In the past three decades institutions for persons with intellectual disabilities (ID) have been downsizing and closing in Ontario, Canada. This trend is reflective of the changes that have occurred in society. As of March 2009 the last institution operated by the Ontario government for persons with ID closed, placing the remaining approximately 1000 persons into the community. The current study was an analysis of part of one study in a four-study research project, called the Facilities Initiative Study, to explore the impact of the closures on the lives of individuals who have been reintegrated into community settings. The goal of the current case study analysis was to describe the impact of changes in social inclusion, choice-making/autonomy, and adaptive/maladaptive functioning of four individuals prior to and following transition to the community. The results suggested that, in most cases, community integration was related to more social inclusion opportunities and autonomy in choice-making, a wider range of adaptive behaviors and fewer maladaptive behaviors. In some cases, the evidence suggested that some of these indices of quality of life were not improving. Overall, the study found that the differences observed were unique to each of the individuals who participated in the case study analysis. Some generalized themes were generated that can be applied to future deinstitutionalization endeavors.

BACKGROUND: Over the past decade, physician-rating websites have been gaining attention in scientific literature and in the media. However, little knowledge is available about the awareness and the impact of using such sites on health care professionals. It also remains unclear what key predictors are associated with the knowledge and the use of physician-rating websites. OBJECTIVE: To estimate the current level of awareness and use of physician-rating websites in Germany and to determine their impact on physician choice making and the key predictors which are associated with the knowledge and the use of physician-rating websites. METHODS: This study was designed as a cross-sectional survey. An online panel was consulted in January 2013. A questionnaire was developed containing 28 questions; a pretest was carried out to assess the comprehension of the questionnaire. Several sociodemographic (eg, age, gender, health insurance status, Internet use) and 2 health-related independent variables (ie, health status and health care utilization) were included. Data were analyzed using descriptive statistics, chi-square tests, and t tests. Binary multivariate logistic regression models were performed for elaborating the characteristics of physician-rating website users. Results from the logistic regression are presented for both the observed and weighted sample. RESULTS: In total, 1505 respondents (mean age 43.73 years, SD 14.39; 857/1505, 57.25% female) completed our survey. Of all respondents, 32.09% (483/1505) heard of physician-rating websites and 25.32% (381/1505) already had used a website when searching for a physician. Furthermore, 11.03% (166/1505) had already posted a rating on a physician-rating website. Approximately 65.35% (249/381) consulted a particular physician based on the ratings shown on the websites; in contrast, 52.23% (199/381) had not consulted a particular physician because of the publicly reported ratings. Significantly higher likelihoods for being aware of the websites could be demonstrated for female participants (P<.001), those who were widowed (P=.01), covered by statutory health insurance (P=.02), and with higher health care utilization (P<.001). Health care utilization was significantly associated with all dependent variables in our multivariate logistic regression models (P<.001). Furthermore, significantly higher scores could be shown for health insurance status in the unweighted and Internet use in the weighted models. CONCLUSIONS: Neither health policy makers nor physicians should underestimate the influence of physician-rating websites. They already play an important role in providing information to help patients decide on an appropriate physician. Assuming there will be a rising level of public awareness, the influence of their use will increase well into the future. Future studies should assess the impact of physician-rating websites under experimental conditions and investigate whether physician-rating websites have the potential to reflect the quality of care offered by health care providers.

Self-determination and decision-making are acknowledged internationally as key rights of persons with disabilities and should play an important role in the development of educational plans and procedures. Not only is the chance for individuals with developmental disabilities to select their own tasks, leisure activities or reinforcers a valuable way of enhancing rights-based education and personal dignity, but choice-making opportunities may also function as a useful clinical or educational tool if they actually improve the efficacy of programmes aimed at the acquisition of socially relevant behaviours and life skills or the reduction of challenging behaviours.

The study reported here assessed whether or not choice affected effectiveness of an educational procedure for three children on the autism spectrum. Following a preference assessment, a number of discrete teaching trials were conducted with each child and, contingent upon targeted responses, either the child or the therapist selected one of three preferred reinforcer items. Reinforcer choice did not affect intervention effectiveness for two of the children; however, performance and motivation improved for the third child. Results re-affirmed the importance of thorough preference assessments prior to intervention and showed that additional stimulus choice contingent on the target response may improve motivation and outcomes for some children.

