BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) is a common cause of complicated skin and skin-structure infection (cSSSI). Increasing antimicrobial resistance in cSSSI has led to a need for new safe and effective therapies. Ceftaroline was evaluated as treatment for cSSSI in 2 identical phase 3 clinical trials, the pooled analysis of which is presented here. The primary objective of each trial was to determine the noninferiority of the clinical cure rate achieved with ceftaroline monotherapy, compared with that achieved with vancomycin plus aztreonam combination therapy, in the clinically evaluable (CE) and modified intent-to-treat (MITT) patient populations. METHODS: Adult patients with cSSSI requiring intravenous therapy received ceftaroline (600 mg every 12 h) or vancomycin plus aztreonam (1 g each every 12 h) for 5-14 days. RESULTS: Of 1378 patients enrolled in both trials, 693 received ceftaroline and 685 received vancomycin plus aztreonam. Baseline characteristics of the treatment groups were comparable. Clinical cure rates were similar for ceftaroline and vancomycin plus aztreonam in the CE (91.6% vs 92.7%) and MITT (85.9% vs 85.5%) populations, respectively, as well as in patients infected with MRSA (93.4% vs 94.3%). The rates of adverse events, discontinuations because of an adverse event, serious adverse events, and death also were similar between treatment groups. CONCLUSIONS: Ceftaroline achieved high clinical cure rates, was efficacious against cSSSI caused by MRSA and other common cSSSI pathogens, and was well tolerated, with a safety profile consistent with the cephalosporin class. Ceftaroline has the potential to provide a monotherapy alternative for the treatment of cSSSI. O Complexo Burkholderia cepacia (CBc) é um grupo de 17 espécies intimamente relacionadas que estão associadas à deterioração pulmonar e aumento da mortalidade em pacientes com Fibrose Cística (FC). Essas espécies variam entre si em relação à prevalência, quadros clínicos e virulência. Pouco é conhecido em relação ao perfil de resistência aos antimicrobianos. Uma vez estabelecida a infecção, a abordagem terapêutica e as medidas de controle atualmente adotadas são baseadas no CBc, sem considerar cada espécie em particular. O objetivo deste estudo foi determinar a prevalência das espécies do CBc em pacientes atendidos em dois centros de referência no Rio de Janeiro, bem como estabelecer perfis de resistência a antimicrobianos e avaliar a diversidade molecular entre as espécies. Cem amostras do CBc isoladas de 38 pacientes com FC no período de janeiro de 2010 a fevereiro de 2012 foram identificadas por métodos fenotípicos e pelo sequenciamento do gene recA. As CIMs para amicacina, aztreonam, ceftazidima, trimetoprim/sulfametoxazol e tobramicina foram determinadas por microdiluição e a genotipagem das espécies foi realizada por PFGE com a enzima SpeI. B. vietnamiensis (44%) foi a espécie mais prevalente, seguida de B. cenocepacia IIIA (36%), B. multivorans (10%), B. cenocepacia IIIB (1%) e B. stabilis (1%). Cinco por cento das amostras não foram identificadas. B. vietnamiensis foi identificada em mais da metade dos pacientes (58,3%). Foram observadas diferenças no perfil de susceptibilidade entre as espécies do CBc. B. cenocepacia IIIA foi a espécie que apresentou as maiores taxas de resistência aos antimicrobianos, sobretudo para trimetoprim/ sulfametoxazol (80,5%), principal antimicrobiano utilizado no tratamento de infecções causadas pelo CBc. Amostras com perfis MDR ocorreram em todas as espécies, destacando-se o perfil A, resistente simultaneamente aos cinco antimicrobianos, observado em 58,8% das amostras de B.cenocepacia IIIA. A análise do polimorfismo genético mostrou que, apesar de B. vietnamiensis ter sido a espécie mais prevalente, a ocorrência de nove grupos clonais sugere que a aquisição dessas cepas tenha se dado a partir de uma fonte ambiental comum. Para B. cenocepacia IIIA, 52,9% das amostras foram atribuídas a um mesmo grupo clonal (BcA), compartilhado entre nove pacientes atendidos em um mesmo centro de referência. Oitenta por cento dessas amostras apresentaram ainda resistência a todos os antimicrobianos testados. Os dados mostram que, mesmo com o emprego de técnicas moleculares, é difícil a identificação do CBc em nível de espécie; que B. cenocepacia IIIA é caracterizada por índices de resistência superiores às outras espécies e que a transmissão cruzada entre os indivíduos aponta para a necessidade do estabelecimento de medidas de vigilância do CBc nos centros de referência.

