Purpose: Samples from different environmental sources were screened for the presence of Aspergillus, and the distribution of the different species-complexes was determined in order to understand differences among that distribution in the several environmental sources and which of these species complexes are present in specific environmental settings. Methods: Four distinct environments (beaches, poultries, swineries and hospital) were studied and analyzed for which Aspergillus complexes were present in each setting. After plate incubation and colony isolation, morphological identification was done using macro- and microscopic characteristics. The universal fungal primers ITS1 and ITS4 were used to amplify DNA from all Aspergillus isolates, which was sequenced for identification to species complex level. SPSS v15.0 for Windows was used to perform the statistical analysis. Results: Thirty-nine isolates of Aspergillus were recovered from both the sand beach and poultries, 31 isolates from swineries, and 80 isolates from hospital environments, for a total 189 isolates. Eleven species complexes were found total. Isolates belonging to the Aspergillus Versicolores species-complex were the most frequently found (23.8%), followed by Flavi (18.0%), Fumigati (15.3%) and Nigri (13.2%) complexes. A significant association was found between the different environmental sources and the distribution of the several species-complexes (p<0.001); the hospital environment had a greater variability of species-complexes than other environmental locations (10 in hospital environment, against nine in swine, eight in poultries and seven in sand beach). Isolates belonging to Nidulantes complex were detected only in the hospital environment, whereas the other complexes were identified in more than one setting. Conclusion: Because different Aspergillus complexes have different susceptibilities to antifungal drugs, and different abilities in producing mycotoxins, knowledge of the species-complex epidemiology for each setting may allow preventive or corrective measures to be taken toward decreasing professional workers or patient exposure to those agents.

Organic waste is a rich substrate for microbial growth, and because of that, workers from waste industry are at higher risk of exposure to bioaerosols. This study aimed to assess fungal contamination in two plants handling solid waste management. Air samples from the two plants were collected through an impaction method. Surface samples were also collected by swabbing surfaces of the same indoor sites. All collected samples were incubated at 27◦C for 5 to 7 d. After lab processing and incubation of collected samples, quantitative and qualitative results were obtained with identification of the isolated fungal species. Air samples were also subjected to molecular methods by real-time polymerase chain reaction (RT PCR) using an impinger method to measure DNA of Aspergillus flavus complex and Stachybotrys chartarum. Assessment of particulate matter (PM) was also conducted with portable direct-reading equipment. Particles concentration measurement was performed at five different sizes (PM0.5; PM1; PM2.5; PM5; PM10). With respect to the waste sorting plant, three species more frequently isolated in air and surfaces were A. niger (73.9%; 66.1%), A. fumigatus (16%; 13.8%), and A. flavus (8.7%; 14.2%). In the incineration plant, the most prevalent species detected in air samples were Penicillium sp. (62.9%), A. fumigatus (18%), and A. flavus (6%), while the most frequently isolated in surface samples were Penicillium sp. (57.5%), A. fumigatus (22.3%) and A. niger (12.8%). Stachybotrys chartarum and other toxinogenic strains from A. flavus complex were not detected. The most common PM sizes obtained were the PM10 and PM5 (inhalable fraction). Since waste is the main internal fungal source in the analyzed settings, preventive and protective measures need to be maintained to avoid worker exposure to fungi and their metabolites.

O presente trabalho de projeto centra-se numa questão silenciosa na sociedade portuguesa: a violência no trabalho. A proposta apresentada visa dar visibilidade a um problema humano, social e económico com reflexos no quotidiano de milhares de pessoas em todo o mundo. Consciencializar as organizações e os/as trabalhadores/as para a necessidade de atitudes preventivas é o principal objetivo da estratégia de comunicação aqui apresentada. Esta estratégia baseia-se numa revisão de literatura e analise da situação da violência no trabalho em Portugal.

Clinical and environmental samples from Portugal were screened for the presence of Aspergillus and the distributions of the species complexes were determined in order to understand how their distributions differ based on their source. Fifty-seven Aspergillus isolates from clinical samples were collected from 10 health institutions. Six species complexes were detected by internal transcribed spacer sequencing; Fumigati, Flavi, and Nigri were found most frequently (50.9%, 21.0%, and 15.8%, respectively). β-tubulin and calmodulin sequencing resulted in seven cryptic species (A. awamorii, A. brasiliensis, A. fructus, A. lentulus, A. sydowii, A. tubingensis, Emericella echinulata) being identified among the 57 isolates. Thirty-nine isolates of Aspergillus were recovered from beach sand and poultry farms, 31 from swine farms, and 80 from hospital environments, for a total 189 isolates. Eleven species complexes were found in these 189 isolates, and those belonging to the Versicolores species complex were found most frequently (23.8%). There was a significant association between the different environmental sources and distribution of the species complexes; the hospital environment had greater variability of species complexes than other environmental locations. A high prevalence of cryptic species within the Circumdati complex was detected in several environments; from the isolates analyzed, at least four cryptic species were identified, most of them growing at 37ºC. Because Aspergillus species complexes have different susceptibilities to antifungals, knowing the species-complex epidemiology for each setting, as well as the identification of cryptic species among the collected clinical isolates, is important. This may allow preventive and corrective measures to be taken, which may result in decreased exposure to those organisms and a better prognosis.

