Randomized, controlled trials have demonstrated efficacy for second-generation antipsychotics in the treatment of acute mania in bipolar disorder. Despite depression being considered the hallmark of bipolar disorder, there are no published systematic reviews or meta-analyses to evaluate the efficacy of modern atypical antipsychotics in bipolar depression. We systematically reviewed published or registered randomized, double-blind, placebo-controlled trials (RCTs) of modern antipsychotics in adult bipolar I and/or II depressive patients (DSM-IV criteria). Efficacy outcomes were assessed based on changes in the Montgomery-Asberg Depression Rating Scale (MADRS) during an 8-wk period. Data were combined through meta-analysis using risk ratio as an effect size with a 95% confidence interval (95% CI) and with a level of statistical significance of 5% (p<0.05). We identified five RCTs; four involved antipsychotic monotherapy and one addressed both monotherapy and combination with an antidepressant. The two quetiapine trials analysed the safety and efficacy of two doses: 300 and 600 mg/d. The only olanzapine trial assessed olanzapine monotherapy within a range of 5-20 mg/d and olanzapine-fluoxetine combination within a range of 5-20 mg/d and 6-12 mg/d, respectively. The two aripiprazole placebo-controlled trials assessed doses of 5-30 mg/d. Quetiapine and olanzapine trials (3/5, 60%) demonstrated superiority over placebo (p<0.001). Only 2/5 (40%) (both aripiprazole trials) failed in the primary efficacy measure after the first 6 wk. Some modern antipsychotics (quetiapine and olanzapine) have demonstrated efficacy in bipolar depressive patients from week 1 onwards. BACKGROUND: Deficits in memory and executive performance are well-established features of bipolar disorder and schizophrenia. By contrast, data on cognitive impairment in schizoaffective disorder are scarce and the findings are conflicting. METHOD: We used the Wechsler Memory Scale (WMS-III) and the Behavioural Assessment of the Dysexecutive Syndrome (BADS) to test memory and executive function in 45 schizophrenic patients, 26 schizomanic patients and 51 manic bipolar patients in comparison to 65 healthy controls. The patients were tested when acutely ill. RESULTS: All three patient groups performed significantly more poorly than the controls on global measures of memory and executive functioning, but there were no differences among the patient groups. There were few differences in memory and executive function subtest scores within the patient groups. There were no differences in any test scores between manic patients with and without psychotic symptoms. CONCLUSIONS: Schizophrenic, schizomanic and manic patients show a broadly similar degree of executive and memory deficits in the acute phase of illness. Our results do not support a categorical differentiation across different psychotic categories with regard to neuropsychological deficits.

Cognitive impairment in schizophrenia and psychosis is ubiquitous and acknowledged as a core feature of clinical expression, pathophysiology, and prediction of functioning. However, assessment of cognitive functioning is excessively time-consuming in routine practice, and brief cognitive instruments specific to psychosis would be of value. Two screening tools have recently been created to address this issue, i.e., the Brief Cognitive Assessment Tool for Schizophrenia (B-CATS) and the Screen for Cognitive Impairment in Psychiatry (SCIP). The aim of this research was to examine the comparative validity of these two brief instruments in relation to a global cognitive score. 161 patients with psychosis (96 patients diagnosed with schizophrenia and 65 patients diagnosed with bipolar disorder) and 76 healthy control subjects were tested with both instruments to examine their concurrent validity relative to a more comprehensive neuropsychological assessment battery. Scores from the B-CATS and the SCIP were highly correlated in the three diagnostic groups, and both scales showed good to excellent concurrent validity relative to a Global Cognitive Composite Score (GCCS) derived from the more comprehensive examination. The SCIP-S showed better predictive value of global cognitive impairment than the B-CATS. Partial and semi-partial correlations showed slightly higher percentages of both shared and unique variance between the SCIP-S and the GCCS than between the B-CATS and the GCCS. Brief instruments for assessing cognition in schizophrenia and bipolar disorders, such as the SCIP-S and B-CATS, seem to be reliable and promising tools for use in routine clinical practice.

