Purpose In Saudi Arabia, the health system is mainly staffed by expatriate nurses from different cultural and linguistic backgrounds. Given the potential risks this situation poses for patient care, it is important to understand how cultural diversity can be effectively managed in this multicultural environment. The purpose of this study was to explore notions of cultural competence with non-Saudi Arabian nurses working in a major hospital in Saudi Arabia. Design Face-to-face, audio-recorded, semistructured interviews were conducted with 24 non-Saudi Arabian nurses. Deductive data collection and analysis were undertaken drawing on Campinha-Bacote’s cultural competence model. The data that could not be explained by this model were coded and analyzed inductively. Findings Nurses within this culturally diverse environment struggled with the notion of cultural competence in terms of each other’s cultural expectations and those of the dominant Saudi culture. Discussion The study also addressed the limitations of Campinha-Bacote’s model, which did not account for all of the nurses’ experiences. Subsequent inductive analysis yielded important themes that more fully explained the nurses’ experiences in this environment. Implications for Practice The findings can inform policy, professional education, and practice in the multicultural Saudi setting.

Purpose Samoan communities in Australia exhibit a disproportionate rate of kidney disease compared with other Australians. This article describes a research project that used a culturally sensitive framework, Fa’afaletui, to help reduce the barriers of language and culture and increase our understanding of the factors contributing to kidney disease, in one Samoan community in Australia. Design Semistructured group interviews were undertaken with Samoan community families and groups. The interviews were analyzed according to key concepts embedded in the Fa’afaletui framework. Findings Four factors associated with health risks in this Samoan community emerged—diet and exercise; issues related to the collective (incorporating the village, church, and family); tapu or cultural protocols; and the importance of language. Conclusions The findings suggest that future kidney health promotion initiatives within this Samoan community will be more effective if they are sensitive to Samoan cultural norms, language, and context.

The concept of cultural sensitivity is located within the tradition of anthropology and the history of colonisation and immigration in Australian society. This history provides a basis for examining the largely uncritical introduction of cultural considerations to the discipline of nursing. This paper argues that contemporary understandings of multiculturalism in nursing and health care policy tend to obscure, ignore and thus perpetuate notions of racial superiority. Recent works in transcultural nursing are med to illustrate the way in which ahistorical and therefore quite arbitrary traits are attributed to particular cultural groups. This perspective, given legitimacy in terms of cultural sensitivity, encourages political neutrality and thereby avoids questioning the discriminatory practices embedded in fundamental social relations.

We welcome Olga Kanitsaki’s comments on our paper ‘Rethinking cultural sensitivity’ (Nursing Inquiry 1996 3: 3-10) and appreciate the opportunity to respond. The main point we seek to emphasize here is the fundamental difference between our position and that of Kanitsaki. Our analysis is based on the recognition that the term ‘culture’ is historically and politically grounded. Its meaning changes over time and its contemporary usage (and popularity) thus demands explanation. The analytical task we undertook in our work was to emphasize the politics of culture rather than posing the political as one of a number of cultural dimensions (alongside the social, religious or technological)...

Esta revisión de la literatura tuvo como objetivo describir las actitudes hacia el VIH/SIDA, el cáncer y la Enfermedad de Alzheimer desde el modelo tripartito. Se revisaron 109 artículos publicados entre 2005 y 2015 en algunas bases de datos especializadas y herramientas de análisis de impacto. También se incluyeron fuentes secundarias ampliándose la búsqueda a los últimos 20 años (1995-2015). Los resultados mostraron que la mayoría de los estudios realizados sobre las actitudes hacia estas tres enfermedades son de tipo cuantitativo y la información se analizó con base en los componentes del modelo tripartito. Algunos aspectos sociodemográficos como el sexo y la edad están asociados con las actitudes hacia las tres enfermedades y predominan las creencias erróneas sobre ellas respecto a sus causas, curso y tratamiento. También predominan actitudes negativas hacia las tres enfermedades y las conductas e intenciones conductuales son diversas hacia cada una de ellas. No se hallaron antecedentes empíricos del estudio de la estructura de las actitudes propuesta por el modelo tripartito hacia las tres enfermedades. La Salud Pública ha liderado la investigación con base en el modelo de conocimientos, actitudes y prácticas propuesto por la OMS.

