We report the case of a 46-year-old man found dead in his apartment in a chair with a gunshot wound in his chest. All circumstances pointed to suicide as the manner of death. Finding the weapon, a SIG Sauer P228 pistol, about 2 m away from the decedent with an obstacle between weapon and corpse however generated speculation about third party involvement. Scene investigations and ballistic calculations showed that with a high probability the weapon must have been purposefully thrown away by the decedent after he fired the lethal gunshot.

O objeto deste estudo consiste na violência autoinfligida em mulheres por queimadura. As lesões por queimadura são consideradas causas externas (acidentes e violências) e tem contribuído para o aumento geral dos índices de morbimortalidade acarretando perda de anos de vida produtiva. São resultantes de múltiplos fatores como condições socioeconômicas, violências e desigualdade de gênero. Esta pesquisa teve como objetivos: analisar o perfil sociodemográfico das mulheres que vivenciaram queimadura autoinfligida; descrever as circunstâncias e o contexto social relacionados à queimadura autoinfligida em mulheres; analisar os fatores motivadores da queimadura autoinfligida em mulheres; e, discutir a queimadura autoinfligida em mulheres na perspectiva de gênero. Trata-se de uma pesquisa de abordagem qualitativa e exploratória. Os cenários da pesquisa foram dois Centros de Tratamento de Queimados (Municipal e Federal) localizados no Estado do Rio de Janeiro. Os sujeitos do estudo foram 10 mulheres com história de queimadura autoinfligida e que não tivessem história de tentativa de suicídio anterior e diagnóstico de sofrimento psíquico, uma vez que estas situações poderiam comprometer a análise das vivências de violência. A coleta de dados foi realizada através de entrevista semi-estruturada, com roteiro previamente elaborado, no período de novembro de 2009 a março de 2010. Os dados foram analisados através da técnica de Análise de Conteúdo de Bardin, tendo emergido duas categorias: a) A vida da depoente antes da queimadura: percepção da sua condição pessoal; relações familiares envolvendo mãe, pai, avós, irmãos e filhos; relações sociais e relação com o companheiro; b) Queimadura autoinfligida em mulheres: uma questão de violência de gênero: fatores motivadores da queimadura autoinfligida na perspectiva da mulher e queimadura autoinfligida como desfecho da vivência de violência conjugal. As participantes do estudo caracterizavam-se por ter uma vida, anterior ao evento da queimadura, marcada pela violência na relação familiar e, principalmente, com o parceiro. A constante vivência de violência presente na relação com o parceiro, manifestadas por diferentes expressões (físicas, sexuais e psicológicas) resultou em intenso sofrimento que culminou na queimadura autoinfligida. Ficou evidenciado que a queimadura autoinfligida, no grupo estudado foi uma tentativa de interromper com a violência de gênero vivenciada. A escolha pelo fogo foi justificada pelas mulheres como um elemento capaz de produzir maior letalidade e ser de fácil acesso no ambiente doméstico. As mulheres afirmam que o evento da queimadura autoinfligida promoveu transformações em suas vidas. Nos agravos à saúde da mulher, em especial na violência autoinfligida por queimaduras, é relevante considerar as questões de gênero como estratégia de ação e ampliação das práticas de cuidado.

Non-suicidal self-injury (NSSI) is the deliberate, self-inflicted destruction of body tissue without suicidal intent and an important clinical phenomenon. Rates of NSSI appear to be disproportionately high in adolescents and young adults, and is a risk factor for suicidal ideation and behavior. The present study reports the psychometric properties of the Impulse, Self-harm and Suicide Ideation Questionnaire for Adolescents (ISSIQ-A), a measure designed to comprehensively assess the impulsivity, NSSI behaviors and suicide ideation. An additional module of this questionnaire assesses the functions of NSSI. Results of Confirmatory Factor Analysis (CFA) of the scale on 1722 youths showed items' suitability and confirmed a model of four different dimensions (Impulse, Self-harm, Risk-behavior and Suicide ideation) with good fit and validity. Further analysis showed that youth׳s engagement in self-harm may exert two different functions: to create or alleviate emotional states, and to influence social relationships. Our findings contribute to research and assessment on non-suicidal self-injury, suggesting that the ISSIQ-A is a valid and reliable measure to assess impulse, self-harm and suicidal thoughts, in adolescence.

