Assessment of laboratory test utilization for HIV/AIDS care in urban ART clinics of Lilongwe, Malawi

Background The 2011 Malawi HIV guidelines promote CD4 monitoring for pre-ART assessment and considering HIVRNA monitoring for ART response assessment, while some clinics used CD4 for both. We assessed clinical ordering practices as compared to guidelines, and determined whether the samples were successfully and promptly processed. Methods We conducted a retrospective review of all patients seen in from August 2010 through July 2011,, in two urban HIV-care clinics that utilized 6-monthly CD4 monitoring regardless of ART status. We calculated the percentage of patients on whom clinicians ordered CD4 or HIVRNA analysis. For all samples sent, we determined rates of successful labprocessing, and mean time to returned results. Results Of 20581 patients seen, 8029 (39%) had at least one blood draw for CD4 count. Among pre-ART patients, 2668/2844 (93.8%) had CD4 counts performed for eligibility. Of all CD4 samples sent, 8082/9207 (89%) samples were successfully processed. Of those, mean time to processing was 1.6 days (s.d 1.5) but mean time to results being available to clinician was 9.3 days (s.d. 3.7). Regarding HIVRNA, 172 patients of 17737 on ART had a blood draw and only 118/213 (55%) samples were successfully processed. Mean processing time was 39.5 days (s.d. 21.7); mean time to results being available to clinician was 43.1 days (s.d. 25.1). During the one-year evaluated, there were multiple lapses in processing HIVRNA samples for up to 2 months. Conclusions Clinicians underutilize CD4 and HIVRNA as monitoring tools in HIV care. Laboratory processing failures and turnaround times are unacceptably high for viral load analysis. Alternative strategies need to be considered in order to meet laboratory monitoring needs.

In this study we intend to address a learning experience that was inspired by the intersection between public art and art education. We intend to make a reflection and analysis of pedagogical actions undertaken during this experience related to a theoretical framework to help us evaluate and improve the pedagogical and methodological strategies. The experience began with a visit to the exhibition "Street Art" at the AXA building in Porto. This visit was made with a group of children of different ages, accompanied by their parents, in May 2014. This exhibition brought new gazes for urban art. By recognizing obvious benefits such as proximity and accessibility, urban art, so rarely addressed in the classroom, may be an educational resource. Our intention in this study asks whether the contact with urban works of art can be motivating for students and facilitators of learning. We used a methodology of action research to approach urban art in visual art education lessons, exploring various art objects of the exhibition "Street Art" and the surrounding city with students. A presente pesquisa pretende realizar um estudo sobre a arte urbana e algumas das questões que compõem seu panorama. Imergindo em sua recente história, a pesquisa propicia maior destaque para a arte urbana que surgiu a partir a década de 1970 com proeminente viés crítico político-social. A Arte Urbana é descrita e problematizada por meio da tensão entre o domínio público e o privado, da violência urbana e do sistema das artes, de modo a se alcançar uma melhor concepção sobre esse fenômeno cultural em crescente evolução. Em seguida percorro por minha trajetória na arte e descrevo minhas experiências com intervenções urbanas. Efetuando uma análise sobre a minha produção como artista urbano, que conta, predominantemente, com trabalhos que intentam estabelecer interlocução com a dinâmica da vida urbana, realçando problemas práticos do cotidiano, tais como a violência, o hiato entre Estado e população e a desigualdade social. Atribuindo o devido destaque ao projeto de intervenção urbana realizado junto a esta pesquisa de mestrado, denominado: "Status quo revolução", que possui como tema principal, a esperança poética de transformação, tendo o amor, e seu maior ícone, o coração, como símbolos

La investigación sobre el graffiti relacionado con el arte urbano, el diseño y la comunicación visual, la cultura social y política en la ciudad de Bogotá, son temas que concentran esta investigación. Opiniones y entrevistas fueron revelando la influencia sobre la comunidad, la sociedad civil, la política, y el enfrentamiento radical con la leyes, su clasificación y su exigencia según el estrato social han hecho de los espacios públicos de Bogotá una ciudad con paredes hablantes. Aunque no es tomado como algo malo, -para algunos- vandalismo para otros, despierta en muchos, opiniones diversas y sobre todo una subestimada valoración de los artistas hacia una expresión liberal y democrática de un sistema que los aleja de cualquier exhibición del arte tradicional.