Syftet: Syftet med studien var att belysa specialistutbildade sjuksköterskors erfarenheter av transkulturell omvårdnad inom psykiatrisk vård. Metod: Studien genomfördes med en kvalitativ metod med enskilda semistrukturerade intervjuer. Specialistutbildade sjuksköterskor som arbetat inom psykiatrisk vård minst ett år fick berätta om sina erfarenheter av transkulturell omvårdnad. Resultat: I resultatet framkom det att specialistutbildade sjuksköterskor inompsykiatrisk vård upplever språket som ett hinder för omvårdnaden eftersom språket är så centralt inom psykiatrisk vård då samtal utgör en stor del under vårdtiden. Det visar också att religiösa och kulturella seder kan ha betydelse i möten med patienter från olika kulturer. Möjligheter att förstå varandra mellan personal, patienter och anhöriga som är delaktiga i vården lyfts fram. Anhöriga kan bidra med information och även stötta patienterna vilket är värdefullt för omvårdnaden. Tolk används ofta för att lösa kommunikationsproblem men de används inte i så stor utsträckning man skulle önska på grund av kostnader. Slutsats: De specialistutbildade sjuksköterskorna har i stor utsträckning, kunnat ta hänsyn till patientens önskemål och erbjudit patienten det bästa möjliga alternativ till god omvårdnad. Studien visar att det finns specialistutbildade sjuksköterskor inom psykiatrisk vård som anser sig behöva mer kunskap inom transkulturell omvårdnad.

A presente dissertação tem por objetivo analisar os aspectos religiosos islâmicos e as implicações das relações de gênero no islam sobre a assistência de saúde às mulheres muçulmanas, e através disto, discutir a importância do conhecimento prévio do islamismo pelos profissionais de saúde para propor uma assistência de saúde congruente a estas mulheres, tendo por referência a Política Nacional de Atenção Integral à Saúde da Mulher. Esta pesquisa tem abordagem qualitativa, com o desenvolvimento de pesquisa de campo e aplicação de um roteiro de perguntas semi-estruturado, com questões relacionadas ao islamismo e à saúde das mulheres. Ao todo, foram entrevistadas dez pessoas, sendo estas: quatro mulheres revertidas ao islam, três mulheres de família muçulmana, dois sheiks e uma assistente social. As entrevistas foram realizadas no Centro de Divulgação do Islam para a América Latina e o Caribe (CDIAL) e na Assembléia Mundial da Juventude Islâmica na América Latina (WAMY).

Tomando como punto de partida la invisibilidad de la producción científica de enfermería, en la perspectiva antropológica, en las revisiones de la literatura sobre antropología de la salud en Brasil, el objetivo de este artículo es realizar un breve análisis crítico de la relación entre enfermería y antropología puntuando las particularidades del contexto brasileño en comparación con países como Estados Unidos, Gran Bretaña y España y, en especial, discutir los dilemas (problemas y límites) en las relaciones existentes entre las dos disciplinas y señalar algunos desafíos para su desarrollo y consolidación. Por último, los argumentos y análisis comparativo de la construcción de esta relación en los diferentes países permiten ver ciertas hipótesis que apuntan a contribuir a los futuros debates y estudios en Brasil.

Justificación: La inmigración en España es un fenómeno de gran importancia que repercute en el área de la salud. Objetivo: Investigar la relación entre la elección de la anestesia epidural y la nacionalidad de la mujer, y a su vez, observar si influyen además otros factores en esta decisión. Diseño y Metodología: Estudio cuantitativo, transversal y descriptivo con una muestra de 634 mujeres que han dado a luz en el Hospital del Noroeste de la Región de Murcia. Las variables seleccionadas fueron: antecedentes obstétricos, edad, tipo de anestesia, nacionalidad de la mujer y motivo por el rechazo de la anestesia epidural. Resultados: El 8,51% de las mujeres que dieron a luz en el hospital del Noroeste de la Región de Murcia durante el año 2010 fueron inmigrantes. En cuanto a la nacionalidad, el 20,37% de las mujeres inmigrantes no han utilizado ningún tipo de anestesia. Se ha encontrado una significante diferencia con las mujeres no inmigrantes españolas en las que sólo el 4,31% la rechazó (P<0,001). En relación a los abortos, las mujeres que no prefieren ningún tipo de anestesia son aquellas que sí han tenido abortos (P<0.05). Las edades más jóvenes de las mujeres, de 16 a 25 años, se relaciona con el no uso de la anestesia epidural (P<0,05). Conclusión: Las mujeres inmigrantes hacen menos uso de la anestesia epidural. Es un reto para la enfermera aprender las diferentes culturas ya que realiza su trabajo en una sociedad cada vez más multicultural.

