State and international entities can have profound effects on the development of a country’s nursing profession. Through a global health governance lens, this paper explores the development of nursing in Brazil during the early twentieth century, and its intersections with national and international interests. Accordingly, we will show how state policies established an environment that fostered the institutionalization of nursing as a profession in Brazil and supported it as a means to increase the presence of females in nation building processes. The State focused on recruiting elite women for nursing, in part due to the Rockefeller Foundation’s involvement in the country. Nurses who worked for Rockefeller came from well-educated classes within US society with specific ideas about who should be a nurse and the roles of nurses in a healthcare system. These women served as the primary vehicles for interacting with Brazilian health authorities responsible for health system development. Their early efforts did not, however, ensure a system capable of producing nursing human resources at a rate that, in present day Brazil, could meet the health needs of the country. Findings from this paper offer new avenues for historians to explore the early roots of professional nursing through a global health governance lens, improve the understanding of the intersection between international politics and professionalization, and highlight how these factors may impact nursing human resources production in the long term.

Ce mémoire porte sur l’évolution de la profession de puéricultrice au Québec entre 1925 et 1985. Il cherche plus spécifiquement à éclairer les raisons de la disparition de cette profession. Celle-ci prend racine dans le contexte de la lutte contre la mortalité infantile et de la médicalisation croissante de la maternité, ce dernier phénomène connaissant de profondes modifications au cours du XXe siècle. En parallèle, les systèmes de santé et d’éducation québécois connaissent d’importants bouleversements. Comment la profession de puéricultrice évolue-t-elle devant ces changements ? Comment tentera-t-elle de faire sa place dans le monde des spécialistes des soins et de l’enfance ? Par ailleurs, quelle place la société, plus spécifiquement le monde médical et l’État, lui réservera-t-elle au fil du temps et pour quelles raisons ? Qu’est-ce qui explique sa disparition ? Voilà les questions auxquelles ce mémoire cherche à répondre. Afin de bien ancrer les origines de la profession, notre analyse s’est d’abord penchée sur les raisons de la création des premières formations en puériculture et des stratégies de professionnalisation des puéricultrices. Notre recherche s’est ensuite intéressée aux discours des acteurs s’étant prononcés sur le sort des puéricultrices à la suite des grandes réformes des années 1960-1970 ainsi qu’au discours des puéricultrices qui cherchent à s’inscrire dans ce nouveau système de santé étatisé. Nous émettons l’hypothèse que la profession de puéricultrice a été victime de plusieurs facteurs dont les multiples réorganisations du système de santé ainsi que de la compétition entre les professions de la santé, mais plus profondément d’une conception de la maternité qui se modifie considérablement au cours de la période étudiée.

A enfermagem é uma profissão voltada para o cuidado das pessoas nas diferentes fases da vida, o ato de cuidar é a essência no fazer da enfermeira, e, possui uma inquietação com o conjunto de prioridade de pesquisa em seu meio. Neste sentido, a saúde materna considerada um indicador sensível à qualidade de vida de uma população é uma delas. O presente estudo é uma revisão integrativa da literatura que teve como objetivo descrever as infecções mais frequentes que a mulher está exposta durante o período puerperal, investigadas em publicações nacional e internacional da área da saúde, além de identificar o nível de evidência cientifica de cada artigo. Para a seleção dos estudos foram utilizadas três bases de dados, ScienceDirect, Pubmed e Lilacs. O recorte temporal foi de 2009 a 2013 e, a amostra foi composta por 19 artigos, relacionado à infecção puerperal. Os dados foram coletados da segunda quinzena de setembro à primeira de outubro. A análise dos estudos permitiu identificar que mais da metade das publicações foi no Brasil. Dois estudos identificaram enfermeiros como autores. A maioria dos periódicos de veiculação dos estudos era da área medica. Doze estudos apresentaram delineamento não experimental, três eram estudo de caso e quatro apresentaram delineamento experimental. As principais infecções puerperais encontradas foram a endometrite, a infecção urinária, a infecção do sítio cirúrgico, a sepse puerperal, a mastite, a cervicite. Os resultados mostraram que são necessárias mais pesquisas com delineamento experimental, principalmente no que tange a área da enfermagem. A avaliação rigorosa da puérpera no pós-parto, a adequada conduta para prevenção da infecção puerperal e/ou manejo das intervenções no cuidado da paciente com morbidade infecciosa, alicerçam ações indispensáveis da enfermeira na obtenção de um atendimento de enfermagem mais seguro, de qualidade, que promova o protagonismo da mulher nesta etapa importante de sua vida e lhe proporcione autonomia em relação aos seus direitos sexuais e reprodutivos contribuindo para a redução da mortalidade materna.

