The observational method in tunnel engineering allows the evaluation in real time of the actual conditions of the ground and to take measures if its behavior deviates considerably from predictions. However, it lacks a consistent and structured methodology to use the monitoring data to adapt the support system in real time. The definition of limit criteria above which adaptation is required are not defined and complex inverse analysis procedures (Rechea et al. 2008, Levasseur et al. 2010, Zentar et al. 2001, Lecampion et al. 2002, Finno and Calvello 2005, Goh 1999, Cui and Pan 2012, Deng et al. 2010, Mathew and Lehane 2013, Sharifzadeh et al. 2012, 2013) may be needed to consistently analyze the problem. In this paper a methodology for the real time adaptation of the support systems during tunneling is presented. In a first step limit criteria for displacements and stresses are proposed. The methodology uses graphics that are constructed during the project stage based on parametric calculations to assist in the process and when these graphics are not available, since it is not possible to predict every possible scenario, inverse analysis calculations are carried out. The methodology is applied to the “Bois de Peu” tunnel which is composed by two tubes with over 500 m long. High uncertainty levels existed concerning the heterogeneity of the soil and consequently in the geomechanical design parameters. The methodology was applied in four sections and the results focus on two of them. It is shown that the methodology has potential to be applied in real cases contributing for a consistent approach of a real time adaptation of the support system and highlight the importance of the existence of good quality and specific monitoring data to improve the inverse analysis procedure.

OBJECTIVE: To develop a behavioural observation method to simultaneously assess distractors and communication/teamwork during surgical procedures through direct, on-site observations; to establish the reliability of the method for long (>3 h) procedures. METHODS: Observational categories for an event-based coding system were developed based on expert interviews, observations and a literature review. Using Cohen's κ and the intraclass correlation coefficient, interobserver agreement was assessed for 29 procedures. Agreement was calculated for the entire surgery, and for the 1st hour. In addition, interobserver agreement was assessed between two tired observers and between a tired and a non-tired observer after 3 h of surgery. RESULTS: The observational system has five codes for distractors (door openings, noise distractors, technical distractors, side conversations and interruptions), eight codes for communication/teamwork (case-relevant communication, teaching, leadership, problem solving, case-irrelevant communication, laughter, tension and communication with external visitors) and five contextual codes (incision, last stitch, personnel changes in the sterile team, location changes around the table and incidents). Based on 5-min intervals, Cohen's κ was good to excellent for distractors (0.74-0.98) and for communication/teamwork (0.70-1). Based on frequency counts, intraclass correlation coefficient was excellent for distractors (0.86-0.99) and good to excellent for communication/teamwork (0.45-0.99). After 3 h of surgery, Cohen's κ was 0.78-0.93 for distractors, and 0.79-1 for communication/teamwork. DISCUSSION: The observational method developed allows a single observer to simultaneously assess distractors and communication/teamwork. Even for long procedures, high interobserver agreement can be achieved. Data collected with this method allow for investigating separate or combined effects of distractions and communication/teamwork on surgical performance and patient outcomes.

OBJECTIVE: To investigate the association between early depressive behavior after stroke onset and occurrence of poststroke depression (PSD) at 3- and 12-month follow-up evaluations. METHODS: The study prospectively included 273 patients with first-ever single uncomplicated ischemic stroke. In the stroke unit, nurses scored crying, overt sadness, and apathy daily using an observational method to include patients with comprehension deficits. The Barthel Index was used to assess disability. Follow-up evaluation at months 3 and 12 included psychiatric assessment based on the Diagnostic and Statistical Manual of Mental Disorders, 4th edition. RESULTS: Crying (19.8%), overt sadness (50.5%), and apathy (47.6%) were observed. Of the patients observed crying, 4 showed pathologic crying, 19 emotionalism, and 12 catastrophic reactions. Crying and overt sadness, but not apathy, were associated with a subjective experience of depression (p < 0.05). Thirty of 52 (58%) patients observed crying, 12 of 19 (63%) patients with emotionalism, and 5 of 12 (41%) patients with catastrophic reactions developed PSD within the first year. Multiple logistic regression analysis showed that only severe functional disability (odds ratio [OR], 4.31; 95% CI, 2.41 to 7.69), crying behaviors (OR, 2.66; 95% CI, 1.35 to 5.27), and an age <68 years (OR, 2.32; 95% CI, 1.30 to 4.13) were (p < 0.05) predictors of late PSD development (13% of the variance). CONCLUSIONS: In the stroke unit, crying and overt sadness are more reliable indicators of depressed mood than apathy. In patients with first-ever stroke, crying behaviors soon after stroke, a younger age, and severe disability are predictors of poststroke depression occurrence within the first year after stroke onset.