In cases of ligature strangulation, the importance of distinguishing self-inflicted death from homicide is crucial. This entails objective scene investigation, autopsy and anamnesis in order to elucidate the manner of death correctly. The authors report a case of unplanned complex suicide by means of self-strangulation and multiple sharp force injury. The use of more than one suicide method, consecutively--termed unplanned complex suicide--gives this case particular significance. A brief discussion on this uncommon method of suicide is presented, particularly relevant to the attending forensic physician. In addition, a short overview of the entity of complex suicide is given.

L’étude qui nous concerne vise à mieux comprendre la violence en milieu carcéral. C’est à l’aide d’entrevues menées auprès de treize hommes incarcérés à l’Unité spéciale de détention, une unité canadienne spécifiquement conçue pour loger des personnes qui ont fait usage de violence intra murale ou sont soupçonné de l’avoir fait, que nous avons tenté d’atteindre l’objectif principal de ce projet. Plus spécifiquement, le point de vue des participants a été recueilli sur l’incarcération, sur les moyens utilisés afin de s’accoutumer aux défis de l’enfermement ainsi que la manière dont les individus justifient l’usage de violence dans ce contexte. Les résultats de nos analyses suggèrent que le milieu carcéral est un monde hostile et imprévisible où les individus se perçoivent constamment vulnérables à de multiples formes d’agression. Placés en hyper vigilance et orientés vers la survie, les détenus font usage de diverses stratégies d’adaptation afin de répondre aux situations difficiles qu’ils rencontrent en contexte de perte de liberté. La violence représente donc un des moyens dont disposent les individus pour survivre à l’intérieur des murs. Les diverses formes d’adaptation violente rencontrées en milieu carcéral peuvent ainsi informer sur les enjeux de survie présents dans un milieu et donc, du risque de violence ou de victimisation dans ce milieu, que cette violence soit interpersonnelle, collective, auto dirigée, axée sur la fuite ou contre les biens. Puisque l’usage de violence par une personne engendre une possibilité accrue de choisir la violence pour une autre, et ce, avec les conséquences qui s’en suivent pour les détenus et les membres du personnel, nous avons tenté d’identifier et de comprendre les diverses logiques d’action qui motivent le choix d’avoir recours à la violence dans les institutions carcérales. Or, il appert que certaines caractéristiques des individus tendent à faire augmenter le risque pour une personne d’avoir recours à la violence carcérale. De même, il semble que certaines institutions sont davantage propices à l’usage de violence que d’autres. De surcroît, des éléments appartenant aux individus et au milieu en interrelation semblent favoriser la possibilité qu’un condamné fasse usage de violence intra murale. Ainsi, le recours à la violence est davantage probable si elle est légitimée par les individus et le milieu, si le niveau d’adhésion aux valeurs des sous-cultures délinquantes des individus est élevé et si la philosophie du milieu en favorise le maintien, si des groupes influents en quête de contrôle et de pouvoir sont présents dans le milieu et qu’un individu désire y être affilié, s’il y a présence de marchés illicites comme réponse à la privation et qu’un individu y participe ou encore, si le milieu et l’individu sont pris dans l’engrenage d’un climat de survie. Par conséquent, une réflexion concernant le sentiment de sécurité préoccupant les reclus, sentiment qui est un besoin fondamental chez tous les êtres humains, un retour sur les divers modèles théoriques en fonction des données que nous avons obtenues ainsi qu’une réflexion portant sur les moyens dont disposent les individus et le milieu afin de diminuer les possibilités que la violence soit utilisée, ont été amorcées en conclusion.

