Hazard perception in driving involves a number of different processes. This paper reports the development of two measures designed to separate these processes. A Hazard Perception Test was developed to measure how quickly drivers could anticipate hazards overall, incorporating detection, trajectory prediction, and hazard classification judgements. A Hazard Change Detection Task was developed to measure how quickly drivers can detect a hazard in a static image regardless of whether they consider it hazardous or not. For the Hazard Perception Test, young novices were slower than mid-age experienced drivers, consistent with differences in crash risk, and test performance correlated with scores in pre-existing Hazard Perception Tests. For drivers aged 65 and over, scores on the Hazard Perception Test declined with age and correlated with both contrast sensitivity and a Useful Field of View measure. For the Hazard Change Detection Task, novices responded quicker than the experienced drivers, contrary to crash risk trends, and test performance did not correlate with measures of overall hazard perception. However for drivers aged 65 and over, test performance declined with age and correlated with both hazard perception and Useful Field of View. Overall we concluded that there was support for the validity of the Hazard Perception Test for all ages but the Hazard Change Detection Task might only be appropriate for use with older drivers.

Most clinically-employed speech materials for testing hearing impaired individuals are recordings made by adult male talkers. The author examined the possible effect of talker age and gender on the speech perception of children through the use of 1) two speech perception tests, each with four talker types (adult males, adult females, 10-12 year olds, 5-7 year olds), and 2) two groups of pediatric listeners: normal-hearing (NH) and cochlear implant users (CI).

New technology in the Freedom (R) speech processor for cochlear implants was developed to improve how incoming acoustic sound is processed; this applies not only for new users, but also for previous generations of cochlear implants. Aim: To identify the contribution of this technology - the Nucleus 22 (R) - on speech perception tests in silence and in noise, and on audiometric thresholds. Methods: A cross-sectional cohort study was undertaken. Seventeen patients were selected. The last map based on the Spectra (R) was revised and optimized before starting the tests. Troubleshooting was used to identify malfunction. To identify the contribution of the Freedom (R) technology for the Nucleus22 (R), auditory thresholds and speech perception tests were performed in free field in soundproof booths. Recorded monosyllables and sentences in silence and in noise (SNR = 0dB) were presented at 60 dBSPL. The nonparametric Wilcoxon test for paired data was used to compare groups. Results: Freedom (R) applied for the Nucleus22 (R) showed a statistically significant difference in all speech perception tests and audiometric thresholds. Conclusion: The reedom (R) technology improved the performance of speech perception and audiometric thresholds of patients with Nucleus 22 (R).

The principal theme of this thesis is the effect of yoked prisms on body posture and egocentric perception. Yoked prisms have been clinically used in the management of a variety of visual and neuro-motor dysfunctions. Most studies have been conducted in pathological populations by studying the effects of prismatic adaptation, without distinguishing short and long term effects. In this study, postural and perceptual prismatic effects have been studied by preventing prism adaptation. A healthy population was selected in order to investigate the immediate prismatic effects, when there is no obvious benefit from their use for the individual. Posturography was used to assess changes in weight distribution and shifts in centre of pressure (barycentre). In addition, photographic analyses were used to assess effects on posture on the x and z axis. Experiments with space board and visual midline shift were used for the evaluation of spatial perception and egocentric localisation. One pair of 8 Δ yoked prisms base left (BL) and one pair of 8 Δ yoked prisms base up (BU) were applied randomly and compared to a pair of plano lenses. Results suggest that immediate prismatic effects take place on a perceptual level and are reflected on an altered body posture respectively without significant changes in weight distribution. Yoked prisms BL showed a rightward rotational effect on spatial perception by expanding space on the z axis when viewing through the base of the prism and constricting space through the apex of the prism. Body posture responded respectively to what was visually perceived by altering posture. A rightward shift and tilt of the head was recorded along with the hips shift and shoulders tilt in the dame direction. Additionally, right shoulder shifted backwards and an angular midline shift to the right was recorded. The egocentric localisation was affected by shifting the midline perception to the left. Yoked prisms BU resulted on a head shift forward and a reduction of the head-neck angle by bringing the chin closer to the chest. The egocentric localisation was altered on the vertical axis providing subjects the perception that their eye level was higher during the experiment. In conclusion, yoked prisms seemed to induce changes in body posture, mainly in the upper body and head, without any significant changes in weight distribution. These changes are partially reflected in spatial perception tests and egocentric localisation before any prismatic adaptation takes place.