Introducción: Determinar el balance de líquidos en pacientes quirúrgicos es un factor de suma importancia, la variabilidad de la onda de pletismografía puede ser un soporte para ayudar a superar este objetivo. La interpretación subjetiva por parte del anestesiólogo de la variabilidad de la onda de pletismografía, puede inferirse como una herramienta confiable para predecir y guiar la administración de líquidos en pacientes que no requieran monitoria invasiva. Metodología: Estudio exploratorio desarrollado por método de observación con la colaboración de 90 médicos anestesiólogos, mediante observación de la variabilidad de la onda de pletismografía en 5 videos aleatorizados. El porcentaje de decisiones correctas se analizó mediante IC del 95% para proporciones. Para evaluar la homogeneidad en la fracción de respuestas correctas se realizan pruebas chi-cuadrado de homogeneidad con un nivel de significancia de 0.05. Resultados: El 75% de la población encuestada con (IC) 95% de la proporción acertó en la estimación del IVP. La conducta de administrar o no líquidos y/o vasopresores fue correcta en el 80% de la población para 4 videos, con una tasa de error de 8,2% por video. El video 4 obtuvo un 32% de acierto y una tasa de error de 10,6%. Conclusiones: El estudio permitió de manera subjetiva, determinar que el uso de la variabilidad de la onda de pletismografía es una herramienta de fácil lectura que ayuda a la administración de líquidos durante el intraoperatorio en el paciente sometido a ventilación mecánica en procedimientos quirúrgicos de baja a intermedia complejidad.

This research evaluated the contribution of the Support Center for Family Health (SCFH) in relation to its effect on the Family Health Units through perception of the Family Health Strategy (FHS) and SCFH professionals, in addition to the satisfaction of users in relation to that role. Data were collected in the public health services of the city of Macaíba-RN in 2012, through semi-structured questionnaire and non participant systematic observation and it counted as investigated subjects 272 individuals (60 FHS professionals, 12 SCFH professionals and 200 users representatives of 20 units). For analysis of the responses of the opened questions was used categorization process and, in relation to the observational method, that was based on checking the space organization, the characteristics of the participating subjects and the specific set of activities performed by SCFH teams through an observation guide. The results point to a good acceptance of the SCFH teams role by most FHS professionals who reported active participation in the health units routine, its integration activities to the FHS teams with resolving based health promotion actions. Regarding the SCFH professionals, they also reported positive contribution by participating actively in the units routine with integrated activities to the ESF teams and developing resolute actions. For users, the SCFH brought assurance services with better access to specialized, resolving and welcoming care. Systematic observation ratified data obtained by questionnaire. It was perceived the need to implement actions related to man`s health, to invest in expanding the number of the SCFH teams, the greater supply of medications, improving the regulatory process and planning together as a key strategy to promote a more effective integration between teams SCFH / FHS

Purpose: To determine palpebral dimensions and development in Brazilian children using digital images. Methods: An observational study was performed measuring eyelid angles, palpebral fissure area and interpupillary distance in 220 children aged from 4 to 72 months. Digital images were obtained with a Sony Lithium movie camera (Sony DCR-TRV110, Brazil) in frontal view from awake children in primary ocular position; the object of observation was located at pupil height. The images were saved to tape, transferred to a Macintosh G4 (Apple Computer Inc., USA) computer and processed using NIH 1.58 software (NTIS, 5285 Port Royal Rd., Springfield, VA 22161, USA). Data were submitted to statistical analysis. Results: All parameters studied increased with age. The outer palpebral angle was greater than the inner, and palpebral fissure and angles showed greater changes between 4 and 5 months old and at around 24 to 36 months. Conclusion: There are significant variations in palpebral dimensions in children under 72 months old, especially around 24 to 36 months. Copyright © 2006 Informa Healthcare.

