimes they all insist on referring to the same group of people and describing the same actions with seemingly contradictory terms. What happens when an addicted individual acts in accordance with his addiction? This thesis also tries to answer this question and make this kind of action and agency understandable. By showing that these three common views fall short of capturing the phenomenon, I will provide characteristics that jointly suffice for something to be labelled addiction, but which are not, however, individually necessary (or sufficient) for addiction. Those characteristics are strong desire, myopia, biased decision-making, and weakness of will. Furthermore, they should be understood in the framework of diachronic, active agency. They contribute to a view on addictive action that consists of different kinds of actions. Acknowledging the variety of nuanced human action, the understanding of addictive action is increased and this may also be utilised in addiction-related policies and treatment. The emphasis of my study lies specifically on understanding addiction as action by the means of analytical philosophy.

Addiction, and the experience of being addicted, is notoriously difficult to describe verbally and explain rationally. Would multifaceted and multisensory cinematic images work better in making addiction understandable? This study enquires how cinematic expression can render visible the experience of being addicted which is invisible as such. The basic data consists of circa 50 mainly North American and European fiction films from the early 1900s to the early 2000s that deal with addictive disorders as defined in the psychiatric DSM-V classification (substance dependence- and gambling disorders). The study develops an approach for analyzing and interpreting a large volume of digital film data: digital cinematic iconography is a framework to study the multifaceted cinematic images by processing and viewing them in the “digital image-laboratory” of the computer. Images are cut and classified by editing software and algorithmic sorting. The approach draws on early 1900s German art historian Aby Waburg’s image research and media archaeology, that are connected to film studies inspired by the phenomenology of the body and Gilles Deleuze’s film-philosophy. The first main chapter, “Montage”, analyses montage, gestural and postural images, and colors in addiction films. The second main chapter, “Thingness”, focuses on the close-ups of material objects and faces, and their relation to the theme of spirituality in cinema and art history, The study argues that the cinema engages the spectator to "feel" what addiction is through everyday experience and art historical imagery. There is a particular, historically transmitted cinematic iconography of addiction that is profane, material, thing-centered, abject, and repetitive. The experience of being addicted is visualized through montages of images characterized by dark and earthy colors, horizontal compositions and downward- directed movements. This is very profane and secular imagery that, however, circulates image-historical traces of Christian iconography, such as that of being in the grip of an unknown power.

Työssä tarkasteltiin tupakkalain vaikutuksia ravintoloiden ilmanvaihtoon ja työntekijöiden tupakansavulle altistumiseen. Lisäksi tutkittiin, mitä muita määräyksiä Suomessa ja muualla on ravintoloiden ilmanvaihdosta ja minkälaisin ilmanvaihdollisin menetelmin voidaan ravintolatyöntekijöiden altistumista tupakansavulle vähentää. Kokeellisena osana tutkittiin Halton Oy:n kehittämän baaritiskin paikallisilmanvaihtojärjestelmän suojaustehokkuutta ja toimivuutta suomalaisessa yökerhossa. Järjestelmään kuului kattoon sijoitettu kohdepuhallus ja baaripöytään sijoitettu ilmaverholaitteisto. Mittaukset suoritettiin käytännön olosuhteissa toimivassa ravintolassa. Mitattuina suureina olivat sisäilman nikotiini- ja hiukkaspitoisuus työskentelyalueella ja asiakkaiden oleskeluvyöhykkeellä. Lisäksi käytettiin merkkiainemenetelmää tupakansavun leviämisen seuraamiseen. Saavutettu suojaustehokkuus oli mittaustavasta ja tilanteesta riippuen 72-95%. Mittaukset osoittivat, että tutkitulla paikallisilmanvaihtoratkaisulla voitiin hyvin oleellisesti vähentää ympäristön tupakansavun kulkeutumista työntekijöiden hengitysvyöhykkeelle. Paikallisilmanvaihto on kuitenkin toteutettava osana koko ravintolan ilmanvaihtoa ja säädettävä oikein hyvän tuloksen saavuttamiseksi.

Background: Measurement of serum cotinine, a major metabolite of nicotine, provides a valid marker for quantifying exposure to tobacco smoke. Exposure to tobacco smoke causes vascular damage by multiple mechanisms, and it has been acknowledged as a risk factor for atherosclerosis. Multifactorial atherosclerosis begins in childhood, but the relationship between exposure to tobacco smoke and arterial changes related to early atherosclerosis have not been studied in children. Aims: The aim of the present study was to evaluate exposure to tobacco smoke with a biomarker, serum cotinine concentration, and its associations with markers of subclinical atherosclerosis and lipid profile in school-aged children and adolescents. Subjects and Methods: Serum cotinine concentration was measured using a gas chromatographic method annually between the ages 8 and 13 years in 538-625 children participating since infancy in a randomized, prospective atherosclerosis prevention trial STRIP (Special Turku coronary Risk factor Intervention Project). Conventional atherosclerosis risk factors were measured repeatedly. Vascular ultrasound studies were performed among 402 healthy 11-year-old children and among 494 adolescents aged 13 years. Results: According to serum cotinine measurements, a notable number of the school aged children and adolescents were exposed to tobacco smoke, but the exposure levels were only moderate. Exposure to tobacco smoke was associated with decreased endothelial function as measured with flow-mediated dilation of the brachial artery, decreased elasticity of the aorta, and increased carotid and aortic intima-media thickness. Longitudinal exposure to tobacco smoke was also related with increased apolipoprotein B and triglyceride levels in 13-year-old adolescents, whose body mass index and nutrient intakes did not differ. Conclusions: These findings suggest that exposure to tobacco smoke in childhood may play a significant role in the development of early atherosclerosis. Key Words: arterial elasticity, atherosclerosis, children, cotinine, endothelial function, environmental tobacco smoke, intima-media thickness, risk factors, ultrasound

Pathological gambling, a form of behavioral addiction, refers to maladaptive, compulsive gambling behavior severely interfering with an individual’s normal life. The prevalence of pathological gambling has been estimated to be 1–2% in western societies. The reward deficiency hypothesis of addiction assumes that individuals that have, or are prone, to addictions have blunted mesolimbic dopamine reward signaling, which leads to compulsive reward seeking in an attempt to compensate for the malfunctioning brain reward network. In this research project, the effects of gambling were measured using brain [11C] raclopride PET during slot machine gambling and possible brain structural changes associated with pathological gambling using MRI. The subjects included pathological gamblers and healthy volunteers. In addition, impulse control disorders associated with Parkinson’s disease were investigated by using brain [18F]fluorodopa PET and conducting an epidemiological survey. The results demonstrate mesolimbic dopamine release during gambling in both pathological gamblers and healthy volunteers. Striatal dopamine was released irrespective of the gambling outcome, whether the subjects won or not. There was no difference in gambling induced dopamine release between pathological gamblers and control subjects, although the magnitude of the dopamine release correlated with gambling related symptom severity in pathological gamblers. The results also show that pathological gambling is associated with extensive abnormality of brain white matter integrity, as measured with diffusion tensor imaging, similar to substance-addictions. In Parkinson’s disease patients with impulse control disorders, enhanced brain [18F] fluorodopa uptake in the medial orbitofrontal cortex was observed, indicating increased presynaptic monoamine function in this region, which is known to influence signaling in the mesolimbic system and reward processing. Finally, a large epidemiological survey in Finnish Parkinson’s disease patients showed that compulsive behaviors are very common in Parkinson disease and they are strongly associated with depression. These findings demonstrate the role of dopamine in pathological gambling, without support for the concept of reward deficiency syndrome.

Tutkimus on lähtökohdiltaan kvalitatiivinen.