The present study aimed to trial the effectiveness of 10 newly developed brief vignettes portraying typical interactions between staff and people with intellectual disabilities in residential care settings to assess the knowledge and understanding of staff about choice diversity, pre- and post-attendance at a staff training workshop. A total of 29 residential staff completed the Vignette Rating Scale and a knowledge questionnaire pre- and post-training. A t- test conducted on the vignettes revealed that respondents identified fewer choices in the post-test vignettes compared to the pre-test vignettes. Results showed no significant difference between the pre- and post-test data on the knowledge questionnaire. The questionnaire revealed a high level of knowledge about choice prior to and following training. The vignettes, however, proved effective in measuring changes in awareness of choice diversity among residential staff following participation in a staff training workshop.

Academically gifted students are recognised as possessing considerable achievement potential. Yet many gifted students fail to perform at a level commensurate with their ability. This phenomenon is known as underachievement and may have far-reaching personal and social costs. Underachievement is particularly prevalent during early adolescence, between the ages of 10 to 14 years, when declining levels of academic achievement are often apparent. Grouping for instructional purposes is advocated as a means of reducing underachievement for gifted students by providing opportunities for like-minded and like-ability individuals to learn together. Despite this, some gifted students continue to underachieve. For some, underachievement appears to be a deliberate choice making them a population of learners at-risk. This multiple case study examines the relationship between self-presentation and underachievement as experienced by three gifted adolescents. It reveals how students perceived and explained the discrepancy between their ability and performance. Further, the study identifies those self-presentation strategies adopted in response to underachievement. Findings may assist parents and educators to provide greater support for gifted adolescents.

In real life strategic interactions, decision-makers are likely to entertain doubts about the degree of optimality of their play. To capture this feature of real choice-making, we present here a model based on the doubts felt by an agent about how well is playing a game. The doubts are coupled with (and mutually reinforced by) imperfect discrimination capacity, which we model here by means of similarity relations. We assume that each agent builds procedural preferences de ned on the space of expected payoffs-strategy frequencies attached to his current strategy. These preferences, together with an adaptive learning process lead to doubt-based selection dynamic systems. We introduce the concepts of Mixed Strategy Doubt Equilibria, Mixed Strategy Doubt-Full Equilibria and Mixed Strategy Doubtless Equilibria and show the theoretical and the empirical relevance of these concepts.

Baseado nos pressupostos subjacentes às práticas pedagógicas centradas na criança, este estudo tem por objetivo analisar a perspetiva dos educadores de infância sobre a promoção de oportunidades de escolha em contextos de ensino pré-escolar, tendo por referência o processo de inclusão de crianças com necessidades adicionais de suporte, bem como examinar os efeitos de um programa de intervenção, centrado na expansão de oportunidades de escolha, na participação de uma criança em situação de incapacidade. A fim de responder ao primeiro objetivo foi auscultada a opinião de 71 educadores de infância, através de uma pesquisa por inquérito. Já para o escrutínio dos efeitos do programa de intervenção, foi desenvolvido um estudo de caso único, tipo AB, avaliando o seu impacte na participação de uma criança de 4 anos com Paralisia Cerebral, através do uso de grelhas de observação e de uma entrevista semiestruturada à educadora. Os resultados sugerem que existe uma diferença significativa na facilidade de implementação de oportunidades de escolha, quando consideradas crianças com desenvolvimento típico e com necessidades adicionais de suporte, que parece também variar em função das diferentes rotinas do contexto pré-escolar e correlacionar-se com o nível de severidade da incapacidade. As características das crianças e as exigências curriculares são entendidas como as principais barreiras à implementação destas práticas. Os resultados sugerem, ainda, que o programa de intervenção – baseado no planeamento de oportunidades de escolha embebidas nos diferentes momentos da rotina do contexto pré-escolar - teve impacte positivo na participação da criança, com expansão dos indicadores de envolvimento, interação/comunicação e satisfação

In the last few decades, there have been significant changes in the way people with intellectual disabilities (ID) live in many countries around the world. Large isolated institutions have been replaced by community-based housing. This study examined the deinstitutionalization process in Ontario and it's effects on the lives of three individuals with ID. A case analysis approach was used allowing for in depth evaluation of the quality of life of these participants following their discharge with a focus on family involvement, community engagement, and choice making. A discrepancy analysis between the Essential Elements Plan (EEP), constructed when they were entering the community placement, and the current living arrangements was also done. The results of this study suggested that with community living comes improvements in family interactions, community engagement, and decision-making. However, these improvements were found to be minimal. Also, little discrepancy was found between the EEPs and their actual placements.