O contato com pessoas acometidas por dermatoses imunobolhosas (DI) é impactante devido ao visível sofrimento causado pelo desconforto/mal-estar e comprometimento da autoimagem. Trata-se de um grupo de doenças de evolução crônica cuja manifestação primária e fundamental consiste no desenvolvimento de bolhas e menos frequentemente de vesículas na pele e/ou mucosas. O desafio de cuidar desta clientela justifica-se pelo caráter desfigurante e desconfortante, complexidade e vulnerabilidade a complicações, dentre as quais infecções e infestações, principalmente no ambiente hospitalar, fato agravado pelas drogas imunossupressoras utilizadas para o tratamento. Neste contexto, sobreleva-se a necessidade de ampliação do cuidado de enfermagem, sem limitar-se ao cumprimento das prescrições dos demais profissionais de saúde, objetivando atender às necessidades desta clientela e contemplá-la em todas as suas dimensões. Assim, este estudo teve como objetivo geral: propor uma Tecnologia de Cuidados de Enfermagem ao Cliente com Dermatoses Imunobolhosas (TCECDI) que reconheça padrões de conforto/bem-estar do cliente hospitalizado, antes e após a sua aplicação. A necessidade de trabalhar com dados imprecisos como a subjetividade do conforto/bem-estar, e com um grupo de doenças de incomum acometimento às pessoas, despertou o interesse pela lógica fuzzy, uma teoria que auxilia na compreensão dos conceitos que extrapolam as barreiras da lógica formal, permitindo estabelecer diferentes graus de pertinência dos atributos julgados relevantes, representando uma ferramenta que pode capturar informações subjetivas convertendo-as em valores de pertinência. Os atributos estabelecidos para avaliação do conforto nos três momentos foram: dor, mobilidade, padrão de sono, exposição do corpo e das lesões, conhecimento sobre a doença e autocuidado. Trata-se de um estudo quase experimental, interinstitucional, realizado no período de junho de 2012 a abril de 2013, em unidades de internação especializadas em dermatologia localizadas no Rio de Janeiro e no Mato Grosso do Sul. O delineamento alternativo para o ensaio clínico utilizado foi destinado a um único grupo não randomizado do tipo série temporal. As aferições foram realizadas antes (T0), 24 horas após (T1) e uma semana após (T2) o recebimento da intervenção. Como inexiste grupo controle, em cada sujeito considerou-se o seu próprio controle. Baseando-se nas classificações advindas da lógica fuzzy, na definição de conforto e nos três sentidos técnicos preconizados por Kolcaba: alívio, calma e transcendência,constatou-se a redução significativa no padrão de desconforto. Assim, pode-se afirmar que a implementação da TCECDI interferiu de forma expressiva e positiva nas necessidades de conforto dos sujeitos do estudo. Ao privilegiar a autonomia do enfermeiro e o seu saber específico, esta pesquisa contribuiu para a enfermagem como profissão, preenchendo lacunas nesta área do conhecimento, possibilitando o ensino qualificado, além de estimular entre os profissionais de saúde a reflexão, compreensão e desenvolvimento de outras pesquisas sobre a prática do cuidado em saúde, principalmente em dermatologia.