A contaminação dos alimentos e transmissão de patógenos via manipuladores pode ocorrer se as condições de higiene, armazenamento e preparo não estiverem satisfatórios. Procedimentos inadequados de manipulação oferecem um perigo potencial à saúde pública devido ao alto risco de ocorrência de doenças de origem alimentar por microrganismos resistentes aos antimicrobianos. Neste estudo foram coletados swabs das mãos de 49 manipuladores de alimentos, isoladas e identificadas por metodologia convencional 244 enterobactérias de 7 cantinas permissionárias em uma Universidade do Município do Rio de Janeiro. Foram utilizados discos contendo os seguintes antimicrobianos: aztreonam, tobramicina, ceftazidima, cloranfenicol, tetraciclina, sulfa, amicacina, ampicilina e sulbactam, ciprofloxacina, gentamicina, cefalotina, cefepime, cefoxitina, imipenem, ampicilina, cefotaxima. A pesquisa de genes que codificam β-lactamases foi realizada pela Reação em Cadeia da Polimerase para os genes blaTEM, blaSHV, blaCTX, blaOXA-1 e blaCMY-2. As espécies mais prevalentes identificadas foram: Enterobacter cloacae (21,3%), Citrobacter braakii (15,2%), Escherichia coli (12,7%), Klebsiella pneumoniae subesp. Pneumoniae (12,2%). O perfil de resistência aos antimicrobianos revelou que 94 (38,5%) cepas identificadas apresentaram resistência a pelo menos dois antibióticos, 55 cepas (22,5%) a pelo menos um antibiótico, 34 cepas (13,9%) a três antibióticos, 14 cepas (5,7%) mostraram-se resistentes a quatro antibióticos. Uma cepa de E. cloacae mostrou resistência múltipla a dez antibióticos e 140 cepas (57,3%) foram resistentes simultaneamente a cefalotina e cefoxitina. Nenhuma cepa foi resistente ao aztreonam e a sulfa e apenas 01 foi resistente ao ciprofloxacina, 01 ao imipenem e 02 a cefotaxima. Foi identificado a presença do gene blaSHV em uma cepa de K. pneumoniae subsp. pneumoniae. Este dado aponta para uma mudança no perfil dessas bactérias isoladas da comunidade que passam a ter características semelhantes com as de origem hospitalar. Os resultados alertam para um perigo à saúde dos consumidores e conseqüentemente à saúde pública, devido a presença de enterobactérias resistentes aos antimicrobianos nas mãos de manipuladores de alimentos.

A Trust strategy to reduce ciprofloxacin use was implemented at a University hospital. This study aimed to investigate whether the susceptibility of Gram-negative organisms (GNO) to alternative antimicrobials (co-amoxiclav, doxycycline, aztreonam, piperacillin/tazobactam, meropenem and gentamicin) changed, and whether there was any relationship between GNO susceptibility to these antimicrobials and ciprofloxacin usage.

Balanoposthitis is defined as the inflammation of the glans penis and its foreskin. In the presence of other underlying medical conditions, this localized infection may spread systemically, serving as a source of fever and bacteremia in neutropenic males. Two rare cases of balanoposthitis caused by a clonally related Pseudomonas aeruginosa isolate co-producing the SPM-1 metallo-beta-lactamase and the novel 16S rRNA methylase RmtD are described. Four multidrug-resistant (MDR) P. aeruginosa isolates were successively recovered from glans/foreskin swabs and urine cultures from two uncircumcised pediatric patients, one with Burkitt`s non-Hodgkin`s lymphoma and one with acute lymphoblastic leukemia. Clinically, preputial colonization by MDR P. aeruginosa evolved to severe balanoposthitis with glans/foreskin lesions as a source of fever. Combination therapy of ciprofloxacin and/or aztreonam (systemic) plus polymyxin B (topical) was effective once reversion of the neutropenic condition was achieved. Although P. aeruginosa remains an unusual cause of balanoposthitis, these cases should alert the physician to the potential pathogenicity of this bacterium. Furthermore, co-production of metallo-beta-lactamase and 16S rRNA methylase has a potential impact on the empirical management of complicated infections caused by P. aeruginosa. Since 1988 to 1992, a study about susceptibility to antimicrobial drugs of bacterias isolated from hospitalized patients was performed. The compared susceptibility to important drugs (ampicillin, cephalotin, cefoxitin, ceftaxizime, ceftriaxone, aztreonam, gentamicin, amikacin, pefloxacin, ciprofloxacin, imipenem, oxacillin and vancomycin) was investigated in 1200 strains (300 of each specie) of the prevalent bacterias: E. coli, Klebsiella pneumoniae, Pseudomonas aeruginosa and S. aureus. Minimal inhibitory concentration (MIC) was determined by agar dilution method, using from 0.05 to 256 mcg of each drug per ml of culture medium (Mueller-Hinton). Ranges of MIC, MIC(50%), MIC(90%) and the proportion of resistant strains were determined and permitted to know the 4 drugs that were found to be more active against bacterias; the CIM(90%) values are: E. coli - aztreonam (0.1 mcg/ml), pefloxacin (0.1), ceftazidime (0.25) and ceftriaxone (0.05); K. pneumoniae-aztreonam (0.25) ceftriaxone (0.25), ceftazidime (0.5) and pefloxacin (2.0); P. aeruginosa-imipenem (4.0), aztreonam (16), ceftazidime (16) and ciprofloxacin (16); S. aureus-vancomycin (1.01, ciprofloxacin (8, 0), amikacin (128) and cephalothin (128 mg/ml). The better 'in vitro' antibacterial activity observed was related to: aztreonam (77-100% of the sensitive strains), ceftazidime (50-99,7%), pefloxacin (73-99,7%), ciprofloxacin (80%), imipenem (93%) and vancomycin (100%).