People with intellectual disabilities (ID) are more likely to be victims of abuse and human rights violations than people without ID. The 3Rs: Rights, Respect, and Responsibility project has developed and is testing a human rights training program for adults with ID. The current project was conducted to make recommendations to adapt the 3Rs rights training program to be used with youth with ID and their families. An interpretive phenomenological framework was employed to investigate youth with ID, parents', and siblings' perceptions of the i r experiences with choice making, an enactment of rights, in the family context. Thematic analysis of interviews revealed that, consistent with previous research, family members consider family values, conventions, and family members' well being when making decisions. A training program should promote a consideration of expanded opportunities for youth with ID to make choices and should be flexible to address individual families' cultures, needs, and desires.

This article is an analysis and reflection on the role of lists and diagrams in Start where you are, a multimedia improvisational piece performed as part of square zero independent dance festival: the second edition/la deuxième édition. This interdisciplinary festival was organised by collective (gulp) dance projects and took place in Ottawa, Canada, in August 2005. Start where you are was the result of a collaboration between the authors: two dance artists (Andrew and MacKinnon, the principals of (gulp)) and a visual communication designer (Gillieson). A sound artist and a lighting technician also participated in the work. This is a post-performance retrospective meant to analyze more closely the experience that meshed the evidentiary weight of words and graphics with the ephemerality and subjectivity of movement-based live performance. It contextualizes some of the work of collective (gulp) within a larger tradition of improvisation in modern dance. It also looks at how choice-making processes are central to improvisation, how they relate to Start, and how linguistic material can intersect with and support improvisational performance. Lastly, it examines some characteristics of lists and diagrams, unique forms of visual language that are potentially rich sources of material for improvisation.

The objective of the study was to investigate the relationship between autonomy markers in adult people with intellectual disabilities (ID) and variables such as intelligence quotient (IQ), early aging, and literacy. Participants were 47 adults with ID (33 men; aged 28-58 years with nonspecific etiology, 34 literate). All participants were trainees at the Center for Training of Capacity and Orientation for Employment at Associação de Pais e Amigos dos Excepcionais de São Paulo, São Paulo State, Brazil. They were divided into two age groups: younger (n = 27, mean age 31.85; +/- 2.23), and older (n = 38, mean age 41.84; +/- 5.54). They were administered intelligence scales twice, and answered questions related to social participation, community integration, and choice-making; the percentage of item responses leaning toward self-determination and freedom in choice-making was considered in defining the autonomy score. For both groups, IQ declined from the first to the second assessment. Autonomy scores were more determined by IQ classification than by literacy. Authors concluded that autonomy in people with ID is a multifactorial phenomenon associated with cognitive decline.

A growing number of empirical studies recently investigated consumers' valuation for local food products. However, different aspects related to the local food consumption still remain vague or unexplored. As such, the objective of the present research is to fulfill the existing literature using a mixed methodological approach for the investigation of consumers' preferences and Willingness to Pay (WTP) for local food products. First of all, local food is still a blurred concept and this factor might be source of individuals' misperception for the local origin meaning. Therefore, a qualitative research has been performed in order to investigate the meaning and the perception of the local food in the Italian food market. Results from this analysis have been used as inputs for the building of a non-hypothetical Real Choice Experiment (RCE) to estimate consumers' WTP for locally and organically produced apple sauce. The contribution of this study is three-fold: (1) consumers' valuation for the local origin is interpreted in terms of regional borders, over the organic food claim in case of an unusual food product in the area of interest, (2) the interaction between individuals' personality traits and consumers’ preferences for local and organic foods is analyzed, (3) the role of Commitment Cost creation in consumers' choice making in case of uncertainty due to the use of a novel food product and of an unconventional food claim is investigated. Results suggest that consumers are willing to pay a higher price premium for organic over locally produced apple sauce, possibly because of the presence of a regulated certification. In accordance with Commitment Cost theory, the organic label might thus decrease consumers' uncertainty for the features of the product in question. Results also indicate that individuals' personality can be source of heterogeneity in consumers' preferences.