The role of the hospital-employed nurse educator is evolving. Factors influencing this change include the introduction of standards for nurse educators by the College of Nurses of Ontario (CNO), a change in the way nurses are educated, the emergence of nursing as a profession, and hospital restructuring as a result of budgetary constraints. Two of these influencing factors: the introduction of the updated Standards of Practice for Registered Nurses and Registered Practical Nurses (1996) and hospital restructuring occurred over the last 7 years at several hospitals in southern Ontario. Current literature as well as the Standards of Practice (1996) were utilized to examine the current roles and responsibilities of nurse educators and subsequently develop a questionnaire to study the impact of these influencing factors on the role of the nurse educator. This questionnaire was piloted and revised before its distribution at 4 hospitals in southern Ontario. Twenty-five of the 41 surveys (61%) distributed were returned for analysis. The data reflected that the Standards of Practice had a positive influence on the role of the nurse educator, while hospital restructuring had a negative impact. In addition, many of the roles and responsibilities identified in the literature were indeed part of the current role of nurse educators, as well as several responsibilities not captured in the literature. The predictions for the future of this role in its current state were not positive given the financial status of the health care system as well as the lack of clarity for the role and the current level ofjob satisfaction among practicing nurse educators. However, a list of recommendations were generated which, if implemented, could add clarity to the role and improve job satisfaction. This could enhance the retention of current nurse educators and the possibility of recruiting competent nurse educators to the role in the future.

Nursing as a profession goes in search on quality of their care through new frameworks, trying to break postures of the health care system so fragmented in the care. To change professional practices, it is necessary to build their own knowledge grounded on Nursing Care System. The aim of this study was to analyze the influence of nurses' knowledge on care systematization in nursing in the University Hospital Natal-RN. It is an analytical descriptive study carried out at the Onofre Lopes University Hospital (HUOL), Natal-RN, 2010, the sample was composed of 40 active nurses working in hospitalization units of the hospital, the inclusion criteria were being in the monthly scale sector and agree to participate in the study. The non-participant observation and another interview were used for collecting data, statistical analysis was descriptive and inferential with reliability test, Pearson test, chi-square and Fischer, the variables that correlated were analyzed in a model Multiple logistic , calculating odds ratio. The results were: predominance of female professionals (90%), predominantly in the age range 39-46 years (37.5%), nurses who have the undergraduate degree at the Federal University of Rio Grande do Norte (80%), and who have expertise training as a minimal degree (62.5%). Among the surveyed, the knowledge showed significance with the graduation time (p = 0.018) and time working in HUOL (p = 0.036). The majority of the professionals surveyed do not know which organ is responsible for the SAE legislation (52.5%), aware of the steps needed to build the nursing diagnosis (92.5%), understand the characteristics of nursing planning (90% ). However the same professionals do not perform physical examination in patients (50.0%) did not classify the clinical findings (68.4%), and identify the problems encountered as a classification (13.2%). The planning of nursing care is carried out by verbal order of nurses (82.5%), 41% of the professionals assess only the intervention stage, in other words, the actions taken. Regarding the practical application of nursing records 53% of nurses do not realize records, 30.8% is incomplete, the other held notes (p = 0.003). The nurses know the nursing process (90% of appropriate responses), despite the actions defined by the theory are not applied in practice. Investigators believe the condition of the hospital teacher (22.5%) could positively affect the implementation of the SAE associated with the interest of professionals (20%). Of the respondents, 17.5% accept as truth the lack of facilities to assist the SAE implementation in the hospital. It was concluded that nurses know the theory that underlies the SAE and the nursing process, but do not develop the service know as well, there is need for action to boost the SAE implementation as practice of nurses in the hospital investigated