A Organização Mundial da Saúde define a literacia em saúde como o conjunto de competências cognitivas e sociais e a capacidade dos indivíduos para compreenderem e usarem informação para a promoção e manutenção da saúde. A transmissão de informação sobre saúde é mais eficaz quando os seus conteúdos são especificamente desenhados para uma pessoa ou para um grupo populacional e quando a mensagem é bem delimitada, realçando os benefícios (ganhos) e os custos (perdas) associados aos comportamentos e às tomadas de decisão. Analisa-se, neste estudo, o conceito de literacia em saúde e a associação da baixa literacia em saúde aos comportamentos em saúde e aos gastos em saúde. Apresenta-se uma análise da literatura científica publicada sobre a baixa literacia em saúde e a sua implicação nos custos na saúde usando, para este objectivo, uma base de dados das ciências da saúde (MEDLINE/PubMed) e quatro plataformas científicas (DOAJ, SCOPUS, SciELO e Web of Science). A literatura científica analisada evidencia que pessoas com baixa literacia em saúde apresentam uma menor capacidade de compreensão dos conteúdos de material informativo sobre alimentos, doenças crónicas ou sobre o uso de medicamentos, por exemplo, bem como maior dificuldade em pesquisar, seleccionar, ler e assimilar a informação em saúde disponível na Internet. A baixa literacia em saúde relaciona-se, então. com a dificuldade na prevenção e na gestão de problemas de saúde, bem como com comportamentos ineficazes de saúde, i.e., com o uso inadequado de medicamentos, com o recurso excessivo aos serviços de saúde (em especial, os de urgências) ou com a ineficácia em lidar com situações de emergência. A baixa literacia está também associada a taxas de hospitalização mais altas, mas também mais longas no tempo (o que implica mais custos associados a internamento prolongado, mais exames de diagnóstico e fraca adesão à terapêutica medicamentosa), a uma diminuição da utilização de medidas preventivas e a uma fraca adesão à prescrição terapêutica. A baixa literacia acaba por afectar igualmente a comunicação (e a relação) médico-doente. Apresentam-se, como complemento, sugestões de melhoria da literacia em saúde e da comunicação médico-doente para efeitos da promoção da saúde.

Cork is a light, porous and impermeable material extracted from the bark of some trees. It is in manufacture of stoppers for wine bottles the main application of cork. It is estimated that the area occupied by cork oaks in the Iberian Peninsula is around 33% in Portugal and 23% in Spain. The world production of cork is focused in the south Europe, with Portugal being the most important producer followed by Spain. According to Companies Directory more than 100 manufactories from Portugal has their branch associated with the preparation and fabrication of cork. Cork workers are at risk for developing diseases of the respiratory tract such as occupational asthma and Suberosis, a form of pulmonary hypersensitivity due to repeated exposure to mouldy cork dust. In this review study papers from 2000 were analyzed to better understand which fungi species are associated with occupational disease in cork workers. The most prevalent fungi species in these workers that are associated with those occupational diseases are Penicilliumglabrum, Chrysoniliasitophila and Trichodermalongibrachiatum. Therefore, a specific knowledge about occupational exposure to fungi in the cork industry is the key to better understand the related diseases and to define preventive measures. Given the importance of this occupational setting in Portugal is essential to evaluate the combined exposure of fungi and particles and their metabolites. Further studies concerning exposure assessment to fungi and particles in the cork industry must be developed.

Aflatoxin B1 (AFB1) is considered by different International Agencies as a genotoxic and potent hepatocarcinogen. However, despite the fact that the fungi producing this compound are detected in some work environments, AFB1 is rarely monitored in occupational settings. The aim of the present investigation was to assess exposure to AFB1 of workers from one Portuguese waste company located in the outskirt of Lisbon. Occupational exposure assessment to AFB1 was done with a biomarker of internal dose that measures AFB1 in the serum by enzyme-linked immunosorbent assay. Forty-one workers from the waste company were enrolled in this study (26 from sorting; 9 from composting; 6 from incineration). A control group (n = 30) was also considered in order to know the AFB1 background levels for the Portuguese population. All the workers showed detectable levels of AFB1 with values ranging from 2.5ng ml−1 to 25.9ng ml−1 with a median value of 9.9±5.4ng ml−1. All of the controls showed values below the method’s detection limit. Results obtained showed much higher (8-fold higher) values when compared with other Portuguese settings already studied, such as poultry and swine production. Besides this mycotoxin, other mycotoxins are probably present in this occupational setting and this aspect should be taken into consideration for the risk assessment process due to possible synergistic reactions. The data obtained suggests that exposure to AFB1 occurs in a waste management setting and claims attention for the need of appliance of preventive and protective safety measures.