La présente étude s’intéresse aux choix de filières de formation des filles comparées aux garçons. La présence des filles dans les filières de formation dans le domaine des sciences, de la technologie, du génie et de la mathématique (STGM) est moins importante que celle des garçons. Ce fait est documenté dans la plupart des pays industrialisés (OCDE, 2013). Les décideurs sont préoccupés par cette sous-représentation des filles et des femmes dans ces domaines et s’affairent à comprendre le phénomène, dans le but d’agir pour changer la situation (Drouin et al., 2008; MCCCF, 2011). Or, les facteurs d’influence pour expliquer cet écart entre les garçons et les filles sont nombreux et ne font pas l’objet d’un consensus dans la littérature (Ceci et al., 2009). Toutefois, plusieurs s’entendent pour dire que les mathématiques, importantes dans les profils de formation en STGM, et la façon dont les filles les perçoivent pourraient expliquer, en partie, leurs choix (Rowan-Kenyon et al., 2012 et Wang et al., 2013). Ces auteurs ont aussi suggéré que le contexte social et les croyances des filles au sujet des mathématiques seraient déterminants dans le processus de choix impliquant cette discipline. Un modèle théorique sociocognitif, inspiré par les travaux de Lent et al, (1994-2006), expliquant le processus de choix scolaires et professionnels a permis de conceptualiser les liens entre les déterminants socio-motivationnels spécifiques aux mathématiques. L’objectif général de la présente étude était de mieux documenter l’importance des mathématiques dans les choix de filières de formation menant aux carrières en STGM. Spécifiquement, nous avons examiné les relations entre le rendement en mathématiques, la perception des élèves quant au contexte social (soutien des parents et enseignants), leurs attentes de réussite, la valeur qu’ils accordent aux mathématiques (sentiment d’autoefficacité, anxiété, perception de l’utilité et intérêt) et les choix de filières de formation générale après leur secondaire (sciences humaines sans mathématiques, sciences humaines avec mathématiques, sciences de la santé et sciences pures). Nous avons exploré les distinctions entre les filles et les garçons. Pour ce faire, 1129 élèves finissants ont été questionnés au sujet de leurs motivations en mathématiques et de leurs intentions de formation post-secondaire. Par la suite, une comparaison entre les 583 filles et les 543 garçons a été réalisée par des analyses de régression logistiques multinomiales. Les résultats montrent que plusieurs déterminants permettent de dégager des similitudes et des différences quant aux choix de filières de formation des filles et des garçons. D’abord, il semble que pour la plupart des élèves, filles ou garçons, un rendement élevé et un important soutien des enseignants tel que perçu par les élèves à la fin du secondaire est davantage lié aux choix de filières en sciences pures et en sciences de la santé qu’en sciences humaines avec ou sans mathématiques. Toutefois, le soutien des parents perçu est plus déterminant pour les filles qui choisissent les sciences de la santé que pour les garçons. Le soutien des enseignants perçu est plus déterminant pour les garçons qui choisissent les sciences humaines que pour les filles. Aussi, un faible sentiment d’autoefficacité en mathématiques serait associé au choix de filières en sciences humaines, alors qu’une forte anxiété en mathématiques chez les filles serait associée aux filières en sciences de la santé. Pour les garçons, c’est davantage l’intérêt en mathématiques qui est déterminant pour choisir la filière des sciences pures. La perception de l’utilité des mathématiques est déterminante à la fois pour les garçons et pour les filles qui choisissent les filières de sciences les menant à des carrières en STGM. En somme, nos résultats suggèrent que le soutien en mathématiques de la part des adultes significatifs, tel que perçu par les élèves, est moins prépondérant que les attentes de réussite (sentiment d’autoefficacité et anxiété) et la valeur accordée aux mathématiques (intérêt et utilité perçue) pour comparer les garçons et les filles dans leurs choix de filières. À la lumière des résultats obtenus, il nous semble que l’implantation de mesures, dans les milieux scolaires, pour renforcer le sentiment d’autoefficacité des jeunes filles en mathématiques et surtout pour diminuer leur taux d’anxiété dans cette matière serait une voie prometteuse pour atteindre la parité entre les garçons et les filles dans les filières en STGM.

Changes in residential accommodation models for adults with intellectual disability (ID) over the last 20 years in Australia, the United Kingdom and the United States have involved relocation from institutions primarily into dispersed homes in the community. But an evolving alternative service style is the cluster centre. This paper reports on the relocation of a matched group of 30 pairs of adults with moderate and severe IDs and challenging behaviour who were relocated from an institution into either dispersed housing in the community or cluster centres but under the same residential service philosophy. Adaptive and maladaptive behaviour, choice-making and objective life quality were assessed prior to leaving the institution and then after 12 and 24 months of living in the new residential model. Adaptive behaviour, choice-making and life quality increased for both groups and there was no change in level of maladaptive behaviour compared with levels exhibited in the institution. However, there were some significant differences between the community and cluster centre group as the community group increased some adaptive skills, choice-making and objective life quality to a greater extent than the cluster centre group. Both cluster centre and dispersed community living offer lifestyle and skill development advantages compared with opportunities available in large residential institutions. Dispersed community houses, however, offer increased opportunities for choice-making, acquisition of adaptive behaviours and improved life quality for long-term institutionalized adults with IDs.