The present study is based on an analysis of professional work relationships in the nursing team from the task/skills of its members as a contribution to understanding the work process in nursing. It is aimed to identify the skills of the nursing team members through the vision of nurses, technicians and nursing assistants, thus it attempts to find strategies to improve the health assistance to patients. It is a descriptive and analytical study with a qualitative approach grounded in theoretical and methodological framework of Symbolic Interactionism. The research was carried out in the participant work place, a Public Hospital of Reference for the SUS, located in the eastern health district of Natal/RN. Nineteen nursing professionals participated in the study, which seven was nurse and twelve nursing technician. As procedure to collect data we used an unstructured interview accompanied by a standard topic guide which was recorded and later transcribed. The content analysis was chosen as the main methodology to analyze the discussion, which gave rise to thematic categories that were considered relevant based on the theoretical framework of this study, and the interactionist theory. This study was in accordance with the ethical principles of the Resolution nº. 196/96, it has obtained an appropriate consent of the UFRN Research Ethics Committee. The results indicate that the professionals seen the nursing as a profession strongly attached to the health care process and as a profession that acquired a scientific status very recently. Regarding to the nursing functions in the work process in nursing, the professionals they identified the manage/administer category as the main activities developed by these professionals, thus the education and complex care in nursing categories. Concerning to the technicians and nursing assistants functions, it was figured out in the professional s opinion that there is not distinction among the attribution of these categories. The interviewed were unanimous in report that these professionals are more involved in direct patient health care through performance of basic duties in nursing care. Finally, with regard to the work relationship among nursing team members, the majority of those interviewed see this relationship as disharmony and quarrelsome and in general, there is not bond between categories that comprise the process of work in nursing. On the basis of our results we consider the importance of knowing the meaning of nursing given by these professionals; also their skills could be useful as basis to identify problems, which source could be detected in the power relationship, deviations of functions, gap between design (knowledge) and performance (doing) work, besides the loss of the global activities view in the process of nursing work

Ao rever a história da Enfermagem percebe-se que sua origem foi sustentada em bases empíricas e em práticas fundamentalmente místicas, constituindo-se como uma profissão caritativa e vocacional, reproduzindo alguns estereótipos ainda presentes no imaginário da população e dos demais profissionais da área da saúde. É possível que a trajetória da Enfermagem tenha corroborado para uma baixa visibilidade profissional e, que a Enfermagem ainda enfrente reflexos de sua gênese profissional. Assim, tem-se como objetivo geral: conhecer a percepção das enfermeiras acerca das possíveis implicações da visibilidade da Enfermagem no cotidiano de trabalho; e como objetivo específico: construir estratégias que podem promover o reconhecimento e a visibilidade da Enfermagem. A pesquisa foi desenvolvida através de uma abordagem qualitativa do tipo exploratória, com 30 enfermeiros atuantes no Hospital Universitário Dr. Miguel Riet Corrêa Jr. (HU-FURG), da Universidade Federal do Rio Grande (FURG). Para a coleta dos dados, utilizou-se a entrevista semiestruturada. O processo de análise compreendeu a análise textual discursiva, e os resultados foram apresentados em dois artigos intitulados: “Implicações da Visibilidade da Enfermagem no Exercício Profissional” e “Estratégias para Promover o Reconhecimento e Visibilidade da enfermagem”. Após as análises concluiu-se que a visibilidade da Enfermagem está relacionada à trajetória histórica da profissão, à falta de reconhecimento da Enfermagem como profissão que possui saberes científicos, à veiculação errônea da imagem da Enfermagem na mídia, à ausência da realização de marketing por parte do enfermeiro, aos posicionamentos inadequados perante a equipe e ainda, à sobrecarga de trabalho. Para garantir o reconhecimento e a visibilidade da enfermagem é preciso demonstrar conhecimentos de forma crítica-reflexiva no cotidiano de trabalho. Evidenciou-se que, para proporcionar a mudança desejada da imagem profissional é necessário que a própria enfermagem desenvolva estratégias para promover sua visibilidade e valorização, as quais são essenciais à consolidação da profissão.