Hazard perception has been found to correlate with crash involvement, and has thus been suggested as the most likely source of any skill gap between novice and experienced drivers. The most commonly used method for measuring hazard perception is to evaluate the perception-reaction time to filmed traffic events. It can be argued that this method lacks ecological validity and may be of limited value in predicting the actions drivers’ will take to hazards encountered. The first two studies of this thesis compare novice and experienced drivers’ performance on a hazard detection test, requiring discrete button press responses, with their behaviour in a more dynamic driving environment, requiring hazard handling ability. Results indicate that the hazard handling test is more successful at identifying experience-related differences in response time to hazards. Hazard detection test scores were strongly related to performance on a driver theory test, implying that traditional hazard perception tests may be focusing more on declarative knowledge of driving than on the procedural knowledge required to successfully avoid hazards while driving. One in five Irish drivers crash within a year of passing their driving test. This suggests that the current driver training system does not fully prepare drivers for the dangers they will encounter. Thus, the third and fourth studies in this thesis focus on the development of two simulator-based training regimes. In the third study participants receive intensive training on the molar elements of driving i.e. speed and distance evaluation. The fourth study focuses on training higher order situation awareness skills, including perception, comprehension and projection. Results indicate significant improvement in aspects of speed, distance and situation awareness across training days. However, neither training programme leads to significant improvements in hazard handling performance, highlighting the difficulties of applying learning to situations not previously encountered.

El presente estudio descriptivo de corte transversal, comparó la percepción de color (Test de Farnsworth-100 y de Ishihara) y la sensibilidad al contraste en un grupo de 35 ojos con implantes de lentes intraoculares claros esféricos y otro grupo de 35 ojos con LIOs amarillos esféricos, para evaluar las diferencias entre tener o no el filtro de luz azul. Adicionalmente, después de un examen oftalmológico completo en el ojo a estudiar, se realizaron preguntas de satisfacción POP para evaluar el resultado subjetivo del procedimiento. Todos los pacientes tenían más de 3 meses POP. No se encontraron diferencias estadísticamente significativas en las pruebas de percepción de color que sugirieran que las alteraciones correspondían al filtro de luz azul, y aunque los pacientes con los LIOs amarillos lograron un mejor desempeño en la prueba de sensibilidad al contraste al analizarlos estadísticamente, clínicamente esto no parece tener importancia. La satisfacción general en ambos grupos tuvo una gran puntuación, y dentro de las molestias referidas más frecuentemente y que estaban presentes desde el procedimiento, se encontraban ardor ocular, lagrimeo y dificultad para ver de cerca sin corrección. Según estudios en modelos animales, hay evidencia que sugiere que los LIOs amarillos, al compararlos con LIOs claros, protegen de mejor manera la retina una vez el cristalino se ha retirado. Según este y otros estudios publicados a la fecha, la agudeza visual, sensibilidad al contraste y percepción de color no varían de manera importante entre ambos tipos de lentes en la población general.

OBJETIVO: Avaliar a condição ocular em população de trabalhadores de uma indústria metalúrgica paulista. MÉTODOS: Amostra de 2516 funcionários de uma fábrica na cidade de São Paulo foi submetida a uma avaliação oftalmológica como exame ocupacional periódico. Foi aplicado um questionário e realizado o exame de acuidade visual (Snellen) e teste de Ishihara. Os funcionários com acuidade visual menor que 0,7 ou com diferenças maior que duas linhas ou que apresentassem alguma queixa ocular, passaram por avaliação complementar (biomicroscopia, refração, tonometria e fundo de olho). Prescreveu-se óculos conforme indicação. RESULTADOS: Houve predomínio do sexo masculino (62,5%) e faixa etária de 20 a 29 anos (41%). A maioria não apresentava antecedentes oculares (97,6%) ou sistêmicos (96,6%).A acuidade visual estava acima de 0,7 em 95,5% dos olhos e 84% não utilizavam correção. O diagnóstico final foi exame normal em 55% dos casos, presbiopia em 13.6%, astigmatismo miópico em 10% e hipermetrópico em 7,7% dos indivíduos. Baixa visão foi encontrada em 2,4%, cegueira unilateral em 0,4%, não havendo casos de cegueira ou de deficiência visual entre os trabalhadores. As principais causas de baixa visão foram erros refrativos e ambliopia. CONCLUSÃO: A maioria dos funcionários da indústria pesquisada apresentava exame oftalmológico normal e nenhum deles apresentava cegueira bilateral.

Several methods are used towards delayed onset muscle soreness appraisal. This study's goal was to investigate, amongst three commonly adopted methods, which one would be the most effective (the one that shows higher values) in the quantification of this phenomenon. 10 male subjects, of age 22.8 ± 3.1 years old, weight 80 ± 12.4 kg, height 174 ± 0.07 cm, without recent experience with strength training (lower limbs) and/or running took place in this study. All subjects carried out a muscle damage induction protocol, which consisted of 30 minute downhill running (-16% or -9.09º) at 80% speed compared to their maximum oxygen consumption. Muscle damage determinants such as isometric peak torque, knee joint range of motion and circumference of the medial portion of the thight were measured before, during, 24, 48, 72 and 96 hours after downhill running. The subjective pain perception was measured simultaneously with the other determinants through three different tests: sitting on and getting up of a chair; climbing and descending from a 45cm step; and self thigh palpation. After going through all subjective pain perception tests, the subjects filled out a visual analog scale with their perception of pain. Muscle damage changes over time were compared through variance analysis (ANOVA) one way for repeated measures. Subjective pain perception values obtained in all three different tests were compared through two way ANOVAs for repeated numbers. The significance level adopted in this study was z ≤ 0.05. The results showed that the step test was the on which better evaluated the delayed onset muscle soreness. No significant differences were found through the ADM and CIR recovery markers. Downhil running determined reduced of PTI (~22.4%). Auditory brainstem implant outcomes and MAP parameters: Report of experiences in adults and children