This paper examines performances that defy established representations of disease, deformity and bodily difference. Historically, the ‘deformed’ body has been cast – onstage and in sideshows – as flawed, an object of pity, or an example of the human capacity to overcome. Such representations define the boundaries of the ‘normal’ body by displaying its Other. They bracket the ‘abnormal’ body off as an example of deviance from the ‘norm’, thus, paradoxically, decreasing the social and symbolic visibility (and agency) of disabled people. Yet, in contemporary theory and culture, these representations are reappropriated – by disabled artists, certainly, but also as what Carrie Sandahl has called a ‘master trope’ for representing a range of bodily differences. In this paper, I investigate this phenomenon. I analyse French Canadian choreographer Marie Chouinard’s bODY rEMIX/gOLDBERG vARIATIONS, in which 10 able-bodied dancers are reborn as bizarre biotechnical mutants via the use of crutches, walkers, ballet shoes and barres as prosthetic pseudo-organs. These bodies defy boundaries, defy expectations, develop new modes of expression, and celebrate bodily difference. The self-inflicted pain dancers experience during training is cast as a ‘disablement’ that is ultimately ‘enabling’. I ask what effect encountering able bodies celebrating ‘dis’ or ‘diff’ ability has on audiences. Do we see the emergence of a once-repressed Other, no longer silenced, censored or negated? Or does using ‘disability’ to express the dancers’ difference and self-determination usurp a ‘trope’ by which disabled people themselves might speak back to the dominant culture, creating further censorship?

The current program of research addresses the need for multi-level programs to target the major increase in injury rates that occurs throughout adolescence. Specifically, it involves the investigation of school connectedness as a protective factor for adolescent injury, and the development of school connectedness as a component of an injury prevention program. To date, school-based risk taking and injury prevention has frequently been limited to addressing adolescents' knowledge and attitudes to risk behaviours, and has largely overlooked the importance of the wider school social context as a protective factor in adolescent development. Additionally, school connectedness has been primarily studied in terms of its impact on student achievement, wellbeing and risk taking behaviour, and research has not yet addressed possible links with injury. Further, school connectedness intervention programs have targeted risk taking behaviours without evaluating their potential impact on injury outcomes. This is the first reported research to develop strategies to increase school connectedness as part of a school-based injury prevention program. The research program was conceptualised as three distinct stages. The development of these research stages was informed by a comprehensive review of the literature on adolescent risk taking, injury and school-based prevention, as well as on school connectedness and its importance in adolescence. A review of the school connectedness literature indicated that students' connectedness is largely influenced by relationships within the school context including with teachers and other school staff, and is therefore a potentially modifiable factor that may be targeted in school-based programs. Overall, the literature shows school connectedness to be a key protective factor in adolescent development. This review established a foundation from which the current program of research was designed. The first stage of the research involved an empirical investigation of the relationship between adolescent risk taking-related injuries and school connectedness. Stage one incorporated two studies. The first involved the development of a measure of adolescent injury, the Extended Adolescent Injury Checklist (E-AIC), for use in the current research as well as in future school-based studies and program evaluation. The results of this study also highlighted the extent of the problem of risk-related injury in adolescence. The second study in Stage one examined the relationship between students' reports of school connectedness, risk taking behaviour and risk taking-related injuries on the E-AIC. The results of this study showed significant relationships between increased school connectedness and reduced reported engagement in transport and violence risk taking, and fewer associated injuries. This study therefore suggested the potential for school-based injury prevention programs to incorporate strategies targeting increased adolescent connectedness to school. The second stage of this research involved the compilation of an evidence base to inform the design of a school connectedness intervention. Stage two also incorporated two studies. The first study in Stage two involved a systematic review of programs that have targeted school connectedness for reduced risk taking and injury. The results of this study revealed that interventions targeting school connectedness can be effective in reducing adolescent risk taking behaviour, and also provided an evidence base for the design of the current school connectedness intervention. The second study in Stage two examined teachers' understanding and perceptions of school connectedness. This qualitative study indicated that teachers consider students' connectedness to be an important factor that relates to their risk taking behaviour; and also provided directions and content for the intervention design stage. The third stage of this research built upon the findings of each of the previous studies, and involved the design, implementation and evaluation of a school connectedness intervention as a component of an adolescent injury prevention program, Skills for Preventing Injury in Youth (SPIY). This connectedness intervention was designed as a professional development workshop for teachers of 13 to 14 year old adolescents, and was developed as a complementary component to the curriculum-based SPIY program. The SPIY connectedness component was implemented and evaluated using process and six-month impact evaluation methodologies. The results of this study revealed that teachers saw value in the program and made use of the strategies presented, and that program school students' self-reported violence risk behaviour was reduced at six-month follow-up. Despite these promising findings, the results of this study did not demonstrate a significant impact of the program on change in students' connectedness to school, relative to comparison schools. The positive impact on self-reported violence risk behaviour was however replicated in additional analyses comparing students participating in the connectedness version of SPIY with students participating in an earlier curriculumonly version of the program. This finding indicated that the connectedness component has additional benefits relating to reduction in violence risks, over and above a curriculum-only version of the program. This research was the first reported to address the relationship between school connectedness and adolescent injury outcomes, and to develop school connectedness as a component of an adolescent injury prevention program. Overall, the results of this program of research have demonstrated the importance of incorporating strategies targeting the wider school social context, including school connectedness, in adolescent injury prevention programs. This research has important implications for future research and practice in adolescent injury prevention.