Introduction: female Genital Mutilation is internationally considered an affront on human rights and an act of violence against women and young girls. Furthermore, it hierarchises and perpetuates inequality and denies the right to bodily and psychosocial integrity of women and young girls. Aims: to detect the key points for the abolition of Female Genital Mutilation as well as the necessary resources for its eradication. Material and Method: a qualitative methodology with an ethnonursing perspective, via semi-structured interviews, held both individually and in groups, in 21 men familiar with Female Genital Mutilation. Findings: through the voices of men familiar with this tradition, five key points are presented for its gradual eradication: sensitisation and awareness building, team action, abolition-promoting media, focusing action on rural areas and applying educational means before punitive ones. Conclusion and practical implications: awareness-raising via the combined efforts of families, communities and governments, together with the promotion of health education programmes in demonstrating the complications derived from this practice, play a vital part in eradicating Female Genital Mutilation.

Objetivo: Explorar el conocimiento de los hombres procedentes de países donde se realiza la mutilación genital femenina sobre las consecuencias negativas en la salud de las mujeres afectadas. Métodos: Metodología cualitativa con enfoque etnometodológico, a través de entrevistas semiestructuradas individuales y grupales a 25 hombres, en relación con la mutilación genital femenina, seleccionados mediante triple muestreo. Se entregó una carta de presentación del estudio a los participantes y la declaración del consentimiento informado, y se les solicitó permiso para grabar la entrevista en audio. El análisis de los datos se realizó con el software informático Atlas. Ti7. Resultados: Los participantes contrarios al corte son conscientes de la diversidad de complicaciones físicas, obstétricas, psicológicas, sobre la sexualidad y sociales en las mujeres sometidas a mutilación. Sin embargo, los hombres que tienen un posicionamiento favorable muestran en general un desconocimiento de los problemas secundarios a esta práctica. Conclusiones: Los participantes procedentes de países donde se realiza la mutilación genital femenina, contrarios a mantener esta práctica, muestran un mayor conocimiento de las consecuencias negativas que los que se manifiestan a favor. El diseño de herramientas y programas de sensibilización destinados a la lucha contra la mutilación genital femenina debe visibilizar las complicaciones sobre la salud de las mujeres y las niñas, e incluir intervenciones familiares que impliquen a los hombres en el proceso de erradicación de esta práctica.

The desire to research on this subject arisen from the experience as nursing in the indigenous health, where I observed that many professionals from all regions of Brazil chose to work within this zone. It was notorious the nurse s difficult to settle in only one place for a long length of time. Probably due to health care in indigenous zones happens from a cultural confront. This confront materialize because both sides are imbued with their own culture: in one hand the nurse professional with its scientific knownledgment on the other the indigenous with their rituals and peculiars habits. In this context nurses should delineate and negotiate the reality through symbolic representations of life, and then make questions on the new reality. In this way, this study set out with the aim of apprehends the nurse s social representations of transcultural care in indigenous health. This knownledgment is important to avoid possible conflicts, shocks, difficulties and health care incongruence within this context. The data collect was carried out on a range of non structured interview guided by a pre-elaborated questionnaire with four questions and a hand drawing related to nurse s health care in the indigenous health. This research had a sample of 17 nurses from the Indigenous Sanitary District of Manaus in the Amazon State. To interpret data we used the Discourse of the Collective Subject, which findings were presented in three chapters: characterization of participants, discussion on themes prevalent in discourse; social representation of nursing care through infographics. The analysis revealed that the care in the indigenous health is challenging because the native people imbued in its world are perceived and processed according to the nurse s cultural lens, leading to materialize of some strangeness and adaptation difficulties, especially in the first contacts. The Social Representation on nursing practice, in many cases, is projected and contrived on the basis of scattered believes and on perception derived from common sense. The findings shows that representions are essential to mitigating the initial strangeness and help nurses to better situate themselves in the new universe. The nurse s practice in the indigenous health care should merge into each other. From the Social Representations is possible to perceive that assimilation, also comprehension on indigenous health system and its traditional knowledge are important to developing strategies to improve access and quality of care for indigenous peoples. After analysis the nurse s discourses and drawings, it is possible to represent the nurse s practice in the indigenous health as anthropophagism, since nurses should literally consuming its patients culture, digesting it and seize it as means to provide culturally congruent care. We highlight the urgent need for preparation and training of professionals to work more effectively with indigenous peoples
