The treatment of massive pulmonary embolism (PE) is a matter of debate. We present our institutional experience of patients suffering from massive PE with the aim of comparing the early results, the outcome and quality of life (QoL) between patients primarily assigned to either pulmonary surgical embolectomy (SE) or thrombolytic therapy (TL). A subgroup of patients (TS) with failed responses to TL requiring SE was separately analysed.

Background During recent years laparoscopic cholecystectomy has dramatically increased, sometimes resulting in overtreatment. Aim of this work was to retrospectively analyze all laparoscopic cholecystectomies performed in a single center in order to find the percentage of patients whose surgical treatment may be explained with this general trend, and to speculate about the possible causes. Methods 831 patients who underwent a laparoscopic cholecystectomy from 1999 to 2008 were retrospectively analyzed. Results At discharge, 43.08% of patients were operated on because of at least one previous episode of biliary colic before the one at admission; 14.08% of patients presented with acute lithiasic cholecystitis; 14.68% were operated on because of an increase in bilirubin level; 1.56% were operated on because of a previous episode of jaundice with normal bilirubin at admission; 0.72% had gallbladder adenomas, 0.72% had cholangitis, 0.36% had biliodigestive fistula and one patient (0.12%) had acalculous cholecystitis. By excluding all these patients, 21.18% were operated on without indications. Conclusions The broadening of indications for laparoscopic cholecystectomy is undisputed and can be considered a consequence of new technologies that have been introduced, increased demand from patients, and the need for practice by inexperienced surgeons. If not prevented, this trend could continue indefinitely.

Desarrollo y redacción de una guía basada en la mejor evidencia disponible sobre el diagnóstico y manejo de la ruptura aguda del ligamento cruzado anterior (LCA) para asistir a la toma de decisiones para controlar la variabilidad en los procesos clínicos de diagnóstico y manejo de dicha patología. Se definió el grupo de desarrollo de la guía (GDG) y de acuerdo con la metodología desarrollada en el Centro de Estudios del Hospital según convocatoria del Ministerio de Salud, se cumplieron las siguientes etapas : Priorización, definición de foco y alcance, formulación de las preguntas clínicas relevantes al estado del arte y puntos de controversia actual en el tema a tratar (Metodología PECOT), revisión sistemática de la literatura, síntesis de la evidencia, evaluación y graduación de acuerdo con la metodología "GRADE" y las platillas propuestas por SIGN. Por consenso se definieron las recomendaciones de acuerdo con la metodología GRADE. Se presenta una Guía de Práctica Clínica (GPC), que responde las preguntas: enfoque de hemartrosis, indicaciones quirúrgicas, terapia física preoperatoria, profilaxis antitrombótica, tipo de injerto, anestésicos intrarticulares, lesiones asociadas, posición del túnel femoral, reconstrucción de uno contra dos haces. De acuerdo con la evidencia encontrada se formulan doce recomendaciones ajustadas al nivel de evidencia. La presente GPC corresponde a un piloto de una metodología para desarrollo de GPC aprobabda para uso a nivel nacional y al primer producto sobre este tema en la literatura encontrada.

Objective: To determinate the corneal diagnoses of patients underwent Penetrating Keratoplasty (PK) in the university hospital of Sao Jose do Rio Preto, São Paulo, Brazil, and their relation to age group, gender and socioeconomic factors. Methods: Retrospective analysis of 1085 registration forms of patients underwent Penetrating Keratoplasty in the university hospital of Sao Jose do Rio Preto, São Paulo, Brazil, from 2000 to 2009. The surgical indications were then correlated to age group, gender and socioeconomic factors, by chi-square test. Results: Fifty-seven percent of all Penetrating Keratoplasties were performed in male patients, and the mean age was 52.3 years. Most surgeries (90.8%) occurred by means of public health care (SUS). The main diagnosis was leukoma (23%), representing 14% of the indications for PK in males and 9% in females. Leukoma was also the diagnosis in 40% of PK performed in patients older than 60 years and in 94.4% of these surgeries in public health care users. Conclusion: The main indication for Penetrating Keratoplasty was leukoma, predominating in males, ages above 60 years and in public health care users.