"I want there now!" Images of the future in the drawings of Ghanaian, British, Finnish and Tanzanian children The aim of the Thesis is to examine what kind of images children aged between 8 and 12 years in Ghana, Great Britain, Finland and Tanzania hold of their future. The study is a qualitative analysis based on data that consists of 214 drawings, digitally photographed and analysed using AtlasTI computer program. The data was coded and divided into " families " whose frequencies were compared in order to maintain results. The assignment was given to the children in the language used in the school and it was similar in each country. The children were introduced to the idea with a set of stimulating questions and after that they were asked to draw and colour with a pen a picture of 1) him/herself as an adult, 2) his/her future home and 3) the people and the animals they think they will be living with. The children were also asked to write down the country and the place they believe to live in their future. They were also encouraged to write down e.g. their dream profession, what they would like to have as hobbies in their adulthood and other important things in their lives. The analysis focuses on the content of the drawings instead of artistic or psychological interpretations. The differences between the drawings from different countries as well as the differences in a single country were significant. The current trends and the experiences children had lived through were present in the drawings. There were no differences in the colour of the skin and the most popular professions were doctor, police and football player. The football was very strongly present in all of the data; almost fourth of the children had either drawn or mentioned football as their future profession or hobby. Different flora was present in 84% of Tanzanian and 70% of Ghanaian drawings compared to the 23% of British and 17% of Finnish drawings. Of all the family forms a dream of the traditional nuclear family could be found among 29% of the drawings. Compared to that a total of 30% wanted either a child of their own without a spouse (15%) or a spouse without a child (15%). Also grandparents, childhood friends, other adults, animals or even space aliens were present and sharing a home with the children in the drawings. Of all children 15% wanted to live totally alone in the future. This was most typical (38%) among the Finnish boys. The cat and the dog were the most popular animals to appear in the drawings. Moving abroad proved to be purely a western phenomenon, dreamed by 57% of the British and 18% of the Finnish children. As results, a negative self-image, violence, lack of expressions of positive feelings and solitude among the Finnish boys in the data were very concerning. ABSTRACT Mental disorders and suicide attempts among acute burn patients were investigated in a collaborative research project between National Institute for Health and Welfare and Departments of Psychiatry and Plastic surgery in University of Helsinki in Finland. This project was realized in two parts. The first cohort of burn patients consisted of all burn patients admitted to the Helsinki Burn Centre during 1989 97. In this retrospective cohort, 5.7% (N=46) of the total of 811 burn patients had attempted suicide. The burn severity of suicide attempters was markedly higher than in the other burn patients. Suicide attempters were more often unemployed or on disability pension and had psychiatric history before the injury. The second sample was a prospective cohort of all acute consecutive burn patients admitted to the Helsinki Burn Centre during 18 months in 2006- 2007. All subjects (N=107) of the cohort were interviewed with the Structured Clinical Interview for DSM-IV for Axis I and II mental disorders (SCID-I and SCID-II) at baseline and then 86 % of all (N=92) with SCID-I at the end of six-month follow-up. Most (61%) patients had at least one lifetime mental disorder before burn; 47 % substance-related, 10% psychotic and 23% personality disorders. The overall prevalence of Axis I mental disorders increased significantly from the month prior to burn to acute care but decreased significantly from acute care to six months. However, more than one half (55%) of the cohort suffered from some mental disorder during follow-up. Less than one half of the burn patients with estimated need for psychiatric care received psychiatric care. Burn severity independently and strongly predicted risk for mental disorders during follow-up and pre-burn psychiatric history, severe burns and estimated need for psychiatric care significantly predicted psychiatric care received. The proportion of patients with self-inflicted burns is not high but mental disorders are common among burn patients. Mental disorders may predispose to burns. After burn injury, more than half of the patients suffer from mental disorders and a strong relationship exists between burn severity and some post-burn mental disorders. A minority of the patients with unequivocal need for psychiatric care actually receive it. Psychiatric consultations and care follow mainly the course of acute burn treatment.