O presente estudo tem como tema de discussão, o trabalho do Agente Comunitário de Saúde (ACS) como essencial para ações de prevenção e controle do câncer, em conjunto com a equipe da Estratégia Saúde da Família. Direcionou-se o olhar, através dos olhos do ACS, para os riscos e atitudes facilitadoras para o desenvolvimento do câncer, bem como para sinais de alarme. A questão que norteou a pesquisa foi: os Agentes Comunitários de Saúde, após receberem devida capacitação, alcançam habilidades suficientes para atuarem no controle do câncer, através de ações da identificação de riscos e educação em saúde? O objetivo principal consistiu em elaborar um modelo de intervenção que contribua para o controle do câncer na Atenção Básica, tendo o ACS como principal mediador. Trata-se de uma pesquisa de intervenção, transversal, de abordagem qualitativa, tendo como objeto de estudo, o trabalho do ACS como um instrumento da ESF para o controle do câncer. Foram selecionados cinco ACS‘s como participantes principais e oito famílias como participantes secundários. Para a produção dos dados usou-se da pesquisa de campo, que transcorreu entre os meses de janeiro a abril de 2014, e contou com o método observacional. Esta produção foi dividida em dois momentos: estudo de eficácia e estudo de eficiência. A análise dos dados foi organizada em duas matrizes interpretativas: considerações acerca das atividades desenvolvidas e o corolário das ações desenvolvidas, cada uma com suas respectivas categorias e temas. Esta análise contou com o suporte teórico-metodológico da análise de conteúdo temática proposta por Bardin (2011). Quanto aos resultados, os ACS‘s, após receberem o trabalho de capacitação, tiveram um bom desempenho nas atividades de investigação e educação em saúde. Em um primeiro momento, a intervenção despertou mudanças positivas no cotidiano das famílias participantes da pesquisa, isto é, após a educação em saúde realizada pelos ACS‘s conseguiram abandonar hábitos de risco que haviam sido identificados. Assim, após implementação do plano prévio de intervenção, evidenciou o modelo aplicável, ou seja, foi possível perceber como deve se organizar e quais os passos devem ser realizados. A presente pesquisa ainda não pode responder se a intervenção elaborada, em definitivo, é capaz de ocasionar mudanças positivas no cotidiano dos participantes a longo prazo e, principalmente, se estas mudanças ajudarão na redução dos índices de câncer. Cabe destacar, para que intervenções como esta tenham sucesso parcerias precisam ser fortalecidas. Pois para que possamos ter uma saúde pública eficiente e, ao mesmo tempo, uma Atenção Básica de qualidade, necessita-se de fato, que todos trabalhem juntos.

We present a remote sensing observational method for the measurement of the spatio-temporal dynamics of ocean waves. Variational techniques are used to recover a coherent space-time reconstruction of oceanic sea states given stereo video imagery. The stereoscopic reconstruction problem is expressed in a variational optimization framework. There, we design an energy functional whose minimizer is the desired temporal sequence of wave heights. The functional combines photometric observations as well as spatial and temporal regularizers. A nested iterative scheme is devised to numerically solve, via 3-D multigrid methods, the system of partial differential equations resulting from the optimality condition of the energy functional. The output of our method is the coherent, simultaneous estimation of the wave surface height and radiance at multiple snapshots. We demonstrate our algorithm on real data collected off-shore. Statistical and spectral analysis are performed. Comparison with respect to an existing sequential method is analyzed.