This work aims at contributing to increase and improve the communication between orthodontists and maxillofacial surgeons, reviewing and discussing the principles of diagnosis and orthodontic movement specific to patients with surgical indication. It describes the elective points in the conduct of the orthodontist so that their decisions could lead to an individualized and appropriate planning, striving for excellence in terms of outcomes for the surgical-orthodontic treatment of dentofacial discrepancies.

Despite improved medical treatment of rheumatoid arthritis, carpal tunnel compression, caput ulnae syndrome and palmar and dorsal tenosynovitis with potential tendon rupture represent urgent surgical indications. While diagnostic and therapeutic synovectomy may guide medical treatment, it should be performed before joint instability and destructive arthritis are established. Swan-neck and Boutonniere deformities as well as ulnar or radial drift of metacarpophalangeal (MCP) joints or the wrist can only be corrected when the involved joints are supple and intact. In the presence of destructive arthritis, partial and total wrist fusion, arthroplasties of the MCP joints and arthrodeses of the distal interphalangeal joints are recommended.

Ankle sprains are the most common injuries in sports, usually causing damage to the lateral ligaments. Recurrence has as usual result permanent instability, and thus loss of proprioception. This fact, together with residual symptoms, is what is known as chronic ankle instability, CAI, or FAI, if it is functional. This problem tries to be solved by improving musculoskeletal stability and proprioception by the application of bandages and performing exercises. The aim of this study has been to review articles (meta-analisis, systematic reviews and revisions) published in 2009-2015 in PubMed, Medline, ENFISPO and BUCea, using keywords such as “sprain instability”, “sprain proprioception”, “chronic ankle instability”. Evidence affirms that there does exist decreased proprioception in patients who suffer from CAI. Rehabilitation exercise regimen is indicated as a treatment because it generates a subjective improvement reported by the patient, and the application of bandages works like a sprain prevention method limiting the range of motion, reducing joint instability and increasing confidence during exercise. As podiatrists we should recommend proprioception exercises to all athletes in a preventive way, and those with CAI or FAI, as a rehabilitation programme, together with the application of bandages. However, further studies should be generated focusing on ways of improving proprioception, and on the exercise patterns that provide the maximum benefit.

Objective: Patients with high cervical spinal cord injury are usually dependent on mechanical ventilation support, which, albeit life saving, is associated with complications and decreased life expectancy because of respiratory infections. Diaphragm pacing stimulation (DPS), sometimes referred to as electric ventilation, induces inhalation by stimulating the inspiratory muscles. Our objective was to highlight the indications for and some aspects of the surgical technique employed in the laparoscopic insertion of the DPS electrodes, as well as to describe five cases of tetraplegic patients submitted to the technique. Methods: Patient selection involved transcutaneous phrenic nerve studies in order to determine whether the phrenic nerves were preserved. The surgical approach was traditional laparoscopy, with four ports. The initial step was electrical mapping in order to locate the "motor points" (the points at which stimulation would cause maximal contraction of the diaphragm). If the diaphragm mapping was successful, four electrodes were implanted into the abdominal surface of the diaphragm, two on each side, to stimulate the branches of the phrenic nerve. Results: Of the five patients, three could breathe using DPS alone for more than 24 h, one could do so for more than 6 h, and one could not do so at all. Conclusions: Although a longer follow-up period is needed in order to reach definitive conclusions, the initial results have been promising. At this writing, most of our patients have been able to remain ventilator-free for long periods of time.

The formation of an intestinal stoma is one of the most frequent operations in visceral surgery. Despite new operative techniques and a more restrictive use of the stoma, the stoma formation remains an often necessary surgical procedure, which results to a dramatic change in the patients' life. The stoma formation and its later closure are associated with a high morbidity. Many complications, such as stoma necrosis, stoma retraction or stoma prolapse, are related to surgical mistakes made during stoma formation. These complications are therefore largely avoidable. The stoma formation needs careful planning together with a professional stoma nursing team. Moreover, it is mandatory that the stoma formation is made with great care and that it meticulously follows the well established surgical principles. A perfectly placed, technically correctly fashioned and easy to care for stoma is essential for a good patients'quality of life.