En este artículo se pretende mostrar cómo la utilización de métodos visuales en la investigación contribuye a potenciar la participación activa de las personas con TMG. Se utiliza como ejemplo un estudio de caso de corte cualitativo que incorpora tres actividades de componente visual (el dibujo “el río de la vida”, las fotografías y el dibujo de proyección de futuro) para favorecer la reflexión narrada que, sobre sus experiencias y vivencias, desarrollan cinco personas con TMG. El uso de las fotografías y dibujos en este estudio permite afirmar que estas estrategias se han mostrado válidas para acceder, en la medida que los participantes han querido, a esferas de vida personales en trayectorias vitales determinadas por la enfermedad mental

Possible new ways in the pharmacological treatment of bipolar disorder and comorbid alcoholism. Azorin JM, Bowden CL, Garay RP, Perugi G, Vieta E, Young AH. Source Department of Psychiatry, CHU Sainte Marguerite, Marseilles, France. Abstract About half of all bipolar patients have an alcohol abuse problem at some point of their lifetime. However, only one randomized, controlled trial of pharmacotherapy (valproate) in this patient population was published as of 2006. Therefore, we reviewed clinical trials in this indication of the last four years (using mood stabilizers, atypical antipsychotics, and other drugs). Priority was given to randomized trials, comparing drugs with placebo or active comparator. Published studies were found through systematic database search (PubMed, Scirus, EMBASE, Cochrane Library, Science Direct). In these last four years, the only randomized, clinically relevant study in bipolar patients with comorbid alcoholism is that of Brown and colleagues (2008) showing that quetiapine therapy decreased depressive symptoms in the early weeks of use, without modifying alcohol use. Several other open-label trials have been generally positive and support the efficacy and tolerability of agents from different classes in this patient population. Valproate efficacy to reduce excessive alcohol consumption in bipolar patients was confirmed and new controlled studies revealed its therapeutic benefit to prevent relapse in newly abstinent alcoholics and to improve alcohol hallucinosis. Topiramate deserves to be investigated in bipolar patients with comorbid alcoholism since this compound effectively improves physical health and quality of life of alcohol-dependent individuals. In conclusion, randomized, controlled research is still needed to provide guidelines for possible use of valproate and other agents in patients with a dual diagnosis of bipolar disorder and substance abuse or dependence.

Supported Employment (SE) is a clear alternative to the work inclusion of people with severe mental disorder (SMD). This article has two purposes: first, to show the possibilities of application of the SE with people with SMD, secondly, to establish the main lines that are configured as elements favoring the inclusion processes of this group work by SE. A piece of research has been carried out based on interviews of professionals with experience in supporting the work inclusion of people with SMD, focusing on the key factors that these professionals perceive as key elements for successful work inclusion processes. The awareness of the disease by the worker with SMD, the organization of the support processes throughout the insertion process, the relationship with the company as well as with the family and the characteristics of the health network are among the factors that, according to the research results, appear to be key factors for successful and less successful work inclusion processes. The information obtained provides insight into how people with SMD develop work inclusion processes with supported employment and help us to suggest some strategies to improve these processes

Background: The relevance of persistent cognitive deficits to the pathogenesis and prognosis of bipolar disorders (BD) is understudied, and its translation into clinical practice has been limited by the absence of brief methods assessing cognitive status in Psychiatry. This investigation assessed the psychometric properties of the Spanish version of the Screen for Cognitive Impairment in Psychiatry (SCIP-S) for the detection of cognitive impairment in BD. Methods: After short training, psychiatrists at 40 outpatient clinics administered the SCIP three times over two weeks to a total of 76 consecutive type I BD admissions. Experienced psychologists also administered a comprehensive battery of standard neuropsychological instruments to clinical sample and 45 healthy control subjects. Results: Feasibility was supported by a brief administration time (approximately 15 minutes) and minimal scoring errors. The reliability of the SCIP was confirmed by good equivalence of forms, acceptable stability (ICC range 0.59 to 0.87) and adequate internal consistency (Chronbach's alpha of 0.74). Construct validity was granted by extraction of a single factor (accounting 52% of the variance), acceptable correlations with conventional neuropsychological instruments, and a clear differentiation between bipolar I and normal samples. Efficiency was also provided by the adequate sensitivity and specificity. Limitations: The sample size is not very large. The SCIP and the neurocognitive battery do not cover all potentially relevant cognitive domains. Also, sensitivity to change remains unexplored. Conclusion: With minimal training, physicians obtained a reliable and valid estimate of cognitive impairment in approximately 15 minutes from an application of the SCIP to type I BD patients.