The auditory brainstem implant (ABI) was first developed to help neurofibromatosis type 2 patients. Recently, its use has been recently extended to adults with non-tumor etiologies and children with profound hearing loss who were not candidates for a cochlear implant (Cl). Although the results has been extensively reported, the stimulation parameters involved behind the outcomes have received less attention. Objective: The aim of this study is to describe the audiologic outcomes and the MAP parameters in ABI adults and children at our center. Methods: Retrospective chart review. Five adults and four children were implanted with the ABI24M from September 2005 to June 2009. In the adult patients, four had Neurofibromatosis type 2, and one had postmeningitic deafness with complete ossification of both cochleae. Three of the children had cochlear malformation or dysplasia, and one had complete ossified cochlea due to meningitis. Map parameters as well as the intraoperative electrical auditory brainstem responses were collected. Evaluation was performed with at least six months of device use and included free-field hearing thresholds, speech perception tests in the adult patients and for the children, the Infant-Toddler Meaningful Auditory Integration Scale (IT-MAIS) and (ESP) were used to evaluate the development of auditory skills, besides the MUSS to evaluate. Results: The number of active electrodes that did not cause any non-auditory sensation varied from three to nineteen. All of them were programmed with SPEAK strategy, and the pulse widths varied from 100 to 300 mu s. Free-field thresholds with warble tones varied from very soft auditory sensation of 70 dBHL at 250 Hz to a pure tone average of 45 dBHL. Speech perception varied from none to 60% open-set recognition of sentences in silence in the adult population and from no auditory sensation at all to a slight improvement in the IT-MAIS/MAIS scores. Conclusion: We observed that ABI may be a good option for offering some hearing attention to both adults and children. In children, the results might not be enough to ensure oral language development. Programming the speech processor in children demands higher care to the audiologist. (C) 2011 Elsevier Ireland Ltd. This study had the aim to investigate the auditory and communicative abilities of children diagnosed with Auditory Neuropathy Spectrum Disorder due to mutation in the Otoferlin gene. It is a descriptive and qualitative study in which two siblings with this diagnosis were assessed. The procedures conducted were: speech perception tests for children with profound hearing loss, and assessment of communication abilities using the Behavioral Observation Protocol. Because they were siblings, the subjects in the study shared family and communicative context. However, they developed different communication abilities, especially regarding the use of oral language. The study showed that the Auditory Neuropathy Spectrum Disorder is a heterogeneous condition in all its aspects, and it is not possible to make generalizations or assume that cases with similar clinical features will develop similar auditory and communicative abilities, even when they are siblings. It is concluded that the acquisition of communicative abilities involves subjective factors, which should be investigated based on the uniqueness of each case.

PURPOSE: To examine the basis of previous findings of an association between indices of driving safety and visual motion sensitivity and to examine whether this association could be explained by low-level changes in visual function. METHODS: 36 visually normal participants (aged 19 – 80 years), completed a battery of standard vision tests including visual acuity, contrast sensitivity and automated visual fields. and two tests of motion perception including sensitivity for movement of a drifting Gabor stimulus, and sensitivity for displacement in a random-dot kinematogram (Dmin). Participants also completed a hazard perception test (HPT) which measured participants’ response times to hazards embedded in video recordings of real world driving which has been shown to be linked to crash risk. RESULTS: Dmin for the random-dot stimulus ranged from -0.88 to -0.12 log minutes of arc, and the minimum drift rate for the Gabor stimulus ranged from 0.01 to 0.35 cycles per second. Both measures of motion sensitivity significantly predicted response times on the HPT. In addition, while the relationship involving the HPT and motion sensitivity for the random-dot kinematogram was partially explained by the other visual function measures, the relationship with sensitivity for detection of the drifting Gabor stimulus remained significant even after controlling for these variables. CONCLUSION: These findings suggest that motion perception plays an important role in the visual perception of driving-relevant hazards independent of other areas of visual function and should be further explored as a predictive test of driving safety. Future research should explore the causes of reduced motion perception in order to develop better interventions to improve road safety.

A number of tests and test batteries are available for the prediction of older driver safety, but many of these have not been validated against standardized driving outcome measures. The aim of this study was to evaluate a series of previously described screening tests in terms of their ability to predict the potential for safe and unsafe driving. Participants included 79 community-dwelling older drivers (M=72.16 years, SD=5.46; range 65-88 years; 57 males and 22 females) who completed a previously validated multi-disciplinary driving assessment, a hazard perception test, a hazard change detection test and a battery of vision and cognitive tests. Participants also completed a standardized on-road driving assessment. The multi-disciplinary test battery had the highest predictive ability with a sensitivity of 80% and a specificity of 73%, followed by the hazard perception test which demonstrated a sensitivity of 75% and a specificity of 61%. These findings suggest that a relatively simple and practical battery of tests from a range of domains has the capacity to predict safe and unsafe driving in older adults.