During the last 30 years governments almost everywhere in the world are furthering a global neoliberal agenda by withdrawing the state from the delivery of services, decreasing social spending and lowering corporate taxation etc. This restructuring has led to a massive transfer of wealth from the welfare state and working class people into capital. In order to legitimize this restructuring conservative governments engage in collective blaming towards their denizens. This presentation will examine some of the well circulated phrases that have been used by the dominant elite in some countries during the last year to legitimize the imposition of austerity measures. Phrases such as, ‘We all partied’ used by the Irish finance minister, Brian Lenihan, to explain the Irish crisis and collectively blame all Irish people, ‘We must all share the pain’, deployed by another Irish Minister Gilmore and the UK coalition administration’s sound bite ‘We are all in this together’, legitimize the imposition of austerity measures. Utilizing the Gramscian concept of common sense (Gramsci, 1971), I call these phrases ‘austerity common sense’. They are austerity common sense because they both reflect and legitimate the austerity agenda. By deploying these phrases, the ruling economic and political elite seek to influence the perception of the people and pre-empt any intention of resistance. The dominant theme of these phrases is that there is no alternative and that austerity measures are somehow self-inflicted and, as such, should not be challenged because we are all to blame. The purpose of this presentation is to explore the “austerity common sense” theme from a Gramscian approach, focus on its implications for the social work profession and discuss the ways to resist the imposition of the global neoliberal agenda.

Cette étude vise à comprendre et à expliquer la résilience des familles endeuillées par le suicide d’un adolescent . Lorsque le suicide d’un adolescent survient, comme il s’agit d’une mort violente, inattendue, auto-infligée et qu’elle ne s’inscrit pas dans l’ordre normal de la vie, la famille est confrontée à une situation de crise qui nécessite, de sa part, une mobilisation importante d’énergie. On remarque que, malgré cette épreuve, la plupart des familles continuent à fonctionner. Le concept de résilience familiale qui fait référence à la capacité d’une famille de rebondir face à une situation de crise peut expliquer ce phénomène. C’est pourquoi le but de cette étude a été de proposer une théorie du processus de résilience à partir de ce que vivent les familles endeuillées par le suicide de leur adolescent. Le modèle McGill en soins infirmiers constitue la toile de fond théorique compte tenu de sa vision familiale, de sa croyance dans les forces de la famille et de l’importance du rôle de collaboration des infirmières. Pour atteindre le but de notre étude, une approche par théorisation ancrée, inspirée de la vision straussienne, a été choisie. L’échantillonnage théorique a été constitué de données obtenues à l’aide : d’entrevues semi structurées (13) avec les membres de sept familles rencontrés soit de façon individuelle, en couple ou en groupe (n=17 participants); de documents personnels remis par les participants (journal intime, homélie …); d’un questionnaire sociodémographique; et de notes de terrain. Une analyse comparative continue des données à travers une triple codification a permis de proposer une théorisation en profondeur du processus de résilience familiale suite au suicide d’un adolescent. Les principaux résultats indiquent que, dans un premier temps, la famille est confrontée à un cataclysme engendré par le suicide lui-même et influencé par le contexte familial, le contexte social et les émotions vécues. S’ensuit une période de naufrage plus ou moins importante compte tenu des bouées de sauvetage présentes au sein et dans l’entourage des familles. La présence de ces bouées intra et extrafamiliales permet un rebondissement plus ou moins rapide des familles. Par la suite, différentes actions intra et extrafamiliales permettent aux familles d’émerger malgré cette blessure indélébile, c’est-à-dire apprendre et grandir à travers cette expérience. L’analyse a également permis de dégager quatre types de résilience familiale selon que le rebondissement est rapide ou tardif et que l’émergence est continue ou non, soit le processus de résilience des familles énergiques centrifuge et centripète, de la famille stupéfaite, de la famille combattante et de la famille tenace. Cette étude permet d’approfondir la compréhension du vécu des familles endeuillées par le suicide d’un adolescent et d’aider les professionnels de la santé à mieux adapter leurs interventions en fonction des besoins des familles, des bouées de sauvetage présentes et des actions susceptibles de favoriser leur émergence.