Per a poder comprendre la dimensió de les possibles transformacions que Internet pot comportar, cal investigar els usos concrets de què es o ha estat objecte. En definitiva, cal realitzar investigacions empíriques que aportin informació sobre qui usa Internet, en quines circumstàncies i amb quins objectius. En aquest context, s’ha iniciat l’anàlisi en profunditat d’un cas específic de comunitat virtual de suport social creada per una persona afectada pel trastorn bipolar. L’objectiu d’una comunitat d’autoajuda virtual és proporcionar informació i recolzament emocional a través d’Internet. Mitjançant una metodologia qualitativa s’ha realitzat l’observació del fòrum virtual que penja de la pàgina web “Bipoloarweb.com”. En les investigacions del fenomen dels grups d’autoajuda a Internet, sovint s’ha destacat que a diferència dels grups ‘presencials’, aquests només poden aportar recolzament emocional i informatiu als seus membres. El tipus de recolzament instrumental o altre tipus d’assistència física, en canvi, no és possible en els casos virtuals. Els primers resultats de la recerca invaliden aquesta afirmació general ja que s’ha pogut observar episodis diferents d’ajut instrumental. En relació a la gestió d’informació i producció de coneixement, ja es pot avançar algunes qüestions interessants. En primer lloc, la quantitat i el detall de la informació que en el fòrum circula sobre el trastorn Bipolar. La majoria d’aquests coneixements, però, sorgeixen directament de compartir l’experiència diària entre els membres del grup. Tot això permet avançar la següent hipòtesi de treball pel futur: a més a més de suport emocional i instrumental, aquest grup ‘empodera’ el seus membres? Si la resposta fos positiva, el nostre cas tindria semblances amb altres fenòmens com son les associacions de malalts.

El deliri és un trastorn neuropsiquiàtric que poden patir els pacients greus a les unitats de cures intensives. La seva aparició s’associa amb un augment de la morbimortalitat. La seva incidència varia entre el 20-80% segons els mètodes diagnòstics i la situació clínica del pacient. L’objectiu principal del estudi fou avaluar la incidència de delirium en la Unitat de Reanimació de l’Hospital Universitari Germans Trias i Pujol mitjançant el test ICDSC i associar el delirium amb la edat, severitat del pacient, urgència de la cirurgia, especialitat quirúrgica i opinió del equip d‘infermeria sobre el test. Durant 3 mesos es van incloure 50 pacients que van ser avaluats amb el ICDSC dues vegades al dia, durant 5 dies. La incidència de delirium a la unitat fou del 18%. Es va trobar una associació estadísticament significativa entre delirium i la edat i el APACHE II score al ingrés. És recomanable monitoritzar diàriament el delirium i tractar-lo com a un signe vital més. El test ICDSC és un test senzill i fàcil que ens ajuda a no infravalorar el delirium del pacient crític.

En aquesta investigació, hem comparat tres mostres (amb diagnòstic de transtorn de la personalitat, amb trets disfuncionals de la personalitat i sans) per tal de valorar quina disposa d’un funcionament parental més acurat. Dels resultats obtinguts, malgrat el petit tamany de la mostra, podem afirmar que els progenitors sans obtenen millors resultats que els altres dos, i que els que disposen de trets disfuncionals tenen un funcionament més adequats que els que tenen un diagnòstic.