Hundred and fifty frozen broiler carcasses of four commercial brands, purchased at retail stores for Salmonella research, were examined: 43 of the carcasses referred to each of the brands A, B, D and 21 of brand C. Thirty-two percent of the samples were found positive; 11 serotypes were identified as S. Agona, S. Anatum, S. Enteritidis, S. Hadar, S. Havana, S. Mbandaka, S. Montevideo, S. Ouakam, S. Poona, S. Schwarzengrund and S.14, 5, 12:-. Antibiogram testing of the isolated strains showed 100% resistance to ampicilin, 75.0% to cefhalotin, 52.1% to cephoxitin, 22.9% to tobramicin, 6.2% to polimixin B and to tetracyclines, 4.2% to gentamicin, and 2.1% to netilmicin, to aztreonam and to amicacin. All strains showed total sensibility to chloramphenicol and to sulfazotrim.

The aim of this study was the assessment of isolation frequency and antimicrobial susceptibility pattern of nonfermenting Gram-negative bacilli. Ninety eight strains of nonfermenting Gram-negative bacilli, isolated from several clinical materials of patients admited at the Dr. Domingos Leonardo Cerávolo University Hospital and at Dr. Odilo Antunes Siqueira State Hospital, as well as from every outpatient; assisted at Laboratory of Clinical Analysis of Unoeste University, Presidente Prudente, São Paulo, in the period of October 1999 to April 2001 were analyzed. The most frequent species were Pseudomonas aeruginosa (65.3%) and Acinetobacter baumannii (23.5%). The frequency of the other isolated species was smaller than 2.5%. In the antimicrobial susceptibility tests, the two species more prevalent showed high resistance. The antibiotic most active in vitro was the imipenem, with 79.6% in microdiluition method, and 76.6% in diffusion method, for Pseudomonas aeruginosa strains and 100.0% in both microdiluition and diffusion methods, for Acinetobacter baumannii. The cephalosporins of third generation, the ciprofloxacin and the aminoglycosides, presented percentage of susceptibility varying from 22.4 to 69.7%. These results bring implications to the emergency use of the antimicrobial agents in the treatment of patients with severe infection.

The aim of this research was to evaluate the susceptibility profile of Pseudomonas spp. and the prevalence of bacterial samples isolated from horizontal surfaces surrounding wash-basins used by dentists in several adjoined consulting-rooms, at points next to and at a distance from the basin, before and after surgical procedures. Our results showed a high percentage of Gram-positive cocci and Gram-negative bacilli; 34.66% were Staphylococcus spp. and 30.12% were non-fermentative Gram-negative bacilli among which Pseudomonas spp. (40.90%) was the commonest genus. Analysis of the susceptibility profile of Pseudomonas spp. isolates by determining the minimal inhibitory concentration (MIC) of 14 antibiotics showed a great variation among the strains and high rates of resistance to cefazolin, ceftazidime and aztreonan. Of the 14 antibiotics tested, 59.03% were found to be active against all the environmental isolates. Strains were resistant to aztreonan (62.82%), while susceptibility to third generation cephalosporins was variable.