Le graffiti est depuis longtemps associé au vandalisme dans les métropoles où il prolifère. Il s’intègre au paysage urbain à un point tel que, même s’il procède d’une logique de visibilité et de promotion, nous parvenons à ne plus le remarquer. Pourtant, sa présence suscite toujours la colère des citoyens propriétaires de murs vandalisés et des autorités municipales qui, chaque année, dépensent de grosses sommes d’argent pour l’effacer. Malgré les restrictions et les amendes, le graffiti est néanmoins devenu un véritable phénomène d’art urbain : des artistes graffiteurs ont atteint la notoriété en dehors de leur sous-culture et ont pu exposer légalement leur travail, du fait d’un intérêt croissant du milieu officiel de l’art. Celui-ci contribue à faire grandir l’engouement et l’enthousiasme pour une pratique d’art urbaine. Le graffiti illustre l’expression d’une identité qui s’approprie et subvertit les surfaces urbaines de la ville, au moyen d’un nom propre fictif qui n’a aucune légitimation juridique et légale. De plus, l’application et la diffusion de la signature graffitique communiquent des valeurs qui guident et définissent toute la sous-culture du graffiti. Ces valeurs sont culturellement considérées comme masculines : le risque, le défi et la dissidence. La figure de l’artiste graffiteur apparaît ainsi comme une figure marginale et rebelle. Un tel portrait laisse alors entrevoir une culture fortement machiste, d’autant plus que les garçons qui exercent le graffiti sont beaucoup plus nombreux. Or, si les femmes artistes de la communauté graffitique représentent une minorité, c’est notamment parce que leur attrait pour une pratique illicite, nocturne et dangereuse est moindre. À partir d’une approche qui touche aux gender studies et à certains concepts traditionnels de l’histoire de l’art, nous cherchons à expliquer l’intérêt généralement plus faible des femmes pour le graffiti. Nous désirons également démontrer comment l’exercice des artistes féminines du graffiti se distingue de celui de leurs homologues masculins. De quelle manière se détermine leur expérience? Leur iconographie sert-elle à les définir? Quelles sont leurs opinions et leurs perceptions sur leur propre culture? Finalement, comment caractérisent-elles leur statut de femme artiste au sein d’une communauté "machiste"?

La tesis indaga sobre la capacidad del arte como elemento de transformación de la ciudad. La práctica cada vez más común de la instalación de obras de arte efímero en el espacio público de las ciudades produce alteraciones reales del paisaje urbano. En el caso de la ciudad de Madrid la proliferación de obras de arte efímero en sus calles y plazas más céntricas se ha materializado de forma intensa desde los primeros años del siglo XXI. Este trabajo de investigación estudia un intervalo de cinco años representativos para Madrid (2007-2011) por la cantidad y la trascendencia de las intervenciones de arte público temporal y de arte urbano en su espacio público. Con la recopilación, el análisis, la categorización y evaluación de casi trescientos ejemplos de manifestaciones artísticas efímeras en Madrid se establecen conclusiones sobre la repercusión del arte en la transformación de ciertos procesos urbanos. La constatación de la existencia de un movimiento de arte urbano en el siglo XXI permite tratar un tema necesario en el momento actual: la revitalización del espacio urbano a través del arte. La tesis trata de describir conceptualmente una forma novedosa de arte que puede ser un motor de procesos participativos urbanos directos y decisorios. ABSTRACT This thesis makes a research about the ability of art as an element of transformation inside the city. The increasingly common practice of ephemeral art installations in urban public spaces is provoking real changes in the cities landscape. In the case of Madrid the proliferation of ephemeral artworks in the most busy streets and squares has been brought into life as an intense activity from the first years of the 21st century. This research paper studies a span of five years which have been quite representative to Madrid (2007-2011) because of the big amount of meaningful interventions that have taken place both as temporary art and as urban art in its public spaces. Through data-gathering, analysis, classification and assessment of almost three hundred examples of ephemeral art forms in Madrid, several conclusions can be stated on the impact of art in transforming certain urban processes. This evidence of the existence of an urban art movement in the 21st century opens a necessary debate subject at this moment: the regeneration of urban space through art. This thesis aims at conceptually describe this newborn artistic expressions of art, seen as a driving force for providing more direct and participatory urban decision-making processes.

“Kahvehane (Coffeehouse) Kongresspark”explores the question of transcultural relations and belonging through an artistic exploration of coffee and its cultural history. The project was commissioned as an urban art intervention by the 'Soho in Ottakring' Biennial in Vienna. Organic and fair-trade coffee, sponsored by Kaffee Alt Wien , was served free of charge to the public in specially designed cups and saucers, which were created at the Ceramics Research Centre at the University of Westminster. The designs reflect coffee’s shared multi-ethnic history and the globalization of coffee.The project includes an interactive lecture-performance exploring the many legends about the origins of coffee tracing its journey from Ethiopia, the Arabian peninsula through the Ottoman Empire (Turkey) to Vienna and across the entire globe. While adopting a humorous take on the globalization of coffee, the performance sheds light on the history of colonialism and question the conditions of contemporary trade. The performance was written and performed in collaboration with Bharatanatyam artist and actor Shane Shambhu.

The 2010 LAGI competition was held on three underutilized sites in the United Arab Emirates. By choosing Staten Island, New York in 2012 the competition organises have again brought into question new roles for public open space in the contemporary city. In the case of the UEA sites, the competition produced many entries which aimed to create a sculpture and by doing so, they attracted people to the selected empty spaces in an arid climate. In a way these proposals were the incubators and the new characters of these empty spaces. The competition was thus successful at advancing understandings of the expanded role of public open spaces in EAU and elsewhere. LAGI 2012 differs significantly to the UAE program because Fresh Kills Park has already been planned as a public open space for New Yorkers - with or without these clean energy sculptures. Furthermore, Fresh Kills Park is already an (gas) energy generating site in its own right. We believe Fresh Kills Park, as a site, presents a problem which somewhat transcends the aims of the competition brief. Advancing a sustainable urban design proposition for the site therefore requires a fundamental reconsideration of the established paradigms public open space. Hence our strategy is to not only create an energy generating, site specific art work, but to create synergy between the public and the site engagement while at the same time complement the idiosyncrasies of the pre-existing engineered landscape. Current PhD research about energy generation in public open spaces informs this work.