INTRODUCCION: La Acalasia es un trastorno motor primario causado por la pérdida selectiva de las motoneuronas del plexo mientérico esofágico que ocasiona aumento en la presión basal y relajación incompleta del esfínter esofágico inferior (EEI), y la desaparición de la peristalsis esofágica. OBJETIVOS. Correlacionar los síntomas clínicos de los pacientes con alteraciones manométricas y los hallazgos morfológicos (radiología y endoscopia) en una cohorte de pacientes con Acalasia. PACIENTES Y MÉTODOS. Estudio retrospectivo de 37 pacientes, 22 (59.4%) hombres y 15 (40,5%) mujeres, con una edad media de 57,45 años con Acalasia estudiados entre 2000-2009, mediante evaluación clínica según escalas de Zanitoto, Atkinson y clasificación manométrica de acuerdo a los criterios de Pandolfino. RESULTADOS. Todos los pacientes consultaron por disfagia esofágica de 34,7 meses de evolución en promedio, moderada en 15 pacientes (40,5%, necesidad de agua para pasar), y en 8 (21,6%) con obstrucción severa. En el momento del diagnóstico el 35% de los pacientes requerían dieta triturada y el 35% sólo podían pasar líquidos; 14 pacientes presentaban pérdida de peso (37,8%). Los estudios morfológicos sólo orientaron el diagnóstico de Acalasia en un 48,6% de los pacientes (endoscopia sugestiva en un 37,8% y TGE en el 10,8%). El 70.2% de los pacientes presentaron un patrón manométrico típico y un 13.5% un patrón de Acalasia vigorosa. El 31% de los pacientes presentaron una pH-metría de 24 h con reflujo gastroesofágico patológico y un 10,5% un patrón sugestivo de retención y fermentación esofágica. CONCLUSIÓN. El diagnóstico de los pacientes con Acalasia es tardío, y se realiza con síntomas clínicos severos de disfagia esofágica en los que las pruebas de diagnóstico morfológico (TEGD, endoscopia) ofrecen un bajo rendimiento. La manometría y pH-metría de 24 h ofrece un diagnóstico precoz y preciso, el tipo de Acalasia y de la existencia de reflujo gastroesofágico o acidificación por retención. La información proporcionada por estas exploraciones funcionales debe ser tenida en cuenta para indicar el tratamiento de los pacientes con Acalasia.

La Esclerosis Sistémica (ES) es un trastorno generalizado del tejido conectivo caracterizado por el engrosamiento y la fibrosis tanto de la piel como de órganos internos asociándose a daño vascular. Tradicionalmente, la afectación vascular en la ES ha sido considerada principalmente microvascular (1). Sin embargo, existe evidencia reciente que muestra que la ES podría también asociarse a lesión macrovascular (2) debido a la situación de inflamación crónica sostenida que predispondría al incremento de la rigidez de la pared arterial (3) y el desarrollo de arteriosclerosis. No obstante, y a pesar de diversos intentos en estudios realizados en la última década, la fisiopatología completa de los mecanismos subyacentes permanece todavía sin esclarecer. El objetivo de nuestro trabajo ha sido analizar; por un lado, dentro de una población de pacientes con ES, una serie de parámetros para evaluar la prevalencia de enfermedad cardiovascular subclínica y la existencia de factores de riesgo cardiovascular y; por otra parte, comparar esos hallazgos frente a una población sana en busca de diferencias que apoyen la hipótesis existente en la actualidad de que, ciertas enfermedades autoinmunes predisponen al desarrollo de arteriosclerosis precoz (3,4).

El transtorn de l’olfacte a la malaltia d’Alzheimer (MA) és freqüent i precoç. Per determinar la relació entre la MA i el volum del bulb i tracte olfactori, és necessari un instrument que ens permeti realitzar el càlcul d’aquest volum a partir d’imatges obtingudes per ressonància magnètica. S’ha desenvolupat un software específic i s’ha realitzat un estudi observacional i transversal de fiabilitat en una mostra de pacients. S’ha calculat el volum de forma semiautomàtica per dos observadors independents. El coeficient de correlació intraclasse obtingut ens permet concloure que la fiabilitat interexaminador és apropiada.