O objetivo deste trabalho foi avaliar a eficácia adaptativa, antes e após intervenção em Psicoterapia Breve Operacionalizada em um paciente portador de Diabetes Mellitus tipo1. Os instrumentos utilizados foram a Entrevista Clínica Preventiva, a Escala Diagnóstica Adaptativa Operacionalizada EDAO e a Psicoterapia Breve Operacionalizada PBO. O delineamento desta pesquisa se deu pelo método clínico combinado ao método descritivo observacional. Foi realizado um estudo de caso com um paciente diagnosticado com Diabetes Mellitus tipo1, que foi submetido a oito entrevistas diagnósticas para a obtenção do diagnóstico clínico e em seguida a doze sessões de Psicoterapia Breve Operacionalizada. Após seis meses do término das sessões o paciente participou de uma entrevista de Follow up. O diagnóstico adaptativo, após as primeiras entrevistas foi: Adaptação Ineficaz Severa, no qual foram detectadas as situações-problemas a serem abordadas na PBO e foi organizado o planejamento desta intervenção. Finalizada a PBO e após seis meses o paciente foi submetido a uma entrevista de Follow up, no qual a classificação diagnóstica foi: Adaptação Ineficaz Leve. Conclui-se que neste estudo de caso que houve uma evolução no diagnóstico adaptativo após a intervenção em PBO, mostrando a importância do atendimento em Psicoterapia Breve Operacionalizada.

El clima organizacional es indispensable para incrementar la productividad de las instituciones, pues los empleados o directivos necesitan de un ambiente de armonía y comprensión para poder establecer el trabajo en equipo, dado que si se presentan fricciones desde el área interna del lugar de trabajo no podrá permanecer la organización a flote, debido a que puede caer en el fracaso por no identificar los problemas, que pueden estar enraizados en las actitudes de sus empleados. La monografía denominada “Análisis del Clima Organizacional en el Ministerio de Turismo Zonal 6 y el Sindicato de Choferes Profesionales del Azuay en el periodo octubre – marzo 2016”, analiza la percepción del clima organizacional existente, entre hombres y mujeres, de las respectivas instituciones, estableciendo diferencias del ambiente laboral entre el sector público y privado. Mediante encuestas se analizó una serie de variables, las cuales permitieron establecer el grado de influencia que ejercen entre los integrantes de las instituciones, además se aplicaron entrevistas a los directivos, y a través de la observación directa no participante se contrastó lo manifestado por los encuestados y las percepciones de las investigadoras.

Abstract OBJECTIVE To determine time standards for interventions and activities conducted by nursing professionals in Family Health Units (FHU) in Brazil to substantiate the calculation of work force. METHOD This was an observational study carried out in 27 FHU, in 12 municipalities in 10 states, in 2013. In each unit, nursing professionals were observed every 10 minutes, for eight work hours, on five consecutive days via the work sampling technique. RESULTS A total of 32,613 observations were made, involving 47 nurses and 93 nursing technicians/assistants. Appointments were the main intervention carried out by nurses, with a mean time of 25.3 minutes, followed by record-keeping, which corresponded to 9.7%. On average, nursing technicians/assistants spent 6.3% of their time keeping records and 30.6 intervention minutes on immunization/vaccination control. CONCLUSION The study resulted in standard times of interventions carried out by the FHU nursing team, which can underpin the determination of nursing staff size and human resource policies. Furthermore, the study showed the panorama of interventions currently employed, allowing for the work process to be reviewed and optimized.

A new statistical parallax method using the Maximum Likelihood principle is presented, allowing the simultaneous determination of a luminosity calibration, kinematic characteristics and spatial distribution of a given sample. This method has been developed for the exploitation of the Hipparcos data and presents several improvements with respect to the previous ones: the effects of the selection of the sample, the observational errors, the galactic rotation and the interstellar absorption are taken into account as an intrinsic part of the formulation (as opposed to external corrections). Furthermore, the method is able to identify and characterize physically distinct groups in inhomogeneous samples, thus avoiding biases due to unidentified components. Moreover, the implementation used by the authors is based on the extensive use of numerical methods, so avoiding the need for simplification of the equations and thus the bias they could introduce. Several examples of application using simulated samples are presented, to be followed by applications to real samples in forthcoming articles.