El presente artículo está orientado a analizar los fenómenos de marcación corporal autoinfligida desde el psicoanálisis anglosajón y bajo la luz de la teoría sobre la relación de objeto y la línea de desarrollo psíquico derivada del psicoanálisis infantil. Dentro de la revisión de la literatura se hacen paralelos entre la teoría del desarrollo del self, los estudios derivados del psicoanálisis culturalista y las investigaciones hechas en el campo de la psicosomática, con el fin de estudiar desde diferentes perspectivas el fenómeno de las marcas corporales autoinfligidas.

Introducción: el Trastorno Límite de la Personalidad afecta del 2% al 6% de los adultos en Estados Unidos. Es una condición de alta relevancia dentro de las patologías psiquiátricas debido a características como impulsividad, inestabilidad en las relaciones interpersonales, disregulación en el estado de ánimo y comportamiento agresivo. Esto determina un impacto negativo en la funcionalidad del individuo siendo la agresividad contra sí mismo o contra otras personas uno de sus componentes claves. Métodos: Revisión sistemática de la literatura de artículos de bases de datos y búsqueda manual de revistas relacionadas que aportaran la mejor evidencia con el fin de encontrar estudios que evaluaran, con instrumentos objetivos, los tratamientos farmacológicos disponibles para el manejo de la agresividad en el TLP .Se evaluó calidad metodológica y los estudios se organizaron en tablas de evidencia. Resultados: La búsqueda arrojo 1081 artículos de los cuales se seleccionaron 52 como potenciales y cinco fueron incluidos en esta revisión. Se clasificaron como nivel de evidencia Ib. El topiramato, el aripiprazol, el divalproato y la fluoxetina mostraron mejores resultados que el placebo especialmente en agresividad e impulsividad. El topiramato fue asociado con pérdida de peso. Los medicamentos fueron seguros y bien tolerados. Discusión: Los medicamentos evaluados mostraron ser mejores que placebo. La diversidad en las escalas utilizadas genera complejidad en la interpretación de resultados. Conclusión: La evidencia sugiere que el tratamiento farmacológico es efectivo en síntomas como agresividad e impulsividad comparado con placebo. Deben considerarse estudios que evalúen combinaciones de fármacos y psicoterapia.

Objective: To report a rare case of self-inflicted eye injury secondary to obsessive-compulsive disorder (OCD).Method: Case report.Results: A 41 year-old patient had significant bilateral visual loss and blunt trauma through touching his ocular cavities due to recurrent doubts regarding their shape and urges to check the format of bones and cartilages. Differential diagnosis with tic disorders is discussed, and the patient's treatment approach is described.Conclusion: Although rare, severe clinical complications can occur in OCD.