The goal of this study was to examine the role of organizational causal attribution in understanding the relation of work stressors (work-role overload, excessive role responsibility, and unpleasant physical environment) and personal resources (social support and cognitive coping) to such organizational-attitudinal outcomes as work engagement, turnover intention, and organizational identification. In some analyses, cognitive coping was also treated as an organizational outcome. Causal attribution was conceptualized in terms of four dimensions: internality-externality, attributing the cause of one’s successes and failures to oneself, as opposed to external factors, stability (thinking that the cause of one’s successes and failures is stable over time), globality (perceiving the cause to be operative on many areas of one’s life), and controllability (believing that one can control the causes of one’s successes and failures). Several hypotheses were derived from Karasek’s (1989) Job Demands–Control (JD-C) model and from the Job Demands–Resources (JD-R) model (Demerouti, Bakker, Nachreiner & Schaufeli, 2001). Based on the JD-C model, a number of moderation effects were predicted, stating that the strength of the association of work stressors with the outcome variables (e.g. turnover intentions) varies as a function of the causal attribution; for example, unpleasant work environment is more strongly associated with turnover intention among those with an external locus of causality than among those with an internal locuse of causality. From the JD-R model, a number of hypotheses on the mediation model were derived. They were based on two processes posited by the model: an energy-draining process in which work stressors along with a mediating effect of causal attribution for failures deplete the nurses’ energy, leading to turnover intention, and a motivational process in which personal resources along with a mediating effect of causal attribution for successes foster the nurses’ engagement in their work, leading to higher organizational identification and to decreased intention to leave the nursing job. For instance, it was expected that the relationship between work stressors and turnover intention could be explained (mediated) by a tendency to attribute one’s work failures to stable causes. The data were collected from among Finnish hospital nurses using e-questionnaires. Overall 934 nurses responded the questionnaires. Work stressors and personal resources were measured by five scales derived from the Occupational Stress Inventory-Revised (Osipow, 1998). Causal attribution was measured using the Occupational Attributional Style Questionnaire (Furnham, 2004). Work engagement was assessed through the Utrecht Work Engagement Scale (Schaufeli & al., 2002), turnover intention by the Van Veldhoven & Meijman (1994) scale, and organizational identification by the Mael & Ashforth (1992) measure. The results provided support for the function of causal attribution in the overall work stress process. Findings related to the moderation model can be divided into three main findings. First, external locus of causality along with job level moderated the relationship between work overload and cognitive coping. Hence, this interaction was evidenced only among nurses in non-supervisory positions. Second, external locus of causality and job level together moderated the relationship between physical environment and turnover intention. An opposite pattern of interaction was found for this interaction: among nurses, externality exacerbated the effect of perceived unpleasantness of the physical environment on turnover intention, whereas among supervisors internality produced the same effect. Third, job level also disclosed a moderation effect for controllability attribution over the relationship between physical environment and cognitive coping. Findings related to the mediation model for the energetic process indicated that the partial model in which work stressors have also a direct effect on turnover intention fitted the data better. In the mediation model for the motivational process, an intermediate mediation effect in which the effects of personal resources on turnover intention went through two mediators (e.g., causal dimensions and organizational identification) fitted the data better. All dimensions of causal attribution appeared to follow a somewhat unique pattern of mediation effect not only for energetic but also for motivational processes. Overall findings on mediation models partly supported the two simultaneous underlying processes proposed by the JD-R model. While in the energetic process the dimension of externality mediated the relationship between stressors and turnover partially, all the dimensions of causal attribution appeared to entail significant mediator effects in the motivational process. The general findings supported the moderation effect and the mediation effect of causal attribution in the work stress process. The study contributes to several research traditions, including the interaction approach, the JD-C, and the JD-R models. However, many potential functions of organizational causal attribution are yet to be evaluated by relevant academic and organizational research. A complete change of career forces a seismic shift in every aspect of your life. From day one, you have to face the loss of long held beliefs, behaviours, the known world of self, and security. We came from professions that themselves are poles apart, and many of the challenges we faced entering the profession were the same: juggling full-time work, part time study, and family commitmemts, taking a pay cut, and loss of social life. But over a short period of time we both transitioned to our new profession successfully. so what make our successful transition possible?
